Background/Objectives: Contemporary patient-specific subperiosteal implants have re-emerged as graftless solutions for oral and maxillofacial rehabilitation, driven by advances in digital planning, CAD/CAM workflows, additive manufacturing, and biomaterial engineering. Their indications have progressively expanded from severely atrophic edentulous jaws to segmental defects, single-tooth replacement, congenital craniofacial anomalies, salvage situations, and oncologic reconstruction. This scoping review aimed to map the current evidence on modern patient-specific subperiosteal implants, focusing on indications, workflow, design principles, materials, outcomes, complications, and maintenance. Methods: A scoping review was conducted according to PRISMA-ScR principles to identify clinical studies, case series, case reports, systematic and scoping reviews, technical notes, finite element analyses, in vitro studies, and relevant translational investigations dealing with contemporary custom-made or CAD/CAM subperiosteal implants. The evidence was narratively synthesized according to clinical indication and thematic domains, including full-arch rehabilitation, sectional and single-tooth applications, congenital and post-oncologic defects, rescue indications, biomechanics, material selection, surface response, prosthetic protocols, and complication management. No quantitative meta-analysis was performed because of the scoping design and the substantial heterogeneity of study types, indications, implant systems, outcome definitions, and follow-up durations. Results: The final evidence map included 116 records, of which 56 were unique human clinical records with extractable denominators and 60 were biomechanical, in vitro, surface-biology, review, consensus, historical, or conceptual records. Of the 56 unique clinical records, 49 were mapped within the six indication-level clinical sections, while seven were retained as cross-cutting clinical evidence addressing patient-reported outcomes, design-related complications, bone apposition, anchorage strategy, comparative graftless rehabilitation, or reconstructive/prosthetic principles. The six indication-level sections included 52 clinical-record assignments: 15 for full-arch rehabilitation, 13 for segmental or sectional rehabilitation, one for single-tooth rehabilitation, four for congenital or craniofacial indications, 13 for post-oncologic or post-ablative reconstruction, and six for rescue or salvage indications. Because three records addressed more than one indication, these counts represent indication-level assignments rather than mutually exclusive clinical records. Reported survival in most short- to mid-term clinical series was generally high, commonly ranging from 90% to 100%, although lower values of 70–80% were reported in selected longer-term cohorts and survival clearly overestimated clinical success in some studies. Expanding applications include posterior mandibular and maxillary defects, lateral incisor agenesis, cleft-related or syndromic deformities, maxillectomy reconstruction, obturator support, and hybrid rehabilitation with endosseous implants; however, evidence for the indications at the extremes of this spectrum—single-tooth replacement and primary oncologic reconstruction—remains limited to small, largely single-group case series and reports. Soft-tissue events, including dehiscence, mucositis, recession, and framework exposure, were the dominant complications and showed wide variability, with reported recession/exposure rates ranging from approximately 10% in some sectional and full-arch series to as high as 65% in bilateral maxillary cohorts; their clinical significance varied from asymptomatic stable findings to progressive inflammatory complications requiring revision. Conclusions: Patient-specific subperiosteal implants represent a promising and increasingly versatile reconstructive option; however, the present findings should be interpreted as evidence mapping rather than as definitive comparative evidence. Their clinical use should remain highly selective, prosthetically driven, and supported by meticulous planning, rigid fixation, soft-tissue management, and structured maintenance. Standardized success criteria, longer follow-up, and comparative prospective studies are required. Full article

Objectives: This study aimed to evaluate and compare the cost-effectiveness of the surgery-first and orthodontic-first approaches in Thai patients with dentofacial deformities undergoing orthognathic surgery. Methods: This retrospective cohort study included 30 patients with dentofacial deformities, divided into the surgery-first approach (SFA, n = 15) and orthodontic-first approach (OFA, n = 15). All underwent double-jaw surgery without genioplasty performed by a single surgeon and orthodontist. Data collected included operative and orthodontic costs, operative time, total treatment and hospital costs, treatment duration, and length of hospital stay. Cost-effectiveness was assessed using the incremental cost-effectiveness ratio (ICER) and incremental time-effectiveness ratio (ITER), with effectiveness measured by quality of life via the Thai Orthognathic Quality of Life Questionnaire (OQLQ) before and at the time of debonding the orthodontic appliance. Results: Overall, the SFA demonstrated greater cost-effectiveness than the OFA. However, the SFA group incurred slightly higher hospital costs. There were no statistically significant differences between the two groups in operative cost, hospital cost, total cost, operative time, or length of hospital stay (p > 0.05). By contrast, orthodontic cost, orthodontic treatment duration, and total treatment duration were significantly lower in the SFA group compared with the OFA group (p < 0.05). Conclusions: Within limits, the SFA is a potentially beneficial alternative to OFA. Patients and clinicians may benefit from using SFA by experiencing shorter treatment duration, lower orthodontic treatment costs, and improvements in certain aspects of quality of life. Further studies with larger sample sizes and longitudinal data are necessary to establish the long-term effectiveness. Full article

Background and Clinical Significance: Vascular lesions of the face, particularly arteriovenous malformations (AVM) and mixed hemangiomas (MH), pose significant diagnostic and therapeutic challenges because of their complex anatomy, unpredictable behavior, and high risk of bleeding. Surgical planning should be individualized and often requires a staged approach with meticulous interdisciplinary coordination to ensure patient safety. The presence of a concomitant odontogenic infection further complicates management, as local inflammation may exacerbate vascular instability and increase the risk of life-threatening complications. Local inflammation and infection might cause some life-threatening conditions, especially when an abscess occurs in the area of any vascular lesion. Ensuring that the oral cavity is free from potential odontogenic infections is a particularly important issue in many complex cases, especially in patients treated for oral, head, and neck cancer or in those with other coexisting morbidities affecting the oral and facial regions. Case Presentation: A 72-year-old man was referred for management of a severe odontogenic infection associated with an extensive facial vascular lesion. The patient’s medical history was significant for arterial hypertension and chronic liver dysfunction (CLD) of unclear etiology. Complete blood testing, including coagulation assessment and liver ultrasonography, was performed, with no contraindication to surgery identified. The scope of odontogenic-related infections was scheduled for simultaneous removal during initial surgery. Preparation for surgery included the local application of sclerotherapy agents. Conclusions: Quite often, a routine panoramic radiograph can help in assessing the status of bone and dentition to undertake all necessary treatment. Severe odontogenic disease, including multiple retained roots, periapical infections, and odontogenic cystic lesions in the context of poor oral hygiene, may lead to the occurrence of possible inflammation. In case of any vascular lesion, a careful diagnostic and therapeutic strategy is needed. This case report highlights that maintaining an infection-free oral environment is a critical component of care in patients with complex facial MH and should be regarded as an essential element of overall treatment planning. In addition to standardized lateral cephalometric radiographs, comprehensive assessment using dental cone-beam computed tomography (CBCT) and CT has become commonplace in the diagnosis and treatment of jaw deformities. Simulation based on cephalometric and CT data is particularly useful in the management of jaw deformities, both for evaluation and prognostic prediction. As such imaging examinations cover a wide anatomical region, it is not uncommon for various incidental pathologies to be discovered. This review emphasizes the necessity of evaluating the entire imaged area in addition to the chief complaint. Furthermore, it outlines the essential anatomical structures that should be assessed during diagnostic imaging performed prior to representative surgical procedures for jaw deformities (e.g., sagittal split ramus osteotomy and Le Fort I osteotomy). This review paper is descriptive in nature, incorporating our facility’s empirical aspects, and presents representative cases in a narrative format; it is not a systematic review. In other word, as the evidence-based literature does not cover all aspects of pretreatment evaluation, these criteria are based on the past experience of the authors. Full article

Background/Objectives: Three-dimensional virtual surgical planning (VSP) is increasingly central to contemporary orthognathic surgery, enhancing diagnostic precision and enabling more reliable forecasts of postoperative outcomes. NemoFAB (Nemotec, Madrid, Spain) is a recently developed digital platform that integrates CBCT data, digital dental models, and facial photographs into a single workflow. Despite its growing clinical use, independent validation of its predictive accuracy remains limited. This study evaluated how closely NemoFAB virtual predictions corresponded to actual postoperative results using standardized cephalometric parameters. Methods: Forty adult patients with dento-maxillofacial deformities requiring combined orthodontic–surgical treatment were included. Eleven cephalometric variables—common to both WebCeph (2D) and NemoFAB (3D)—were measured preoperatively, virtually in NemoFAB, and postoperatively. AI-assisted landmark placement was manually verified by two orthodontists. Statistical analyses included repeated-measures ANOVA, paired t-tests, Bland–Altman plots, and intraclass correlation coefficients (ICC) to evaluate agreement and predictive accuracy. Results: Overjet, overbite, maxillary incisor inclination, maxillary incisor exposure, mandibular incisor projection to the True Vertical Line, and occlusal plane angulation all showed statistically significant changes after surgery (p < 0.05). Bland–Altman analysis demonstrated the narrowest limits of agreement in Nemo–Post comparisons, indicating strong predictive alignment. ICC values showed excellent agreement for incisor angulation (ICC = 0.921–0.984) and Pogonion projection (ICC = 0.943). Consistently poor pre-Nemo agreement reflected expected discrepancies between initial anatomy and planned surgical correction. Conclusions: When predicting skeletal and dentoalveolar changes brought about by orthognathic surgery, NemoFAB showed a high degree of agreement, especially for parameters that are directly impacted by jaw repositioning. Its strong concordance with postoperative outcomes supports its reliability for virtual planning, interdisciplinary coordination, and surgical execution. Advances in soft-tissue modeling and AI-driven automation may further enhance predictive accuracy. Full article

This article presents the results of research on the possibility of extruding oxygen-free copper pipes in bridge dies. The possibility of continuous production of a finished product of any length with a uniformly deformed wall was analysed. One of the most important elements of the work was to determine the shape of the tool (die and bridge) that would allow durable connection of the material. Numerical studies conducted using the commercial computer programme FORGE®NxT 2.1, including analysis of the distribution of material temperature and hydrostatic pressure in the welding zone of the bridge die affecting the copper joint during the manufacture of tubular profiles, confirmed the validity of the research issue. The results of the numerical studies were supplemented by laboratory tests, confirming the accuracy of the selected variant of the finished product manufacturing process. The process of bonding under conditions of two-part material compression was used for physical modelling of copper welding. The tests were conducted using the Gleeble 3800 metallurgical process simulator with the PocketJaw module. Based on the analysis of the obtained results, it was found that for tubes with a wall-thickness-to-inner-diameter ratio of 0.5, it is justified to use tools with a longer sizing section and welding chamber, as well as a larger mandrel generating-line angle within the welding chamber. Background: Central giant cell granuloma of the jaw is a benign but potentially aggressive lesion that can cause pain, facial deformity, tooth loss, and jaw destruction. Many treatment methods are described in the literature, but the less invasive ones are associated with a higher recurrence rate. For several decades, extensive bone resection procedures have been the most effective treatment to date. This study aimed to evaluate a minimally invasive treatment protocol combining multiple weekly intralesional steroid injections with surgical removal of residual tumor tissue and chemical cauterization using Carnoy’s solution. Methods: Thirteen patients with histologically confirmed central giant cell granulomas of the jaws were treated according to the protocol, including weekly triamcinolone injections and, when necessary, fenestration of the cortical bone to access residual lesions. Patients were monitored clinically and radiologically over six years, with reconstruction of bone defects using autogenous grafts and platelet-rich fibrin. Results: The treatment effectively reduced tumor size, restored cortical bone, and allowed preservation of jaw structure. Only one recurrence was observed, and complications were minor and transient. The protocol was equally effective for both aggressive and non-aggressive lesions, regardless of patient age or comorbidities. Conclusions: These findings suggest that combining pharmacological and surgical approaches with chemical cauterization provides a safe, effective, and tissue-preserving strategy for managing central giant cell granulomas, minimizing recurrence while reducing surgical morbidity. Targeting the problem of lack of theoretical model for wear calculation of key actuators in the knotter, a tripping mechanism of knotter based on the principle of elastic deformation was designed. A rigid–flexible coupling dynamic analysis model of the tripping mechanism was established based on the modal stress method, and the contact force time history curves and dynamic stress results between the groove cam and the ball roller, as well as between the knotter jaw and the tripping plate slot, were simulated. Based on MSC, Marc MENTAT, a finite element wear calculation model of the tripping mechanism, was constructed. Through 600 simulations equivalent to 6000 working cycles, the wear cloud maps of the tripping plate and the large gear groove cam were obtained, and the key wear areas and expansion trends were analyzed. The rapid wear tests were conducted by using a self-made knotter fatigue wear test bench, which showed that the maximum deviation between the measured value and the simulated value of the contact pair wear was less than 10%. This verified that the proposed wear model for the tripping mechanism can be used for calculating the contact pair wear of the mechanism, providing a reference for the heat treatment process of the surface hardness of the parts. Background: Orthognathic surgery aims to align the jaws with the facial skeleton and correct dental occlusion. This paper introduces the concept of planning the maxillomandibular complex (MMC) as a whole, utilizing a t-forming set of landmarks: the maxillary central incisor, the chin, and the occlusal plane. Methods: The background, hypothesis, and rationale of the new T concept are explained. A case of a 28-year-old male with skeletal class III malocclusion and an open bite was used to illustrate the application of the T concept in step-by-step surgical planning. The planning encompasses four phases: Phase One involves correcting frontal deformity and various asymmetries, Phase Two involves correcting chin anterior–posterior deformity, Phase Three involves correcting anterior–posterior and vertical MMC position, and Phase Four involves correcting MMC rotation. Results: The T concept provided a structured approach to plan MMC as a whole and integrate all structures into harmony. Conclusions: The T concept provides a logical approach to MMC positioning in orthognathic surgery, addressing functional and aesthetic concerns. It acts as a checkpoint to verify MMC position, helping surgeons achieve better results and avoid compensatory procedures. Full article

The paper presents an innovative theoretical concept of a bio-inspired soft gripper system with two parallel jaws, a fixed and a mobile one. It is conceived for gripping fragile or soft objects with complex, irregular shapes that are easily deformable. This novel gripper is designed for handling small objects of masses up to 0.5 kg. The maximum gripping stroke of the mobile jaw is 13.5 mm. The driving motor is a pneumatic muscle, an actuator with inherently compliant, spring-like behavior. Compliance is the feature responsible for the soft character of the gripper system, ensuring its passive adaptability to the nature of the object to be gripped. The paper presents the structural, kinematic, static, and dynamic models of the novel gripper system and describes the compliant behavior of the entire assembly. The results of the dynamic simulation of the gripper have confirmed the attaining of the imposed motion-related performance. Full article

Aim: Pacifiers play a critical role in the early stages of craniofacial and palate development during infancy. While they provide comfort and aid in soothing, their use can also have significant impacts on the growth and function of the oral cavity. This study aimed to simulate and predict the behavior of six different types of pacifiers and their functional interaction with the tongue and palate, with the goal of understanding their potential effects on orofacial growth and development. Materials and Methods: Biomechanical analysis using Finite Element Analysis (FEA) mathematical models was employed to evaluate the behavior of six different commercial pacifiers in contact with the palate and tongue. Three-dimensional solid models of the palate and tongue were based on the mathematical framework from a 2007 publication. This allowed for a detailed investigation into how various pacifier designs interact with soft and hard oral tissues, particularly the implications on dental and skeletal development. Results: The findings of this study demonstrate that pacifiers exhibit different interactions with the oral cavity depending on their geometry. Anatomical–functional pacifiers, for instance, tend to exert lateral compressions near the palatine vault, which can influence the hard palate and contribute to changes in craniofacial growth. In contrast, other pacifiers apply compressive forces primarily in the anterior region of the palate, particularly in the premaxilla area. Furthermore, the deformation of the tongue varied significantly across different pacifier types: while some pacifiers caused the tongue to flatten, others allowed it to adapt more favorably by assuming a concave shape. These variations highlight the importance of selecting a pacifier that aligns with the natural development of both soft and hard oral tissues. Conclusions: The results of this study underscore the crucial role of pacifier geometry in shaping both the palate and the tongue. These findings suggest that pacifiers have a significant influence not only on facial bone growth but also on the stimulation of oral functions such as suction and feeding. The geometry of the pacifier affects the soft tissues (tongue and muscles) and hard tissues (palate and jaw) differently, which emphasizes the need for careful selection of pacifiers during infancy. Choosing the right pacifier is essential to avoid potential negative effects on craniofacial development and to ensure that the benefits of proper oral function are maintained. Therefore, healthcare professionals and parents should consider these biomechanical factors when introducing pacifiers to newborns. Full article

Orthognathic surgery (OS) is a complex procedure commonly used to treat dentofacial deformities (DFDs). These conditions, related to jaw position or size and often involving malocclusion, affect approximately 15% of the population. Due to the complexity of OS, accurate planning is essential. Digital assessment using computer-aided design (CAD) and computer-aided manufacturing (CAM) tools enhances surgical predictability. However, limitations in soft tissue simulation often require surgeon input to optimize aesthetic results and minimize surgical impact. This study aimed to evaluate the accuracy of virtual surgery planning (VSP) by analyzing the relationship between planning deviations and surgical satisfaction. A single-center, retrospective study was conducted on 16 patients who underwent OS at San Pedro University Hospital of La Rioja. VSP was based on CT scans using Dolphin Imaging software (v12.0, Patterson Dental, St. Paul, MN, USA) and surgeries were guided by VSP-designed occlusal splints. Outcomes were assessed using the Orthognathic Quality of Life (OQOL) questionnaire and deviations were measured through pre- and postoperative imaging. The results showed high satisfaction scores and good overall outcomes, despite moderate deviations from the virtual plan in many cases, particularly among Class II patients. A total of 63% of patients required VSP modifications due to poor soft tissue fitting, with 72% of these being Class II DFDs. Most deviations involved less maxillary advancement than planned, while maintaining optimal occlusion. This suggests that VSP may overestimate advancement needs, especially in Class II cases. No significant differences in satisfaction were observed between patients with low (<2 mm) and high (>2 mm) deviations. These findings support the use of VSP as a valuable planning tool for OS. However, surgeon experience remains essential, especially in managing soft tissue behavior. Improvements in soft tissue prediction are needed to enhance accuracy, particularly for Class II DFDs. This study presents the design, fabrication, and experimental validation of a two-finger robotic gripper featuring a 135° V-shaped fingertip profile tailored for lightweight waste collection in laboratory-scale environmental robotics. The gripper was developed with a strong emphasis on cost-effectiveness and manufacturability, utilizing a desktop 3D printer and off-the-shelf servomotors. A four-bar linkage mechanism enables parallel jaw motion and ensures stable surface contact during grasping, achieving a maximum opening range of 71.5 mm to accommodate common cylindrical objects. To validate structural integrity, finite element analysis (FEA) was conducted under a 0.6 kg load, yielding a safety factor of 3.5 and a peak von Mises stress of 12.75 MPa—well below the material yield limit of PLA. Experimental testing demonstrated grasp success rates of up to 80 percent for typical waste items, including bottles, disposable cups, and plastic bags. While the gripper performs reliably with rigid and semi-rigid objects, further improvements are needed for handling highly deformable materials such as thin films or soft bags. The proposed design offers significant advantages in terms of rapid prototyping (a print time of approximately 10 h), modularity, and low manufacturing cost (with an estimated in-house material cost of USD 20 to 40). It provides a practical and accessible solution for small-scale robotic waste-collection tasks and serves as a foundation for future developments in affordable, application-specific grippers. Background/Objectives: Anterior tooth inclination plays a critical role in treatment planning for surgical orthodontic cases. However, post-treatment outcomes in patients with jaw deformities often deviate from cephalometric values. This study aimed to compare anterior tooth inclination and skeletal morphology among patients with Class II and Class III malocclusions and to establish reference values for individualized treatment plans. Methods: A total of 122 patients (Class II: n = 40; Class III: n = 41; Class I: n = 41 as a control) were retrospectively analyzed. Cephalometric parameters, including U1 to FH and L1 to MP, were measured pre- and post-treatment. Group differences were analyzed using one-way ANOVA and Tukey’s multiple comparison test. Multiple regression analysis was used to establish prediction models for anterior tooth inclination. The threshold for statistical significance was set at p < 0.05. Results: Post-treatment, upper anterior teeth were more lingually inclined in Class II patients and more labially inclined in Class III patients compared to Class I controls (U1 to FH: Class II, 106.8°; Class III, 120.4°; Class I, 111.1°; p < 0.01). Lower anterior teeth were more lingually inclined in Class III patients compared to Class I patients (L1 to MP: 84.9°; p < 0.01). Regression models demonstrated good predictive value (R2 > 0.5) in non-extraction cases. Conclusions: Regression equations developed in this study, alongside the cephalometric averages of Class I individuals, may offer reliable reference values tailored to individual craniofacial morphology, contributing to optimized treatment planning in surgical orthodontic cases. Background/Objectives: Jaw deformities affect not only facial aesthetics but also various oral functions. While previous studies have demonstrated that mandibular prognathism (MP) alters masticatory-induced brain blood flow (BBF), the temporal characteristics of these hemodynamic changes have remained unclear. In this cross-sectional observational study, we investigated the following two specific objectives: (1) whether food hardness affects not only the magnitude but also the temporal patterns of BBF changes during mastication and (2) how malocclusion is associated with these temporal hemodynamic responses. Methods: Twenty-six participants with normal occlusion (NORM) and twenty patients with MP participated in this study. BBF was measured using functional near-infrared spectroscopy, while participants chewed soft paraffin or hard gummy candy. Maximum oxygenated hemoglobin (oxy-Hb) values and time-to-peak BBF were analyzed. Results: While food hardness did not significantly affect maximum oxy-Hb within groups, the MP group showed significantly lower responses during hard gummy candy mastication compared to the NORM group. The occlusal contact area exhibited significant positive correlation with maximum oxy-Hb values, while the ANB angle, an indicator of intermaxillary skeletal relationship, showed no significant correlation with BBF parameters. The hard gummy candy/paraffin ratio of maximum oxy-Hb was significantly higher in the NORM group compared to the MP group. Time-to-peak BBF was approximately twice as long for hard gummy candy compared to paraffin in both groups, with no significant differences between groups. Conclusions: These findings reveal that while MP attenuates the magnitude of masticatory-induced BBF, particularly during hard food mastication, the temporal adaptation to increased food hardness is preserved. This dissociation between magnitude and timing effects suggests that intact basic neurovascular coupling mechanisms would be maintained even in the condition of altered masticatory function in a MP subject, which is providing new insights for rehabilitation strategies in orthognathic surgery cases.
