Background/Objectives: this paper investigates the local vs. global visual processing preference in typically developing (TD) children, youth with Down syndrome (DS), and youth with Williams syndrome (WS). In particular, the global precedence effect (GPE) and the global interference effect (GI) have recently been described as two distinct and at least partially independent effects. Methods: in this study, 50 participants (TD = 25, DS = 13, WS = 12) completed two experiments requiring the identification of either the global or local level of hierarchical stimuli, which consisted of letters and schematic faces. For each stimulus type, two separate blocks were conducted, one with the task to focus on the local elements and the other with the task to focus on the global shape. Results: our results indicate that TD children demonstrate a global precedence effect for letters but not for schematic faces, suggesting a developmental modulation of configural processing. In contrast, both DS and WS groups showed a global processing bias for schematic faces and a significant global interference effect in both conditions, likely reflecting deficits in inhibitory control. Conclusions: these findings challenge the notion that DS and WS individuals can be classified strictly as global or local processors, respectively, emphasizing the influence of stimulus type and cognitive demands. Implications for neurodevelopmental research and clinical interventions are discussed. Full article

Background: The interplay between neuronutrition, physical activity, and mental health for enhancing brain resilience to stress and overall human health is widely recognized. The use of brain mapping via quantitative-EEG (qEEG) comparative analysis enables researchers to identify deviations or abnormalities and track the changes in neurological patterns when a targeted drug or specific nutrition is administered over time. High-functioning mild-to-borderline intellectual disorders (MBID) and autism spectrum disorder (ASD) constitute leading global public health challenges due to their high prevalence, chronicity, and profound cognitive and functional impact. Objective: The objectives of the present study were twofold: first, to characterize an extremely vulnerable group of children with functioning autism symptoms, disclosing their overall pattern of cognitive abilities and areas of difficulty, and second, to investigate the relevance of the effects of a mushroom (Hericium erinaceus) biomass dietary supplement on improvement on neurocognitive behavior. Methods: This study used qEEG to compare raw data with a normative database to track the changes in neurological brain patterns in 147 children with high-functioning autistic attributes when mushroom H. erinaceus biomass supplement was consumed over 6 and 12 months. Conclusions: H. erinaceus biomass in children with pervasive developmental disorders significantly improved the maturation of the CNS after 6 to 12 months of oral use, decreased the dominant slow-wave activity, and converted slow-wave activity to optimal beta1 frequency. Therefore, despite the lack of randomization, blinding, and risk of bias, due to a limited number of observations, it may be concluded that the H. erinaceus biomass may generate a complex effect on the deficits of the autism spectrum when applied to high-functioning MBID children, representing a safe and effective adjunctive strategy for supporting neurodevelopment in children. Pipeline infrastructure constitutes the primary transportation system within the oil and gas industry, where operational safety is critically dependent on advanced in-line inspection technologies. This study presents a comprehensive analysis of eddy current testing (ECT) applications for pipeline integrity assessment. The fundamental principles of ECT are first elucidated, followed by a systematic comparative evaluation of five key ECT methodologies: conventional, multi-frequency, remote field, pulsed, and array eddy current techniques. The analysis examines their detection mechanisms, technical specifications, comparative advantages, and current developmental trajectories, with particular emphasis on future technological evolution. Subsequently, integrating global pipeline infrastructure development trends and market requirements, representative designs of pipeline inspection tools are detailed and we review relevant industry applications. Finally, persistent challenges in ECT applications are identified, particularly regarding adaptability to complex operational environments, quantification accuracy for micro-scale defects, and predictive capability for defect progression. This study proposes that future ECT equipment development should prioritize multi-modal integration, miniaturization, and intelligent analysis to enable comprehensive pipeline safety management throughout the entire asset lifecycle. Background/Objectives: Introducing a demanding home-based program (HBP) of Vojta therapy (VT) into their daily activities is a life-altering event for parents of children with global developmental delay (GDD). This study aims to document the experiences of parents of children with GDD administering a HBP of VT. Methods: A multicentre study with a qualitative case design based on an interpretative approach is presented. Interviews were conducted with 10 parents using photo-elicitation (PE). Inductive and thematic analyses were used. Results: Four common experiential themes were identified. Firstly, crying was identified as the most significant barrier to administering therapy (despite parents accepting that crying was not pain-related). Secondly, parents described the intense and variable emotional impact of being responsible for the therapy and its effects on their child. Thirdly, parents unanimously felt that their involvement was worthwhile, with the developmental results they perceived outweighing the emotional, physical and time demands of administering the VT. Finally, parents developed a narrative of hope stemming from the therapy and its observed effects. Conclusions: The physical, emotional and time demands on parents when administering a HBP of VT are very significant. The main barriers to adherence to the program are identified as the child’s crying during therapy and time management. Intense emotional experiences, both positive and negative, arise while administering a HBP of VT. Parents are not only able to overcome all emotional and logistical challenges when they recognize improvements in their child, but they also begin to hope for further improvement. Implications for the professional design of HBPs of VT include the following: advanced warning that crying is normal, part of the therapy and not pain-related; training and ongoing support from a qualified therapist; training in recognising developmental improvement; and psychological support to deal with the emotional journey. Background and Objectives: Adolescent pregnancy remains a major global public-health concern, particularly in low- and middle-income countries. Romania consistently reports the highest teenage birth rate in the European Union, with pronounced regional disparities. This study aimed to assess the incidence, sociodemographic predictors, and obstetric outcomes of adolescent pregnancies over a ten-year period in southeastern Romania. Materials and Methods: A retrospective, population-based study was conducted at the Clinical County Hospital “Saint Andrew the Apostle”, Constanța, from 1 January 2014 to 31 December 2023. All deliveries involving mothers aged ≤19 years were identified from institutional databases. Demographic variables (age, residence, education) and obstetric outcomes (mode of delivery, gestational age, neonatal parameters) were analyzed. Statistical tests included Pearson’s χ2, t-tests or Mann–Whitney U tests, and multivariate logistic regression to identify independent predictors of cesarean delivery and preterm birth. Results: Among 13,416 hospital deliveries, 1640 (12.2%) involved adolescent mothers (mean age 16.3 ± 1.4 years). Most originated from rural areas (64.6%) and had only primary education (42.8%). Cesarean section was performed in 58.3% of cases—significantly higher among rural (61.2%) and low-education (62.4%) groups (p < 0.05). The leading indications were cervical dystocia (19.2%) and cephalopelvic disproportion (16.9%). Preterm birth occurred in 30.5% and low birth weight in 27.1% of neonates. Multivariate analysis identified primiparity (OR 2.10; 95% CI 1.45–3.05; p < 0.001) and low education (OR 1.56; 95% CI 1.09–2.21; p = 0.015) as independent predictors of cesarean delivery, while rural residence and low education predicted prematurity (OR 1.84; 95% CI 1.12–3.02; p = 0.016). Conclusions: Adolescent pregnancy in southeastern Romania remains a persistent public-health challenge concentrated among rural and low-education populations. These patterns are consistent with previously described vulnerabilities in adolescent populations, including developmental and healthcare-access challenges, although such factors were not directly measured in this study. Community-based prevention, comprehensive sexual education, and adolescent-friendly obstetric pathways are urgently needed to reduce the burden of teenage pregnancy in Eastern Europe. These associations should be interpreted with caution, as the retrospective design precludes causal inference. Full article

Background/Objectives: Prematurity is a significant global health concern, often associated with neurodevelopmental challenges. Solute Carrier Family 6 Member 4 (SLC6A4), the gene encoding the serotonin transporter, a key component in serotonin reuptake in the synaptic cleft, plays a key role in stress response and neurodevelopment. Epigenetic regulation of stress-related genes, such as SLC6A4, influences neonatal stress adaptation and developmental outcomes. This study aimed to quantify and compare DNA methylation levels at 13 CpG sites in the promoter region of the SLC6A4 gene between preterm and term neonates at three time points. Methods: A cohort of 46 preterm infants and a cohort of 49 full-term infants were analyzed. Blood samples collected at birth (D0), the fifth day (D5), and the thirtieth day (D30) were used to analyze DNA methylation, using bisulfite conversion and pyrosequencing. Results: Significant differences in SLC6A4 methylation were observed. At D0, CpGs 12 and 13 showed higher methylation in preterm infants. CpG 9 showed lower methylation in preterm infants at D5. Extremely preterm infants had the highest values of methylation at the three time points. Longitudinal mixed-effects analysis revealed distinct temporal patterns between groups. Total and site-specific methylation at CpGs 2, 8, and 9 increased over time in full-term infants, while methylation remained stable over time in very preterm and extremely preterm infants. Conclusions: This study reveals significant differences in SLC6A4 methylation between very preterm, extremely preterm, and full-term infants, highlighting the impact of prematurity and early-life stress on the epigenome. These findings contribute to improving our understanding of the epigenetic mechanisms shaping neurodevelopment and stress adaptation in neonates. Full article

Leigh syndrome is a rare, progressive mitochondrial disorder of childhood. Early diagnosis is often challenging due to nonspecific clinical manifestations. We report a 1-month-old male infant initially referred for suspected congenital muscular torticollis who ultimately received a diagnosis of Leigh syndrome. Despite unremarkable perinatal history, he subsequently developed persistent feeding difficulties, recurrent vomiting, failure to thrive, and global developmental delay. Early neurological assessment revealed poor repertoire patterns on General Movement Assessment. The Neonatal Oral-Motor Assessment Scale (NOMAS) demonstrated dysfunctional oral-motor control, whereas the video fluoroscopic swallowing study (VFSS) revealed aspiration during swallowing. Brain MRI revealed symmetric basal ganglia lesions. Expanded whole-exome sequencing identified a pathogenic MT-ATP6 m.8993T>G variant with high heteroplasmy level (>90% in blood), confirming the diagnosis of Leigh syndrome. The variant was maternally inherited, although neither the mother nor the older sibling exhibited clinical features of mitochondrial disease. Leigh syndrome can initially manifest with subtle systemic features rather than overt neurological features. Persistent feeding difficulties and growth delay in infancy warrant thorough evaluation, including neuroimaging and comprehensive genomic testing, to enable timely diagnosis and optimize clinical management. Soil salinity poses a critical threat to global agricultural productivity, exacerbating food security challenges in arid and semi-arid regions. This review synthesizes current knowledge on the physiological and biochemical impacts of salinity stress in plants, with a focus on the role of gibberellic acid (GA3) in mitigating these effects. Salinity disrupts ion homeostasis, induces osmotic stress, and generates reactive oxygen species (ROS), leading to reduced chlorophyll content, impaired photosynthesis, and stunted growth across all developmental stages, i.e., from seed germination to flowering. Excess sodium (Na+) and chloride (Cl) accumulation disrupts nutrient uptake, destabilizes membranes, and inhibits enzymes critical for carbon fixation, such as Rubisco. GA3 emerges as a key regulator of salinity resilience, enhancing stress tolerance through various mechanisms like scavenging ROS, stabilizing photosynthetic machinery, modulating stomatal conductance, and promoting osmotic adjustment via osmolyte accumulation (e.g., proline). Plant hormone’s interaction with DELLA proteins and cross-talk with abscisic acid, ethylene, and calcium signaling pathways further fine-tune stress responses. However, gaps persist in understanding GA3-mediated floral induction under salinity and its precise role in restoring photosynthetic efficiency. While exogenous GA3 application improves growth parameters, its efficacy depends on the concentration- and species-dependent, with lower doses often proving beneficial and optimum doses potentially inhibitory. Field validation of lab-based findings is critical, given variations in soil chemistry and irrigation practices. Future research must integrate biotechnological tools (CRISPR, transcriptomics) to unravel GA3 signaling networks, optimize delivery methods, and develop climate-resilient crops. This review underscores the urgency of interdisciplinary approaches to harness GA3’s potential in sustainable salinity management, ensuring food security and safety in the rapidly salinizing world. Prepubescent children and young adolescents (ages 7–14) are in a critical developmental stage for establishing the foundations of healthy sexual behavior. Increasing rates of precocious puberty, combined with limited access to accurate and age-appropriate sexual health education, heighten the vulnerability of this age group to sexual health risks. These risks include early and often coerced sexual initiation, exposure to sexual abuse, and consequent outcomes such as sexually transmitted infections and early pregnancies. However, comprehensive cross-regional analyses and evidence-based interventions addressing the sexual health needs of this age group remain limited. Addressing this gap promotes mutual learning, context-specific adaptation, and global alignment of sexual health support efforts, crucial for achieving the Sustainable Development Goal target of universal access to sexual and reproductive health care. This scoping review aims to map the available evidence on the scope and characteristics of sexual health interventions for prepubescents and young adolescents in the United States and Sub-Saharan Africa. The proposed scoping review will be conducted in accordance with the Arksey and O’Malley framework and Joanna Briggs Institute (JBI) methodology for scoping review. A systematic search of English-language articles published from 2010–2025 will be conducted across PubMed, CINAHL, PsycINFO, Scopus, Web of Science, ERIC, and African Index Medicus. Five reviewers will screen the articles in Covidence and independently assess full-text articles using a standardized data extraction form. Discrepancies will be resolved through discussion and with a sixth reviewer. The review will follow the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. Data will be summarized and synthesized to identify patterns in interventions, delivery methods, outcomes, and implications for practice, research, and policy. Comparative and gap analyses will highlight similarities, differences, and priorities for future research and interventions. An adolescent sexual health expert will help validate and contextualize findings. Results can guide strategies to address shared challenges and advance universal access to sexual health support for prepubescents and young adolescents. Full article

Pediatric obesity is rising globally, and emerging evidence suggests that sleep timing may influence metabolic health through epigenetic mechanisms. This study investigated epigenome-wide DNA methylation patterns associated with bedtime in children and explored their biological relevance. Children aged 6–10 years were classified as early (≤8:30 PM) or late (>8:30 PM) bedtime groups. Saliva-derived DNA was analyzed using the Illumina Infinium MethylationEPIC BeadChip Array, and the Sparse Wrapper Algorithm (SWAG) was applied to identify differentially methylated loci. A total of 1006 CpG sites, representing 571 unique genes, were significantly associated with bedtime (p < 0.001). Significant methylation differences were observed between early and late bedtime groups, with ABCG2, ABHD4, MOBKL1A, AK3, SDE2, PRAMEF4, CREM, CDH4, BRAT1, and SDK1 showing the most consistent variation. Functional enrichment analyses (Gene Ontology, KEGG, and DisGeNET) conducted on the SWAG-identified gene set revealed enrichment in biological processes including peptidyl-lysin demethylation, regulation of sodium ion transport, DNA repair, and lipo-protein particle assembly. Key KEGG pathways included circadian entrainment, neurotransmission (GABAergic, dopaminergic, and glutamatergic), growth hormone synthesis, and insulin secretion. DisGeNET analysis identified associations with neurodevelopmental disorders and cognitive impairment. Cross-comparison with established sleep and obesity gene sets identified ten overlapping genes(CDH4, NR3C2, ACTG1, COG5, CAT, HDAC4, FTO, DOK7, OCLN, and ATXN1). These findings suggest that variations in bedtime during childhood may epigenetically modify genes regulating circadian rhythm, metabolism, neuronal connectivity, and stress response, potentially predisposing to later-life developmental, and metabolic challenges. Global temperature rise has become a critical challenge to agricultural sustainability, severely affecting crop growth, productivity, and survival. Human-induced climate change and greenhouse gas emissions cause heat stress, disrupting plant metabolism and physiology at all developmental stages from germination to harvest. Elevated temperatures during germination impair water uptake, enzyme activity, and energy metabolism, leading to poor or uneven seedling emergence. At key phases such as flowering and grain filling, heat stress limits photosynthesis and transpiration by inducing stomatal closure, restricting carbon dioxide intake, and reducing photosynthetic efficiency. The reproductive stage is particularly vulnerable to high temperatures, impairing pollen viability, preventing anther dehiscence, and reducing fertilization success. Membrane instability further accelerates chlorophyll degradation and leaf senescence. Heat stress also alters biochemical and hormonal balances by disrupting the synthesis and signaling of auxins, gibberellins, and abscisic acid (ABA). Elevated ABA promotes stomatal closure to enhance stress tolerance, while increased ethylene levels trigger premature leaf senescence and abscission. These hormonal shifts and oxidative stress hinder plant growth and reproduction, threatening global food security. Although plants employ adaptive mechanisms such as heat shock protein expression and stress-responsive gene regulation, current strategies remain inadequate, highlighting the urgent need for innovative approaches to improve crop resilience under rising temperatures. Promoting cycling in mixed-traffic environments remains a global challenge, hinging on the development of well-connected, low-stress networks. However, existing evaluation frameworks often lack comprehensiveness, overlooking the multifaceted nature of cyclists’ experiences. This study addresses this gap by proposing a novel multidimensional evaluation framework for assessing the effective connectivity of urban cycling networks. The framework integrates four critical dimensions: (1) structural connectivity of the basic road network, (2) dynamic interference from mixed traffic, (3) comfort of the cycling environment, and (4) cross-barrier connectivity. Using Nanjing, China, as a case study, we applied a hybrid Analytic Hierarchy Process (AHP)–Grey Clustering method to derive objective indicator weights and conduct a comprehensive evaluation. The results yield a composite score of 3.2568 (on a 0–4 scale), classifying Nanjing’s cycling network connectivity at the “Four-Star” level, indicating a generally positive developmental trajectory. Nevertheless, spatial disparities persist: the urban core faces intense traffic interference, while peripheral areas are hindered by network fragmentation and poor permeability. Key challenges include frequent vehicle–cyclist conflicts at intersections, inadequate nighttime illumination, suboptimal pavement conditions, and excessive detours caused by natural barriers such as the Yangtze River. This study provides urban planners and policymakers with a robust and systematic diagnostic tool to identify deficiencies and prioritize targeted interventions, ultimately contributing to sustainable urban mobility by enhancing the resilience, equity, and attractiveness of cycling networks in complex mixed-traffic settings. The 2030 Bilingual Policy was introduced in Taiwan to strengthen citizens’ English communication skills, especially among young people, and to enhance their global competitiveness. Within Taiwan’s educational context, several challenges have emerged. In response, researchers have examined the key factors contributing to effective bilingual education, including policy implementation, bilingual instruction models, teacher professional development, and the availability of teaching resources. Despite their important role in bilingual instruction, bilingual professional learning community (BPLC) remains an underexplored topic of discussion. In particular, how BPLC can support subject teachers with limited English proficiency, a common phenomenon in Taiwan, is a question that needs further investigation. Thus, this study aims to implement a sustained BPLC to examine its functional role and influence on bilingual instructional transformation among teachers with limited English proficiency. The study draws on classroom observation notes, BPLC discussions, and interview data collected from fall 2024 to summer 2025. The findings show that regular classroom observations and bilingual professional conversation meetings benefit teachers across various areas, including classroom management, lesson planning, and language development. The BPLC provides real-time feedback and long-term developmental guidance, thereby facilitating more effective bilingual instruction. To conclude, this study contributes to the understanding of the dynamic relationship between bilingual instruction and BPLC. It also offers insights into how BPLC can support teachers with limited English proficiency in various ways. Full article

Background/Objectives: Antimicrobial resistance (AMR) is a major global health threat, with children at higher risk due to developmental differences in drug metabolism, limited treatment options and inappropriate antibiotic use. Pediatric antimicrobial stewardship programs (ASPs) face implementation challenges, often relying on adult-based guidelines and limited pediatric-specific evidence. This study aimed to identify and prioritize the most critical areas for pediatric ASP intervention development through a structured, multi-round Delphi consensus process with experts in antimicrobial stewardship and infectious diseases. Method: A four-round modified Delphi process was conducted to identify and prioritize key pediatric ASP interventions. Experts in antimicrobial stewardship and infectious diseases were recruited through an existing clinical trial. Using an iterative survey and in-person discussions, experts provided input on priority areas, which were thematically grouped and refined across rounds. Structured feedback supported real-time refinement and consensus-building. Results: Twenty experts participated in the process, generating 25 priority items in Round 1 through open-ended responses. These were narrowed to seven key priorities through structured voting and discussion. The final items were clustered into three intersecting themes: Care Settings, Prescriptions, and Strategies. Care Settings focused on high-impact areas such as outpatient clinics and intensive care units, where misuse is common and/or care is complex. The prescriptions theme prioritized shorter durations and narrow-spectrum agents. The strategy theme highlighted the need for outcome-based metrics, improved diagnostic stewardship, and routine tracking of patient outcomes to guide and assess stewardship efforts. Conclusions: This expert consensus identified key priorities for pediatric ASPs, providing a foundation for future interventions. Findings can be used to inform policy and practice, improving the appropriate use of antimicrobials in pediatrics and combating AMR. Full article

With buildings accounting for 40% of global energy consumption, heating, ventilation, and air conditioning (HVAC) systems represent the single largest opportunity for emissions reduction, consuming up to 60% of commercial building energy while maintaining occupant comfort. This critical balance between energy efficiency and human comfort has traditionally relied on rule-based and model predictive control strategies. Given the multi-objective nature and complexity of modern HVAC systems, these approaches fall short in satisfying both objectives. Recently, reinforcement learning (RL) has emerged as a method capable of learning optimal control policies directly from system interactions without requiring explicit models. However, standard RL approaches frequently violate comfort constraints during exploration, making them unsuitable for real-world deployment where occupant comfort cannot be compromised. This paper addresses two fundamental challenges in HVAC control: the difficulty of constrained optimization in RL and the challenge of defining appropriate comfort constraints across diverse conditions. We adopt a safe RL with a neural barrier certificate framework that (1) transforms the constrained HVAC problem into an unconstrained optimization and (2) constructs these certificates in a data-driven manner using neural networks, adapting to building-specific comfort patterns without manual threshold setting. This approach enables the agent to almost guarantee solutions that improve energy efficiency and ensure defined comfort limits. We validate our approach through seven experiments spanning residential and commercial buildings, from single-zone heat pump control to five-zone variable air volume (VAV) systems. Our safe RL framework achieves energy reduction compared to baseline operation while maintaining higher comfort compliance than unconstrained RL. The data-driven barrier construction discovers building-specific comfort patterns, enabling context-aware optimization impossible with fixed thresholds. While neural approximation prevents absolute safety guarantees, reducing catastrophic safety failures compared to unconstrained RL while maintaining adaptability positions this approach as a developmental bridge between RL theory and real-world building automation, though the considerable gap in both safety and energy performance relative to rule-based control indicates the method requires substantial improvement for practical deployment.
