Background/Objectives: Spinal diffusion tensor imaging (sDTI) remains a challenging method for the selective evaluation of key anatomical structures, like pyramidal tracts (PTs) and dorsal columns (DCs), and for reliably quantifying diffusion metrics such as fractional anisotropy (FA), radial diffusivity (RD), mean diffusivity (MD), and axial diffusivity (AD). This prospective, single-center study aimed to assess the reproducibility, robustness, and reliability of an optimized axial sDTI protocol, specifically intended for long fiber tracts. Methods: We developed an optimized Stejskal–Tanner sequence for high-resolution, axial sDTI of the cervical spinal cord at 3.0 T. Using advanced standardized evaluation and post-processing methods, we estimated DTI values for PTs, DCs, and AHs at the level of the second cervical vertebra. Reliability was evaluated through repeated measurements in 16 healthy volunteers and by comparing results from two 3.0 T scanners (Magnetom Skyra and Magnetom Prisma, Siemens Healthineers, Erlangen, Germany). Reproducibility was assessed using paired t-tests, intraclass correlation coefficients (ICCs), Bland–Altman analysis, and coefficients of variation (CVs). Results: The optimized sDTI protocol demonstrated high consistency for FA between test–retest sessions and across scanners. For the Skyra, the DC region showed the highest reliability (average ICC = 0.858) followed by the PT region (average ICC = 0.789). On the Prisma, the PT region reached an average ICC of 0.854, with the DC region at 0.758. Pooled inter-scanner data indicated good-to-excellent agreement, particularly in the PT region (average ICC = 0.860). FA CVs remained low (<10%) across all regions and scanners. RD showed good-to-excellent ICC values for PTs and DCs (average ICC for Skyra 0.642 and 0.769 and 0.926 and 0.830 for Prisma, respectively) but showed a higher CV between 14.6 and 19.4% for these two scanners. Conclusions: Improved sDTI offers highly reproducible FA measurements for all metrics with scanner independence, supporting its potential as a robust tool for detecting and monitoring spinal cord pathologies. This study examined the criterion validity of the Chronojump photocell compared to the Witty photocell and studied the instrument’s within-session test–retest reliability. Forty-five university students and physically active males performed ten trials on 10 m linear sprints. Times were recorded simultaneously by both devices to obtain paired outcomes. The main results displayed the significant mean differences in physically active university students between devices (diff = 0.05 s, p < 0.001; trivial ES = 0.1), high Spearman’s correlations (rs = 0.99; p < 0.0001), substantial CCC (0.98), and SEE = 0.05 s. Bland-Altman’s plot denoted low systematic errors (0.05 s) with no heteroscedasticity (R2 = 0.004). The within-session reliability (internal consistency) was high (ICC = 0.88; SEM = 1.14 s; CV = 0.23). In addition, the sensitivity of the instrument showed values of SWC = 0.08 s and SDC = 0.4 s. In conclusion, the Chronojump photocell is a valid instrument for the estimation of time in the acceleration phase of a linear sprint in physically active university students. Additionally, this device is shown to be a reliable tool in the measurement of consecutive trials of linear sprints. Full article

This research compares speech discrimination abilities between 17 children who are hard-of-hearing (CHH) and 13 children with normal hearing (CNH), aged 9 to 36 months, using either a conditioned head turn (CHT) or condition play paradigm, for two phoneme pairs /ba-da/ and /sa-ʃa/. As CHH were tested in the aided and unaided conditions, CNH were also tested on each phoneme contrast twice to control for learning effects. When speech discrimination abilities were compared between CHH, with hearing aids (HAs), and CNH, there were no statistical differences observed in performance on stop consonant discrimination, but a significant statistical difference was observed for fricative discrimination performance. Among CHH, significant benefits were observed for /ba-da/ speech discrimination while wearing HAs, compared to the no HA condition. All CHH were early-identified, early amplified, and were enrolled in parent-centered early intervention services. Under these conditions, CHH demonstrated the ability to discriminate speech comparable to CNH. Additionally, repeated testing within 1-month did not result in a change in speech discrimination scores, indicating good test–retest reliability of speech discrimination scores. Finally, this research explored the question of infant/toddler listening fatigue in the behavioral speech discrimination task. The CHT paradigm included returning to a contrast (i.e., /a-i/) previously shown to be easier for both CHH and CNH to discriminate to examine if failure to discriminate /ba-da/ or /sa-ʃa/ was due to listening fatigue or off-task behavior. Pregnancy-related cytomegalovirus (CMV) infection may have negative consequences on the developing fetus and child. In Romania, screening for CMV infection is included in the first prenatal evaluation. We aimed to evaluate the seroprevalence rates in pregnant women who underwent screening over 15 years (May 2008–February 2023). We evaluated 28,012 pregnant women, from whom 23,970 (85.57%) had an index CMV screening, and 4082 had at least two presentations during the same or consecutive pregnancies. A total of 32,290 paired anti-CMV IgM/IgG serological tests were performed. Passed infection with IgG positivity represented 90.15% (29,110) of all tests, corresponding to 28,649 women (88.72%). The seroprevalence increased with women’s age, was more frequently associated with rural residence, and decreased in time intervals. A total of 2322 women (9.69%) displaying an IgM/IgG negative pattern were at risk of acquiring the infection during pregnancy. Only 144 out of 2342 (6.14%) women at risk presented twice during the same pregnancy, of which 12 women (0.51%) displayed a pattern of primary infection. Our population from Northwest Romania shows a high rate of immunization against CMV infection and a low risk of primary infection. We found a low adherence to retesting in cases of probable primary CMV infections, which highlights the need for infection mitigation by hygiene measures and improvement of the existing protocols. Background and Objectives: The social and economic attitudinal dimensions of sustainability are difficult to estimate and are often overlooked in consumer research. To date, few scales can be used to assess in a combined manner the importance given by consumers to social, economic and environmental sustainability. In the frame of the EU-funded HealthFerm Project, a scale to estimate the relative importance of three dimensions of sustainability was necessary. Therefore, the objective was to develop and test a short scale that would englobe all the keywords that are present in the definition of sustainable development and that would assess the importance of each dimension in the eyes of consumers. Methods: A test-retest reliability study with a sample of volunteers (n = 15) who agreed to fill out the questionnaires twice. Cronbach’s alpha was used to evaluate internal consistency. A paired samples t-test was used to evaluate the repeatability of the scale. Results: Of the 15 respondents, seven were women (46.7%); the mean age was 23.5 years (s.d.: 2.4); 66.7% lived in an urban area and 33.3% in a suburban area; 53.3% had a higher education; 46.7% of the sample described their economic situation as neither easy nor difficult. Regarding their diet, 26.7% were flexitarian, 66.7% omnivore and 6.7% pescetarian. There was no difference (paired samples t-test; p-value > 0.05) between the two time points, indicating that the scale is reliable. Furthermore, the scale showed very good internal consistency (Cronbach’s alpha of 0.98). Conclusion: The short scale can be used to evaluate how important the different dimensions of sustainability of fermented plant-based foods are to consumers. Full article

Despite the availability of nutritional recommendations, studies have reported inadequate nutrition in athletes. The existing literature highlights the importance of the nutritional knowledge of both athletes and coaches in influencing athletes’ food choices and behavior, as well as its direct and indirect impact on athletes’ performance and health. To adequately assess nutritional knowledge, monitoring via valid and reliable questionnaires is required. As no questionnaire tailored to German-speaking athletes and coaches exists, this study aimed at developing a new General and Sports Nutritional Knowledge Questionnaire for Athletes and Coaches (GSNKQ-AC). The development followed a literature-based, ten-step validation approach. The initial questionnaire (63 items) was revised and reduced to 29 items in the final version after conducting construct verification in the target group (n = 84 athletes and coaches), evaluating content validity by a panel of nutrition experts (n = 8), verifying face validity by think-out-loud interviews in the target group (n = 7), and conducting classical test theory for item reduction analysis (n = 53). For the final GSNKQ-AC, internal consistency, calculated as Cronbach’s alpha, was 0.87. Students with a focus on sports nutrition (n = 31) scored significantly higher than athletes and coaches (n = 53), revealing good construct validity (77% vs. 62%, p < 0.001). Test–retest reliability (n = 42, matched pairs) showed a Spearman’s correlation coefficient of r = 0.61 (p < 0.01). The brief GSNKQ-AC can be used for status quo or longitudinal assessment of nutritional knowledge among athletes and coaches to reveal gaps and ensure purposeful planning of educational interventions. Low back pain (LBP) is one of the main musculoskeletal pain conditions, and it affects 23–28% of the global population. Strong evidence supports the absence of a direct relationship between the intensity of pain and tissue damage, with psychosocial factors also playing a crucial role. In this context, the Pain Attitudes and Beliefs Scale for Physiotherapists (PABS-PT) is a useful tool for evaluating physiotherapists’ treatment orientations and beliefs regarding the management of low back pain (LBP). It helps identify practitioners who may benefit from additional education in modern pain neuroscience. However, there is not a Spanish validation of this scale for physiotherapists. Thus, the aims of this study were to translate and culturally adapt the Pain Attitudes and Beliefs Scale for Physiotherapists (PABS-PT) into Spanish and to evaluate its psychometric properties. This validation study used three convenience samples of physiotherapists (PTs) (n = 22 for the pilot study, n = 529 for the validity study and n = 53 for assessing the instrument’s responsiveness). The process of translating and adapting the PABS-PT into Spanish followed international guidelines and produced a satisfactory pre-final version of the questionnaire. Factor analysis confirmed the two-factor structure of the original version, with the biomedical (BM) factor explaining 39.4% of the variance and the biopsychosocial (BPS) factor explaining 13.8% of the variance. Cronbach’s alpha values were excellent for the BM factor (0.86) and good for the BPS factor (0.77), indicating good internal consistency. Test–retest reliability was excellent for both factors, with intraclass correlation coefficients (ICCs) of 0.84 for BM and 0.82 for BPS. The standard error of measurement (SEM) was acceptable for both factors (3.9 points for BM and 2.4 points for BPS). Concurrent validity was moderate and in the expected direction and had significant correlations with the Health Care Providers’ Pain and Impairment Relationship Scale (HC-PAIRS) and Revised Neurophysiology Pain Questionnaire (R-NPQ). Sensitivity to change was demonstrated by significant improvements in both factors after an educational intervention, with medium-to-large effect sizes. The PABS-PT also showed good discriminative ability, as it was able to distinguish between physiotherapists with and without pain education. Cut-off values for the BM and BPS factors were determined. In conclusion, the translated and adapted Spanish version of the PABS-PT demonstrated good psychometric properties and can be reliably used to assess the attitudes and beliefs of Spanish-speaking physiotherapists regarding LBP. The questionnaire is recommended for use in clinical and educational research in the Spanish language context. Full article

The disease status, progression, and treatment effect of essential tremor (ET) patients are currently assessed with clinical scores, such as the Fahn–Tolosa–Marin Clinical Rating Scale for Tremor (FTM). The use of objective and rater-independent monitoring of tremors may improve clinical care for patients with ET. Therefore, the focus of this study is to develop an objective accelerometry-based method to quantify ET, based on FTM criteria. Thirteen patients with ET and thirteen matched healthy participants underwent FTM tests to rate tremor severity, paired with tri-axial accelerometric measurements at the index fingers. Analogue FTM assessments were performed by four independent raters based on video recordings. Quantitative measures were derived from the accelerometric data, e.g., the area under the curve of power in the 4–8 Hz frequency band (AUCP) and maximal tremor amplitude. As such, accelerometric tremor scores were computed, using thresholds based on healthy measurements and FTM criteria. Agreement between accelerometric and clinical FTM scores was analyzed with Cohen’s kappa coefficient. It was assessed whether there was a relationship between mean FTM scores and the natural logarithm (ln) of the accelerometric outcome measures using linear regression. The agreement between accelerometric and FTM scores was substantial for resting and intention tremor tests (≥72.7%). However, the agreement between accelerometric postural tremor data and clinical FTM ratings (κ = 0.459) was low, although their logarithmic (ln) relationship was substantial (R2 ≥ 0.724). Accelerometric test–retest reliability was good to excellent (ICC ≥ 0.753). This pilot study shows that tremors can be quantified with accelerometry, using healthy thresholds and FTM criteria. The test–retest reliability of the accelerometric tremor scoring algorithm indicates that our low-cost accelerometry-based approach is a promising one. The proposed easy-to-use technology could diminish the rater dependency of FTM scores and enable physicians to monitor ET patients more objectively in clinical, intraoperative, and home settings. Background: For children with inflammatory bowel disease (IBD), understanding their condition may lead to better outcomes. Knowledge assessment is imperative to identify where education may be required. An IBD knowledge assessment tool (IBD-KID2) is available in English; the aim of this study was to translate IBD-KID2 in to Italian and assess its validity/reliability among children with IBD. Methods: IBD-KID2 has fifteen items, scoring one point per correct answer. IBD-KID2 items were assessed for cultural comprehension/relevance by Italian gastroenterologists using a content validity index; those items with a maximum score proportion <0.78 were reviewed. IBD-KID2 was then translated using ‘forward–backward’ process and reviewed for content/meaning. A prospective study among Italian children with IBD enabled score comparisons with established populations (z test), and reliability was assessed using test–retest completion (Pearson correlation (r), paired t-test). Results: Twenty-five children participated: 16 (64%) male, mean age 14.9 years (SD2.4), Crohn’s disease 13 (52%). The mean IBD-KID2 score was 8.8 (SD2.8), with no association with independent variables. Test–retest showed strong correlation between scores (r = 0.78, p < 0.001), with no mean difference (p = 0.39). Comparison with other pediatric IBD populations (NZ/Australia/Canada) showed no score difference (p = 0.62, CI −0.9 to 1.5). Conclusions: The translation of IBD-KID2 to Italian used a rigorous methodology. Scores showed the translated tool has equivalence and generalizability to Italian children with IBD. Full article

A murine model of myelofibrosis in tibia was used in a co-clinical trial to evaluate segmentation methods for application of image-based biomarkers to assess disease status. The dataset (32 mice with 157 3D MRI scans including 49 test–retest pairs scanned on consecutive days) was split into approximately 70% training, 10% validation, and 20% test subsets. Two expert annotators (EA1 and EA2) performed manual segmentations of the mouse tibia (EA1: all data; EA2: test and validation). Attention U-net (A-U-net) model performance was assessed for accuracy with respect to EA1 reference using the average Jaccard index (AJI), volume intersection ratio (AVI), volume error (AVE), and Hausdorff distance (AHD) for four training scenarios: full training, two half-splits, and a single-mouse subsets. The repeatability of computer versus expert segmentations for tibia volume of test–retest pairs was assessed by within-subject coefficient of variance (%wCV). A-U-net models trained on full and half-split training sets achieved similar average accuracy (with respect to EA1 annotations) for test set: AJI = 83–84%, AVI = 89–90%, AVE = 2–3%, and AHD = 0.5 mm–0.7 mm, exceeding EA2 accuracy: AJ = 81%, AVI = 83%, AVE = 14%, and AHD = 0.3 mm. The A-U-net model repeatability wCV [95% CI]: 3 [2, 5]% was notably better than that of expert annotators EA1: 5 [4, 9]% and EA2: 8 [6, 13]%. The developed deep learning model effectively automates murine bone marrow segmentation with accuracy comparable to human annotators and substantially improved repeatability. Full article

Quantitative MRI biomarkers are sought to replace painful and invasive sequential bone-marrow biopsies routinely used for myelofibrosis (MF) cancer monitoring and treatment assessment. Repeatability of MRI-based quantitative imaging biomarker (QIB) measurements was investigated for apparent diffusion coefficient (ADC), proton density fat fraction (PDFF), and magnetization transfer ratio (MTR) in a JAK2 V617F hematopoietic transplant model of MF. Repeatability coefficients (RCs) were determined for three defined tibia bone-marrow sections (2–9 mm; 10–12 mm; and 12.5–13.5 mm from the knee joint) across 15 diseased mice from 20–37 test-retest pairs. Scans were performed on consecutive days every two weeks for a period of 10 weeks starting 3–4 weeks after transplant. The mean RC with (95% confidence interval (CI)) for these sections, respectively, were for ADC: 0.037 (0.031, 0.050), 0.087 (0.069, 0.116), and 0.030 (0.022, 0.044) μm2/ms; for PDFF: 1.6 (1.3, 2.0), 15.5 (12.5, 20.2), and 25.5 (12.0, 33.0)%; and for MTR: 0.16 (0.14, 0.19), 0.11 (0.09, 0.15), and 0.09 (0.08, 0.15). Change-trend analysis of these QIBs identified a dynamic section within the mid-tibial bone marrow in which confident changes (exceeding RC) could be observed after a four-week interval between scans across all measured MRI-based QIBs. Our results demonstrate the capability to derive quantitative imaging metrics from mouse tibia bone marrow for monitoring significant longitudinal MF changes. Full article

Background: HER2-Low status is found in approximately half of breast cancer patients and shows potential benefits from novel antibody–drug conjugates (ADCs). Data on the accuracy of HER2-Low status between core needle biopsy (CNB) and surgical excision specimen (SES) samples are lacking. We aimed to investigate the accuracy of HER2-Low status diagnosis between CNB and SES samples. Methods: Consecutive early-stage breast cancer patients who underwent surgery from January 2009 to March 2022 with paired CNB and SES samples were retrospectively reviewed. HER2-Low was defined as IHC 1+ or IHC2+ and FISH-negative. Concordance rates were analyzed by the Kappa test. Further clinicopathological characteristics were compared among different HER2 status and their changes. Results: A total of 5610 patients were included, of whom 3209 (57.2%) and 3320 (59.2%) had HER2-Low status in CNB and SES samples, respectively. The concordance rate of HER2 status in the whole population was 82.37% (Kappa = 0.684, p < 0.001), and was 76.87% in the HER2-Negative patients (Kappa = 0.372, p < 0.001). Among 1066 HER2-0 cases by CNB, 530 patients were classified as HER2-Low tumors. On the contrary, in 3209 patients with HER2-Low tumor by CNB, 387 were scored as HER2-0 on the SES samples. ER-negative or Ki67 high expression tumor by CNB had a high concordance rate of HER2-Low status. Conclusions: A relatively low concordance rate was found when evaluating HER2-Low status between CNB and SES samples in HER2-Negative breast cancer patients, indicating the necessity of retesting HER2 low status at surgery, which may guide further therapy in the era of anti-HER2 ADCs. Somatic mutations in PIK3CA are present in ~40% breast cancers (BC); their detection in hormone receptor (HR)+/HER2− tumors allows for selecting patients with advanced disease eligible for PIK3CA targeting with alpelisib. The choice of what type of PIK3CA testing approach to adopt and which tissue sample to analyze is a new task in breast pathology. In this methodological study, we sought to assess the performance of next-generation sequencing (NGS) and RT-PCR for PIK3CA testing on archival formalin-fixed paraffin-embedded (FFPE) primary tumors and corresponding metastases. Sixteen HR+/HER2− BC with known PIK3CA-mutated status (ex. 7, 9, and 20) on metastatic samples by means of amplicon-based targeted NGS were selected, and n = 13 of these samples were re-tested with a commercially available CE-IVD RT-PCR assay. All available primary tumors (n = 8) were tested with both methods. NGS detected mutations in all samples, while RT-PCR in n = 2 sample-pairs and overall, in n = 5/8 (62.5%) primary tumors and 7/13 (53.8%) metastases (κ = 0.09; 95% CI, −0.69–0.87). Slight agreement (κ = 0; 95% CI, −0.59–0.59) was observed between NGS and RT-PCR, with the former being generally more sensitive in cases with low DNA quality and quantity. Post hoc visual inspection of the RT-PCR data increased the concordance to 76.9%. Targeted NGS offers reliable and robust PIK3CA testing on both tumor and metastasis FFPE samples; the accuracy of RT-PCR depends on the DNA quantity and quality. In HR+/HER2− BC, both the selection of the PIK3CA testing strategy of FFPE tissues and which sample to analyze should consider several technical parameters and should be tailored for each case. The Perception Neuron Studio (PNS) is a cost-effective and widely used inertial motion capture system. However, a comprehensive analysis of its upper-body motion capture accuracy is still lacking, before it is being applied to biomechanical research. Therefore, this study first evaluated the validity and reliability of this system in upper-body capturing and then quantified the system’s accuracy for different task complexities and movement speeds. Seven participants performed simple (eight single-DOF upper-body movements) and complex tasks (lifting a 2.5 kg box over the shoulder) at fast and slow speeds with the PNS and OptiTrack (gold-standard optical system) collecting kinematics data simultaneously. Statistical metrics such as CMC, RMSE, Pearson’s r, R2, and Bland–Altman analysis were utilized to assess the similarity between the two systems. Test–retest reliability included intra- and intersession relations, which were assessed by the intraclass correlation coefficient (ICC) as well as CMC. All upper-body kinematics were highly consistent between the two systems, with CMC values 0.73–0.99, RMSE 1.9–12.5°, Pearson’s r 0.84–0.99, R2 0.75–0.99, and Bland–Altman analysis demonstrating a bias of 0.2–27.8° as well as all the points within 95% limits of agreement (LOA). The relative reliability of intra- and intersessions was good to excellent (i.e., ICC and CMC were 0.77–0.99 and 0.75–0.98, respectively). The paired t-test revealed that faster speeds resulted in greater bias, while more complex tasks led to lower consistencies. Our results showed that the PNS could provide accurate enough upper-body kinematics for further biomechanical performance analysis. Full article

The activity of excitatory and inhibitory neural circuits in the motor cortex can be probed and modified by transcranial magnetic stimulation (TMS) and repetitive TMS (rTMS), noninvasively. At present, not only has a consensus regarding the interhemispheric effect of high frequency rTMS not been reached, but the attributes of these TMS-related circuits are also poorly understood. To address this question comprehensively, we integrated a single- and paired-pulse TMS evaluation with excitatory 20-Hz rTMS intervention in order to probe the interhemispheric effect on the intracortical circuits by high-frequency rTMS. In the rest state, after 20-Hz rTMS, a significant increase of single-pulse MEP and paired-pulse intracortical facilitation (ICF) in the non-stimulated hemisphere was observed with good test–retest reliability. Intracortical inhibition (measured by the cortical silent period) in the unstimulated hemisphere also increased after rTMS. No significant time–course change was observed in the sham-rTMS group. The results provide the evidence that 20-Hz rTMS induced a reliable interhemispheric facilitatory effect. Findings from the present study suggest that the glutamatergic facilitatory system and the GABAergic inhibitory system may vary synchronously. Full article
