Special Issue Information

Dear Colleagues,

Arterial hypertension remains one of the most prevalent and consequential drivers of cardiovascular and kidney disease worldwide. Current evidence indicates that hypertension-mediated organ damage is not merely a hemodynamic consequence of elevated blood pressure, but the result of a complex network involving renal sodium handling, renin–angiotensin–aldosterone system dysregulation, sympathetic overactivity, oxidative stress, immune inflammation, metabolic dysfunction, and genetic/epigenetic susceptibility. Recent work on salt sensitivity and multi-omics also points to clinically meaningful heterogeneity in hypertensive phenotypes. In parallel, the growing burden of obesity, insulin resistance, diabetes, and metabolic dysfunction-associated steatotic liver disease has reinforced the view of hypertension as a cardio–kidney–metabolic disorder rather than an isolated vascular condition.

This Special Issue highlights recent advances in the renal mechanisms that initiate and sustain hypertension, the metabolic drivers that accelerate vascular and renal injury, and the organ-protective strategies that may improve long-term outcomes. We welcome contributions addressing molecular and cellular pathways, biomarkers and omics-based stratification, kidney–heart–metabolic crosstalk, and emerging therapeutic approaches, including optimized pharmacological combinations, mineralocorticoid receptor antagonism, SGLT2 inhibition, incretin-based therapies, and device-based interventions such as renal denervation. By integrating mechanistic insight with translational and clinical perspectives, this Special Issue seeks to advance a more precise and organ-protective approach to arterial hypertension.
