Dear Colleagues,
Disorders of consciousness (DoCs) following ischemic stroke range from transient confusion to coma, markedly affecting patient outcomes and management strategies. Their pathophysiology involves multiple mechanisms, including primary anatomical injury and secondary brain injury, encompassing cerebral edema, mass effect, compromised cerebral perfusion, and other systemic complications. Accurate prognostication is challenging but critical for guiding management and shared decision making. Various modalities are used to evaluate stroke, including neuroimaging (such as CT, MRI, and CT), to identify ischemic regions and secondary complications comprising hemorrhagic transformation or edema that contribute to altered consciousness. Prognosis varies with factors such as medical comorbidities, stroke severity, infarct location, etc. However, emerging biomarkers and advanced imaging techniques hold promise for enhancing prognostic precision. Understanding the spectrum of DoCs and associated outcomes is critical for developing tailored treatment algorithms, optimizing recovery potential, and informing caregivers about realistic expectations for patient recovery and rehabilitation trajectories. This topic collection consolidates evidence on evaluating, managing, and predicting outcomes in patients with ischemic stroke and DoCs.
