Dear Colleagues,
In 2026, personalized medicine in urologic oncology has matured into a precision-driven discipline. As Guest Editor, I would like to emphasize this Special Issue’s main aim to highlight the shift from reactive to proactive care, powered by AI-integrated genomic profiling and liquid biopsy monitoring. A cornerstone of this advancement is the use of patient-derived organoids, which allow clinicians to conduct ex vivo drug sensitivity testing, ensuring patients receive the most effective agents first.
Therapeutically, we are witnessing a paradigm shift. Radioligand therapies have transitioned to first-line settings for prostate cancer, while perioperative immunotherapy combinations now define the standard for muscle-invasive bladder cancer. Furthermore, the emergence of personalized mRNA vaccines in 2026 represents a landmark in preventing recurrence. By targeting unique molecular signatures rather than just organ sites, these contemporary tools represent a transformative leap toward truly individualized urological care, significantly improving both survival outcomes and patient quality of life.
