Dear Colleagues,
Our understanding of heart failure (HF) is evolving from earlier concepts of simplified hemodynamic parameters and established neurohormonal pathomechanisms to metabolic disturbances of fatty acid metabolism, mitochondrial dysfunction and new paracrine and endocrine pathways. Our understanding of the etiology of HF is also shifting to new horizons, such as microcirculation and metabolism. Many achievements of translational science on HF originate from biomarker research. Such achievements have produced novel HF therapies and can lead to a better understanding of intricate pathways involved in systolic and diastolic cardiac diseases. The clinical use of classical biomarkers is also under constant improvement, and several new substances are being put into routine testing. Several candidate substances have been shown to compete well with classical biomarkers.
Urgent research targets include a better discrimination of acute versus chronic HF, as well as the differentiation between several forms and etiologies of HF. Biomarker-guided HF therapy and the recognition of prognostic potential of classical and novel substances is also urgently needed. This Special Issue on metabolic markers on HF focuses on these aims, as well as original research on new candidate substances. We welcome submissions from clinical as well as basic research. Insightful reviews are also very welcome.
