Dear Colleagues,
Prostate cancer (PCa) is the second most commonly diagnosed malignancy in men worldwide and affects one in nine men in the United States. While localized PCa is often managed effectively with surgery, radiation, or active surveillance, metastatic disease remains lethal, with a five-year survival rate of ~31%. Because prostate tumors rely on androgen signaling for growth, androgen deprivation therapy (ADT) is the mainstay treatment for advanced disease. However, many tumors progress to metastatic castration-resistant prostate cancer (mCRPC), where androgen receptor pathway inhibitors (ARPIs) are used. Unfortunately, resistance to ARPIs typically develops within a year, emphasizing the urgent need for new strategies.
Recent advances—including next-generation AR inhibitors, PARP inhibitors, radiopharmaceuticals, and immunotherapies—offer promising avenues. Yet, treatment resistance, limited predictive biomarkers, and uncertainty around optimal therapy sequencing remain significant challenges.
This Special Issue of the International Journal of Molecular Sciences will focus on the genetic and molecular markers driving PCa development, progression, and therapeutic response. We welcome original research, reviews, and translational or clinical studies that
- Identify diagnostic, prognostic, and predictive biomarkers;
- Investigate resistance mechanisms to ADT and ARPIs;
- Explore novel genetic targets for therapy;
- Advance precision oncology through personalized approaches.
Our goal is to promote insights that improve clinical outcomes in prostate cancer.
