While much of the current research on early specialization focuses on physical outcomes, training models, and policy implications, little is known about how athletes themselves make sense of their developmental experiences. This study aims to examine how ice hockey players perceive and experience early specialization within competitive youth sport contexts, with the goal of generating a nuanced, inductively grounded understanding of athlete development from the athlete perspective. A qualitative study design was used where eight current and former ice hockey players with experience of early specialization participated. Data were collected using semi-structured interviews and analyzed using qualitative conventional content analysis. Three overarching themes emerged, highlighting experiences of loneliness, pressure, and elevated expectations within elite sport environments, alongside the vital importance of support networks and team community: ‘Thrown into adulthood with premature expectations’, ‘Balancing Support and Pressure in Athlete Development’, and ‘The Struggle Between Dream and Reality’. Players described feeling pressured, isolated, and prematurely professionalized, often at the expense of personal development. The findings highlight the psychological and structural challenges of early specialization in elite ice hockey. While support systems played a crucial role, they also contributed to performance anxiety and external expectations. These insights underscore the need for youth sport systems that prioritize long-term athlete well-being over short-term success. Full article

Background: Ice hockey is characterized by repeated short-duration, high-intensity efforts interspersed with brief recovery periods, requiring a complex interaction of aerobic and anaerobic energy systems. The aim of this study was to determine the energetic structure of repeated high-intensity on-ice sprint exercise in ice hockey players by quantifying the relative contributions of the oxidative, glycolytic and ATP–PCr energy systems. Methods: 14 male semi-professional ice hockey players performed the 30–15IIT followed by the Repeated High-Intensity Effort (RHIE) on-ice. Oxygen uptake was measured breath-by-breath, blood lactate concentration and energy system contributions were estimated using a three-component PCr–La–O2 model. Results: The RHIE on-ice was characterized by a dominant aerobic contribution (63.1 ± 2.6%), followed by phosphagen metabolism (29.8 ± 2.9%), with a relatively small glycolytic contribution (7.4 ± 1.5%). Conclusions: No significant relationships were observed between maximal oxygen uptake (VO2max) and the RHIE performance parameters, energy system contributions or lactate responses, except for a moderate relationship between absolute VO2max and absolute aerobic work. In contrast, parameters determined at the anaerobic threshold showed more consistent relationships with absolute metabolic work. These findings indicate that repeated high-intensity on-ice performance in ice hockey is largely independent of VO2max and is more closely related to individual energetic profiles and metabolic tolerance. Piriformis syndrome, a neuromuscular disorder caused by sciatic nerve compression by the piriformis muscle, remains understudied in athletic populations despite anecdotal reports of elevated prevalence in hockey players. This study investigated the prevalence of piriformis syndrome symptoms and potential risk factors in actively competing (current) and retired (former) high-level hockey players. A cross-sectional survey was conducted among 67 actively competing and retired professional, collegiate, and junior hockey players (58 males, 9 females; mean age 25.6 ± 4.0 years; mean playing experience 17.8 ± 3.7 years). Active playing status was defined as currently participating in organized competitive hockey at any level, while retired status was defined as having ceased competitive participation for at least one season. The survey instrument was based on a validated clinical assessment scoring system, consisting of 12 questions assessing piriformis syndrome-related symptoms. Participants were classified as “high score” (≥6 affirmative responses) or “low score” (<6 responses). Multiple linear regression analysis was used to evaluate associations between demographic variables (age, playing status, years played, competitive level) and total symptom scores. Overall, 25.4% (n = 17) of participants met criteria for high symptom burden, with sitting-induced buttock pain being the most prevalent specific symptom (40.3%). Mean total score was 4.8 ± 1.8 (range 2–10). Multiple regression analysis revealed no statistically significant associations between piriformis syndrome scores and any demographic variable (overall model: R2 = 0.065, p = 0.374). Retired players showed a non-significant trend toward higher scores compared to actively competing players (β = −1.388, 95% CI: −2.793 to 0.018, p = 0.053). No correlations were observed with age (r = −0.045, p = 0.719), years played (r = −0.054, p = 0.666), or competitive level (p = 0.666). In conclusion, this study revealed substantial piriformis syndrome symptom burden (25.4%) in high-level hockey players without significant demographic associations. Full article

Intense training loads alter the skin microbiome and defence mechanisms in athletes, yet adaptation profiles remain insufficiently characterised. This study evaluated the relationships between skin bacterial microbiome structure, antimicrobial activity, dermcidin levels, and acne severity in male professional hockey players compared with amateur athletes and non-athletes. One hundred men (18–57 years) were examined and allocated to six subgroups by exercise intensity and acne status. Microbiota composition was assessed by culture-based methods and MALDI-TOF identification, antimicrobial activity measured spectrophotometrically, dermcidin quantified by ELISA, and sweat proteome characterised by HPLC-MS. Staphylococcus epidermidis and Micrococcus luteus predominated in all groups. Exercise intensity, rather than acne, was the main determinant of total bacterial colonisation, which increased approximately tenfold from non-athletes to professional hockey players. In non-athletes, higher antimicrobial activity correlated with greater acne severity, whereas in professionals this relationship was absent and dermcidin levels showed an inverse association with acne severity. Proteomic analysis identified 17 polypeptides; dermcidin and prolactin-inducible protein were dominant in all groups, and calprotectin (S100-A8/A9) was detected exclusively in healthy professionals. Background: Field hockey is a high-intensity intermittent sport, where nutrition and sleep play an important role in the performance and health of all players, especially in women, who often exhibit a dietary pattern characterized by low energy and carbohydrate intake, along with poor nighttime habits. The purpose of this study is to evaluate the profile of female field hockey players by analyzing their energy and macronutrient intake, prevalence of LEA risk, use of SS, and sleep characteristics. Methods: A cross-sectional, observational, and descriptive study was carried out with 75 female players. Validated questionnaires were used to determine general sports nutrition knowledge (NUKYA) and specific knowledge of carbohydrates in sports (CEAC-Q), sleep quality (ASSQ), low energy availability risk (LEAF-Q), and use of sport supplements. In addition, a 7-day dietary record was analyzed. Results: Players showed a high general nutrition knowledge (NUKYA mean: 66.0 ± 8.5 points; 68.0% scored high), but a pronounced deficiency in specific carbohydrate knowledge (CEAC-Q mean: 24.3 ± 14.9 points; 84.0% scored low). Dietary intake analysis revealed significant deficiencies: mean energy intake was 31.9 ± 10.8 kcal/kg/day, resulting in insufficient intake for 78.7% of players. Carbohydrate intake was particularly low (3.6 ± 1.5 g/kg/day), with 86.7% failing to meet recommendations. Furthermore, 33.3% of players were at risk of Low Energy Availability (LEA). LEA risk (LEAF-Q score) was negatively correlated with both protein (r = −0.363; p = 0.001) and carbohydrate intake (r = −0.347; p = 0.003). Regarding sleep, the mean disturbance score (SDS) was 6.5 ± 2.9 (mild disturbance), with 33.3% showing moderate–severe disturbance, and 92.0% sleeping ≤8 h. Finally, 78.7% of players used supplements, with usage correlated with CEAC-Q scores (r = 0.233; p = 0.044), and 86.4% were guided by non-professional sources. Conclusions: Female hockey players do not meet dietary recommendations for energy and carbohydrates and exhibit a high prevalence of low energy availability and sleep disturbances, despite having acceptable general sports nutrition knowledge. It is recommended to implement specific educational and nutritional strategies to enhance the knowledge, performance, and health of female hockey players. Full article

Field hockey involves repetitive high-intensity movements and physical contact, posing a high risk of injury. However, studies developing injury prediction models without relying on expensive tools such as GPS remain limited. This study aimed to develop an explainable AI model that predicts injury occurrence using only simple questionnaire-based data and visually identifies key predictors. Survey data were collected from 239 adult players registered with the Korea Field Hockey Association in 2024, including university and professional team athletes. Ten variables were used: sex, team affiliation, playing experience, player level, warm-up duration, weekly training hours and days, and physical indicators (age, height, weight). Injury was defined as an event within the past year that resulted in being unable to train for more than 24 h. Logistic Regression, Random Forest, and XGBoost models were compared. The final model—Logistic Regression—underwent SHAP-based visualization for interpretability. The Logistic Regression model showed the best performance in recall (0.6810 ± 0.0983), F1-score (0.6260 ± 0.0499), and AUC (0.6515 ± 0.0393). SHAP analysis identified Group, Training Time, Weight, and Player Level as key predictors, and visualized their contributions to individual predictions. This study demonstrates that a lightweight, interpretable injury prediction model using only simple survey data can achieve practical performance. This approach offers valuable insights for real-world applications and the development of injury prevention strategies. Full article

Background: Oral health (OH) has been linked to overall health status and common physical performance (PP). Aim: The objective was to compare the OH and PP of two third league professional team players from different sports (soccer, ice hockey). Methods: Twenty-nine professional soccer players (mean ± standard deviation; age: 24.3 ± 4.1 years) and twenty-one ice hockey players (age: 27.7 ± 3.6 years) were investigated regarding several dental scores (DMFT, PSI, API, PBI) by one calibrated examiner. The PP diagnostic includes measurement of grip strength as well as postural stability and regulation using posturography. Results: Concerning dental scores, relevant sport-specific differences for PSI (1 vs. 2; p < 0.001), API (29 vs. 48; p = 0.001), and PBI (14 vs. 42; p < 0.001) in favor of soccer players were found. Ice hockey players reported significantly more tension of the temporomandibular joint (p = 0.004) and showed lower readiness for regular check-ups (p < 0.001) and additional oral hygiene (p = 0.045). In contrast, ice hockey players were more balanced and displayed a higher level of weight distribution (p < 0.001), especially in the anterior-posterior direction (p = 0.002). Conclusions: Based on more intensive oral hygiene and dental care, the investigated professional soccer players showed partially better OH conditions compared to the ice hockey players examined. In summary, the OH and PP results reflect the different sport-specific requirements and related training impacts on athletes. Full article

Background/Objectives: COVID-19 poses significant physiological challenges for athletes, particularly those engaged in high-intensity intermittent sports such as ice hockey. This study aimed to evaluate the impact of SARS-CoV-2 infection—especially symptomatic cases—on aerobic and anaerobic performance in professional ice hockey players. Methods: Fifty athletes from the Polish Hockey League were assigned to three groups: control (CG, n = 13), asymptomatic COVID-19 (NSG, n = 28), and symptomatic COVID-19 with post-infection SpO2 < 90% (WSG, n = 9). Each underwent assessments at three time points—pre-preparatory period 2020/2021, post-preparatory period 2020/2021, and pre-preparatory period 2021/2022. Aerobic capacity was measured via a progressive cycle ergometer test (VO2max, VO2 at lactate threshold [VO2Lt], minute ventilation [V’E], breathing frequency [BF], and lactate clearance rate [ΔLa]), and anaerobic capacity via a 30 s Wingate test (relative mean power). Results: Compared with CG and NSG, symptomatic athletes exhibited significant post-infection declines in VO2max (48.2 ± 2.9 vs. 56.2 ± 6.2 and 54.6 ± 3.9 mL/kg/min; p = 0.006, d = 1.56 vs. CG; p < 0.024, d = 1.79 vs. NSG) and VO2Lt (p < 0.05). Relative mean power also decreased in WSG (p < 0.05). In contrast, CG and NSG improved or maintained these metrics over the same period. Symptomatic players showed elevated BF post infection (p = 0.022, d = 1.72) and reduced V’E (p = 0.035; d = 0.83), while ΔLa was markedly lower (p = 0.0004; d = 2.86). Conclusions: SARS-CoV-2 infection, particularly symptomatic cases, can significantly impair both aerobic and anaerobic capacity in elite hockey players. Targeted recovery protocols are essential for restoring performance in affected athletes. Full article

Background: Field hockey is a popular sport, globally. Players repeatedly perform a combination of forward flexion and rotational movements, predisposing them to pain/injury. This study aimed to determine the 3-month period prevalence and incidence (number of new cases) of low back pain (LBP) in male adolescent field hockey players, its characteristics and association with selected risk factors, and treatment approaches. Methods: A questionnaire-based study was administered to 112 male adolescent players, and included sections on demographics, injury characteristics and professional care sought. Results: Sixty-eight questionnaires were completed (67% participation rate); period prevalence of LBP was 63.2% (35.0%: at the beginning of the season, 32.4%: at mid-season, 22.1%: at end-season); incidence was 38.2%. The most common location for LBP was the middle to low back region (39.5%); the most common duration of pain was a few hours (32.6%). Most participants (79.1%) did not classify their pain as a disability; only 44.2% of participants received medical treatment. Conclusions: Despite focusing on a specific group of adolescents, this study revealed a comparable, yet slightly elevated, prevalence of LBP compared to earlier research predominantly involving female populations. Consequently, we recommend the development, implementation, and rigorous evaluation of targeted strategies for the prevention and management of LBP within the sport of field hockey. Full article

This real-life data collection aimed to expand an existing reference database regarding an extensively evaluated ice hockey-specific complex test (IHCT). One hundred and thirty-eight third-league professional ice hockey field players (mean ± SD; age: 26.4 ± 5.24 years; forwards: n = 94, defenders: n = 44) were investigated. IHCT data were collected over eight seasons from three third-league teams. The IHCT included parameters for the load (e.g., 10 m and 30 m sprint times, transition and weave agility times with and without a puck, slap and wrist shots on goal) and stress (e.g., lactate, heart rate). The only relevant (p < 0.002, ηp2 ≥ 0.10) difference between forwards and defenders for performance was found for 30 m backward sprint without a puck (p < 0.001, ηp2 = 0.10, d = 0.74). As expected, in this regard, defenders performed better than forwards. Significant differences were also found in 10 m backward sprint without a puck (p = 0.005), weave agility with a puck (p = 0.014), heart rate recovery minute 10 (p = 0.057), and goals after the test (p = 0.041). This study provides expanded position-specific third-league reference data for the IHCT. On this basis, coaches are able to evaluate players’ performance (forwards vs. defenders) and the effect of the training periods. Further research is necessary to extend this database to first- and second-league players in order to enhance the scope of the IHCT. Full article

The hemostatic system is characterized by a delicate balance between pro- and anticoagulant forces, and the smallest alteration can cause serious events such as hemorrhages or thrombosis. Although exercise has been shown to play a protective role in athletes, several factors may increase the risk of developing venous thromboembolism (VTE), including hemoconcentration induced by exertion, immobilization following sports injuries, frequent long-distance flights, dehydration, and the use of oral contraceptives in female athletes. Biomarkers such as D-dimer, Factor VIII, thrombin generation, inflammatory cytokines, and leukocyte count are involved in the diagnosis of deep vein thrombosis (DVT), although their interpretation is complex and may indicate the presence of other conditions such as infections, inflammation, and heart disease. Therefore, the identification of biomarkers with high sensitivity and specificity is needed for the screening and early diagnosis of thromboembolism. Recent evidence about the correlation between the intensity of physical activity and VTE is divergent, whereas the repeated gestures in sports such as baseball, hockey, volleyball, swimming, wrestling, or, on the other hand, soccer players, runners, and martial art training represent a risk factor predisposing to the onset of upper and lower DVT. Anticoagulant therapy is the gold standard, reducing the risk of serious complications such as pulmonary embolism. The aim of this review is to provide a general overview about the interplay between physical exercise and the risk of thromboembolism in athletes, focusing on the main causes of thrombosis in professional athletes and underlying the need to identify new markers and therapies that can represent a valid tool for safeguarding the athlete’s health. Comparing the Effects of an Off-Ice Sprint-Change of Direction Task on Trunk Kinematics and Gait Laterality in Collegiate Ice Hockey Players

Laterality preferences are intrinsic in most physical activities, and ice hockey is one domain wherein these preferences might influence performance. Biomechanical laterality between dominant and nondominant (or preferred and nonpreferred) limbs is believed to be an advantageous attribute that is linked with skilled performance. Yet little is known about the implications of motor asymmetries for skilled performers in dynamic, time-constrained, team-based activities in an off-ice environment. This can be extended to when player position is considered, notably for those playing in a defensive or an offensive position. In this study, fourteen semi-professional collegiate male ice hockey players (age: 21.87 ± 2.98 years; BMI: 25.26 ± 3.21 kg/m) performed a randomized repeated 15 m sprint-change of direction task. Assessments of lower limb laterality were carried out as participants commenced the 15 m sprint change of direction task in both a right and left foot rear setback position. Biomechanical laterality between right and left rear foot setback positions was inferred by an ActiGraph GTx3 triaxial accelerometer that was located on the participants’ spinous process, representing the trunk centre of mass (CoM). Overall, ANOVA results indicated significant differences across all sprint split times between the right and left foot rear setback positions, with times significantly quicker when players commenced in a right rear foot setback position (p < 0.001). ANOVA revealed significant differences in trunk CoM acceleration between in a right and left rear setback position, specifically during the initial 0–10 m sprint split, with offensive players observed to have lesser trunk anteroposterior and vertical CoM acceleration (p = 0.05) and during the final 5 m sprint split (p = 0.002, d = 0.7), despite overall smaller effect sizes seen in the left foot rear setback position. It appears that starting with the foot in a right rear setback position results in quicker 15 m performance times and concurrent lower magnitudes of trunk CoM acceleration. Although we demonstrated that offensive players were quicker and displayed less trunk CoM acceleration, we recommend that future studies use a greater number of participants for inter-limb symmetry in these movement tests. This report presents the case of an ice hockey player diagnosed with a tear in the anterior horn of the lateral meniscus at 19 years of age. In addition, we suggest a rehabilitation exercise program that enabled an early return to play for an ice hockey athlete in a university sports league. The patient underwent arthroscopic meniscal repair and was discharged one day after surgery. A three-stage rehabilitation program was developed, as follows: Phase I, the protective phase (reduce inflammation symptoms and restore range of motion to enable weight-bearing); Phase II, the restorative phase (improve the strength of the quadriceps muscle and recover cardiovascular endurance); Phase III, the return to full functional activity (familiarize the patient with sport-specific skills and recover a symmetric functional level on both sides). After completing the 10-week rehabilitation program, the patient returned to play and took part in games 71 and 84 days after surgery. The patient had no meniscus reinjury in the 10 years after surgery while playing in university and professional ice hockey leagues. The functional performance-focused rehabilitation program presented in this case report resulted in an early return to sport and good long-term outcomes. Full article

The aim of the present study was to analyse differences in acceleration and movement variability caused by adding whole-body vibration (WBV) and an implement (stick) while performing a slide-board (SB) skating exercise. A total of 10 professional ice-hockey players (age 20.4 ± 2.07 years) participated in the study. Participants performed 30 s of lateral sliding on a slide vibration board (SVB). Four conditions were analysed: no vibration and no stick (NVNS), no vibration with a stick (NVS), vibration without a stick (VNS) and vibration with a stick (VS). Peak acceleration, mean acceleration and movement variability (MV) were analysed in the dominant and non-dominant legs in each condition. Peak acceleration was higher in the non-dominant leg (p < 0.01). However, MV was higher in the dominant leg (p < 0.01). Regarding differences between conditions, mean acceleration was higher in VNS and VS than in NVS (p < 0.05). Regarding MV (sample entropy), there were differences in NVNS compared to VNS and VS (p < 0.01) and in NVS compared to VNS and VS (p < 0.01), with the values being superior in VNS and VS. The addition of WBV during an SB skating exercise results in an increase in MV and mean acceleration. The dominant leg shows greater MV regardless of the addition of vibration and a stick during sliding on an SVB. Full article

Ice hockey is a high-risk sport known for its dominant macho culture. The purpose of this study was to examine experiences surrounding medication use among male, elite ice hockey players in Norway. A mixed-method design was employed, which first examined medications registered on doping control forms (DCFs) (n = 177) and then involved semi-structured focus group interviews (n = 5) with elite athletes (n = 25). Overall, 68% of the DCFs contained information about ≥1 medication. Among the most registered medications were NSAIDs and hypnotics (20% and 19% of all DCFs, respectively). During the interviews, numerous athletes reported using analgesics to manage injuries and pain caused by the sport, often being motivated by sacrificing themselves for the team during important matches and playoffs. Hypnotics were used due to high cumulative stress due to heavy training and competition load, late-night matches, and playing in a semi-professional league. Athlete support personnel (ASP), including physicians and trainers, were the athletes’ main sources of information. The athletes often displayed a profound and non-critical trust in the advice and products provided to them by their team physician. The findings indicate that male, elite ice hockey players, through their excessive and somewhat ignorant use of medications, expose themselves to health risks and inadvertent doping. Full article
