Objective: The purpose of the study was to conduct a systematic review assessing the impact of pharmacological and non-pharmacological interventions on Relative Energy Deficiency in Sport (REDs). The study design follows a systematic review and meta-analysis. The data sources are CINAHL, MEDLINE, SportDiscus, ERIC, and Embase from inception until July 2025. Eligibility criteria for selecting the studies include the experimental, quasi-experimental, and pre-experimental literature that investigated interventions designed to support the symptoms of REDs. Results: A total of nineteen studies (fifteen non-pharmacological interventions, four pharmacological interventions), with a total of 759 females, were included in the review. Non-pharmacological interventions demonstrated positive benefits on menstrual function recovery, energy availability, fat mass, and body fat percentage. Meta-analyses quantified nutrition intervention benefits on an individual’s fat mass (kg), 1.36 (95% CI 0.68, 2.04), and body percentage fat (%), 2.21 (95% CI 1.34, 3.08). It was also possible to identify the impact of non-pharmacological interventions on total triiodothyronine (T3) biomarkers (nmol/L), −2.37 (95% CI −5.57, 0.83). It should be noted, however, that non-pharmacological interventions were limited by quality and certainty assessment, identifying included evidence as low to moderate. Pharmacological interventions demonstrated some positive (at times very strong effect sizes) results for impact on bone mineral density, but conclusions are currently limited by well-powered experimental studies. Conclusions: The current evidence base favors non-pharmacological management as an initial response for managing REDs. Initial pharmacological management appears to identify limited but potentially (depending on the drug) promising evidence for the impact on bone mineral density; further evidence is required to be more certain about the impact on hormonal profiling and menstrual recovery function. Further research is needed to help develop a greater understanding. Full article

Background: Relative Energy Deficiency in Sport (REDs) results from an imbalance between energy intake and expenditure, leading to low energy availability (LEA) and impairments of physiological and/or psychological functions in female and male athletes. While physical determinants of REDs are well documented, psychological factors such as body image (BI) have received comparatively little attention. The aim of this scoping review was to synthesize the current scientific evidence on the relationship between BI and REDs. Methods: A scoping review examined the current literature, including quantitative and qualitative studies. The scoping review was conducted in April 2025 in PubMed, Web of Science, MEDLINE, SPORTDiscus, APA PsycArticles, APA PsycInfo, CINAHL and OpenDissertations. Studies were included if they examined BI aspects in relation to LEA or REDs in a sporting context, regardless of participants’ gender, age, level or sport. Inclusion criteria were based on the Population—Context—Concept (PPC) framework. Results: Seven studies met the inclusion criteria, covering athletes from various ages, genders, sports, and performance levels. Findings indicate that BI dissatisfaction—manifesting, for example, as a drive for thinness or muscularity, exercise dependence, and disordered eating—represents a relevant psychological factor associated with LEA in both female and male athletes. Conclusions: The relationship between BI and REDs is complex and insufficiently explored. Future research should address this link systematically across sports, performance levels, genders, and age groups. In sports medicine practice, screening tools should systematically incorporate psychological risk factors such as BI disturbances to enable early detection, targeted intervention, and prevention of long-term health consequences. This review synthesizes current evidence on energy and macronutrient requirements, hydration strategies, and key micronutrients, including iron, calcium, and vitamin D, which are essential for supporting health and performance in youth sport. It explores the physiological risks associated with low energy availability (LEA), while emphasizing the importance of carbohydrate and protein timing, quality, and distribution. The review also evaluates the role of dietary supplements and ergogenic aids, including creatine and energy drinks, highlighting safety concerns and advocating for a food-first approach. Practical strategies for nutrition education, interdisciplinary collaboration, and individualized care are presented to guide healthcare professionals, coaches, and caregivers in fostering sustainable, performance-supportive eating habits. Background/Objectives: Pressure to stay lean may lead adolescent athletes to dietary restraint and disordered eating. Lack of nutrition awareness can also contribute to suboptimal dietary habits, increasing the risk of eating disorders and Relative Energy Deficiency in Sport [RED-S], though evidence in competitive athletes is limited. This study explored eating disorder symptoms and RED-S knowledge in adolescent artistic gymnasts. Methods: Eighty-four female artistic gymnasts, thirty-nine international and national level gymnasts [high-level; 14 [14, 15] y] and forty-five recreational and club level gymnasts [low-level; 14 [13, 15] y] completed the Eating Disorder Examination Questionnaire [EDE-Q 6.0], the RED-S knowledge Questionnaire and provided training details. Results: Seventeen gymnasts (20.2%) scored above the cutoff point on the EDE-Q. In addition, high-level gymnasts scored higher than low-level on EDE-Q [2.21 ± 1.37 (35.9%) vs. 1.19 ± 0.79 (6.7%), respectively; p < 0.001] and on its subscales: Restraint, Eating Concerns, Weight Concerns, and Shape Concerns [p < 0.001 to 0.009], thus indicating more severe disordered eating symptoms. No group differences were found in binge eating and compensatory behaviors. An important percentage of gymnasts reported at least one episode of binge eating and excessive training [39.3–58.3%], while four gymnasts reported self-induced vomiting. RED-S knowledge did not differ between groups. On average, gymnasts were unaware of correct answers related to RED-S [51.5%], its definitions [79.8–92.9%], and its association with menstrual disturbances and bone health [54.8–86.9%]. However, gymnasts reported better awareness of the impact of food restriction on illness and performance [47.6–84.5%]. Conclusions: Elite artistic gymnasts exhibited a higher prevalence of eating disorder symptoms than lower-level peers. Gymnasts at all levels demonstrated limited knowledge of the effects of RED-S on menstrual and bone health. Failure to recognize these risks may influence gymnasts’ eating behaviors and delay RED-S detection and management. Full article

Background/Objectives: The primary aim of this study was to characterize athletes approaching an outpatient interdisciplinary and multidisciplinary consultation structure for athletes with a suspected relative energy deficiency in sport (REDs) cross-sectionally and longitudinally to prove treatment efficacy. Methods: Data of 58 athletes suspected of REDs were collected at the onset (t0) and completion (t1) of interdisciplinary and multidisciplinary REDs treatment (clinical practice) between January 2019 and December 2022. The data included extracted information from medical records, anthropometric characteristics, physical performance diagnostics, laboratory values, dietary records, and partially gynecological and psychosomatic diagnostics. Results: The sample primarily consisted of female athletes (97%) under 18 years of age (66%) who were underweight with a body mass index (BMI) < 18.5 kg/m2 and BMI percentile below the 10th percentile for their age and gender-specific norms (59%), and experienced menstrual disorders (93%). The dietary behavior is characterized by plant-based and low-energy-dense foods. Eating disorders (anorexia nervosa and bulimia nervosa) were diagnosed in 40% of the athletes according to the International Statistical Classification of Diseases and Related Health Problems 11th revision criteria. During the program, 64% of the athletes exhibited a mean weight gain of 7 (±6) kg (p < 0.001), excluding those still undergoing treatment (36%). Conclusions: The proposed interdisciplinary and multidisciplinary treatment approach proved effective and holds promise for future evidence-based developments in REDs treatment. Full article

Background/Objectives: As an endurance multi-sport race, triathlon places significant energy demands on athletes during performance and training. Insufficient energy intake from food can lead to low energy availability (LEA) and Relative Energy Deficiency in Sport (RED-S). We aimed to measure symptoms related to LEA, examine the risk of RED-S, and find how diet relates to the risk of RED-S in highly trained female amateur triathletes. Methods: Our sample was 20 top-performing female triathletes competing in Quarter Ironman (IM), Half IM, IM, or Double IM triathlons for 5.5 ± 2.5 y who were during the preparatory phase of training (training load 11 ± 3.76 h/week, a single workout 84 ± 25 min). Triathletes completed 3-day food diaries, training diaries, and the Low Energy Availability in Females Questionnaire (LEAF-Q). Exercise energy expenditure was estimated using wrist-worn activity trackers. To examine dietary patterns related to the first signs of LEA, predating RED-S, we created two groups: the L-LEA group (LEAF-Q score 0–5, no symptoms related to LEA, low risk of RED-S, n = 10) and the H-LEA group (LEAF-Q ≥ 6, at least one LEA-related symptom, high risk of RED-S, n = 10). Results: The risk of RED-S was prevalent in 30% of female triathletes, and 50% showed at least one symptom related to LEA. Macronutrient intake was similar in all participants, but triathletes from the H-LEA group tended to eat more plant-sourced protein and fiber. They consumed less saturated fatty acids but ingested more significant amounts of n-6 polyunsaturated fatty acids (PUFAn6). Conclusions: We conclude that foods higher in plant proteins, fiber, and PUFAn6 might predispose female triathletes to LEA by reducing the diet’s energy density. Full article

Background/Objectives: Like in the general population, the prevalences of eating- and weight-related health issues in the armed forces are increasing. Relevant medical conditions include the eating disorders (EDs) anorexia nervosa, bulimia nervosa, binge eating disorder, and avoidant restrictive food intake disorder (ARFID), as well as body dysmorphic disorder, muscle dysmorphia, and the relative energy deficiency in sport (RED-S) syndrome. Methods: We performed a narrative literature review on eating- and weight-related disorders in the armed forces. Results: Entry standards might exclude people with obesity, with EDs, or at high risk for EDs from entering the armed forces for military reasons and to protect the individual’s health. Relevant potential risk factors of eating- and weight-related disorders in the military are the emphasis on appearance and fitness in the military, high levels of stress, military sexual trauma, post-traumatic stress disorder, deployment, relocation, long commutes, consumption of ultra-processed foods and beverages, limitations on food selection and physical exercise, and intensive combat training and field exercises. Eating- and weight-related disorders negatively impact professional military appearance and lead to problems with cardiorespiratory and neuromuscular fitness; daytime sleepiness; and a higher risk of musculoskeletal injuries, and other physical and mental health problems. Current and potential future therapeutic options include occupational health measures, psychosocial therapies, neuromodulation, and drug treatments. Conclusions: Even though randomized controlled trials (RCTs) have been performed to test treatments for obesity in the armed forces, RCTs for the treatment of EDs, body dysmorphic disorder, muscle dysmorphia, and RED-S syndrome are lacking in the military context. Background and Objectives: A sustained mismatch between energy intake (EI) and exercise energy expenditure (EEE) can lead to Low Energy Availability (LEA), as well as health and performance impairments characteristic of Relative Energy Deficiency in Sport (RED-S). Research in females has identified specific LEA cut-points for the risks of developing physiological and performance disturbances. Cut-points in males have yet to be evaluated; therefore, this study examined the prevalence of LEA in highly trained male cross-country skiers. The key purpose of this study was to analyze EI, resting energy expenditure (REE), EEE, and energy availability (EA) in highly trained cross-country skiers during the preparation and competition periods. The secondary objective of our study was to evaluate the relative contribution of fats and carbohydrates to EI, REE, and EEE. Materials and Methods: EI was determined by an estimated 24 h diet recall method, REE was assessed by indirect calorimetry, and EEE was estimated from heart rate in 27 cross-country skiers. Results: EI amounted to 4050 ± 797 kcal/day on a typical training day (TD) and 5986 ± 924 kcal/day (p < 0.001) on a typical competition day (CD). REE on TDs (2111 ± 294 kcal/day or 30 ± 6 kcal/day/kg) was higher (p < 0.05) than on CDs (1891 ± 504 kcal/day or 27 ± 7 kcal/day/kg). The EA in the athletes was <15 kcal∙kg FFM–1·d–1 on TDs and <65 kcal∙kg FFM–1·d–1 on CDs. EI was not optimal, as indicated by low EA throughout TDs (June). This could be associated with insufficient EI along with a high amount of EEE (3690.7 ± 485.2 kcal/day). During the transition from TD to CD, an increase in the contribution of fats to EI and EEE was observed in cross-country skiers. Conclusion: The conception of LEA and REDs and their potential implication for performance is underestimated among coaches and athletes. The importance of appropriate dietary strategies is essential to ensure that enough calories are consumed to support efficient training. Full article

Relative energy deficiency in sport (RED-S) is a condition that arises from persistent low energy availability (LEA), which affects the hypothalamic–pituitary axis and results in alterations of several hormones in both male and female athletes. As frequent blood hormone status determinations using venipuncture are rare in sports practice, microsampling offers promising possibilities for preventing and assessing RED-S. Therefore, this study aimed at developing a liquid chromatography–high-resolution tandem mass spectrometry (LC-HRMS/MS) method for quantifying relevant steroids and thyroid hormones in 30 μL of capillary blood obtained using Mitra® devices with volumetric absorptive microsampling technology (VAMS®). The results of the study showed that all validation criteria were met, including a storage stability of more than 28 days in a frozen state (−18 °C) and 14 days at room temperature (20 °C). The validated assay provided precise (<12%) and accurate (<13%) results for all the target analytes. Furthermore, as a proof of concept, autonomously collected VAMS® samples from 50 female and male, healthy, active adults were analyzed. The sensitivity of all analytes was adequate to quantify the decreased hormone concentrations in the RED-S state, as all authentic samples could be measured accordingly. These findings suggest that self-collected VAMS® samples offer a practical opportunity for regular hormone measurements in athletes and can be used for early RED-S assessment and progress monitoring during RED-S recovery. Energy imbalance exposes athletes to relative energy deficiency in sports (REDs) syndrome. Data on energy consumption, REDs, and bone mineral density (BMD) in adolescent acrobatic gymnasts, especially in males, are scarce. Our aim was to examine the eating habits, energy balance, body composition, and BMD of these athletes. In this study, 18 healthy adolescents participating in competitive acrobatic gymnastics completed a questionnaire, underwent a dual-energy X-ray absorptiometry scan (DXA), received a food log, and had their activities monitored for 3 days. Eighteen acrobats were enrolled (mean age: 14.3 ± 1.2 years; males: 6/18). The mean total body BMD Z-score was 0.4 ± 1.0. Top-position acrobats (7/18) had significantly lower total body BMD Z-scores than base-positioned acrobats (−0.2 ± 0.3 vs. 0.8 ± 0.3, p = 0.032), though their forearms were not significantly different (0.2 ± 0.5 vs. 0.8 ± 0.7, p = 0.331). No sex differences were found for BMD Z-scores, BMI, or energy availability. The BMD parameters of the acrobats were within the normal range for a healthy pediatric population, although three had low BMDs (<−1 SD) for healthy athletes. Total body and LS BMD Z-scores were significantly lower in top-position athletes compared to base-position athletes. These findings suggest personalized (top vs. base) training programs (high-impact training) that may achieve better health outcomes. Full article

The female athlete triad (TRIAD) is a spectrum of disorders involving low energy availability (LEA), low bone mineral density, and menstrual disorders. It is increasingly common to use the term ‘relative energy deficiency in sport’ (RED), emphasising the extensive impact of LEA on the body. The aim of this narrative review was to gather original research encompassing female athletes across various sports as well as to collect findings on the potential of a nutrition-focused approach to prevent or treat the aforementioned disorders. A comprehensive search was conducted in PubMed and Scopus. Several challenges were identified regarding the adequacy of the energy availability, protein, and carbohydrate requirements in the diets of female athletes. Moreover, insufficient intake of vitamin D has been observed across all athlete groups studied. This insufficiency also extends to the average requirement for Ca, Mg, the Ca/P ratio, Zn, and Fe. To address those concerns, a nutritional approach is proposed in the latter part of this review. The factors that can improve the absorption of micronutrients have also been discussed. The TRIAD/REDs affect an ever-growing number of women and require appropriate therapeutic management, particularly through nutritional care. Therefore, cooperation within an interdisciplinary team comprising a physician, nutritionist, physiotherapist, and psychologist is crucial. Full article

Longitudinal measurements of Relative Energy Deficiency in Sport (REDs) among adolescent male elite athletes are lacking. We aimed to monitor REDs indicators and their possible impact on performance in elite high-school cross-country skiing and biathlon athletes (n = 13) (16.3 ± 0.4 years, 179.4 ± 7.6 cm, 63.6 ± 8.2 kg body mass (BM), and peak oxygen uptake (VO2peak): 61.5 ± 5.3 mL/kg BM/min) every 6 months for 3 years. Protocols included assessments of energy availability (EA), body composition and bone mineral density (BMD), resting metabolic rate (RMR), disordered eating behavior, exercise addiction, VO2peak, and muscle strength. Data were analyzed using a linear mixed model. At baseline, 38% had low lumbar BMD (Z-score ≤ −1), and overall, bone health increased only slightly. VO2peak and muscle strength improved (p < 0.001), RMR decreased (p = 0.016), and no change was observed in EA or physiological or psychological REDs indicators. Conclusively, many of these young male athletes had poor bone health at baseline, and most either lost or did not achieve the expected pubertal bone mineral accrual, although no other indication of REDs was observed, while performance improved during the study period. Our findings highlight the importance of elite sports high schools focusing on screening for early detection of impaired bone health in male athletes. Full article

Introduction and Purpose: Athletes, especially in weight-dependent sports, are at higher risk of developing eating disorders (ED). Relative Energy Deficiency in Sport (RED-S) results from a mismatch between energy intake (EI) and exercise, leading to an athlete triad. Material and Methods: Our study includes ten high-performance athletes who came to the Center for Sports Nutrition and Supplementation because of ED. Nutritional and mental status were evaluated individually (physician’s examination, body composition, eating and supplement habits, blood analysis, accelerometer, and three-day diet diary). Results: Nine female and one male athlete with an average age of 17 years are classified as having anorexia nervosa (5), bulimia nervosa (4), or an eating disorder not otherwise specified (1). Female athletes had an average BMI of 18.4 kg/m2 and an F% of 19.7. All athletes had RED-S with an average EI of 1660 kcal/day and an energy expenditure of 2300 kcal/day. Representing different sports (swimming, volleyball, tennis, basketball, jazz ballet, and synchronized swimming), 7/10 athletes stopped training and 5/10 needed hospitalization because of ED exacerbation. In 7/9 athletes, there were <6 menses/12 months. An antidepressive drug (SSRIs) was indicated in six athletes. The average serum iron level was low, at 13.5 micromol/L. Carbohydrates, fat, and proteins were present in EI at 42.8%, 35.4%, and 21.8%. Athletes showed an intake deficit of cholesterol, magnesium, biotin, chrome, iron, fiber, folate, iodine, potassium, vitamins D, E, and K, pantothenic acid, and pyridoxine. Conclusion: Early identification of ED is associated with better outcomes. Educating athletes, sports entourages, and especially parents about healthy eating, pathological eating behaviors, and their consequences is crucial. Full article

The purpose of this narrative review is to identify health and performance consequences associated with LCA in female endurance athletes. The intake of carbohydrates (CHO) before, during, and after exercise has been demonstrated to support sport performance, especially endurance activities which rely extensively on CHO as a fuel source. However, low energy availability (LEA) and low carbohydrate availability (LCA) are common in female athletes. LEA occurs when energy intake is insufficient compared to exercise energy expenditure, and LEA-related conditions (e.g., Female Athlete Triad (Triad) and Relative Energy Deficiency in Sport (RED-S)) are associated with a myriad of health and performance consequences. The RED-S model highlights 10 health consequences and 10 performance consequences related to LEA. The independent effect of LCA on health and performance has been under-researched, despite current CHO intake being commonly insufficient in athletes. It is proposed that LCA may not only contribute to LEA but also have independent health and performance consequences in athletes. Furthermore, this review highlights current recommendations for CHO intake, as well as recent data on LCA prevalence and menstrual cycle considerations. A literature review was conducted on PubMed, Science Direct, and ResearchGate using relevant search terms (i.e., “low carbohydrate/energy availability”, “female distance runners”). Twenty-one articles were identified and twelve met the inclusion criteria. The total number of articles included in this review is 12, with 7 studies illustrating that LCA was associated with direct negative health and/or performance implications for endurance-based athletes. Several studies included assessed male athletes only, and no studies included a female-only study design. Overall, the cumulative data show that female athletes remain underrepresented in sports science research and that current CHO intake recommendations and strategies may fail to consider female-specific adaptations and hormone responses, such as monthly fluctuations in estrogen and progesterone throughout the menstrual cycle. Current CHO guidelines for female athletes and exercising women need to be audited and explored further in the literature to support female athlete health and performance. Full article

Relative energy deficiency in sport (RED-S), which underpins the concept of low energy availability (LEA), can negatively affect athletes’ health and performance. This study aims to investigate the prevalence of RED-S among national athletes in Malaysia. A total of 192 national athletes (97 males, 18–39 years old) responded to an online survey regarding the RED-S risk. Most athletes (67.2%) were classified as having a medium/high RED-S risk. Female (41.2%), weight-class (23.9%), and intermittent sports (20.3%) had the highest prevalence of medium/high RED-S risk. Overall, most athletes spent 2–5 h daily (55.2%) and 6–7 days weekly (53.6%) practicing or playing their sport, and 47.9% reported feeling tired recently. The athletes were also terrified of being overweight (61.5%), worried about what they eat (64.7%), concerned about having fat on their bodies (62.6%), and thinking about burning calories while exercising (69.3%). Only 16.7% of the athletes had a prior history of stress fractures, and 40.6% continued to participate in activities 6–7 days per week while injured. The majority of female athletes (88.3%) had regular menstrual cycles. These findings support the notion that RED-S screening should be addressed in the athletic community. Future research is needed to clinically assess these athletes and investigate the impacts of RED-S on their health and sports performance. Full article
