Dear Colleagues,
Chronic diseases, including diabetes, cardiovascular conditions, cancer, and respiratory illnesses, represent a leading global health challenge and continue to impose a significant burden on individuals, families, and healthcare systems worldwide. Effective health management strategies are crucial not only for improving clinical outcomes but also for enhancing the quality of life (QoL) of those living with long-term conditions. This Special Issue invites original research, reviews, and practice-based innovations that explore holistic, patient-centered approaches to managing long-term chronic disease conditions.
We seek contributions that examine health literacy, self-management behaviors, digital health interventions, community-based care interventions, and interprofessional collaboration models of care in improving health outcomes. We particularly welcome manuscripts that highlight culturally responsive care, equity in care access, policy implications, and innovations in nursing and allied health practices.
This Special Issue of the International Journal of Environmental Research and Public Health (IJERPH) focuses on the current state of knowledge and aims to advance understanding and evidence that contribute to the development of sustainable, equitable, and integrated care models for chronic disease populations across life the lifespan and diverse settings. We welcome interdisciplinary perspectives that bridge clinical practice, research, education, and health policy. We will accept manuscripts from various disciplines that contribute to shaping the future of chronic disease care and improving lives through evidence-based health management.

Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-blind peer-review process. International Journal of Environmental Research and Public Health is an international peer-reviewed open access monthly journal published by MDPI.
