Special Issue Information

Dear Colleagues,

Intermediate care is a growing concept in health care. In recent years, there has been a continuous increase in the need of institutions devoted to this kind of care as well as the complexity of the patients attended by them. Increased life expectancy has implied that people are living with chronic diseases for a longer time and need close medical attention and polypharmacy. Therefore, in this setting, the work of the pharmacist is of paramount importance and has specific characteristics that differ in some issues to the one of acute health care or that of nursing homes. Pharmacists have to face different challenges in their job, such as the complexity of patients (multiple comorbidities, polypharmacy, etc.), high budget requirements, low staff, and an increase in the tasks to perform, among others.

We invite you to share your experience, approaches, and successes in your everyday work and projects in pharmacy practice in intermediate care institutions. We hope that this Special Issue can bring experiences together and contribute to expanding and improving this practice and therefore health outcomes in patients.
