Dear Colleagues,
Gestational diabetes mellitus (GDM) is one of the most frequent metabolic complications of pregnancy, with a prevalence that continues to rise in parallel with increasing maternal age, obesity, and sedentary lifestyle. Although traditionally viewed as a transient disorder resolving after delivery, GDM is now recognised as a powerful marker of future cardiometabolic risk. Women affected by GDM have a markedly increased lifetime risk of developing type 2 diabetes and cardiovascular disease, while offspring exposed to intrauterine hyperglycaemia face a higher probability of obesity and metabolic dysfunction later in life.
Recent advances in the literature highlight the heterogeneity of GDM, suggesting distinct phenotypes driven by predominant insulin resistance or impaired β-cell dysfunction, each potentially associated with different maternal and neonatal outcomes. Novel approaches are focusing on early risk stratification through first-trimester biomarkers, metabolomics, genetic and epigenetic profiling, as well as the integration of artificial intelligence-based prediction models. Continuous glucose monitoring and digital health interventions are emerging as promising tools for improving glycaemic assessment and individualising care.
This Special Issue aims to explore recent updates in the pathophysiology, diagnosis, phenotypic stratification, technological innovations, and long-term consequences of GDM.
