Energy Drinks and Arrhythmias in Adolescents and Young Adults: A Narrative Review of the Last Decade

Background and Objectives: Energy drinks (EDs), widely consumed by adolescents and young adults, contain caffeine and other stimulants marketed to enhance energy and performance. ED consumption has been associated with adverse cardiovascular effects, although most evidence derives from adult populations. This review examines the association between ED intake and arrhythmias or electrocardiographic (ECG) alterations in young people, including the influence of consumption patterns and the reversibility of reported changes. Materials and Methods: A review of the literature of the last ten years was conducted in the PubMed, Scopus, Cochrane Library and Google Scholar databases, using various combinations of the key words: “Energy Drinks”, “arrhythmias”, “ECG”, “electrocardiogram”, “dysrhythmias”, “children”, “teen”, “teenagers”, “adolescents”, “young adults”, “young people”. Results: 22 studies met the inclusion criteria: 10 RCTs, 5 non RCTs, 1 observational study and 6 case reports. Evidence suggests that ED consumption may be associated with cardiac rhythm disturbances and ECG alterations in young individuals. Reported events include atrial fibrillation, ventricular fibrillation, and, in rare cases, cardiac arrest, often following moderate-to-high ED intake. Experimental studies have demonstrated acute cardiovascular effects, including QTc interval changes (6–20 ms), increased supraventricular ectopic activity, alterations in heart rate (3–10 bpm) and ST–T changes. Most reported alterations appeared acute and reversible; however, their occurrence may be influenced by ED dose, composition, caffeine exposure, and individual susceptibility. Conclusions: ED consumption among adolescents and young adults may increase the risk of arrhythmias. There is consistent evidence across studies showing that the most common effects are HR and QTc alterations, with the composition and quantity of the drinks likely influencing these effects. Important gaps remain regarding frequent consumption, long-term outcomes, the contribution of less-studied ingredients, and the impact on younger populations. Stronger regulations may also be warranted to protect young consumers, particularly those at increased cardiovascular risk. Full article

Background: The use of sports supplements among adolescents is rising globally, driven by fitness trends and social media influence, yet knowledge gaps persist. This study aimed to assess supplement usage patterns, knowledge, attitudes, information sources, and the impact of a social media educational intervention among Serbian secondary school students. Methods: A single-group pre–post educational intervention study was conducted in secondary school students (aged 14–18) in Vojvodina, Serbia. A 21-question anonymous questionnaire was distributed to 1000 students along with parental informed consent forms. Pre-intervention survey assessed sociodemographics, physical activity and social media habits, supplement use information sources, and awareness of risks and banned substances. Based on the initial findings, an educational campaign delivered 56 short videos (≈70 s each) on Instagram and TikTok covering most frequently used supplements (e.g., creatine, proteins, caffeine, energy drinks). After, the intervention survey was repeated. The data were analyzed using the McNemar–Bowker test of symmetry. Results: In this study, 65% of Serbian secondary school adolescents reported being physically active, engaging predominantly in gym workouts and team sports. The majority of participants initiate dietary supplement use independently, without consulting healthcare professionals or adults. The most commonly used supplements were vitamins and minerals, while energy drinks ranked notably high. Social media intervention had a limited impact due to its short duration; however, certain changes were detected. Conclusions: Serbian adolescents frequently use sports supplements without adequate professional guidance. Long-term TikTok/Instagram interventions could be used in the future in order to influence behaviors and improve knowledge about sport supplement use. Full article

Caffeine is the world’s most widely consumed psychoactive compound, yet it presents a growing challenge in forensic toxicology due to the proliferation of highly concentrated energy drinks, dietary supplements, and pure anhydrous powders. This review provides a comprehensive evaluation of the diverse sources of caffeine, complex pharmacokinetics, and critical medico-legal implications. While moderate consumption is socially accepted, fatal intoxications are increasingly reported. The toxic effects of caffeine primarily manifest through a number of mechanisms which range from mild effects to sudden cardiac death. Forensic interpretation is inherently complex due to significant inter-individual variability. Analytical determination relies on advanced techniques, with LC-MS/MS serving as the preferred platform for its high sensitivity and ability to quantify primary metabolites like paraxanthine. A critical challenge in postmortem cases is postmortem redistribution, where central blood levels may be artifactually elevated compared to peripheral sites, necessitating more complex matrix analysis. To standardize the evaluation of the role of caffeine in causing death, a proposed Toxicological Significance Score (TSS) framework is discussed to enhance the transparency and defensibility of expert testimony. Ultimately, accurately determining the manner of death requires a nuanced, case-by-case assessment that integrates toxicological findings with circumstantial evidence and individual medical histories. Background/Objectives: Caffeine consumption has been reported to have beneficial effects in metabolic disorders; however, its effects on food intake are not fully elucidated. This study evaluated the impact of chronic caffeine consumption on weight gain, food intake, and metabolic parameters in C57BL/6 male mice. Methods: Eight-week-old male mice (28 animals) were divided into four groups: control (chow diet), caffeine (chow diet + 1 g/L caffeine in drinking water), high-fat diet (HFD), and HFD + caffeine (HFD + 1 g/L caffeine in drinking water). Diets and caffeine were provided ad libitum for 8 weeks. Food and water intake were recorded weekly, and blood glucose was measured every 4 weeks. After 8 weeks of diet and caffeine exposure, metabolic tests were conducted, and tissues were collected for biochemical analysis. Results: HFD consumption for 8 weeks induced an increase in body weight and adiposity compared to the chow diet, without changes in food intake. Caffeine consumption prevented body weight gain and adiposity, although it increased food intake. Caffeine also improved glucose tolerance in the HFD mouse model, without changes in random blood glucose, triglyceride, or cholesterol levels. Analysis of hypothalamic neuropeptide (Agrp, NPY, Pomc, Cart), involved in the control of food intake, showed no differences in expression. There were also no changes observed in locomotion nor in anxiety-like behavior. Conclusions: In conclusion, chronic high-fat diet (HFD) exposure induced obesity characterized by increased body weight and adiposity without altering food intake. Chronic caffeine consumption counteracted HFD-induced weight gain and fat accumulation and improved glucose tolerance, despite increasing food intake. Importantly, caffeine consumption in the HFD group did not affect locomotor activity or anxiety-like behavior, suggesting that its metabolic effects are not driven by changes in general activity or emotional state. Overall, these findings indicate that chronic caffeine consumption improves metabolic homeostasis in HFD-fed mice. Full article

Tea (Camellia sinensis L.) infusion is the second most commonly consumed drink in the world after water, valued for its sensory qualities and health-promoting properties. Tea contains a range of chemical compounds that give it specific nutritional and refreshing properties. These include alkaloids, polyphenolic compounds, carbohydrates, amino acids, enzymes, and aromatic compounds. The content of individual compounds in tea leaves is impacted by factors such as the variety, region, and cultivation method, as well as specific processing operations. The aim of the present study was to investigate the content of bioactive compounds in a selection of organic and conventional tea infusions characterized by different degrees of leaf fragmentation. The analysis of selected phenolic acids, catechins, quercetin, and caffeine in black tea and black Earl Grey tea infusions was conducted using high-performance liquid chromatography (HPLC). The study confirmed that the chemical composition of tea infusions is significantly impacted by the type of tea, cultivation practices, and form of the leaves, and revealed some previously underexplored interactions between the leaf fragmentation and cultivation system effects. From a consumer or product design perspective, organic loose-leaf Earl Grey teas appear to offer the most favourable balance of catechins, and flavonoids whereas conventional bagged black teas provide higher phenolic acid content. Caffeine is one of the most well-known biologically active compounds in coffee beans, and its content largely determines the taste and stimulating properties of the drink. However, the amount of caffeine in beans can vary significantly depending on growing conditions, even within the same coffee variety. The growing global demand for coffee and the current market dynamics emphasize the necessity to investigate how the origin of coffee beans influences beverage quality. Arabica beans, particularly the Red Bourbon variety, are known to exhibit variations in chemical composition, sensory characteristics, and technological behavior depending on their cultivation environment. The study aimed to evaluate the physicochemical and sensory properties of Arabica Red Bourbon beans sourced from distinct geographic regions, considering factors such as altitude and local environmental conditions. The sensory characteristics of the resulting beverages were evaluated using the capping method, and water activity, density, moisture content, color, pH, extractivity and caffeine content were determined. Roasted bean color ranged from 61.4 to 62.5, while ground coffee color was 72.5–75.4. Moisture content was highest in Col and R (3.4%) and lowest in Con (3.1%). The greatest moisture loss during roasting occurred in S and R (13.4%). Water activity decreased from 0.50–0.56 in green beans to 0.18–0.30 post-roasting. Extraction yield ranged from 20.03 to 21.21%, and total dissolved solids (TDS) varied at 1.23–1.30%. The least acidic sample was S (pH 5.04). Colombian beans contained unusually high caffeine. The conducted research confirmed that the geographical origin of Arabica Red Bourbon beans significantly impacts their physicochemical and sensory attributes. Variations in moisture, acidity, and caffeine content were observed among the samples, despite a consistent roasting profile. Full article

This literature review examined the relationship between energy drink consumption and cardiovascular health in young people. Following PRISMA 2020, we searched Scopus for articles published from 2020 to 2025 and included 33 original studies after screening 133 records. Evidence from observational, clinical, and experimental research was synthesized into six themes: youth consumption; direct cardiovascular outcomes; composition and toxicity; animal or cellular experiments; perceptions and habits; and occupational or sociodemographic factors. Across studies, habitual intake was linked to acute blood-pressure rises, arrhythmias, endothelial dysfunction, and metabolic disturbances, sometimes within 24 h of a single can. Risks were amplified by high caffeine and taurine doses and by co-use with alcohol or intense exercise. Adolescents and young adults were most vulnerable, due to heightened sympathetic responses, frequent use under academic or work stress, and limited risk perception. Authors highlighted five actions: longitudinal research; tighter ingredient monitoring and transparent labeling; consumer education; protection of vulnerable groups; and clinical guidance for responsible use. These results were observed across regions and study designs. Overall, the findings indicate that unregulated energy-drink consumption is a preventable cardiovascular risk in youth, justifying the use of coordinated public-health measures, including curriculum-based education, marketing restrictions, ingredient oversight, and clinical screening to mitigate harm. Caffeine is a natural alkaloid consumed primarily for its stimulant and metabolic effects. Some everyday products, such as coffee, tea, soft drinks, sports supplements, and even pain relievers, contain caffeine. However, excessive caffeine consumption, greater than 400 mg per day, can cause adverse effects. Therefore, this work presents an electrochemical sensor based on a molecularly imprinted polymer (MIP) electropolymerized on gold nanoparticles functionalized with p-aminothiophenol (AuNPs-pATP) for caffeine quantification. AuNPs-pATP synthesized show a spherical morphology with an average diameter of 2.54 nm. Stages of MIP formation were monitored by cyclic voltammetry (CV) and electrochemical impedance spectroscopy (EIS) using a potassium ferrocyanide redox probe, where the following were observed: (i) an increase in conductivity upon modification of the GCE with AuNPs-pATP, (ii) the blocking of active sites during the electropolymerization step, and (iii) the release of specific cavities upon template removal, revealing consistent differences between the MIP and the control polymer (NIP). SEM images revealed three-dimensional spherical cavities on MIP surface, while the NIP showed a more compact rough surface. Caffeine quantification was performed using square wave voltammetry (SWV) with LOD of 0.195 µmol L−1 and LOQ of 0.592 µmol L−1. Interference studies indicated high selectivity and a high density of caffeine-specific binding sites in the MIP. Additionally, MIP sensor demonstrated reusability, good reproducibility, and stability, as well as promising results for analysis in soft drink and sports supplement samples. Full article

In left ventricular hypertrophy (LVH), the combined external administration of hydrogen sulfide (H2S) and nitric oxide (NO) has been shown to reverse LVH by activating the endothelial nitric oxide synthase pathway (eNOS/NO), independent of the cystathionine γ-lyase (CSE/H2S) pathway. Individually, both H2S and NO have also been reported to significantly improve RCBP, restore renal excretory performance, and enhance α-adrenergic receptor responsiveness in rats. The induction of LVH was performed over a period of two weeks using drinking water with caffeine and isoprenaline. Five weeks later, the rats were fed with L-arginine (1.25 g/L) as a nitrogen oxide donor. Vascular reactions to methoxamine, phenylephrine, and noradrenaline were assessed in presences and absence of 5-methylurapidil (5-MeU), BMY7378, and chloroethylclonidine (CeC) and α1-adrenoceptor antagonists. In both the Control WKY and LVH-WKY groups, combined H2S+NO therapy significantly (p < 0.05) upregulated the renal mRNA of CSE and eNOS when compared with untreated LVH rats. The treatment also markedly increased RCBP in LVH-H2S+NO rats relative to LVH controls. Furthermore, H2S+NO administration enhanced the activity of α1A, α1B, and α1D adrenergic receptors in mediating renal vasoconstriction. Even under receptor blockade with high doses (HDs) of 5-MeU, CeC, and BMY 7378, renal vasoconstriction responses to adrenergic agonists like NA, PE, and ME in the LVH-H2S+NO group remained comparable to those observed in the counterpart Control-H2S+NO group. The findings of current study suggest that simultaneous exogenous administration of H2S and NO donors improve renal cortical blood flow, support renal function, and augment α1A, α1B, and α1D adrenergic receptor responsiveness to adrenergic agonists like NA, PE, and ME in LVH rats. This effect appears to rely primarily on the eNOS/NO pathway, with partial contribution from the CSE/H2S pathway. Full article

High-intensity functional training (HIFT) has grown in popularity in the past several decades, yet previous research has largely focused on the dietary habits and body composition of elite HIFT athletes and utilized only quantitative study designs, potentially limiting our understanding of typical HIFT athletes. This study aimed to comprehensively describe the common dietary habits and body composition of HIFT athletes. Data were only analyzed descriptively. Among 62 HIFT athletes (age: 36 ± 11.7 years), we estimated body fat percentage (BF%) using a Siri 3-compartment model, and we assessed dietary habits, dietary supplement (DS) use, and open-response rationales for DS use/disuse via an online questionnaire. Qualitative data from open-response questions were coded and grouped via inductive thematic analysis. Body composition varied among both male (n = 36, BF% = 6.5–27.6%) and female participants (n = 26, BF% = 10.6–37.6%). Most participants reported regular consumption of lean meats and home-cooked meals, yet few participants (~20%) regularly consumed the recommended twice daily servings of dairy, fruits, vegetables, and whole grains. Most (77.4%) HIFT athletes reported DS use, with the average HIFT athlete using approximately six DS; dairy protein, creatine, caffeine, and electrolyte drinks were the most reported DS. Improving health, recovery, and nutrient intake were common reasons for using DS, whereas a lack of noticeable results was the most common reason for discontinuation. Some HIFT athletes may rely on DS to address nutrient gaps rather than whole foods. Full article

Background: In Poland, the consumption of energy drinks among young people has changed significantly following the introduction of a ban on sales to minors. This regulatory measure was intended to address growing concerns about the health effects of high caffeine consumption among teenagers. The aim of the study was to assess the frequency, preferences, and determinants of energy drink consumption among Polish adolescents aged 15–17 years, following the introduction of the legal ban on the sale of energy drinks to minors. Methods: The study was conducted in Poland in 2024, following the introduction of a law prohibiting the sale of energy drinks to minors. The study group consisted of 999 high school students aged 15–17 who completed an anonymous, author-designed survey. Data were then collected and analyzed for descriptive statistics, and chi-square tests were used for categorical variables, Mann–Whitney U tests and Kruskal–Wallis tests for group comparisons, and Spearman’s rank correlation for ordinal data. Results: In total, 52% of respondents declared that they consume energy drinks, and 68% reported a reduced intake after the introduction of the regulations prohibiting sales to minors. These drinks are more frequently consumed by males. Additionally, age and frequency of consumption were positively correlated. The factors most often influencing the choice of energy drinks were price, taste, package size, caffeine content, composition, and recommendations from friends. Conclusions: Although a large percentage of minors still consume energy drinks, the new regulations have had an impact on reducing their intake. Full article

Background/Objectives: The rising prevalence of mental health disorders among adolescents underscores the need for early intervention. Of concern is the increasing consumption of sugar-sweetened beverages (SSBs) and high-caffeine drinks, which may adversely affect adolescent mental health. This study examined their association with stress perception, depressive symptoms, suicidal ideation, and loneliness among Korean adolescents. Methods: Data were derived from the 2022 Korea Youth Risk Behavior Survey, a nationally representative dataset comprising 51,850 Korean adolescents aged 12–18 years. Beverage consumption frequency was categorized into three groups: none, 1–4 times per week, and ≥5–6 times per week. Logistic regression models were employed to estimate adjusted odds ratios (AORs) and 95% confidence intervals (CIs) for mental health conditions, adjusting for potential confounders. Results: Compared with non-consumers, adolescents who consumed SSBs ≥5–6 times per week exhibited significantly higher odds of stress perception (AOR 1.35, 95% CI 1.24–1.46), depressive symptoms (AOR 1.32, 95% CI 1.19–1.46), suicidal ideation (AOR 1.23, 95% CI 1.09–1.39), and loneliness (AOR 1.44, 95% CI 1.28–1.61). Similarly, frequent consumption of high-caffeine drinks (≥5–6 times per week) was associated with increased odds of stress perception (AOR 2.13, 95% CI 1.99–2.29), depressive symptoms (AOR 1.75, 95% CI 1.62–1.88), suicidal ideation (AOR 2.04, 95% CI 1.86–2.24), and loneliness (AOR 1.72, 95% CI 1.59–1.87). Conclusions: Frequent consumption of SSBs and high-caffeine drinks is significantly associated with adverse mental health outcomes among Korean adolescents. Given the increasing intake of these beverages, targeted public health interventions are warranted to mitigate their potential negative impact on adolescent mental well-being. Full article

Functional beverages comprise a special category of drinks free of alcohol that contain bioactive components from plant, animal, marine, or microorganism sources that contribute to the reinforcement of human health. Functional beverages are mainly divided into the following basic categories: (i) dairy-based beverages and (ii) non-dairy-based beverages. Functional beverages have several positive functional properties such as the rehydration of the body, recovery of lost energy, the increase of athletic performance, the prevention of pain in joints, the improvement of heart health, the improvement of immunity and the digestive system, and the creation of the feeling of satiety and boosting mood. However, according to health experts, there are also functional beverages that induce obesity and heart diseases because of their high content of sugars, sweeteners, and other components such as caffeine, taurine, taurine combined with caffeine, creatinine, etc. The scope of this review was to highlight the main components and the functional properties of energy drinks along with the effects of functional beverages on human health. Limited review articles address this overall hypothesis in the recent literature, thus comprising the significance of the current study. Full article

Quantification of Caffeine in Energy and Cola Drinks via Rapid High Performance Liquid Chromatography Assays with Ultra Violet Diode Array Detection

There is currently a lack of regulation of the caffeine found in cola and energy drinks by the FDA, which fails to protect the consumers of these products. Due to this lack of regulation, cola and energy drinks can have noticeable differences in their caffeine content when compared to the average amount per serving labelled on the product. In this study, we demonstrate the ability to analyse caffeine rapidly in under 20 s, and with HPLC pressures under 3500 psi (241 bar). To facilitate a high-throughput routine HPLC analysis of the caffeine content found in energy and cola drinks, two HPLC column technologies are studied, a conventional run HPLC column, and a newly commercialised Radial Flow Splitting end fitted HPLC column. The Radial Flow Splitting fitted column demonstrated the following benefits: a 37% reduction in pressure, an increased signal intensity sensitivity of 35%, a reduced analysis time by 20%, and improved metrics in assay precision based on triplicate injections associated with retention time, peak area, and peak height precision %RSD values. Both rapid HPLC methods offer greater opportunity for expanded beverage testing, which can ultimately help protect the consumer. The quantified energy drinks that were tested had a higher caffeine content, on average, than the labelled caffeine content, with an approximately ±16 mg difference per serving size for the energy drinks. In the case of the cola drinks, which did not include caffeine levels on the food label, we compared the levels to the USDA guidance and found up to double the recommended amount of caffeine in one serving for the samples studied. This highlights the need to have stricter regulations for caffeinated beverages to protect consumers and provide transparency regarding the caffeine content. Full article

Background/Objectives: The high consumption rate of energy drinks among pupils is a serious public health concern in various countries, including South Africa. Excessive consumption of energy drinks that contain elevated caffeine and sugar levels has the potential to lead to the development of addictions, strokes, dehydration, sleeping disorders, mental health and central nervous disorders, hypertension, digestive problems, and anxiety. Most pupils regard energy drinks as regular soft drinks and lack knowledge of the active ingredients contained in energy drinks and their side effects. The objective of this study was to investigate factors influencing energy drink usage amongst pupils in the Mahikeng sub-district, Northwest Province. Methods: A quantitative cross-sectional survey was conducted amongst 505 pupils in the Mahikeng sub-district, Northwest, using self-administered questionnaires. Data were analysed using STATA software version 18 to examine associations between variables. Results: The energy drinks consumed most by pupils were Dragon (38.21%), Switch (28.97%), and Red Bull (14.62%). Factors and reasons influencing energy drink usage among pupils include all-night parties (3.1%), concentration (20.3%), being awake (43.1%), curiosity (2.2%), energy levels (23.1%), exams (13.8%), sports (8.7%), fatigue (6.9%), and health (2.3%). There was a strong association (p ≤ 0.05) between energy drink usage and sports activities amongst pupils. Conclusions: It is concluded that health education and promotion intervention programmes are required to educate pupils about the dangers of energy drink usage to prevent public health risks. Further studies, including research on primary school pupils, are necessary, considering that a substantial number of pupils were exposed to energy drinks at an early age. Full article
