Comments and Suggestions for Authors
I have read with interest for this case report. The application of HA spacer in the surgical setting is novel. Some minor revision is required:
1. It would be interested to understand the post-operative continent function for his patient as well.
2. What is the follow-up for this patient? What is the latest status, especially the bowel function, after discharge and at subsequent follow-up?
3. Is there any post-operative imaging, e.g. 6 months, to highlight the post-operative anatomical changes and the spacer status?

This brief communication on hyaluronic acid rectal spacer for rectal protection in salvage cystoprostatectomy is certainly of interest since the evolution of clinical practice, above all concerning radiotherapy, for the treatment of pelvic cancers. The topic is not extensively argued but this is a breaf communication. Nevertheless, since in the introduction T3 bladder cancers ere mentioned, I’d like to find in the discussion, possible data regarding the safety in using this device for these patients.

Good case report to support the call for more work to be done in this area of rectal protection in these complex post radiation cases involving the bladder and the prostate
