Anterior Redisplacement After Intramedullary Nail Fixation for Trochanteric Femoral Fractures: Incidence and Risk Factors in 598 Older Patients

Background/Objectives: Anterior redisplacement, defined as a postoperative anterior shift of the distal fragment despite intraoperative reduction, is occasionally observed after cephalomedullary nailing for trochanteric femoral fractures. However, its incidence and associated risk factors remain unclear. This study aimed to determine the incidence of anterior redisplacement following intramedullary nail fixation in geriatric trochanteric fractures, and to identify independent risk factors. Methods: This study retrospectively reviewed data from 598 consecutive hips in 577 patients (aged ≥ 65 years) who underwent intramedullary nail fixation for trochanteric fractures at a single center (2012–2023). Sagittal reduction on the lateral radiographic view was classified as posterior, anatomical, or anterior according to the position of the distal fragment, and was recorded preoperatively and postoperatively. Anterior redisplacement, the primary outcome, was defined as a change in alignment from a posterior or anatomical position postoperatively to an anterior position on any subsequent follow-up radiograph. Independent risk factors were identified by logistic regression. Results: Among the 543 hips reduced posteriorly (n = 204) or anatomically (n = 339), anterior redisplacement occurred in 73 (13.4%). The incidence of anterior redisplacement was significantly higher following anatomical compared to posterior reduction (19.5% vs. 3.4%; p < 0.001), and also higher in fractures that were anteriorly aligned preoperatively (18.0%) compared to anatomical (8.5%; p < 0.01) and posterior (6.2%; p < 0.01) alignment. Multivariate analysis revealed two independent predictors: preoperative anterior alignment (odds ratio [OR] 1.87, 95% confidence interval [CI] 1.24–2.81; p = 0.003) and postoperative anatomical (vs. posterior) reduction (OR 6.49, 95% CI 2.92–14.44; p < 0.001). Age, sex, Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association classification, Evans–Jensen classification, nail length, and canal-filling ratio were not associated with redisplacement. No lag‑screw cutout occurred during the follow-up. Conclusions: Anterior redisplacement occurred in one of seven geriatric trochanteric fractures despite apparently satisfactory fixation. An anatomical sagittal reduction—traditionally considered “ideal”—increases the risk more than sixfold, whereas a deliberate posterior-buttress is protective. Unlike patient-related risk factors, sagittal reduction is under the surgeon’s control. The study findings provide evidence that choosing a slight posterior bias can significantly improve stability. Background/Objectives: Vitamin D testing has increased significantly in developed countries in recent decades. We aimed to describe trends in vitamin D testing rates and factors associated with testing and vitamin D deficiency in Queensland, Australia (2011–2019). Methods: We used data from the QSkin Sun and Health Study (n = 40,417), a prospective population-based cohort study with linkage to the Medicare Benefits Schedule, Pharmaceutical Benefits Scheme, and pathology laboratories. Main outcomes included age-standardized incidence rate of vitamin D testing; having ≥1 vitamin D test during follow-up; vitamin D deficiency (25-hydroxyvitamin D concentration <50 nmol/L) in the first vitamin D test; and repeat vitamin D tests. Results: The age-standardized incidence rate of testing increased by 2% per quarter during follow-up. Of the 35,250 participants analyzed for associations with testing (median age of 57 years, 52% female), 45% had ≥1 vitamin D test. Among those tested, 56% had no apparent clinical indication for their initial vitamin D test, 21% were vitamin D deficient in their initial test, and 58% had a repeat test. Repeat testing occurred in 56% who were not deficient in their prior test, while only two-thirds of those deficient received a follow-up assessment. Participants who visited a general practitioner ≥2 times in the year prior to follow-up were 60% more likely to have ≥1 vitamin D test compared with those with no visit, but general practitioner (GP) visits were not associated with risk of vitamin D deficiency. Conclusions: These results suggest that initiatives are needed to help clinicians target vitamin D testing in alignment with clinical guidelines. Full article

Developing ultra-high-temperature geothermal resources is challenging, as traditional drilling fluids, including foam systems, lack thermal stability above 160 °C. To address this key technical bottleneck, this study delves into the screening principles for high-temperature-resistant foaming agents and foam stabilizers. Through high-temperature aging experiments (foaming performance evaluated up to 240 °C and rheological/filtration properties evaluated after aging at 200 °C), specific additives were selected that still exhibit good foaming and foam-stabilizing performance under high-temperature and high-salinity conditions. Building on this, the foam drilling fluid system formulation was optimized using an orthogonal experimental design. The optimized formulations were systematically evaluated for their density, volume, rheological properties (apparent viscosity and plastic viscosity), and filtration properties (API fluid loss and HTHP fluid loss) before and after high-temperature aging (at 200 °C). The research results indicate that specific formulation systems exhibit excellent high-temperature stability and particularly outstanding performance in filtration control, with the selected foaming agent FP-1 maintaining good performance up to 240 °C and optimized formulations demonstrating excellent HTHP fluid loss control at 200 °C. This provides an important theoretical basis and technical support for further research and field application of foam drilling fluid systems for deep high-temperature geothermal energy development. In marine engineering applications, substituting conventional crushed stone coarse aggregates with coral aggregates offers dual advantages: reduced terrestrial quarrying operations and minimized construction material transportation costs. However, the inherent characteristics of coral aggregates—low bulk density, high porosity, and elevated water absorption capacity—adversely influence concrete workability and mechanical performance. To address these limitations, this investigation employed microbial-induced carbonate precipitation (MICP) for aggregate modification. The experimental design systematically evaluated the impacts of substrate concentration (1 mol/L) and mineralization period (14 days) on three critical parameters, mass gain percentage, water absorption reduction, and apparent density enhancement, across distinct particle size fractions (4.75–9.5 mm, 9.5–20 mm) and density classifications. Subsequent application trials assessed the performance of MICP-treated aggregates in marine concrete formulations. Results indicated that under a substrate concentration of 1 mol/L and mineralization period of 14 days, lightweight coral aggregates and coral aggregates within the 4.75–9.5 mm size fraction exhibited favorable modification effects. Specifically, their mass gain rates reached 11.75% and 11.22%, respectively, while their water absorption rates decreased by 32.22% and 34.75%, respectively. Apparent density increased from initial values of 1764 kg/m3 and 1930 kg/m3 to 2050 kg/m3 and 2207 kg/m3. Concrete mixtures incorporating modified aggregates exhibited enhanced workability and strength improvement at all curing ages. The 28-day compressive strengths reached 62.1 MPa (11.69% increment), 46.2 MPa (6.94% increment), and 60.1 MPa (14.91% increment) for the 4.75–9.5 mm, 9.5–20 mm, and continuous grading groups, respectively, compared to untreated counterparts. Objective: The aim of this study was to compare micronutrient intake and inadequacy estimates using household consumption and expenditure survey (HCES) and quantitative 24-h recall (24HR) data among women of reproductive age (WRA) in Kasungu district, Malawi. Methods: We conducted a secondary data analysis utilizing HCES dietary data from a subsample of households in rural areas of Kasungu district, which were sourced from the 2019/20 Malawi Fifth Integrated Household Survey (n = 183); and 24HR data were obtained from WRA in a community-based Addressing Hidden Hunger with Agronomy (AHHA) trial in the same district (n = 177). Micronutrient intakes and inadequacy were estimated under two alternative scenarios of large-scale food fortification (LSFF). We standardized apparent nutrient intakes from the HCES data using the adult female equivalent metric. Results: Estimated prevalence of micronutrient inadequacy fell within 20 percentage points between HCES and 24HR for iron (Fe), zinc (Zn), vitamins B2 and B9 under both no fortification and fortification scenarios. There were some discrepancies for the remaining B vitamins, being consistently large for vitamin B3. Conclusions: In the absence of 24HR data, HCES data can be used to make inferences about some micronutrient intakes and inadequacies among rural WRA in Malawi and to inform decisions regarding LSFF, including vehicle selection and coverage. However, additional efforts are needed to improve HCES for dietary nutrient surveillance given existing limitations. Full article

The retinal pigment epithelium (RPE), a monolayer of pigmented cells, is critical for visual function through its interaction with the neural retina. In healthy eyes, RPE cells exhibit a uniform hexagonal arrangement, but under stress or disease, such as age-related macular degeneration (AMD), dysmorphic traits like cell enlargement and apparent multinucleation emerge. Multinucleation has been hypothesized to result from cellular fusion, a compensatory mechanism to maintain cell-to-cell contact and barrier function, as well as conserve resources in unhealthy tissue. However, traditional two-dimensional (2D) imaging using apical border markers alone may misrepresent multinucleation due to the lack of lateral markers. We present high-resolution confocal images enabling three-dimensional (3D) visualization of apical (ZO-1) and lateral (α-catenin) markers alongside nuclei. In two RPE damage models, we find that seemingly multinucleated cells are often single cells with displaced neighboring nuclei and lateral membranes. This emphasizes the need for 3D analyses to avoid misidentifying multinucleation and underlying fusion mechanisms. Lastly, images from the NaIO3 oxidative damage model reveal variability in RPE damage, with elongated, dysmorphic cells showing increased ZsGreen reporter protein expression driven by EMT-linked CAG promoter activity, while more regular RPE cells displayed somewhat reduced green signal more typical of epithelial phenotypes. Senescent cells secrete pro-inflammatory cytokines, collectively referred to as the senescence-associated secretory phenotype (SASP). Certain pro-inflammatory SASP factors are known to inhibit the differentiation of bone-forming osteoblast while promoting the differentiation of bone-resorbing osteoclasts, thereby causing osteoporosis. In this study, we screened cabozantinib, a tyrosine kinase inhibitor used to treat medullary thyroid cancer, for its ability to reduce doxorubicin-induced cellular senescence in both osteoblast and osteoclast progenitors. This non-cytotoxic agent suppressed the secretion of SASP factors (e.g., TNFα, IL1α, IL1β, IL6, and CCL2) from senescent osteoblast and osteoclast progenitors, resulting in enhanced osteoblast differentiation and reduced osteoclast differentiation. Furthermore, intraperitoneal administration of cabozantinib to age-related estrogen-deficient mice subjected to ovariectomy prevented bone loss without apparent side effects, increasing osteoblast numbers and reducing osteoclast numbers along the surface of the trabecular bone. In summary, our findings suggest that anti-aging cabozantinib has potential as a preventive anti-osteoporotic agent by promoting osteogenesis and inhibiting osteoclastogenesis through the repression of SASP. Objective: The primary objective was to identify whether there were differences in performance for the individual subcomponents of the instrumented timed “Up and Go” (iTUG) between adults with traumatic brain injury (TBI) and healthy controls. Methods: Fifteen adults with moderate-to-severe TBI and fifteen age- and sex-matched controls completed two separate trials of the iTUG. Paired t-tests or Wilcoxon signed rank tests were used to determine the differences between groups. Results: Adults with moderate-to-severe TBI took more time to complete the iTUG (14.50 ± 2.36 s vs. 9.85 ± 1.71 s; p-value = 0.0002), had slower chest flexion angular velocities (63.52 ± 23.25 s vs. 88.19 ± 29.20 s; p-value = 0.0486) and vertical acceleration (2.22 [1.23–2.74] s vs. 3.89 [3.36–5.02] s; p-value = 0.0005) during the sit-to-stand movements, and had slower angular velocities during the turns (p-value < 0.05 for both mean and peak turn angular velocities) compared with the controls. Conclusions: Adults with moderate-to-severe TBI completed the iTUG more slowly than healthy controls. Significant differences were noted in the sit-to-stand and turn subcomponents for adults with moderate-to-severe TBI compared with healthy controls, which would not be apparent from evaluating the total time taken alone. Ecdysterone controls moulting and reproduction in insects, crustaceans, and helminths. It is also produced by many plants, probably as an insect deterrent. The steroid is not made by vertebrates but has anabolic effects in mammals and could be useful for treating sarcopenia in aged horses. However, ecdysterone is banned in horseracing and equestrian sports, and with no limit of reporting, the risk of unintended exposure to this naturally occurring prohibited substance is a concern. To explore this risk, pasture plants and hay samples were analysed for ecdysterone content, as well as samples of blood, faeces, and intestinal mucosa from horses (euthanized for non-research purposes) with varying degrees of endo-parasite infestation. The variability in serum ecdysterone concentrations between different horses after administering a fixed dose was also examined. Ecdysterone was detected in 24 hay samples (0.09 to 3.74 µg/g) and several weeds, with particularly high concentrations in Chenopodium album (244 µg/g) and Solanum nigrum (233 µg/g). There was a positive correlation between faecal ecdysterone and faecal egg counts, but no effect of anthelmintic treatment and no relation to the number of encysted cyathostome larvae in the large intestine mucosa. Certain horses maintained an unusually high serum ecdysterone concentration over several weeks and/or displayed an abnormally large response to oral ecdysterone administration. Thus, the risk of environmental exposure to ecdysterone is apparent, and several factors must be considered when determining an appropriate dosage for clinical studies or setting a reporting threshold for equine sports. The objective of this study was to determine the effects of two levels of a precision biotic (PB) within a reduced CP diet for performance, digestibility, and blood biomarkers in Dekalb White laying hens from 24 to 42 weeks of age. A total of four diets were fed: (1) an industry-standard positive control (PC) diet with a formulated CP level of 17.5%; (2) a negative control (NC) diet with a formulated CP level of 15.5%; (3) NC with 0.5 kg/ton PB (NC+PB1); (4) NC with 0.7 kg/ton PB (NC+PB2). The NC diet decreased hen–day egg production compared to the PC diet, and supplementation with both levels of PB led to intermediate levels of production. There was a similar trend for the feed conversion ratio (FCR), measured in kilograms of feed per dozen eggs. Feed cost was lower in the reduced CP diets compared to the PC. Apparent total tract digestibility was decreased with PB supplementation without differences in apparent ileal digestibility. The increased plasma uric acid levels in the NC+PB2 group suggest a shift in protein metabolism toward higher efficiency in ammonia detoxification. While more research is needed on PB, this research suggests that supplementation with PB in laying hens could potentially improve performance parameters. Full article

Our previous study has shown that supplementation of 0.50% benzoic acid (BA) increased growth performance, promoted rumen fermentation, and improved the composition and function of rumen microbiota. This research was designed to conduct a deeper exploration of the impacts of dietary supplementation with BA on the apparent digestibility of nutrients and the composition of rectal microbiota in weaned Holstein dairy calves. Sixteen Holstein heifer calves with similar body weights (91.2 ± 0.7 kg) were selected and randomly allocated into two groups, each comprising eight calves. Calves in the control group (CON group) were fed with a basal diet, while those in the benzoic acid group (BA group) were fed with the basal diet supplemented with 0.50% benzoic acid (on a dry matter basis). The experimental period started at 60 days of age and ended at 102 days of age, lasting for a total of 42 days. The calves were weaned at 60 days of age, with a transition period of 7 days. Feed samples were collected every two weeks, fecal samples were collected from 99 to 101 days of age, and blood samples were collected at 102 days of age. The results showed that supplementation with BA did not influence the digestibility of dry matter, crude protein, ether extract, neutral detergent fiber, acid detergent fiber, calcium, and phosphorus between the two groups. Compared with the CON group, BA supplementation tended to decrease the total cholesterol (TC) in the serum of the calves (p = 0.067). Supplementation with BA increased the relative abundances of the two beneficial bacteria, Bifidobacterium and Bifidobacterium pseudolongum (p < 0.05, LDA > 2), but decreased that of the harmful bacterium, Clostridium sensu stricto 1, in the rectum of dairy calves. The microbial functional prediction revealed that the fecal microbial metabolism involved in primary bile acid biosynthesis was higher in the calves from the BA group. In conclusion, the present study demonstrated that adding 0.50% BA to the diet did not influence the apparent nutrient digestibility, but improved rectal microbiota health, which finally promoted the growth performance in weaned Holstein dairy calves. Equine piroplasmosis (EP) is an endemic parasitic disease in southern European countries, such as Spain. Andalusia, the most southwestern region of Europe, is the community with the highest number of registered horses and farms in Spain and one of the main international exporters of Andalusian (Spanish Purebred) horses worldwide. Considering the current expansion of this disease and the possible effect of climate change on its prevalence, studying the EP prevalence in this region is compelling. Molecular (PCR) and serological methods (cELISA and IFAT) were used to study the true and apparent prevalences during a period of three consecutive years, evaluating the effects of age, sex, season, year of testing, and province. Results showed different EP prevalences between western and eastern provinces, as well as among seasons. Moreover, a positive association was observed between age and T. equi seropositivity, without any effect of sex. Our findings demonstrate that Andalusia is an EP endemic region, but prevalences were lower compared to central and northern Spanish regions. Moreover, EP prevalence has not increased in Andalusia in recent years despite climate changes. Full article

An Evaluation of the Rheological and Filtration Properties of Cow Bone Powder and Calcium Carbonate as Fluid-Loss Additives in Drilling Operations

Some additives currently used to enhance drilling mud’s rheological qualities have a substantial economic impact on society. Carboxymethyl cellulose (CMC) and calcium carbonate (CaCO3) are currently imported. Food crops have influences on food security; hence, this research explored the potential of utilizing cow bone powder (CBP), a bio-waste product and a renewable resource, as an environmentally friendly fluid-loss additive for drilling applications, in comparison with CaCO3. Both samples (CBP and CaCO3) were evaluated to determine the most efficient powder sizes (coarse, medium, and fine powder), concentrations (5–15 g), and aging conditions (before or after aging) that would offer improved rheological and fluid-loss control. The results obtained showed that CBP had a significant impact on mud rheology when compared to CaCO3. Decreasing the particle size (coarse to fine particles) and increasing the concentration from 5 to 15 g positively impacted mud rheology. Among all the conditions analyzed, fine-particle CBP with a 15 g concentration produced the best characteristics, including in the apparent viscosity (37 cP), plastic viscosity (29 cP), and yield point (25.5 lb/100 ft2), and a gel strength of 16 lb/100 ft2 (10 s) and 28 lb/100 ft2 (10 min). The filtration control ability of CaCO3 was observed to be better than that of the coarse and medium CBP particle sizes; however, fine-particle-size CBP demonstrated a 6.1% and 34.6% fluid-loss reduction at 10 g and 15 g concentrations when compared to respective amounts of CaCO3. The thermal behavior of the Mud Samples demonstrated that it positively impacted rheology before aging. In contrast, after aging, it exhibited a negative effect where samples grew more viscous and exceeded the API standard range for mud properties. Therefore, CBP’s excellent rheological and fluid-loss control ability makes it a potential, sustainable, and economically viable alternative to conventional materials. This superior performance enhances the thinning properties of drilling muds in stationary and circulating conditions. Full article

Background/Objectives: The COVID-19 pandemic has caused sickness and death among many health care workers. However, the apparent resistance of health care workers to SARS-CoV-2 infection despite their high-risk work environment remains unclear. To investigate if inflammation and circadian disruption contribute to resistance or diminished susceptibility to the SARS-CoV-2 virus, we retrospectively evaluated a cohort of volunteer hospital nurses (VHNs). Methods: A total of 246 apparently healthy VHNs (mean age 37.4 ± 5.9 years) who had received the BNT162b2 mRNA vaccine were asked to report their sleep quality, according to the Pittsburgh Sleep Quality Index, and number of SARS-CoV-2 infections during the observational study period (from the end of December 2020 to April 2025). The expression of inflammation-associated mediators and circadian transcription factors in peripheral blood mononuclear cells, as well as sleep quality, were examined. Results: Our findings revealed no anthropometric, biochemical, or inflammation-associated parameters but demonstrated significantly greater levels of NFE2L2, also known as nuclear factor erythroid-derived 2-like 2 (NFR2), gene expression in peripheral blood mononuclear cells among VHNs who had never been infected with SARS-CoV-2 (n = 97) than in VHNs with only one (n = 119) or with two or more (n = 35) prior SARS-CoV-2 infections (p < 0.01). This result was confirmed through one-to-one propensity score matching (p < 0.01). Moreover, NRF2 gene expression was not associated with the number of COVID-19 vaccinations (p = 0.598). Finally, NRF2 gene expression was higher among participants who reported better sleep quality (p < 0.01). Conclusions: Our findings suggest possible interactions among NRF2 gene expression, protection against SARS-CoV-2 infection, and the modulation of COVID-19 vaccination efficacy. Full article

As research on national cultural parks advances, the significance of conducting multi-dimensional perception evaluations of their cultural ecosystem services (CESs) becomes increasingly apparent. This study examines the eight dimensions of CESs within the Grand Canal National Cultural Park from the perspective of soundscape preference. Using Tongzhou Grand Canal Forest Park as a case study, five categories of soundscapes comprising 19 sound sources were identified through the analysis of online textual data. This study then collected public preferences and perceptions of these five soundscapes via on-site questionnaires and analyzed the data using SPSS26 for correlation and IPA analyses. The results indicate that the overall evaluation of the park’s CESs is positive. There is a significant mutual influence between soundscape preference and CES perception. Specifically, the preference for natural soundscape significantly impacts the evaluation of each CES dimension, while satisfaction with leisure and entertainment is positively correlated with preferences for all types of soundscapes. Additionally, there are notable differences in soundscape preference among different age groups. These findings not only enhance our understanding of soundscape planning in national cultural parks but also provide valuable guidance for their management and design. Full article
