Cover Story: Atypical fibroxanthoma (AFX) and pleomorphic dermal sarcoma (PDS) are two genetically linked ends of the same spectrum. While AFX is an early-stage tumor, PDS is a more advanced and aggressive form of tumor, with a higher recurrence rate and a greater propensity for metastasis. Due to a substantial difference in clinical behavior, it is critical to distinguish between AFX and PDS accurately. Since Mohs micrographic surgery is often employed to treat these tumors, it is crucial to perform the histopathologic evaluation of the debulk specimen in order to avoid PDS being overlooked. This contribution discusses the distinction between these two tumors and underscores the importance of the histopathologic evaluation of debulk specimens. These measures could aid in the prevention of adverse patient outcomes and enable the use of additional treatment modalities. View this paper
