The Social Internet of Things (SIoT) has emerged as a promising paradigm for addressing interoperability, adaptability, and intelligent collaboration challenges in smart city environments. However, existing solutions often provide only partial support for semantic interoperability, dynamic social relationships, and context-aware service coordination across heterogeneous IoT ecosystems. This paper presents SEMIWARE, a semantic social network-oriented middleware designed to support collaborative, interoperable, and context-aware SIoT applications. SEMIWARE adopts a layered architecture that combines a FIWARE-based middleware backbone with modular services for context management, semantic annotation, semantic reasoning, service discovery, social relationship management, profiling, security, and ontology alignment. Its semantic backbone is provided by an OWL2 ontology that models IoT entities, users, services, contextual information, and trust-aware social relationships. The middleware is validated through two representative applications in distinct domains: smart mobility, where semantic reasoning supports adaptive eco-friendly route computation, and healthcare, where semantically integrated wearable and environmental data support health-event detection for people with dementia. Experimental evaluation further examines the performance of semantic annotation, semantic reasoning, and context management services under increasing workloads. The results provide prototype-level evidence that SEMIWARE supports semantic interoperability, cross-domain adaptability, and graph-based processing under controlled workloads, indicating its potential suitability for complex, data-intensive SIoT applications. This study examines how the sense of home for people with dementia is shaped not only by physical settings but by dynamic atmospheric compositions emerging through memory, sensation, and everyday practices. Building on a preliminary literature mapping that identified three dimensions of home in later-life care environments—safe space, small world, and connection—we developed a multisensory co-design toolkit combining key-element cards and curated olfactory prompts. The study was conducted in a dementia-friendly residential care facility in Italy. Nine residents with mild–moderate dementia (aged 75–84) participated in two group sessions and six individual sessions, facilitated by two design researchers with care staff present. Data consist of audio-recorded and transcribed interviews, guided olfactory sessions, and researcher fieldnotes. Across sessions, participants articulated “small worlds” as micro-environments composed of meaningful objects, bodily comfort, routines, and sensory cues that supported emotional regulation and identity continuity. Olfactory prompts, administered through a low-intensity and participant-controlled protocol, supported scene-based autobiographical recall for some participants, often eliciting memories of domestic rituals, places, and relationships. Rather than treating home-like design as a fixed architectural style, we interpret home as continuously re-made through situated sensory–temporal patterns and relational practices. We translate these findings into atmospheric design directions for dementia care: designing places of self and refuge, staging accessible material memory devices, embedding gentle olfactory micro-worlds within daily routines, and approaching atmosphere as an ongoing process of co-attunement among residents, staff, and environmental conditions. The study contributes a methodological and conceptual framework for multisensory, narrative-driven approaches to designing home-like environments in long-term care. Full article

The global increase in dementia cases, which is predicted to exceed 152 million by 2050, poses substantial challenges to healthcare systems and caregiving structures. Concurrently, the expansion of mobile health (mHealth) technologies offers scalable, cost-effective opportunities for dementia care. This study systematically reviews 100 publicly available dementia-related mobile applications on the Apple App Store (iOS) and the Google Play Store (Android), categorised using the Mobile App Rating Scale (MARS), as well as the targeted end-users, Internet of Things (IoT) integration, data protection, and cost burden. Applications were evaluated for their utility in cognitive training, memory support, carer education, clinical decision-making, and emotional well-being. Findings indicate a predominance of carer resources and support tools, while clinically integrated platforms, cognitive assessments, and adaptive memory aids remain underrepresented. Most apps lack empirical validation, inclusive design, and integration with electronic health records, raising ethical concerns around data privacy, transparency, and informed consent. In parallel, the study identifies promising pathways for energy-optimised IoT systems, Artificial Intelligence (AI), and Ambient Assisted Living (AAL) technologies in fostering dementia-friendly, sustainable environments. Key gaps include limited use of low-power wearables, energy-efficient sensors, and smart infrastructure tailored to therapeutic needs. Application domains such as cognitive training (19 apps) and carer resources (28 apps) show early potential, while emerging innovations in neuroadaptive architecture and emotional computing remain underexplored. The findings emphasize the need for co-designed, evidence-based digital solutions that align with the evolving needs of people with dementia, carers, and clinicians. Future innovations must integrate sustainability principles, promote interoperability, and support global aging populations through ecologically responsible, person-centred dementia care ecosystems. Background/Objectives: The rising prevalence of dementia presents significant emotional, psychological, and economic challenges for families. Dementia-friendly communities (DFCs) aim to alleviate these burdens by fostering social inclusion and mutual support for people with dementia (PWD) and their caregivers. This study explores the social network structures within DFCs in rural Taiwan, utilizing social network theory as its framework. Methods: A qualitative design was employed, involving semi-structured interviews with eleven participants, including eight caregivers and three case managers. Data were analyzed using grounded theory. Results: The results indicate that the DFC social network is grounded in reciprocal relationships and mutual support between families, workplaces, and neighborhoods. Durability depends on stable relationships, sustained support systems, and the preservation of local culture. Cultural alignment and engagement reduce stigma and foster understanding, while diverse activities strengthen social bonds and participation. Conclusions: The study highlights the importance of government policy, infrastructure, and public awareness in sustaining dementia-friendly environments. The results offer valuable insights for enhancing community design and policy to better support PWD and their families. Full article

Background/Objectives: The role of environmental modifications and design in mitigating behavioural symptoms is increasingly being recognised as a way to address the psychosocial needs of individuals with dementia. This study aims to investigate various nature-based interventions for reducing agitation in people with dementia in long-term residential care environments. Methods: Database searches were conducted on MEDLINE, PsycINFO, Scopus, and Web of Science. A literature search was conducted with the following inclusion criteria: (i) peer-reviewed journal publication written in English; (ii) random controlled trials (RCTs) and quasi-experimental design with results for pre- and post-testing reported; (iii) interventions using natural elements, where the effectiveness of the reduction in agitation was measured using a validated instrument; and (iv) participants aged 65 and older with dementia residing in long-term care facilities. Results: This meta-analysis included 29 studies with 733 participants. The results showed that such interventions had a significant negative mean effect on lowering agitation in this population. Additionally, intervention settings (indoor vs. outdoor) and the presence of social interaction were significant predictors of the effect size for agitation reduction. At the same time, no significant differences in effect size were observed between the types of experiences with nature (indirect vs. direct) or the duration of the interventions. Conclusions: This study demonstrates that, when thoughtfully applied, nature-based interventions can significantly alleviate agitation in patients with dementia residing in long-term residential care facilities. This review lays the groundwork for future research aimed at developing design guidelines and planning strategies to integrate natural elements into dementia-friendly environments effectively. Dementia’s escalating incidence, coupled with its economic burden, highlights the need for architectural designs and forms that benefit people living with dementia. This research explores strategies and design principles that focus on establishing supportive spaces for the prolonged autonomy, well-being, and safety of dementia patients. Contemporary research emphasizes the implementation of effective navigational techniques, via visual cues, familiar landmarks, and simplified layouts. For greater navigational signage, the intentional use of contrasting colors, textures, and lighting are recommended to demarcate functional areas within a structure. Incorporating familiar objects in personalized areas enhances treatment outcomes, e.g., reminiscence therapy leverages familiar objects, environmental cues, scents, sounds, and tactile features to trigger and retain memory. Integrating safety precautions such as slip-resistant flooring, handrails, and accessible bathrooms helps mitigate falls for the cognitively impaired. From a therapeutic perspective, this study draws attention towards incorporating gardens and outdoor spaces, which offers sensory stimulation, encourages physical activity, and fosters social engagement. Additionally, the integration of scents, sounds, and tactile features enriches the sensory experience for individuals with dementia. Through comprehensive consideration of design elements, this research highlights how intentionally crafted, dementia-friendly environments can convert spaces into empowering therapeutic settings tailored to address the unique need of this vulnerable group. This architectural approach acts as a valuable complement to the medical and therapeutic interventions in the treatment and care of people living with dementia and their families. Full article

Objective: Visual processing deficits arising in dementia are associated with particular functional disability. This study aimed to investigate the effects of the built environment on mobility and navigation in people with dementia-related visual loss. Methods: Participants with posterior cortical atrophy (PCA; “visual-variant Alzheimer’s”; n = 11), typical Alzheimer’s disease (tAD; N = 10), and controls (n = 13) repeatedly walked down routes within a simplified real-world setting. Participant groups were of comparable age and gender. Routes were of different complexity (straight, U-shaped, and S-shaped), overhead lighting levels (low and high) and with or without a dynamic LED (light-emitting diode) cue (trial n = 24). Ratios of walking times for each experimental condition (each complex route vs the straight route, high lighting vs low, and LED cue vs no cue) were compared between participant groups. Kinematic measures were produced from a total of 10,813 steps using wearable inertial measurement units (IMUs). Results: The walking time ratios relating to route complexity were higher in the PCA group than in controls: 30.3% (95% CI [13.5%, 49.5%] higher for U-shaped vs straight and 31.9% [21.1%, 55.3%] for S-shaped vs straight, averaged over other conditions). The analogous results relating to route complexity for the tAD group were intermediate between those for the PCA and control groups. There was no evidence that walking time ratios differed according to lighting level or the presence of the LED cue. Conclusions: Findings contribute to evidence-based design for dementia-friendly environments, emphasizing consequences of environmental complexity for functional independence and mobility in people with dementia-related visual loss. Findings inform recommendations for environmental design to support the independence of individuals with dementia. Full article

According to statistical forecasts, one in six people on the planet will be 60 years of age or older by 2030. By 2050, the number of people in this age group is predicted to double to 2.1 billion. Furthermore, it is projected that the number of people 80 years of age and older would treble between 2020 and 2050, reaching 426 million. For architects, this demographic transition poses a serious problem, especially when it comes to creating ergonomic living environments that adhere to universal design principles. With a focus on prefabricated large-panels technology built in Poland between 1960 and 1989, this study examines and presents effective design ideas for senior residential complexes and contrasts them with Polish and European approaches. The study addresses the unique needs of senior citizens, taking into account both physical and cognitive limitations, and provides a thorough analysis of statistical data to highlight the difficulties in designing for seniors. The article showcases creative design solutions made for people with cognitive impairments including Alzheimer’s disease and dementia. One of the key studies undertaken in the article was the analysis of the existing Popowice Północne housing estate in Wrocław, constructed using large-panel technology. Additionally, it sets out the fundamentals for developing age-friendly cities and provides thorough guidelines for a comprehensive approach to constructing living areas that are age-friendly. The article utilizes literature and statistical data analysis, case study research, and a comparative method. To summarize the analyses and research, a SWOT analysis was conducted on the accessibility of large-panel buildings for the older people. Full article

Health-promoting outdoor environments designed for people living with dementia (PLwD) has proved to be an effective non-pharmacological intervention for treatment of symptoms and improved well-being. However, for individuals with Young-Onset Dementia (YOD), who have particular symptoms and needs, the content and design of these environments are underexplored. This study aimed to explore the needs of individuals with YOD in a garden setting, to generate design-related knowledge for ‘dementia-friendly’ outdoor environments, while contributing to the field of Evidence-Based Design (EBD). An 8-week long nature-based program was carried out in Alnarp’s rehabilitation garden, a specifically developed garden based on research from e.g., landscape architecture, environmental psychology and medical science. The study used a triangulation of qualitative methods including six participants with YOD and a multidisciplinary team of five staff members. Content analysis was used for all gathered data, including 17 semi-structured interviews with participants with YOD and with staff. Data collection and analysis was performed based on the evidence-based Quality Evaluation Tool (QET). The study led to a target group adapted version of the QET for people with YOD containing 20 developed environmental qualities for designers to pursue in therapeutic gardens, including the additional quality of Calmness. A progression was noted, as a result of perceived positive effects during the intervention, indicating possible change and development of the group’s needs and preferences in the outdoors. South Korea is an aging society with a rapidly increasing number of people with dementia. In that context, this study aimed to compare dementia nursing home building standards in South Korea and Germany, based on the minimum physical and architectural requirements specified by both countries’ relevant laws, to gain insights into improving South Korean building standards. I examined South Korea’s Welfare of Senior Citizens Act and its Enforcement Regulations, and Germany’s Federal Nursing Home Act (HeimG), Regulation on Minimum Standards for Nursing Homes (HeimMindBauV), and the corresponding state regulations. The analysis revealed differences regarding the basic requirements, facility sizes, composition of necessary rooms, and detailed regulations pertaining to these rooms. South Korea emphasized autonomy, including barrier-free and compensatory environments, as well as safety and security. Meanwhile, Germany enhanced similar aspects of autonomy to those in South Korea, including the barrier-free standard DIN 18040-2. Additionally, Germany incorporated features such as familiarity, sensory stimulation, legibility, and social interaction, reflecting aspects that offer orientational cues for autonomy. Improving South Korean building standards requires stronger regulations on the autonomy of individuals with dementia. Further, additional regulations on familiarity, sensory stimulation, legibility, and social interactions should be considered. The results provide foundational data for cross-national comparisons to establish building standards for dementia-friendly built spaces in dementia nursing homes in South Korea. Further surveys on spatial utilization, discussions, and the application of proposed improvements in Korea would contribute to enhancing the dementia-friendly nursing home design in the country. An ageing population is a global phenomenon. Like other developed economies, Hong Kong Special Administrative Region (HKSAR), China, also faces a severe ageing problem. One initiative to enhance the safe living and well-being of the growing elderly population is to assist them by building ageing-friendly living environments with the application of smart home technologies (SHTs). Therefore, this study focused on investigating the perception of professionals on the use of SHTs to improve and enhance the “ageing-in-place” (AIP) of elderly residents in HKSAR, China. A questionnaire survey was employed to obtain the perception of professionals with requisite knowledge of the older people facility needs regarding SHTs in achieving AIP for the elderly. The data retrieved were analysed with different statistical analyses. Based on the results of the analyses, all the professionals had similar perceptions of the use of SHTs for the safety and well-being of the elderly, except for the incongruence observed between the government employees, contractors and academic regarding how SHTs may not help to better monitor elderly daily activities. The possible reasons for the inconsistent opinions of the academics with other groups were linked to the knowledge of human behaviours and early dementia symptoms in gerontology. The findings will help care receivers, healthcare professionals, social workers, policymakers, smart home designers and developers to improve and enhance AIP in elderly residences in HKSAR, China. The concept of dementia-friendly cities and communities has achieved great dissemination in the international context since 2016. Although it is usually related with community networks and services, evidence and guidelines show the close relationship between the built environment design, health promotion, and the preservation of relationships with the local surroundings. Recent publications emphasize best practices in urban areas and care management. However, this is a very complex reality in each country depending on the sociosanitary services, the demographic, and geographic structure and many other different aspects including cultural ones. Moreover, design should also consider not only basic aspects such as habitability or universal accessibility, but also heritage, identity, and the feeling of normalized living. Knowledge about international experiences and innovative approaches is, as yet, an object of study as demographic ageing is still challenging all the welfare systems, especially in Europe. This study presents eight descriptive study cases in three different European countries—the United Kingdom, Belgium, and The Netherlands—to analyze the relationship between dementia-friendly initiatives and their intersection with design, urban planning and the provision of care. The results can provide strategic lines for development and innovation towards dementia-friendly societies and cities achieving SDG numbers 3 and 11 simultaneously. Full article

Black churches have traditionally been a haven for Black American families; however, many churches do not currently have programs to support families living with dementia. Alter™ was established to assist faith communities in meeting the needs of these families and becoming a viable resource to promote their health and wellness. Alter™ achieves this aim through a three-pronged approach: (1) conducting educational sessions, (2) modifying Black churches to be dementia-inclusive spaces, and (3) providing ongoing support. The principal goal of Alter™ is to offer guidance to churches in adapting their community to reflect a supportive environment for families affected by dementia. Alter™ uses a partnership checklist to encourage activities that incorporate dementia education opportunities, environmental modifications, and dementia resources and support. This paper reports on a formative mixed-methods evaluation of church partners enrolled in Alter™. Church partner ambassadors within the faith communities participated in the evaluation survey (n = 8) and two focus groups (n = 11). Ambassadors are appointed by church leadership to lead the implementation of program activities. Data were collected concerning the levels of helpfulness, difficulty, usefulness, or utility of checklist activities and modifications and the ease of program implementation. The evaluation also assessed the COVID-19 pandemic’s impact on each church’s ability to implement activities and modifications. The survey results revealed that most required partnership activities and modifications were found to be at least moderately helpful. Some of the items (themes) that differed across church partners included barriers to implementing activities, the support provided and needed, and the use of program funding. This evaluation provides key insights to consider in developing and refining community-based, dementia-friendly communities (including faith communities). As implementation science expands and improves, the need to evaluate the implementation of programs continues to be highlighted. Our formative evaluation shed light on key areas in which modifications to our original programming would lead to program improvement and sustainability. Additionally, implementing the modifications identified in our evaluation will facilitate the achievement of the mission of Alter™ to improve the well-being of older adults affected by dementia and their families. Other programs would reap substantial benefits from engaging in similar formative evaluation efforts. Full article

Alternative care environments for regular nursing homes are highly warranted to promote health and well-being of residents with dementia that are part of an age-friendly and dementia-friendly city and society. Insight is lacking on how to translate evidence-based knowledge from theory into a congruent conceptual model for innovation in current practice. This study reports on the co-creation of an alternative nursing home model in the Netherlands. A participatory research approach was used to co-create a conceptual framework with researchers, practitioners and older people following an iterative process. Results indicate that achieving positive outcomes for people with dementia, (in)formal caregivers, and the community is dependent on how well the physical, social and organizational environment are congruently designed. The theoretical underpinnings of the conceptual model have been translated into “the homestead,” which is conceptualized around three main pillars: activation, freedom and relationships. The Homestead Care Model is an illustrative example of how residential care facilities can support the development of age-friendly communities that take into consideration the needs and requirements of older citizens. However, challenges remain to implement radical changes within residential care. More research is needed into the actual implementation of the Homestead Care Model. Full article

Given their stability, their ability to influence public attitudes and capacity to mobilise human resources, it seems likely that many faith communities can have a significant impact on the wellbeing of people living with dementia (PLWD) and their carers, who may be dependent on the commitment of informal communities of support over months or years. This paper reports on a pre–post cohort study undertaken in an Anglican (Church of England) diocese in the UK. Representatives (N = 61) of 11 church communities completed the Dementia Awareness Scale immediately before and 12 months after an intervention to promote ‘Dementia-Friendly Church’ in their congregation. Results showed a robust improvement in scores on both the ‘Knowledge’ and ‘Comfort’ subscales, with the largest effect size in the latter. Within each subscale, improvements were evenly distributed across the individual items. These findings indicate that such an intervention is likely to significantly improve the environment for people living with dementia and their companions who are members of or engage with the Church community. They also suggest that a similar programme could be extended to other intentional communities, whether in faith-based organisations or societies with shared interests. Full article
