Ponderosa pine (Pinus ponderosa Lawson & C. Lawson) forests in the western United States have experienced reduced fire frequency since Euro-American settlement, usually because of successful fire suppression policies and even without such human impacts at remote sites in the Great Basin and Mojave Deserts. In an effort to improve our understanding of long-term environmental dynamics in sky-island ecosystems, we developed tree-ring chronologies from ponderosa pines located in the Sheep Mountain Range of southern Nevada, inside the Desert National Wildlife Refuge (DNWR). After comparing those dendrochronological records with other ones available for the south-central Great Basin, we analyzed their climatic response using station-recorded monthly precipitation and air temperature data from 1950 to 2024. The main climatic signal was December through May total precipitation, which was then reconstructed at annual resolution over the past five centuries, from 1490 to 2011 CE. The mean episode duration was 2.6 years, and the maximum drought duration was 11 years (1924–1934; the “Dust Bowl” period), while the longest episode, 19 years (1905–1923), is known throughout North America as the “early 1900s pluvial”. By quantifying multi-annual dry and wet episodes, the period since DNWR establishment was placed in a long-term dendroclimatic framework, allowing us to estimate the potential drought resilience of its unique, tree-dominated environments. Anthracyclines remain a cornerstone of cancer therapy but are associated with a significant risk of cardiotoxicity, which can lead to overt heart failure. The risk is modulated by cumulative dose, pre-existing cardiovascular disease, and patient-specific factors. As cancer survival improves, the long-term cardiovascular consequences of anthracycline exposure have become a growing concern, underscoring the need for effective preventive strategies. This narrative review examines lifestyle and pharmacological interventions aimed at mitigating anthracycline-induced cardiotoxicity. Evidence suggests that structured exercise programs and antioxidant-rich diets may enhance cardiovascular resilience, while beta-blockers, renin-angiotensin system inhibitors, and dexrazoxane remain central pharmacological options. Emerging therapies, including sodium-glucose co-transporter 2 inhibitors and sacubitril/valsartan, show promise but require further investigation. A comprehensive approach that integrates lifestyle modifications with pharmacological strategies within a multidisciplinary cardio-oncology framework may provide optimal protection, improving long-term cardiovascular outcomes in cancer patients receiving anthracyclines. Background: Implant-based breast reconstruction (IBBR) following conservative mastectomy is the most common approach for women undergoing breast cancer surgery. The aim of this study was to compare the oncological outcomes of conservative mastectomy combined with prepectoral IBBR to the subpectoral technique. Methods: The clinical and demographic data of consecutive breast cancer patients who underwent conservative mastectomy with either prepectoral or subpectoral IBBR between January 2018 and December 2023 were retrospectively analyzed. The primary outcome was the impact of conservative mastectomy with prepectoral IBBR on local recurrence-free survival (LRFS). Secondary outcomes included distant disease-free survival (DDFS) and overall survival (OS). Results: A total of 842 women (with a median age of 46 years and a range of 20–79 years) were included in the study. Of these, 648 patients (77.0%) underwent prepectoral IBBR, while 194 (23.0%) received subpectoral IBBR. The median follow-up was 32 months (3–74). Locoregional relapse occurred in 19 patients (2.9%) in the prepectoral group and 14 (7.2%) in the subpectoral group. Distant metastases were observed in 21 (3.2%) patients in the prepectoral group and 11 (5.7%) in the subpectoral group. Deaths were reported in eight patients (1.2%) in the prepectoral group and five (2.6%) in the subpectoral group. There were no statistically significant differences between the two groups in terms of the LRFS, DDFS, and OS (p = 0.676; p = 0.994; p = 0.940, respectively). Conclusions: Our study indicates that conservative mastectomy combined with prepectoral IBBR produces similar results to those of the subpectoral approach, with no significant differences in LRFS, DDFS, and OS. Introduction: Bronchiectasis, characterized by airway dilation, mucus hypersecretion, and recurrent exacerbations, is increasingly recognized in children and adolescents. Recent guidelines from the European Respiratory Society (ERS) and Thoracic Society of Australia and New Zealand (TSANZ) emphasize early diagnosis and optimized management. This review explores therapeutic strategies for pediatric bronchiectasis. Materials and methods: Our review involved a comprehensive search of English-language literature in the PubMed and EMBASE databases until December 2023, focusing on observational studies, interventions, reviews, and guidelines in pediatric bronchiectasis. Results: Management strategies encompass airway clearance techniques, mucoactive agents, pulmonary rehabilitation, bronchodilators and inhaled corticosteroids tailored to individual needs and age-appropriate techniques. Antibiotics play key roles in preventing exacerbations, eradicating pathogens, and managing acute exacerbations, which are guided by culture sensitivities and symptoms. Long-term antibiotic prophylaxis, particularly macrolides, aims to reduce exacerbations, although concerns about antibiotic resistance persist. Vaccinations, including pneumococcal and influenza vaccines, are crucial for preventing infections and complications. Surgery and lung transplantation are reserved to severe, refractory cases after failure of medical therapies. Conclusions: The optimal management of pediatric bronchiectasis requires a multidisciplinary approach, including physiotherapy, pharmacotherapy, and vaccinations, tailored to individual needs and guided by evidence-based guidelines. Further research is needed to refine diagnostic and therapeutic strategies and improve outcomes for affected children and adolescents. Full article

Background: Breast cancer in young women aged < 40 years is rare and often aggressive with less favorable survival rates. The lack of systematic screening, later stage at diagnosis, and a more aggressive disease biology may all contribute to their poor prognosis. Data on the best management remain conflicting, especially those regarding surgical management, either breast-conserving or mastectomy. To our knowledge, there are limited studies surrounding the treatment of young women with early breast cancer, and this analysis evaluated the oncological outcomes for those patients who underwent surgery upfront. Methods: We conducted a retrospective study including 130 young women with early breast cancer from a total of 373 consecutive patients treated with upfront surgery between January 2016 and December 2021 at our institution. Local recurrence-free survival (LR-FS), distant metastasis-free survival (DMFS), disease-free survival (DFS), and overall survival (OS) were evaluated. Results: The median follow-up was 61.1 months (range, 25–95). A total of 92 (70.8%) patients underwent breast-conserving surgery, while 38 (29.2%) patients underwent conservative mastectomy with immediate implant breast reconstruction. In total, 8 of 130 patients (6.2%) developed a local recurrence in the treated breast, an7 (5.4%) patients presented distant metastasis. Overall, two (1.6%) patients died due to breast cancer recurrence. Conclusions: The results of our study interestingly support breast-conserving surgery in young patients with early-stage breast cancer. While appropriate breast-conserving surgery can achieve favorable oncological outcomes and can always be considered a valid alternative to conservative mastectomy in upfront surgery, a younger age at diagnosis should never be used alone to choose the type of surgery. Correctly estimating the maximum lifespan of plant species is a necessary component of demographic and life-history studies, which, in turn, are needed for understanding climatic impacts. Arboreal monocotyledons, which can grow to >30 m in height and >5 m in trunk perimeter, are difficult to age because they do not undergo seasonal dormancy; hence, their longevity has been estimated using various size-related methods. In this study, we tested radiocarbon (14C) dating with Accelerator Mass Spectrometry (AMS) as an additional tool for determining the age of two iconic monocotyledons: the Canary Island palm and the dragon tree. A total of 25 samples were collected from the basal stem of four palms and five dragon trees on Gran Canaria and Tenerife and then processed using the most advanced 14C-AMS analysis available. Calibration curves provided by the “IntCal group” were used to determine the oldest possible age of each sample, and 16 of them were found to be “modern”, i.e., formed after the 1950s. Nine samples that were either collected from exterior, but lignified, palm tissues or from interior, and lignified, dragon tree tissues suggested ages > 300 years. Given the constant improvement of 14C-AMS tools, they can contribute to the further refinement of existing scientific knowledge on Macaronesian charismatic megaflora. Full article

The development of stem heartwood and the factors that control it play an important role in tree physiology, thereby impacting demographic and ecological processes of woody species. We investigated the relationships of stem heartwood with site- and tree-level variables in Pinus canariensis plantations. A total of 30 plots were sampled in the island of Gran Canaria, Spain, over a large elevation range (995–1875 m) and on terrain with different slopes (4%–70%) and exposures. The 15 pines closest to each plot center were measured and cored to quantify growth rates and the size of heartwood, also known as “tea”. We used generalized linear mixed models (GLMMs) to account for both fixed and random effects while evaluating the best predictors of heartwood presence. Stem diameter was the variable most correlated with heartwood radius, and allowing for a random slope and intercept of this relationship accounted for spatially related variability. Furthermore, the GLMM model became more effective when the relationship between stem diameter and heartwood was modeled using the presence/absence of the “tea” rather than its measured size. Other site- and tree-level variables either were not statistically significant or improved the model relatively little. Because stem heartwood affects both wood quality and the amount of carbon that trees can store, our findings have implications for forest management and carbon-conscious policies. Full article

The Calabrian Alpine newt (Ichthyosaura alpestris inexpectata) is a glacial relict with small and extremely localised populations in the Catena Costiera (Calabria, Southern Italy) and is considered to be “Endangered” by the Italian IUCN assessment. Climate-induced habitat loss and recent fish introductions in three lakes of the Special Area of Conservation (SAC) Laghi di Fagnano threaten the subspecies’ survival in the core of its restricted range. Considering these challenges, understanding the distribution and abundance of this newt is crucial. We surveyed the spatially clustered wetlands in the SAC and neighbouring areas. First, we provide the updated distribution of this subspecies, highlighting fish-invaded and fishless sites historically known to host Calabrian Alpine newt populations and two new breeding sites that have been recently colonised. Then, we provide a rough estimate of the abundance, body size and body condition of breeding adults and habitat characteristics in fish-invaded and fishless ponds. We did not detect Calabrian Alpine newts at two historically known sites now invaded by fish. Our results indicate a reduction in occupied sites and small-size populations. These observations highlight the need for future strategies, such as fish removal, the creation of alternative breeding habitats and captive breeding, to preserve this endemic taxon. Full article

In the Mediterranean basin, agriculture and other forms of human land use have shaped the environment since ancient times. Intensive and extensive agricultural systems managed with a few cultured plant populations of improved varieties are a widespread reality in many Mediterranean countries. Despite this, historical cultural landscapes still exist in interior and less intensively managed rural areas. There, ancient fruit tree varieties have survived modern cultivation systems, preserving a unique genetic heritage. In this study, we mapped and characterized 106 living fruit trees of ancient varieties in the Basilicata region of southern Italy. Tree ages were determined through tree ring measurements and radiocarbon analyses. We uncovered some of the oldest scientifically dated fruit trees in the world. The oldest fruit species were olive (max age 680 ± 57 years), mulberry (647 ± 66 years), chestnut (636 ± 66 years), and pear (467 ± 89 years). These patriarchs hold a unique genetic resource; their preservation and genetic maintenance through agamic propagation are now promoted by the Lucan Agency for the Development and Innovation in Agriculture (ALSIA). Each tree also represents a hub for biodiversity conservation in agrarian ecosystems: their large architecture and time persistence guarantee ecological niches and micro-habitats suitable for flora and fauna species of conservation significance. The year-to-year variability of precipitation has significant consequences for water management and forest health. “Whiplash” describes an extreme mode of this variability in which hydroclimate switches abruptly between wet and dry conditions. In this study, a pool of total-ring-width indices from five conifer species (Abies magnifica, Juniperus grandis, Pinus ponderosa, Pinus jeffreyi, and Tsuga mertensiana) in the Sierra Nevada is used to develop reconstructions of water-year precipitation using stepwise linear regression on lagged chronologies, and the reconstructions are analyzed for their ability to track whiplash events. A nonparametric approach is introduced to statistically classify positive and negative events, and the success of matching observed events with the reconstructions is evaluated using a hypergeometric test. Results suggest that reconstructions can effectively track whiplash events, but that tracking ability differs among species and sites. Although negative (dry-to-wet) events (1921–1989) are generally tracked more consistently than positive events, Tsuga stands out for strong tracking of positive events. Tracking ability shows no clear relationship to variance explained by reconstructions, suggesting that efforts to extend whiplash records with tree-ring data should consider optimizing reconstruction models for the whiplash signal. Background: Conservative mastectomy with immediate prosthetic breast reconstruction (IPBR) is an oncologically accepted technique that offers improved esthetic results and patient quality of life. Traditionally, implants have been placed in a submuscular (SM) plane beneath the pectoralis major muscle (PMM). Recently, prepectoral (PP) placement of the prosthesis has been increasingly used in order to avoid morbidities related to manipulation of the PMM. The aim of this study was to compare outcomes of SM vs. PP IPBR after conservative mastectomy in patients with histologically proven breast cancer treated with neoadjuvant chemotherapy (NAC). Methods: In this retrospective observational study, we analyzed two cohorts of patients that underwent mastectomy with IPBR after NAC in our institution from January 2018 to December 2021. Conservative mastectomy was performed in 146 of the 400 patients that underwent NAC during the study period. Patients were divided into two groups based on the positioning of implants: 56 SM versus 90 PP. Results: The two cohorts were similar for age (mean age 42 and 44 years in the SM and PP group respectively) and follow-up (33 and 20 months, respectively). Mean operative time was 56 min shorter in the PP group (300 and 244 min in the SM and PP group). No significant differences were observed in overall major complication rates. Implant loss was observed in 1.78% of patients (1/56) in the SM group and 1.11% of patients (1/90) in PP group. No differences were observed between the two groups in local or regional recurrence. Conclusions: Our preliminary experience, which represents one of the largest series of patients undergoing PP-IPBR after NAC at a single institution documented in the literature, seems to confirm that PP-IPBR after NAC is a safe, reliable and effective alternative to traditional SM-IPBR with excellent esthetic and oncological outcomes; it is easy to perform, reduces operative time and minimizes complications related to manipulation of PPM. However, this promising results need to be confirmed in prospective trials with longer follow-up. Full article

This study reports a characterization of the nutritional quality of several vegetables belonging to Brassica genus and other species cultivated in the central Italy. The aim of this trial is to investigate the antioxidant capacity and phytochemical content of several vegetable products during two consecutive years. The sensorial quality is investigated with the measuring of soluble solid content (SSC), titratable acidity (TA) and pH; the nutritional quality is investigated by the measurement of the total antioxidant capacity (TAC), the total phenols content (TPH), the total anthocyanins content (ACY), and the vitamin C content. The results confirm the highest antioxidant capacity of Brassica genus, in particular, the red curly kale (13.68 and 11.97 mM Trolox/kg fw in the two locations tested); among other vegetables analyzed, the most interesting are chicory and borage (10.3 and 11.94 mM Trolox/kg fw in the first year of cultivation in Valdaso, respectively). A high intake of these vegetables may bring a lot of health benefits linked to their antioxidative capacity and the vitamin C content. Full article

Brassicaceae plants are rich with antioxidant compounds that play a key role for human health. This study wants to characterize two Italian broccoli cultivars (Roya and Santee) and black cabbage, evaluating the variation of antioxidants in different portion and at different developmental stage of the plants: for broccoli, heads and stems were sampled, while for black cabbage, leaves and seeds were analyzed. Roja cultivar was also analyzed at the first and second harvest to evaluate the variation of phytochemical compounds over time. Nutritional and sensorial qualities were investigated. Black cabbage seeds showed higher value of total antioxidants, total phenols, and total anthocyanins than leaves. Similarly, phenolics and anthocyanins content in head was higher than in stem in broccoli. In Roja cultivar, the harvest date seemed to influence the antioxidant capacity and the phytochemical compounds content, with broccoli sampled in the second harvest showing better results for all the nutritional parameters. These local vegetables represent a significant source of antioxidants and may contribute to health benefits of the consumers. Full article

Early T-cell precursor (ETP) is an aggressive form of acute lymphoblastic leukemia (ALL), associated with high risk of relapse. This leukemia subtype shows a higher prevalence of mutations, typically associated with acute myeloid leukemia (AML), including RAS and FLT3 mutations. FLT3-ITD was identified in 35% cases of adult ETP-ALL, but data in the pediatric counterpart are lacking. ETPs frequently lack immunoglobulin (IG) and T-cell receptor (TR) gene rearrangements, used for minimal residual disease (MRD) monitoring. Among 718 T-ALL enrolled in Italy into AIEOP-BFM-ALL2000, AIEOP-ALLR2006, and AIEOP-BFM-ALL2009 consecutive protocols, 86 patients (12%) were identified as ETP and 77 out of 86 children were studied for the presence of FLT3-ITD. A total of 10 out of 77 (13%) ETP cases were FLT3-ITD positive. IG/TR MRD monitoring was feasible only in four cases. FLT3-ITD MRD monitoring was performed using real-time PCR in all FLT3-ITD positive ETP cases. A comparison between IG/TR and FLT3-ITD resulted in comparable findings. Our study demonstrated that the FLT3-ITD prevalence in children was lower (13%) than that reported in adult ETP-ALL. FLT3-ITD can be used as a marker for sensitive molecular MRD monitoring in ETP-ALL when IG/TR markers are not available, potentially selecting those patients who should spare allogeneic hematopoietic stem cell transplantation (HSCT). Finally, the FLT3 pathway is a robust druggable target in this aggressive form of leukemia. Full article

Recently, the consumption of healthy foods has been related to the prevention of cardiovascular, degenerative diseases and different forms of cancers, underlying the importance of the diet for the consumer’s health. Fruits and vegetables contain phytochemicals that act as protective factors for the human body, through different mechanisms of action. Among vegetables, Brassica received a lot of attention in the last years for the phytochemical compounds content and antioxidant capacity that confer nutraceutical value to the product. The amount of healthy bioactive compounds present in the Brassica defines the nutritional quality. These molecules could belong to the class of antioxidant compounds (e.g., phenols, vitamin C, etc.), or to non-antioxidant compounds (e.g., minerals, glucosinolates, etc.). The amount of these compounds in Brassica vegetables could be influenced by several factors, depending on the genotypes, the environmental conditions and the cultivation techniques adopted. The aim of this study is to highlight the main phytochemical compounds present in brassicas used as a food vegetable that confer nutritional and sensorial quality to the final product, and to investigate the main factors that affect the phytochemical concentration and the overall quality of Brassica vegetables.
