This computational study examined how variations in the seatback angle of two generic child restraint systems (CRSs) affect spinal loading in young occupants (1.5 YO and 3 YO) during frontal impacts, performed according to the specifications included in UNECE R129. CRS seatback angle dictates torso recline, which in turn influences head, chest, and spine kinematics and loading. While manufacturers typically recommend 30–45° for rear-facing CRSs and an upright position for forward-facing CRSs, little is known about the biomechanical implications of deviating from these guidelines. Using PIPER human body models representing a 1.5-year-old in a rear-facing CRS and a 3-year-old in a forward-facing CRS, simulations were performed under UN-R129 frontal impact conditions. The seatbacks were rotated 5° and 10° more upright or reclined relative to the nominal angle, with occupants restrained by a five-point harness and CRSs secured with ISOFIX, top tether, or three-point belt. The results showed that reclined configurations generally increased the predictions of spinal loading (forces and/or moments) given by the PIPER model, while nominal or more upright angles reduced loads, particularly in the lumbar spine of the 3-year-old model. Overall, the study highlights how computational tools can guide CRS design improvements to optimize spinal protection and enhance child safety beyond current regulatory requirements. Full article

This study presents the results of an experimental evaluation of ride comfort for children transported in child restraint systems (CRS) during passages over speed bumps, with particular emphasis on the energy contained in vibrations. The tests were carried out under real operating conditions using two vehicles with different suspension characteristics and three loading levels corresponding to different stages of child development. Vertical accelerations were recorded at key points of the vehicle–seat system and subsequently analyzed in accordance with ISO 2631-1. Based on the vibration signals, root mean square acceleration (RMS), vibration dose value (VDV), seat effective amplitude transmissibility (SEAT), and root mean quad (RMQ) indices were calculated, enabling not only the assessment of discomfort levels but also the estimation of mechanical energy transmitted through the seat structure. The results showed that, depending on the type of vehicle, bump geometry, and load mass, the vibration energy can be significant and, in many cases, corresponds to levels classified as “severe” or “extreme discomfort.” At the same time, this energy constitutes a potential power source for low-power sensors in “smart seat” systems, such as those monitoring the child’s posture or environmental conditions. The findings highlight the need to consider vibration comfort criteria and the potential for vibration energy harvesting in the design and homologation of CRS, which aligns with the concept of sustainable transport and the development of energy self-sufficient technologies. This study aims to compare the restraint performance of two child restraint systems (CRSs) used on airplanes—a rear-facing child seat (RFCS) and the child aviation restraint system (CARES)—for 1.5-year-old children, along with their compatibility with different types of aircraft seats. 16 g longitudinal dynamic tests were conducted on two types of aircraft seats with CRSs. Results indicate poor compatibility of CARES with Type A seats, significantly increasing the risk of head, neck, and abdominal injuries, with Nij exceeding the acceptable limit. In contrast, CARES exhibited good compatibility with Type B seats and effectively protected children. RFCS tests demonstrated effective injury risk reduction on both types of seats. It can be found that the performance of CARES depends on restraint status and seat dimensions; RFCSs provide adequate protection for 1.5-year-olds. Optimal protection can be achieved with smaller restrain angles of CRS and using thinner seat cushions. Compared to CARES, RFCSs better adapt to various aircraft seat structures, offering superior child protection. Full article

The overall objective of the current study was to investigate the behaviours and knowledge of parents/carers in relation to safe child occupant travel in the Emirate of Dubai in the United Arab Emirates (UAE). A community survey was completed by 786 participants who were responsible for the safety of 1614 children (aged 10 years and younger). The survey included questions related to the type, frequency and appropriateness of restraint use for their eldest child. Overall, 24 percent of participants reported that they ‘never/almost never’ restrained their eldest child while travelling in a motor vehicle, with this proportion increasing with child age. For example, though 89 percent of participants reported that they restrained their infants (<1 year) in an ‘appropriate’ restraint for their age, this rate was much lower for children aged between 5 and 7 years (10%). Overall, these findings suggest that a large proportion of child occupants, especially those aged five years and older, are not appropriately restrained in vehicles, and therefore are at an increased risk of death or serious injury in the event of a crash. Future research will validate this self-reported child restraint use data with objective data from observations of real-world child restraint use behaviour in the UAE. Full article

Background: Child road traffic injuries are a major global public health problem and the issue is particularly burdensome in middle-income countries such as South Africa where injury death rates are 41 per 100,000 for under 5′s and 24.5 per 100,000 for 5–14-year-old. Despite their known effectiveness in reducing injuries amongst children, the rates of use of child restraint systems (CRS) remains low in South Africa. Little is known about barriers to child restraint use especially in low- and middle-income countries. Methods: We carried out observation studies and parent/carer surveys in 7 suburbs of Cape Town over a three month period to assess usage rates and explore the knowledge and perceptions of parents towards child restraint legislation, ownership and cost; Results: Only 7.8% of child passengers were observed to be properly restrained in a CRS with driver seatbelt use and single child occupancy being associated with higher child restraint use. 92% of survey respondents claimed to have knowledge of current child restraint legislation, however, only 32% of those parents/carers were able to correctly identify the age requirements and penalty. Reasons given for not owning a child seat included high cost and the belief that seatbelts were a suitable alternative. Conclusions: These findings indicate the need for a tighter legislation with an increased fine paired with enhanced enforcement of both adult seatbelt and child restraint use. The provision of low-cost/subsidised CRS or borrowing schemes and targeted social marketing through online fora, well baby clinics, early learning centres would be beneficial in increasing ownership and use of CRS. The child restraint legislation in Malaysia becomes mandatory from 1 January 2020. Prior to commencement of the rule, a survey showed that only 36% of Malaysian parents were aware of the importance of a child restraint system (CRS) and only 27% usage was reported during travel. The Malaysian Institute of Road Safety report showed that children transported in private vehicles were the leading groups of casualties among children aged 1 to 4 years old (43.8%) and 5 to 9 years old (30.2%), respectively. We performed a narrative review using the PubMed, ScienceDirect and Google Scholar databases using keywords such as child restraint system, unrestrained injuries, Malaysia and epidemiology. The objectives of this review were: (1) to determine the prevalence on the use of CRS in Malaysia, (2) to evaluate the injuries related to unrestrained children and (3) to show the nation’s preparation towards implementation of the child restraint law. Six papers on prevalence, one paper on injury and six mainstream newspaper were included in this study. The prevalence of a CRS use was shown between 5% to 41.8%. In relation to injury, the only publication from this country showed that among 19 children involved in a car crash, five (26.3%) children had non-craniomaxillofacial (CMF) injuries, ten (52.6%) with CMF injuries only, two (10.5%) with both CMF and non-CMF injuries and two (10.5%) without any injury. Overall, the Injury Severity Score (ISS) range was between 0 to 13 (median, 1.00; interquartile range, 1). Preparation to comply with the best practice of the child restraint law is still ongoing, especially those addressing the issues related to the low-income parents in the country. Due to scarcity of publication and data on the CRS use and injuries related to its non-usage, it is advocated that parallel with the implementation legislation, vigorous forms of public education as well as good data management must be performed and monitored regularly by the road safety authority in this country. This study presents a comparison of the common Child Restraint Systems (CRS) which reduces the value of dynamic loads affecting the child’s body during car accidents. The analyzed systems were: child seats, booster seats, and straps—adjustable vehicle seat belts adapted to children’s stature. The effectiveness of the analyzed devices was assessed on the basis of experimental tests carried out in the accredited laboratory approving the Child Restraint Systems. The tests were carried out in accordance with the new Regulation No. 129 UN/ECE. The authors examined whether the tested devices meet the guidelines of the new Regulations No. 129 despite approval in accordance with Regulation No. 44. Based on the research results, better safety parameters of some new solutions dedicated to children’s safety could be observed. Almost all the selected CRS met requirements stated in Regulation No. 129. The only exception was for abdominal pressure in one of the tested devices. Head Resultant Acceleration in tested devices was 14.7–39.0% less than the limit determined in Regulation No. 129 whereas Chest Resultant Acceleration was 17.4–37.6% less. Abdominal pressure was 46.4–81.4% beneath the limit (apart from one case which did not meet the requirements). The HPC parameter (Head Performance Criterion) was 45.4–74.5% less. The final results show that there are still some possibilities for improving the safety of young vehicle passengers. Full article

Suboptimal compliance with child restraint system (CRS) recommendations can increase risk for injury or death in a motor vehicle crash. The purpose of this study was to examine scenarios associated with incomplete CRS use and non-use in children ages 4–10 years. We used a cross-sectional online survey with a convenience sample of parent/caregivers from the United States, age ≥18 years, with a child age 4–10 years in their home, who could read and spoke English, and drove child ≥6 times in previous three months. We used descriptive statistics and Mann-Whitney U to describe and compare the distribution of responses to situational use of CRSs among car seat users and booster seat users. We also used descriptive statistics and the Mann-Whitney U to describe and compare the distribution of responses to carpooling items among booster seat users and non-booster seat users. There were significant differences among those who reported most often using booster seats (n = 282) and car seats (n = 127) in situations involving rental cars, driving just around the corner, car too crowded to fit the CRS, not enough CRSs in the vehicle, the CRS is missing from the car, or the child is in someone else’s car without a CRS (p < 0.05). Among those who reported most often using booster seats and who carpooled other children (n = 159), 71.7% (n = 114) always used a booster seat for their own child. When carpooling other children, booster seat users were significantly more likely to use booster seats for other children ages 4–10 than the non-booster seat users (p < 0.01). Continued education and programs surrounding CRS use is critical, particularly for children who should be in booster seats. Full article

Objectives: Child Restraint System (CRS) misuse is common. We characterized caregivers’ use of child passenger safety informational and instructional resources and determined whether there were differences in the quality of CRS installations associated with prior exposure to specific resources as evaluated in a standardized CRS installation environment. Methods: Caregivers completed self-report surveys and installed a forward-facing CRS in a controlled environment. Installations were evaluated for security (tightness) and accuracy (no errors) by a child passenger safety technician (CPST). Results: CRS manuals were the most common way caregivers learned to install a CRS. Primary care providers (PCP)s were the most frequently endorsed source of CRS safety information. There was no strong pattern of associations between prior exposure to resources and installation quality (security or accuracy), although some evidence supports protective effects of learning from CPSTs; 13% (19 out of 151) installations were secure and 57% (86 out of 151) installations were accurate. Conclusions: A focus on developing effective and lasting behavioral interventions is needed. Full article

Childhood death from vehicle crashes and the delivery of information about proper child restraint systems (CRS) use continues to be a critical public health issue. Safe Seat, a sequential, mixed-methods study identified gaps in parental knowledge about and perceived challenges in the use of appropriate CRS and insights into the preferences of various technological approaches to deliver CRS education. Focus groups (eight groups with 21 participants) and a quantitative national survey (N = 1251) using MTurk were conducted. Although there were differences in the age, racial/ethnic background, and educational level between the focus group participants and the national sample, there was a great deal of consistency in the need for more timely and personalized information about CRS. The majority of parents did not utilize car seat check professionals although they expressed interest in and lack of knowledge about how to access these resources. Although there was some interest in an app that would be personalized and able to push just-in-time content (e.g., new guidelines, location and times of car seat checks), content that has sporadic relevance (e.g., initial installation) seemed more appropriate for a website. Stakeholder input is critical to guide the development and delivery of acceptable and useful child safety education. Full article

Objective: to examine parents’ use of child restraint systems (CRS), and determine if parents’ knowledge of, attitude toward, and use behavior of child restraint systems have improved following enactment of child restraint use laws in other cities. Design: Observations and a cross-sectional survey of drivers transporting children 17 years and under were conducted at the gate of the schools and parking lots of hospitals in Shantou. Observers recorded the seating location of child passengers, the type of restraint, and appropriate use of CRS and safety belts based on the observation. Knowledge of and attitudes towards use of CRS were reported by the driver following observation. Results: Approximately 6.6% of passengers aged 0–12 were in CRS; rate of forward-facing CRS in children aged 3–5 (9.9%) was higher than rear-facing CRS for children aged 0–2 (1.1%) and booster seat use among children aged 6–12 (0.1%). Children younger than four years old (OR = 3.395, 95% CI = 2.125–5.424), drivers having a college or higher lever education (OR = 2.908, 95% CI = 1.878–4.500) and drivers wearing seatbelt (OR = 3.194, 95% CI = 1.605–6.356) had greater odds of CRS use. Over half (56.6%) of parents might or would use CRS if they could rent CRSs with fees. Conclusions: The rate of CRS is still low in Shantou. Comprehensive public education programs supported by legislation might be an effective way to improve child passenger safety. Renting CRSs to parents could be a new approach to encourage use.
