Background: Canine cranial cruciate ligament (CCL) rupture is a common orthopedic condition leading to stifle joint dysfunction, discomfort, and reduced mobility. Diagnosis typically involves radiography, computed tomography (CT), and magnetic resonance imaging (MRI). In this study, we conducted a retrospective analysis to evaluate the use of infrared thermography in assessing local temperature and thermal patterns in dogs with acute-onset lameness due to CCL rupture compared to those with intact ligaments. Methods: The study involved 12 dogs with cranial cruciate ligament rupture and nine dogs with intact ligaments. The stifle area of all dogs was clipped and scanned using a FLIR E50 thermographic camera. Two regions of interest (ROI), designated El1 and Bx1, were analyzed with FLIR Tools software 5.X by comparing the average of the maximum and of the mean temperature values between the two groups. Results: Thermal imaging revealed differences between the two groups of dogs, which were further supported by significantly higher temperatures in the El1 (lateral aspect of the stifle joint) and Bx1 (cranial aspect of the stifle joint) areas in the study group compared to the control group using a comparative analysis—two-sample t-test. In the El1 area, the study group showed a temperature increase of 1.8 °C compared to the control group, while in the Bx1 area, the difference was 1.76 °C. Conclusions: Infrared thermography shows potential to differentiate dogs with acute-onset lameness due to CCL rupture from dogs with intact ligaments, but further studies are needed to assess its accuracy in distinguishing it from other stifle pathologies. Background: Understanding the mechanisms underlying dental pain caused by pulpitis in humans has led to the development of animal models, such as the rat, which enable the study of the mechanisms underlying inflammation; the use of these models is considered ethically justified when the anticipated scientific benefits outweigh the potential impacts on animals in the harm/benefit balance. Objective: To develop a rat model of mechanically induced pulpitis and to evaluate the potential impact on animal well-being. Methods: Pulpitis was mechanically induced in male Lewis rats (13–16 weeks, 350–400 g) which were anesthetized and endotracheally intubated. Following pulp exposure, the cavity was sealed with either amalgam (n = 10) or zinc phosphate cement (n = 10). Following recovery and return to their housing, behavioral assessments and histological evaluations using Hematoxylin and Eosin (H&E) staining were conducted in separate cohorts at two time points: 3 h and 5 days following the procedure. Results: A standardized model of mechanically induced pulpitis was established and verified clinically and by histopathological analysis, which showed evidence of the inflammatory process and revealed no statistically significant differences in the scoring of pain, discomfort, or distress, nor in the measurements of food and water consumption or body weight. Conclusions: The behavioral assessments conducted in this study supported the implementation of a safe and easily reproducible model for future research aimed at elucidating the mechanisms underlying pulp inflammation. Background: Local anesthesia is essential for most dental procedures, but its parenteral administration is often painful. Topical anesthetics are commonly used to minimize local anesthesia pain; however, commercial formulations fail to fully prevent the discomfort of local anesthetic injection. Methods: We developed and characterized a novel lidocaine and epinephrine co-loaded liquid crystalline precursor system (LCPS) for topical anesthesia. The formulation was structurally characterized using polarized light microscopy (PLM) and small-angle X-ray scattering (SAXS). Rheological behavior was assessed through continuous and oscillatory rheological analyses. Texture profile analysis, in vitro mucoadhesive force evaluation, in vitro drug release and permeation studies, and an in vivo toxicity assay using the chicken chorioallantoic membrane (CAM) model were also conducted. Results: PLM and SAXS confirmed the transition of the LCPS from a microemulsion to a lamellar liquid crystalline structure upon contact with artificial saliva. This transition enhanced formulation consistency by over 100 times and tripled mucoadhesion strength. The LCPS also provided controlled drug release, reducing permeation flow by 93% compared to the commercial formulation. Importantly, the CAM assay indicated that the LCPS exhibited similar toxicity to the commercial product. Conclusions: The developed LCPS demonstrated promising physicochemical and biological properties for topical anesthesia, including enhanced mucoadhesion, controlled drug delivery, and acceptable biocompatibility. These findings support its potential for in vivo application and future clinical use to reduce pain during dental anesthesia procedures. Full article

Globally, endometriosis affects almost 10% of reproductive-aged women, leading to chronic pain and discomfort. Endocrine-disrupting compounds (EDCs) seem to play a pivotal role as a causal factor. The current manuscript aims to explain potential molecular pathways, synthesize current evidence regarding EDCs as causative agents of endometriosis, and highlight implications in the general population and clinical work. A thorough review of experimental, epidemiologic, and mechanistic research studies was conducted to explain the association between EDCs and endometriosis. Among the primary EDCs under investigation are polychlorinated biphenyls, dioxins, phthalates, and bisphenol A (BPA). Despite methodological heterogeneity and some discrepancies, epidemiologic evidence supports a positive association between some increased levels of BPA, phthalates, and dioxins in urine or in blood, and endometriosis. Experiments support some effect of EDCs on endometrial cells and causing endometriosis. EDCs function as xenoestrogens, alter immune function, induce oxidative stress, and disrupt progesterone signaling. Epigenetic reprogramming may play a role in mediating EDC-induced endometriosis. Endocrine, immunological, and epigenetic pathways link EDCs and endometriosis. Prevention techniques require deeper comprehension of those factors. Causal linkages and possible treatment targets should be based on longitudinal studies and multi-omics techniques. Restriction of EDCs could be beneficial for endometriosis prevalence limitation. This study investigates the influence of façade-design parameters on daylighting performance in hot arid climates, with a particular focus on Egypt. A total of nine façade configurations of a residential building were modeled and simulated using Autodesk Revit and Insight 360, varying three critical variables: glazing type (clear, blue, and dark), Window-to-Wall Ratio (WWR) of 15%, 50%, 75%, and indoor wall finish (light, moderate, dark) colors. These were compared to the Leadership in Energy and Environmental Design (LEED) daylighting quality thresholds. The results revealed that clear glazing paired with high WWR (75%) achieved the highest Spatial Daylight Autonomy (sDA), reaching up to 92% in living spaces. However, this also led to elevated Annual Sunlight Exposure (ASE), with peak values of 53%, exceeding the LEED discomfort threshold of 10%. Blue and dark glazing types successfully reduced ASE to as low as 0–13%, yet often resulted in underlit spaces, especially in private rooms such as bedrooms and bathrooms, with sDA values falling below 20%. A 50% WWR emerged as the optimal balance, providing consistent daylight distribution while maintaining ASE within acceptable limits (≤33%). Similarly, moderate color wall finishes delivered the most balanced lighting performance, enhancing sDA by up to 30% while controlling reflective glare. Statistical analysis using Pearson correlation revealed a strong positive relationship between sDA and ASE (r = 0.84) in highly glazed, clear glass scenarios. Sensitivity analysis further indicated that low WWR configurations of 15% were highly influenced by glazing and finishing types, leading to variability in daylight metrics reaching ±40%. The study concludes that moderate glazing (blue), medium WWR (50%), and moderate color indoor finishes provide the most robust daylighting performance across diverse room types. These findings support an evidence-based approach to façade design, promoting visual comfort, daylight quality, and sustainable building practices. Full article

This study examines how Ontario’s Health and Physical Education curriculum addresses the needs of young adults with disabilities. A total of 54 individuals aged 18–35 years old with developmental, learning, or physical disabilities who had completed secondary school in Ontario participated in a cross-sectional mixed-methods survey. Participants were recruited through disability-focused community networks and a university psychology participant pool. They completed the Sex Education subscale of the Sexual Knowledge, Experience, Feelings and Needs Scale, a 35-item sexual knowledge questionnaire, and open-ended questions. Quantitative data were analyzed using descriptive statistics and independent samples t-tests; qualitative responses were examined using thematic analysis. Participants reported limited factual knowledge, minimal classroom representation, and heavy reliance on independent learning. Barriers included inaccessible materials, teacher discomfort, and the absence of disability narratives in sexuality units. Findings point to three priorities: revising curriculum content, expanding educator training, and creating disability-affirming resources. These measures will help ensure comprehensive and rights-based sexuality education that supports the autonomy and well-being of students with disabilities. Full article

Background: This systematic review investigates the role of virtual reality (VR)-based multisensory cognitive training in cognitive function, executive function and wayfinding ability among people diagnosed with mild cognitive impairment (MCI) and Alzheimer’s disease (AD). Methods: The review was carried out using PRISMA guidelines. PubMed, Scopus, Embase, and Google Scholar were searched up from inception to February 2025 using terms related to MCI, AD, VR, and cognitive functions. Studies were included if they involved participants with MCI or early AD, used VR-based training, collected baseline data, and reported cognitive outcomes. Results: Nine studies with MCI were included, but no eligible studies focused on AD. Seven out of nine eligible studies in MCI reported significant improvements in global cognitive function (MoCA, CERAD-K, MMSE). Some studies showed improvements in executive function (EXIT-25, TMT-A/B, and SCWT), while others found no significant differences. One study reported improved depression/mental status (GDS, MOSES, QoL-AD). Just one study reported improvement in functional ability (IADL). One study reported enhanced cognition and reduced discomfort (SSQ). VR programs were generally well-tolerated, with no significant adverse events reported. Conclusions: VR shows promise for improving cognitive function in MCI. VR also showed potential benefits in executive function and psychological outcomes like depression and quality of life, though consistency varied. Full article

In this report of two cases, we describe two patients with spinal involvement of gout. The first case involved a 67-year-old female who presented to the emergency department with a one-week history of weakness in both the upper and lower limbs, despite no prior history of gout. Cervical spine MRI revealed spinal cord compression at the C4 level from a posterior lesion. During surgery, chalky white deposits consistent with gouty tophi were observed in the ligamentum flavum within the epidural space at C4. These intraoperative findings correlated with elevated serum uric acid levels. The second case concerned a 68-year-old male who presented with a five-day history of right lower limb pain along with bilateral knee discomfort. Radiologic and laboratory evaluations revealed elevated inflammatory markers, negatively birefringent crystals in knee joint aspirate, spondylodiscitis at the L5-S1 level, and a right-sided synovial cyst at the T10–T11 level causing spinal cord compression. Following the initiation of anti-gout therapy, the patient experienced significant clinical improvement, normalization of inflammatory markers, and radiologic resolution of the thoracic synovial cyst. Demand Side Management (DSM) plays a crucial role in modern energy systems, enabling more efficient use of energy resources and contributing to the sustainability of the power grid. This study examines DSM strategies within a multi-environment context encompassing residential, commercial, and industrial sectors, with a focus on diverse appliance types that exhibit distinct operational characteristics and user preferences. Initially, a single-objective optimization approach using Genetic Algorithms (GAs) is employed to minimize the total energy cost under a real Time-of-Use (ToU) pricing scheme. This heuristic method allows for the effective scheduling of appliance operations while factoring in their unique characteristics such as power consumption, usage duration, and user-defined operational flexibility. This study extends the optimization problem to a multi-objective framework that incorporates the minimization of CO2 emissions under a real annual energy mix while also accounting for user discomfort. The Non-dominated Sorting Genetic Algorithm II (NSGA-II) is utilized for this purpose, providing a Pareto-optimal set of solutions that balances these competing objectives. The inclusion of multiple objectives ensures a comprehensive assessment of DSM strategies, aiming to reduce environmental impact and enhance user satisfaction. Additionally, this study monitors the Peak-to-Average Ratio (PAR) to evaluate the impact of DSM strategies on load balancing and grid stability. It also analyzes the impact of considering different periods of the year with the associated ToU hourly schedule and CO2 emissions hourly profile. A key innovation of this research is the integration of detailed, category-specific metrics that enable the disaggregation of costs, emissions, and user discomfort across residential, commercial, and industrial appliances. This granularity enables stakeholders to implement tailored strategies that align with specific operational goals and regulatory compliance. Also, the emphasis on a user discomfort indicator allows us to explore the flexibility available in such DSM mechanisms. The results demonstrate the effectiveness of the proposed multi-objective optimization approach in achieving significant cost savings that may reach 20% for industrial applications, while the order of magnitude of the trade-offs involved in terms of emissions reduction, improvement in discomfort, and PAR reduction is quantified for different frameworks. The outcomes not only underscore the efficacy of applying advanced optimization frameworks to real-world problems but also point to pathways for future research in smart energy management. This comprehensive analysis highlights the potential of advanced DSM techniques to enhance the sustainability and resilience of energy systems while also offering valuable policy implications. Oral health during pregnancy is essential for maternal and child well-being, as hormonal and physiological changes increase women’s susceptibility to oral diseases. Despite the recognized importance of prenatal dental care, adherence to dental services remains a challenge in the public health context. This study aimed to analyze oral health and the use of dental services during pregnancy through the perception of pregnant women. It represents the qualitative phase of a mixed-method study conducted with 25 pregnant women (with and without dental care) receiving prenatal care in the Brazilian Unified Health System (SUS). Participants were selected through saturation sampling, and data were collected via semi-structured interviews, followed by content analysis. The findings revealed four major themes: barriers and facilitators to dental care, changes during pregnancy and oral health. Discomfort from oral changes was a common concern. Barriers included misinformation, fear, cultural beliefs, and service organization. In contrast, facilitating factors were identified, such as care prioritization, support from healthcare teams, health education, and access through SUS. This study concludes that emotional, cultural, and contextual aspects shape the use of dental services during pregnancy. Access through SUS is perceived as an important facilitator, which simultaneously presents organizational weaknesses that need to be addressed. Full article

The endoscopic capsule is a miniaturized device used for medical diagnosis, which is less invasive compared to traditional gastrointestinal endoscopy and can reduce patient discomfort. However, it faces challenges in communication transmission, such as high power consumption, serious signal interference, and low data transmission rate. To address these issues, this paper proposes an optimized modulation scheme that is low-cost, low-power, and robust in harsh environments, aiming to improve its transmission rate. The scheme is analyzed in terms of the in-body channel. The analysis and discussion for the scheme in wireless body area networks (WBANs) are divided into three aspects: bit error rate (BER) performance, energy efficiency (EE), and spectrum efficiency (SE), and complexity. These correspond to the following issues: transmission rate, communication quality, and low power consumption. The results demonstrate that the optimized scheme is more suitable for improving the communication performance of endoscopic capsules. Full article

Road irregularities generate vibrations that are transmitted to cyclists’ hands. This paper describes a purpose-designed laboratory setup and data processing method to assess vibration-induced numbness and discomfort. The rear wheel of a road bike was coupled with a smart trainer for indoor cycling, while the front wheel was supported by a vibrating platform to simulate road–bike interaction. The vibrotactile perception threshold (VPT) is measured in the fingers, and a questionnaire was used to assess the discomfort in different parts of the hand using a unipolar scale. To validate the method, ten male volunteers underwent two one-hour cycling sessions, one for each of the two handlebar designs tested. VPT was measured in the index and little fingers of the right hand at 8 and 31.5 Hz before and after each session, while the discomfort questionnaire was completed at the end of each session. The discomfort scores showed a strong inter-subject variability, indicating the necessity to combine them with the objective measurements of the VPT, which is shown to be sensitive in identifying the perception shift due to vibration exposure and the differences between the fingers. This study demonstrates the effectiveness of the proposed method for assessing hand numbness and discomfort in cyclists. Full article

Hemorrhoidal disease remains a prevalent yet often overlooked condition, affecting millions worldwide and imposing a substantial healthcare burden. Despite the availability of multiple treatment options, gaps persist in patient education, early symptom recognition, and optimal treatment selection. Recent advancements are evolving the pharmacist’s role in hemorrhoid management beyond traditional over-the-counter (OTC) and prescription approaches. The 2024 American Society of Colon and Rectal Surgeons (ASCRS) guidelines introduce updates on the use of phlebotonics, a class of venoactive drugs gaining recognition for their role in symptom management, yet largely underutilized in U.S. clinical practice. In parallel, novel clinical tools are reshaping how pharmacists engage in assessment and care. The integration of digital decision-support platforms and structured evaluation algorithms now empowers them to systematically evaluate symptoms, identify red flag signs, and optimize patient triage. These tools reduce diagnostic variability and improve decision-making accuracy. Given their accessibility and trusted role in frontline healthcare, pharmacists are well-positioned to bridge these critical gaps by adopting emerging treatment recommendations, leveraging algorithm-driven assessments, and reinforcing best practices in patient education and referral. This narrative review aims to equip pharmacists with updated insights into evidence-based hemorrhoid management strategies and provide them with structured assessment algorithms to standardize symptom evaluation and treatment pathways. By integrating these innovations, pharmacists can enhance treatment outcomes, promote patient safety, and contribute to improved quality of life (QoL) for individuals suffering from hemorrhoidal disease. Background/Objective: Puberphonia is a voice disorder characterized by the persistence of a high-pitched voice in sexually mature males. In phoniatrics and speech-language pathology, it is also known as post-mutational voice instability, mutational falsetto, persistent fistulous voice, or functional falsetto. The absence of an age-appropriate vocal pitch may adversely affect psychological well-being and hinder personal, social, and occupational functioning. The aim of this study was to evaluate of the impact of phoniatric and logopedic rehabilitation on voice quality in patients with puberphonia. Methods: The study included 18 male patients, aged 16 to 34 years, rehabilitated for voice mutation disorders. Phoniatric and logopedic rehabilitation included voice therapy tailored to each subject. A logopedist led exercises aimed at lowering and stabilizing the pitch of the voice and improving its quality. A phoniatrician supervised the therapy, monitoring the condition of the vocal apparatus and providing additional diagnostic and therapeutic recommendations as needed. The duration and intensity of the therapy were adjusted for each patient. Before and after voice rehabilitation, the subjects completed the following questionnaires: the Voice Handicap Index (VHI), the Vocal Tract Discomfort (VTD) scale, and the Voice-Related Quality of Life (V-RQOL). They also underwent an acoustic voice analysis. Results: Statistical analysis of the VHI, VTD, and V-RQOL scores, as well as the voice’s acoustic parameters, showed statistically significant differences before and after rehabilitation (p < 0.005). Conclusions: Phoniatric and logopedic rehabilitation is an effective method of reducing and maintaining a stable, euphonic male voice in patients with functional puberphonia. Effective voice therapy positively impacts selected aspects of psychosocial functioning reported by patients, improves voice-related quality of life, and reduces physical discomfort in the vocal tract. Infrared thermography is a common approach used in building inspection for identifying building envelope thermal anomalies that cause energy loss and occupant thermal discomfort. Detecting these anomalies is essential to improve the thermal performance of energy-inefficient buildings through energy retrofit design and correspondingly reduce operational energy costs and environmental impacts. A thermal bridge is an unwanted conductive heat transfer. On the other hand, an infiltration/exfiltration anomaly is an uncontrollable convective heat transfer, typically happening around windows and doors, but it can also be due to a defect that comprises a building envelope’s integrity. While the existing literature underscores the significance of automatic thermal anomaly identification and offers insights into automated methodologies, there is a notable gap in addressing an automated workflow that leverages building envelope component segmentation for enhanced detection accuracy. Consequently, an automatic thermal anomaly identification workflow from visible and thermal images was developed to test it, utilizing segmented building envelope information compared to a workflow without any semantic segmentation. Therefore, building envelope images (e.g., walls and windows) were segmented based on a U-Net architecture compared to a more conventional semantic segmentation approach. The results were discussed to better understand the importance of the availability of training data and for scaling the workflow. Then, thermal anomaly thresholds for different target domains were detected using probability distributions. Finally, thermal anomaly masks of those domains were computed. This study conducted a comprehensive examination of a campus building in Syracuse, New York, utilizing a drone-based data collection approach. The case study successfully detected diverse thermal anomalies associated with various envelope components. The proposed approach offers the potential for immediate and accurate in situ thermal anomaly detection in building inspections. Full article
