The hydrodynamic response of closed and semi-closed (open-bottom) rigid cylindrical aquaculture platforms was examined through combined model tests and numerical simulations. Free decay tests in calm water quantified natural periods and damping ratios for heave and pitch motions. Subsequent regular wave testing characterized response amplitude operators (RAOs) and wave elevations at interior and exterior wave gauges. Finally, the motion and wave elevation characteristics of the two types of aquaculture platforms under irregular waves were analyzed under extreme sea conditions. Results demonstrated that bottom openings significantly altered hydrodynamic responses of aquaculture platforms, with a 59% enhancement in heave damping ratio and a 47% reduction in heave natural period. Semi-closed cages exhibited asymmetric internal sloshing profiles along the mid-transverse axis, with lateral sloshing amplitudes increasing by 200–300% at lateral wave gauges. Under irregular wave conditions, compared to closed aquaculture platform, semi-closed aquaculture platform increased the heave, pitch motion, and internal sloshing response but reduced run-up on the outer wave-facing side. Polycystic ovary syndrome (PCOS), the most common endocrine disorder in reproductive-age women, is characterized by menstrual irregularities and polycystic ovarian morphology. It is also associated with insulin resistance, chronic inflammation, and oxidative stress, all of which may promote autoimmunity. Several studies have suggested a higher occurrence of antinuclear antibodies (ANA) in PCOS but the main challenge in this field is the inconsistency of findings due to heterogeneous study designs and assay methods. However, to date, no systematic review has synthesized this evidence. We conducted a systematic review to evaluate the prevalence and serum levels of ANA in women with PCOS. A comprehensive search was performed in PubMed, Embase, and Scopus, and 13 studies were ultimately included, comprising 924 women with PCOS and 1172 controls. ANA were elevated in about half of the studies, while the remainder found no significant differences between PCOS and controls. Anti-dsDNA antibodies were the most consistently investigated ANA subtype, with most studies reporting higher levels or prevalence in PCOS. For other ANA subtypes, the evidence was limited and inconclusive, largely due to methodological variability across studies. This systematic review suggests that ANA may be elevated in a subset of women with PCOS, but the current evidence remains inconsistent. These findings highlight the need for methodological standardization in ANA assessment to enable clearer conclusions and to clarify whether ANA positivity has clinical relevance in this population. Background/Objectives: The ongoing prevalence of dental issues within the school-age population continues to present significant public health challenges. This study aims to thoroughly evaluate the oral health conditions of schoolchildren aged 7–10 years, with a particular focus on identifying the prevalence and severity of dental caries, as well as their relationship with dental hygiene and dietary habits. Methods: A cross-sectional approach was employed involving 700 children aged from 7 to 10 years, recruited from nine urban and rural educational institutions across two Romanian counties, namely Mureș and Bistrița-Năsăud. Data were collected regarding the sociodemographic characteristics of participants, including parental educational background and occupational status. Furthermore, information on dental hygiene routines was gathered, encompassing tooth brushing habits, flossing usage, frequency of brushing, along with dietary details such as daily meal count, snacking frequency, and sugar intake levels. An oral examination was performed by a trained specialist to assess the condition of the children’s teeth, documenting the number of decayed, missing, and filled teeth. The presence of plaque was evaluated using a plaque index, and the dental caries status was quantified using the Decayed, Missing, and Filled Teeth (DMFT) index. Results: The findings revealed that the average plaque score was marginally higher in boys (mean = 0.69 ± 0.36) compared to girls (mean = 0.65 ± 0.40). Additionally, children from urban environments demonstrated a mean plaque score of 0.61 ± 0.32, whereas their rural counterparts exhibited a higher score of 0.73 ± 0.38. In multivariable models, irregular brushing, higher daily sugar consumption, and ≥3 snacks/day were independently associated with both caries prevalence (DMFT > 0) and greater DMFT counts (p < 0.05). Conclusions: The evidence denotes a concerning association between poor oral hygiene practices and higher rates of tooth decay among children, particularly among those with elevated sugar intake and frequent snacking behaviors. Dental public health professionals are encouraged to incorporate considerations of oral hygiene habits when developing future health promotion strategies aimed at improving the oral health status of children. Background/Objectives: Epilepsy is identified by irregular neuronal hyperexcitability, generating recurrent seizures. Despite many available pharmacological treatments, certain patients with drug-resistant epilepsy may require novel therapeutic approaches. In the present study, we aimed to evaluate the effects of lacosamide, low-frequency repetitive transcranial magnetic stimulation, and their combination on intracellular calcium dynamics in an in vitro model of neuronal excitability, hypothesizing that these interventions could mitigate potassium chloride-induced neuronal excitation. Methods: We utilized differentiated SH-SY5Y human neuroblastoma cells as an in vitro model of neuronal excitability. Neuronal excitability was induced with 50 mM KCl, and cells were treated with lacosamide (300 µM), rTMS (1 Hz), or their combination. Intracellular calcium levels were quantified using fluo-4 AM fluorescence calcium imaging, with changes expressed as percentage change in fluorescence intensity (%ΔF/F) relative to baseline. Results: The combination of lacosamide and rTMS was the most effective, significantly reducing KCl-induced calcium elevation (ΔF/F = 9.15) compared to lacosamide alone (ΔF/F = 17.11), rTMS alone (ΔF/F = 23.70), and the untreated cells serving as controls (ΔF/F = 66.70). The combination showed a statistically significant effect, with enhanced suppression of neuronal excitability compared to individual treatments. Conclusions: Lacosamide and low-frequency rTMS (1 Hz) effectively attenuated KCl-induced changes in intracellular calcium levels in vitro, with their combination demonstrating the highest efficacy. These findings suggest a promising foundation in the management of drug-resistant epilepsy. Future studies are necessitated to validate these results and benefit clinical translation. Full article

Background/Objectives: University students in health sciences are particularly vulnerable to poor sleep quality and elevated stress due to academic and clinical demands. This follow-up study aimed to compare sleep quality and perceived stress levels among health science students during online and frontal (in-person) education periods and to examine the influence of behavioral and demographic factors. Methods: A prospective, follow-up design was applied involving students from the University of Pécs, Faculty of Health Sciences, across nursing, radiography, laboratory analytics, paramedicine, and physiotherapy programs. Data were collected via anonymous online questionnaires during two intervals: February–March 2023 (online education) and April–May 2023 (frontal education). The Athens Insomnia Scale (AIS) and Perceived Stress Scale (PSS-10) were used to assess sleep quality and stress, respectively. Internal consistency was evaluated using Cronbach’s alpha (AIS: α = 0.81–0.84; PSS: α = 0.87–0.90). Data were analyzed using descriptive statistics and paired sampled t-tests (p < 0.05). Results: AIS scores were significantly higher during online learning compared to in-person (5.47 ± 2.67 vs. 4.25 ± 2.48; p = 0.001), indicating poorer sleep quality. In contrast, PSS scores were higher during the frontal period (29.48 ± 8.67 vs. 24.31 ± 7.15; p < 0.05). Increased screen time, irregular routines, and lack of physical activity were associated with poorer outcomes. Conclusions: Online education may compromise sleep quality, while in-person learning appears to increase perceived stress. These findings highlight the need for targeted health promotion strategies adapted to different educational modalities. Full article

Background: Polycystic ovary syndrome (PCOS) is an endocrine disorder characterized by hyperandrogenism, irregular ovulation, and polycystic ovarian morphology. The severity of periodontal inflammation in PCOS may be linked to elevated levels of inflammatory mediators, like interleukins (IL-6, IL-17), and matrix metalloproteinase-8 (MMP-8), found in both serum and saliva samples. This systematic review aims to assess the presence, nature, and variations in salivary inflammatory biomarkers in individuals with PCOS and their potential connection to periodontal disease (PD). Materials and Methods: Selected databases were PubMed, Scopus, Google Scholar, and Web of Science. The search strategy included the following terms: “oral inflammatory biomarkers”, “Salivary mediators,” “metabolic indicators,” “periodontal diseases,” “periodontitis,” “polycystic ovary syndrome,” “PCOS,” and “ovulatory dysfunction.” Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were used. Results: Several salivary inflammatory biomarkers are present in women with PCOS, including cytokines, C-reactive protein (CRP), reactive oxygen species (ROS), MMPs, and microbial diversity alterations. Additionally, the reviewed studies suggest a correlation between PCOS and PD, as patients with PCOS exhibit greater periodontal alterations compared to healthy women. The heightened periodontal response in PCOS appears to be associated with a systemic inflammatory state, probably increasing the susceptibility to PD. Conclusions: Salivary inflammatory biomarkers in PCOS patients are a useful diagnostic tool for evaluating the heightened risk of periodontal disease. Further research with stricter protocols is necessary to better define the diagnostic potential of these biomarkers for PCOS patients and determine their role in the early detection of periodontal disease. Background: Ustekinumab (UST) is a monoclonal antibody (mAb) used in the treatment of inflammatory bowel disease (IBD). Elevated serum concentrations are typically associated with improved therapeutic outcomes, and therapeutic drug monitoring (TDM) is a useful tool for guiding mAbs treatment. This study aimed to develop a dried blood spot (DBS) method for TDM of UST in patients with IBD. Methods: The commercial enzyme-linked immunosorbent assay for plasma samples was optimized for DBS samples and subsequently validated according to international guidelines for classical and DBS-specific validation parameters. It was then applied to analyze serum and DBS samples obtained from venous and capillary blood of IBD patients undergoing UST therapy. Results: The method was linear (3–12 mg/L) with acceptable inter-day accuracy (90.1–106%) and precision (<12%). We confirmed that there was no hematocrit effect and that DBS samples were stable for one month under room conditions. A linear model was developed between venous DBS and serum UST concentrations, which showed no systemic bias, and 71% of the samples were within ±20% of the mean. In addition, a linear correlation between venous DBS and capillary DBS samples was established, showing no significant bias, with 84% of samples within ±20% of the mean. Finally, a novel strategy was developed to overcome the limitations of poor-quality samples (irregular shapes) based on area image analysis. Conclusions: The newly developed DBS method is the first to enable reliable measurement of UST in capillary blood, appropriate clinical interpretation of the measured concentrations, and remote monitoring of patients in the early phase of therapy. Polycystic ovary syndrome (PCOS) is a prevalent endocrinological condition among women of reproductive age, characterized by several well-known symptoms, including hyperandrogenism, anovulation, irregular menstrual cycles, and insulin resistance. In addition, women suffering from PCOS are also at an increased risk of developing several autoimmune diseases, including thyroid disorders, type 1 diabetes, and rheumatoid arthritis. Furthermore, an elevated prevalence of diverse autoantibodies is observed in women diagnosed with PCOS. These include antibodies specific to autoimmune diseases, e.g., anti-thyroid peroxidase (anti-TPO), anti-thyroglobulin (anti-TG), and antinuclear antibodies (ANAs), as well as those that are non-specific, such as anti-malondialdehyde-modified human serum albumin (anti-HSA-MDA) or anti-α-crystallin. It appears that several mechanisms may be responsible for this phenomenon. PCOS has been observed to co-occur with autoimmune diseases, potentially attributable to shared genetic susceptibility or the presence of hormonal disorders resulting from autoimmune diseases. Moreover, PCOS is a chronic low-grade inflammatory disease that may contribute to immune dysfunction and subsequent overproduction of autoantibodies. A further intriguing aspect may be the yet-unknown role of autoantibodies in the pathogenesis of PCOS, considering PCOS as a disease with an autoimmune etiology. Full article

Post-operative pancreatic fistula (POPF) is a serious yet far too common complication following distal pancreatectomy (DP), as it affects 20–30% of patients after DP. POPF raises the risk of other complications and causes delays to a patient’s oncologic care. In this review, we present the latest data on patient risk factors for developing POPF, such as obesity, smoking, young age, thick pancreas, lack of epidural anesthesia, hypoalbuminemia, and elevated drain amylase levels. Other risk factors that have been identified with irregular consistency include open surgical approach, non-malignant or neuroendocrine tumor pathology, simultaneous splenectomy, simultaneous vascular resection, and long operative time. We also review the consequences of POPF, which include hemorrhage, infection, delayed gastric emptying, re-operation, re-admission, delays in adjuvant chemotherapy initiation, reduced progression-free survival, and reduced overall survival. Finally, we present strategies that have been studied for avoiding POPF after DP, including reducing pressure at the sphincter of Oddi, the use of sealants and patches, optimizing pancreatic transection, strategies for post-operative drain placement, the use of post-operative somatostatin analogues, and pre-clinical studies of experimental devices and techniques that may inform future trials. This review informs readers on the current state of the art with regard to POPF after DP and sets the stage for future studies to improve patient outcomes. Full article

Photoaging is a multifactorial and progressive skin aging process primarily triggered by prolonged exposure to ultraviolet (UV) radiation. This condition leads to both structural and functional impairments in the skin, including the formation of wrinkles, loss of elasticity, pigmentation irregularities, and an elevated risk of skin malignancies. At the core of photoaging is the accumulation of reactive oxygen species (ROS), which generate oxidative stress, initiate chronic inflammation, cause DNA damage, and accelerate the breakdown of the extracellular matrix—largely through the activity of matrix metalloproteinases (MMPs). The review provides a comprehensive analysis of various natural substances, including antioxidants, anti-inflammatory agents, photoprotective compounds, and emerging regenerative treatments, based on in vitro and in vivo research. Special emphasis is placed on natural substances, including polyphenols, cannabinoids, carotenoids, retinoids, and vitamins, highlighting their potential in preventing and treating photoaging. The longitudinal equivalent bending stiffness is a critical parameter for assessing the longitudinal responses of Double-O-Tube (DOT) shield tunnels under adjacent construction activities. Based on a longitudinal equivalent continuous model and the characteristics of the DOT shield tunnel cross-section, an analytical solution for the longitudinal equivalent bending stiffness (LEBS) of the DOT shield tunnel has been derived. Given that the cross-section of the DOT shield tunnel is an irregular structure, two scenarios are considered: one in which the neutral axis is located at the waist of the tunnel and another where it is situated at the lower arch. Using the structural design of the DOT shield tunnel for Shanghai Metro Line M8 as a case study, the effects of bolt number, segment thickness, segment width, and pillar height on the longitudinal equivalent bending stiffness have been investigated. Additionally, formulas for calculating the deformation and stress indices of the DOT shield tunnel have been established. The results indicate that increasing the number of bolts and widening the segments can enhance the longitudinal equivalent bending stiffness efficiency (LEBSE), resulting in an upward shift of the neutral axis. Conversely, as the segment thickness increases, the LEBSE decreases linearly while the neutral axis moves downward; however, the value of LEBS itself increases. With an increase in the pillar height angle, the neutral axis shifts upward, leading to an increase in the LEBS. When the pillar height angle is increased from 10° to 45°, the LEBSE decreases rapidly, followed by a gradual increase with further elevation in the pillar height angle. When the tunnel curvature radius exceeds 15,000 m, the bolts, segments, and joint openings remain in a safe state. However, when the curvature radius decreases to 5233 m, the maximum tensile stress on the bolts reaches their yield limit, and the joint openings exceed the warning threshold. Full article

Post-Coarctectomy Pseudoaneurysm with Recurrent Coarctation Treated with Open Surgery: A Comprehensive Literature Review and Case Report

Introduction: Patch aortoplasty for aortic coarctation may have a high rate of late aneurysm and pseudoaneurysm formation, with an elevated risk of rupture and subsequent mortality when it occurs. Case: A 55-year-old male with irregular follow-up after patch aortoplasty for aortic coarctation 15 years ago, presented with a late post-coarctectomy aortic arch pseudoaneurysm and re-coarctation. Through a redo thoracotomy and under left heart bypass, the pseudoaneurysm and previous patch were partially excised, and reconstruction with interposition of a “Y” shaped Dacron graft was performed, in order to preserve the patency of the left subclavian artery. The postoperative course was uneventful, and at 6 months, the CT-angio control showed adequate graft patency. Comprehensive Literature Review: A comprehensive literature review with the primary focus on the different late complications after CoA repair (post-aortic coarctation aneurysms and recoarctation), and outcomes of open and endovascular approaches was performed. Conclusions: Open surgical repair for post-coarctectomy pseudoaneurysm is feasible with excellent results. However, due to the technical challenges, it may be performed with better results in high-volume aortic centers. Full article

From Diversity to Homogenisation: Assessing Two Decades of Temperate Native Forest Replaced by Exotic Plantations in the Nahuelbuta Mountain Range

The Nahuelbuta Mountain Range in the south-central zone of Chile is a biodiversity hotspot that has undergone intense land use/cover transformation. This study analyses two decades of land use change (1999–2018) in the watersheds of the Lebu and Leiva rivers. The magnitude and spatial configuration of these changes were assessed using detailed spatial information, transition matrices, and landscape metrics. The results show that between 1999 and 2018, temperate native forest decreased by 30.3% in Lebu and 22.8% in Leiva, being replaced mainly by exotic forest plantations, which increased by 20.2% and 13.5%, respectively. The spatiotemporal analysis revealed losses concentrated in the lowland and middle zones of both watersheds, with persistence of temperate native forest in higher elevations. Landscape metrics showed an increase in diversity (SIDI: Lebu 0.41–0.65; Leiva 0.29–0.57) and a decrease in aggregation (AI: 92–86%; 95–90%). At the class level, the temperate native forest presented greater fragmentation, with a reduction in size and proximity, an increase in density, and more irregular shapes. In contrast, exotic forest plantations increased in size and proximity, with a slight decrease in density and greater complexity of form, consolidating their expansion and spatial continuity in both watersheds. These findings emphasise the need to implement territorial planning and conservation strategies adapted to the Nahuelbuta context, through native forest management plans that promote ecological conservation, the recovery of degraded landscapes, and the strengthening of ecosystem services, thus contributing to the well-being of local communities and long-term environmental sustainability. Polycystic ovary syndrome (PCOS) is a complex condition affecting women of reproductive age, characterized by menstrual irregularities, hyperandrogenism, polycystic ovarian morphology, and low-grade inflammation accompanied by oxidative stress and increased autoimmune risk, particularly Hashimoto’s thyroiditis. Many studies have examined thyroid autoantibodies—anti-thyroid peroxidase antibodies (anti-TPO) and anti-thyroglobulin antibodies (anti-TG)—in PCOS; however, observed differences in baseline thyroid-stimulating hormone (TSH) levels and body mass indices (BMIs) impede a direct interpretation of the results. This systematic review and meta-analysis aimed to summarize the available evidence on the prevalence and levels of anti-TPO and anti-TG in women with PCOS. We conducted a systematic search of PubMed, Scopus, and Embase, which yielded 40 eligible, observational studies including 6045 women with PCOS and 4527 controls. Subgroup analyses were conducted separately for TSH- and BMI-matched populations. Anti-TPO prevalence (odds ratio [OR] = 2.03; 95% confidence interval [CI]: 1.35–3.04; p = 0.0006) and levels (standardized mean difference [SMD] = 0.63; 95% CI: 0.37–0.88; p < 0.00001) were significantly higher in PCOS patients. Anti-TG prevalence (OR = 1.92; 95% CI: 1.23–3.01; p = 0.004) and levels (SMD = 0.41; 95% CI: 0.18–0.64; p = 0.0004) were also significantly elevated. In matched subgroups, prevalence differences were no longer significant, though anti-TPO levels remained significantly elevated and anti-TG levels were borderline significant in the TSH-matched subgroup of PCOS women. Although differences in thyroid autoantibody prevalence in women with PCOS appear to be driven by elevated TSH levels and BMIs, the persistently increased antibody levels in the majority of matched subgroups suggest that PCOS itself may contribute independently to heightened autoimmune activation. In this study, a novel direct laser beam turning (DLBT) approach is proposed for the precision machining of AISI 308L austenitic stainless steel, which eliminates the need for cutting tools and thereby eradicates tool wear and vibration-induced surface irregularities. A nanosecond-pulsed Nd:YAG fiber laser (λ = 1064 nm, spot size = 0.05 mm) was used, and Ø1.6 mm × 20 mm cylindrical rods were processed under ambient conditions without auxiliary cooling. The experimental framework systematically evaluated the influence of scanning speed, pulse frequency, and the number of laser passes on dimensional accuracy and material removal efficiency. The results indicate that a maximum diameter reduction of 0.271 mm was achieved at a scanning speed of 3200 mm/s and 50 kHz, whereas 0.195 mm was attained at 6400 mm/s and 200 kHz. A robust second-order polynomial correlation (R2 = 0.99) was established between diameter reduction and the number of passes, revealing the high predictability of the process. Crucially, when the scanning speed was doubled, the effective fluence was halved, considerably influencing the ablation characteristics. Despite the low fluence, evidence of material evaporation at elevated frequencies due to the incubation effect underscores the complex photothermal dynamics governing the process. This work constitutes the first comprehensive quantification of pass-dependent diameter modulation in DLBT and introduces a transformative, noncontact micromachining strategy for hard-to-machine alloys. The demonstrated precision, repeatability, and thermal control position DLBT as a promising candidate for next-generation manufacturing of high-performance miniaturized components. Full article
