Background: Maternal overweight and obesity, which show a rising trend globally, are associated with adverse pregnancy outcomes and long-term health risks for both mother and child. Awareness and understanding of these risks among women of reproductive age are essential for effective prevention and early intervention. Methods: We conducted a cross-sectional survey among 958 women planning pregnancy, currently pregnant or breastfeeding to assess their knowledge and attitudes regarding overweight and obesity in the perinatal period. The questionnaire covered lifestyle behaviors, breastfeeding practices, and knowledge related to overweight and obesity in pregnancy. Results: Overall knowledge regarding the consequences of maternal overweight and obesity was low, with notable deficits in understanding the associated health risks and frequent misconceptions about dietary recommendations during pregnancy. Awareness gaps were particularly noticeable in domains related to fetal outcomes and recommended energy requirements across pregnancy. Excessive gestational weight gain was reported in over 75% of pregnancies, including among women with normal body mass index. Participation in antenatal classes, current breastfeeding and older age were significantly associated with higher knowledge; however, these factors together explained only 6.2% of variability. Still, several key aspects were not well recognized despite high educational attainment and frequent contact with maternity care services. Conclusions: Our study highlights a clear and urgent need for better, more targeted educational strategies to improve women’s understanding of metabolic health and nutrition before and during pregnancy. The low explained variance indicates that maternal knowledge is influenced by multifactorial and not easily captured determinants, emphasizing the need for comprehensive and individualized educational approaches. Enhancing maternal awareness could support better health outcomes for both mothers and their offspring. Full article

Background: Overweight and obesity in childhood and adolescence represent one of the most significant public health challenges of our century. Affected children and adolescents often face psychosocial maladaptation, including low self-esteem, depressive and anxiety symptoms, and behavioral problems, many of which may persist till later in life. The aim of our study was to evaluate the impact of a multidisciplinary, personalized lifestyle intervention program on psychosocial and behavioral symptoms, assessed through standardized psychometric questionnaires, and to investigate their relation with cardiometabolic parameters in children and adolescents with overweight and obesity. Methods: In this prospective cohort study, 537 children and adolescents (6–18 years; females: 52.9%; pubertal: 43.6%) with obesity (n = 44.3%), overweight (n = 33.7%), or normal BMI (n = 22%) participated in a personalized lifestyle intervention program for one year. Clinical and laboratory evaluations, including anthropometric, cardiometabolic, and endocrinologic parameters, as well as psychosocial functioning assessed by the Child Behavior Checklist (CBCL) and Youth Self-Report (YSR), were performed at the beginning and the end of the study. Linear regression analyses identified predictors of psychometric change. Results: At initial evaluation, children and adolescents with obesity displayed a less favorable cardiometabolic profile and greater emotional/conduct difficulties compared to their overweight and normal-BMI counterparts. Following the intervention, significant improvements were observed in BMI, anthropometric and cardiometabolic parameters, as well as reductions in internalizing, externalizing, and total problem scores across multiple CBCL and YSR domains (p < 0.05). The improvements in psychosocial functioning were partly independent of BMI reduction. Linear regression analyses identified cardiometabolic and endocrine markers as significant predictors of psychometric change (p < 0.05), highlighting interactions between metabolic recovery, pubertal hormones, and stress physiology. Conclusions: A personalized, multidisciplinary lifestyle intervention program implemented for 1 year led to parallel improvements in psychosocial and cardiometabolic health in children and adolescents with overweight and obesity. Identification of specific metabolic and endocrine predictors provides novel insights into potential biological mechanisms associated with adiposity, emotional well-being, and neurodevelopment. Background/Objectives: This study examines factors associated with child overweight/obesity (OW/OB), pre-COVID-19 and during the COVID-19 pandemic, among all U.S. children aged 10–17 years, with or without developmental disabilities (DD) and, separately, among the subgroup of children diagnosed with a DD. Methods: Using data from the National Survey of Children’s Health (NSCH, 2018–2021), we applied descriptive statistics and multivariate logistic regression analyses to estimate the odds ratios of associations between family resilience, screen time, and childhood overweight/obesity. Family resilience measures families’ communication and problem-solving behaviors. Screentime is time spent on TV, computer, cellphone or electronic devices. Results: In descriptive analyses, during COVID-19, 35.8% of all children were identified as OW/OB compared to 32.8% pre-COVID-19—a weighted increase of 3.0%. Among children with developmental disabilities, OW/OB increased from 37.4% to 39.3%. Children reporting ≥4 h of screentime use increased from pre-COVID-19 to during COVID-19 in both groups (All Children: pre-COVID: 33.5%, during COVID: 41.6%; Developmental Disabilities: pre-COVID: 39.9%, during COVID: 49.4%). Among all children, there was a positive and strong association between screentime use and OW/OB at both pre- and during COVID-19 years. Children belonging to households with low family resiliency had 1.31 times the odds of being overweight/obese (95% CI, 1.06–1.63, p < 0.05) before the pandemic. However, these results were not significant after the pandemic. Conclusions: Prevalence of overweight/obesity in all children and children with DD during the COVID-19 pandemic continued to rise. Screentime was found to be a key determinant in increased weight status. Contrary to our hypothesis, family resilience failed to emerge as a significant protective factor for OW/OB; additional research is needed to explore the protective role of family resiliency on childhood obesity. Study findings may provide insights into developing best practices and tailored interventions with early OW/OB screening and programs tailored towards the youngest group of children aged 10–12 years or below. Background: Childhood undernutrition and overnutrition continue to be major public health challenges in South Africa. There is limited evidence on how socio-economic factors and dietary behaviors influence nutritional outcomes among school-aged children, particularly in rural areas such as Thulamela Municipality. Objective: This study aimed to examine the socio-demographic determinants, dietary patterns, and nutritional status among school-aged children in Thulamela Municipality, Limpopo Province, South Africa. Methods: A cross-sectional survey was conducted with 347 children aged 8–12 years. Simple random sampling was used to select eight villages from a total of 227 within the municipality. A snowball sampling method was used to recruit eligible children. Data on socio-demographic characteristics, including the child’s sex, parental education level, marital status, and employment status, were collected. Additionally, their dietary habits and meal frequency patterns were collected using structured questionnaires. Anthropometric measurements including height, weight, and BMI-for-age were obtained following WHO growth standards. Associations between variables were assessed using chi-square tests, with p-values < 0.05 considered statistically significant. Results: The prevalence of severe and moderate stunting was 20.5% and 21.0%, respectively. Overweight conditions and obesity affected 32.6% and 16.2% of participants, respectively. Parental education (p = 0.027), marital status (p = 0.001), and household income (p = 0.043) showed significant associations with height-for-age and BMI-for-age Z-scores. Additionally, regular breakfast consumption and the frequent intake of vegetables and dairy products were positively associated with improved nutritional outcomes (p < 0.05). Conclusions: The nutritional profile of school-aged children in Thulamela Municipality reflects a double burden of malnutrition, with concurrent high rates of stunting, overweight conditions, and obesity. Interventions that promote balanced diets and address socio-economic disparities are crucial for improving child growth and overall health. Socio-economic factors, including parental education, marital status, and household income, were significantly associated with children’s height-for-age and BMI-for-age. Furthermore, the regular consumption of breakfast, vegetables, and dairy products was associated with better nutritional outcomes, highlighting the influence of both dietary behaviors and socio-demographic determinants on child growth and health. Implementing nutrition education programs within schools that emphasize the value of balanced diets and highlighting the significance of eating breakfast regularly and incorporating vegetables and dairy products into daily meals is important. These programs should include both children and their caregivers to support regular healthy eating behaviors at home and in school. Additionally, schools should carry out regular growth monitoring and nutritional assessments to identify early indications of undernutrition or overnutrition, enabling prompt referrals and interventions for children who may be at risk. Background: There is a high prevalence of the double burden of malnutrition (DBM) in children and adolescents in South Asia. This research aims to explore which sociodemographic factors are attributed to DBM in urban Bangladesh, a South Asian country. Methods: We conducted secondary analyses of data obtained from the national survey of childhood obesity among school-age children in Bangladesh (2012–2013). The sample includes 4140 children (aged 5–9 years) and adolescents (10–19 years) randomly recruited from the city corporation (urban) areas in all administrative divisions. At the population level, DBM was defined as the coexistence of underweight and overweight/obesity among children and adolescents. At the household level, DBM was defined as maternal underweight co-occurring with child overweight/obesity within the same mother-child dyad. A multivariable logistic regression model was fitted to estimate odds ratios and 95% confidence intervals. A rapid policy review was conducted to understand the implication of the results obtained from the analysis. Results: The prevalence of DBM at the population level was 45.2% (95% CI: 42.5–45.5%), ranging between 40.0% and 47.6% across seven divisions (p < 0.001). At the household level, DBM prevalence was 16.6% (95% CI: 14.7–18.7%), ranging between 14.0% and 19.0% across seven divisions (p = 0.015). At the population level, DBM odds were 56% higher among younger children (5–9 years) than adolescents (10–19 years) (OR: 1.56; 95% CI: 1.37–1.78), and this association was found in four divisions. At the household level (mother-child pairs), DBM odds were 64% higher in younger children than adolescents (OR: 1.64; 95% CI:1.38–1.95); and higher in children living at a lower-middle socioeconomic status (SES) and middle SES, than upper SES. The policy review revealed that Bangladesh has made substantial commitments to improve nutrition; however, reference to DBM is absent from policy documents. Conclusions: The prevalence of DBM is high among children in urban areas in Bangladesh, disproportionately affecting younger children and households with low SES. In the current policy space, Bangladesh should revise national nutrition frameworks to recognize DBM as a public health priority and implement region-sensitive strategies for preventing and reducing malnutrition among school-aged children. Children and adolescents with mental illness are at increased risk of developing overweight and obesity, a relationship that is complex, bidirectional, and often exacerbated by the weight-related side effects of psychotropic medications. This review addressed the research question: How are overweight and obesity addressed in children and adolescents with mental illness? Following JBI and PRISMA-ScR guidelines, a systematic search of PubMed, CINAHL, EMBASE, and PsycINFO was conducted, including studies in English or Scandinavian languages, across all designs, that focused on non-pharmacological approaches for this population aged 0–19 years. The search was completed in May 2025. Nine studies met the inclusion criteria, comprising four qualitative studies, four cohort studies, and one cross-sectional study. Based on inductive content analysis, three overarching themes were developed: approaches to weight and health, showing a predominant focus on individual lifestyle modifications; roles, resources, and prioritization, reflecting how constrained resources influence healthcare professionals’ decisions; and critical repercussions and future directions, highlighting the consequences for children, adolescents, and their families. Overall, interventions mainly target behavior change, with limited attention to structural or systemic factors. These findings underscore the need for tailored guidance and clear clinical strategies to support healthcare professionals and families in addressing weight-related issues in child and adolescent mental health care. Background: Malnutrition and micronutrient deficiencies remain significant public health burdens among Indonesian children under five years of age. This study assessed the relationship between growth indicators and micronutrient status of children aged 0–59 months. Methods: A cross-sectional design was performed, utilizing the 2018 Indonesian Basic Health Research (Riskesdas) data and biological specimens, including 550 samples from children aged between 0 and 59 months old. The Riskesdas data used in this study were demographic characteristics, anthropometric measurements, and nutritional status. The biological samples were used to measure micronutrients (iron, zinc, calcium, vitamin A (VA), and vitamin D (VD). Results: Overall, 23.1% of children were stunted, and 10.5% were wasted. This study also revealed that the prevalence of micronutrient deficiency was 34.2%, 38.7%, 8.2%, 7.8%, and 0.4% for ferritin, zinc, calcium, VA, and VD, respectively. Moreover, wasting was significantly higher in girls (13.5%) than in boys (8.2%, p = 0.044). Stunted children had significantly lower ferritin values, while wasted children had lower VA concentrations. In contrast, VD was lower in overweight children. Lastly, this study found that height for age Z-scores (HAZ score) and Body Mass Index (BMI) for age Z-scores (BAZ score) correlated positively with ferritin, zinc, and calcium levels. Conclusions: Stunting, wasting, and multiple micronutrient deficiencies remain prevalent among Indonesian children under five years of age. Strengthening nutrition interventions, in particular for iron, zinc, VA, and VD, is essential to improve child growth and health outcomes in this population. Full article

Background: Childhood obesity is a global issue, influenced by energy-dense foods and powerful cues that affect brain areas regulating food intake. The olfactory system, linked to food preferences and consumption, is inversely related to body mass index. However, no studies have assessed the possible effect of eating behavior traits on the relationship between olfactory capacity and obesity. Objectives: The aim of this study was to examine whether olfactory capacity, eating behavior traits, and body mass index are associated with obesity in adolescents. Methods: An analysis of 204 Chilean adolescents was undertaken in a cross-sectional study. The proportion of participants with normal weight was found to be 39.2%, that of overweight was 25.9%, and that of obesity was 34.8%. Anthropometric measurements (weight, height, BMI Z-score), eating behavior, and olfactory capacity were evaluated. The Child Eating Behavior Questionnaire (CEBQ) and Food Reinforcement Value Questionnaire (FRVQ) were used to assess eating behavior. The Sniffing sticks test was used to assess olfactory capacity. Results: In the global sample, 1.0% had anosmia, 20.5% had hyposmia, 61.0% had normosmia, and 17.5% were supersmellers. Girls showed higher odor identification percentages than boys (p = 0.01). No gender differences were found in olfactory threshold, discrimination, identification, or TDI (threshold–discrimination–identification) scores, nor nutritional status. Stratified analysis revealed that girls with obesity had significantly lower odor discrimination capacity compared to those with normal weight. Conclusions: the study highlights a potential link between olfactory function and obesity, with obese girls showing reduced odor discrimination compared to normal-weight girls. Further research is needed to explore these mechanisms and their implications for targeted obesity interventions. Full article

The prevalence of maternal obesity is rapidly increasing, which represents a major public health concern worldwide. Currently more than 50% of all adult women are overweight or obese, and this trend is reflected in women of child-bearing age. Maternal obesity is characterized by metabolic dysfunction and chronic inflammation, and is associated with health problems in both the mother and the offspring. Intrauterine programming occurs during embryonic and fetal development, a critical period not only for the formation of tissues and organs but also for the etiology of diseases later in life. The principal mechanisms underlying fetal programming in the offspring of obese mothers appear to involve DNA methylation and chromatin remodeling within progenitor cells. Aberrant DNA methylation patterns have been identified in genes involved in insulin signaling, lipid metabolism, and appetite regulation in the placenta and fetal tissues. Histone modifications, such as acetylation and methylation of histone tails, may also play a crucial role in modulating chromatin structure and accessibility of transcriptional machinery to DNA. The persistence of such modifications throughout life, and potentially across generations, can lead to permanent alterations in gene expression, thereby contributing to the intergenerational transmission of metabolic disorders. The aim of this paper is to present an overview of the current knowledge regarding the effects of maternal obesity on fetal development and the occurrence of fetal complications, as well as long-term complications observed in adulthood related to intrauterine exposure to maternal obesity, including hypertension and cardiovascular diseases, impaired insulin secretion and resistance, diabetes mellitus, and metabolic syndrome. Background: Foundational learning spaces in South Africa, designed to nurture growth and development, continue to grapple with malnutrition, a persistent barrier to the health, cognitive potential, and wellbeing of preschool-aged children, amidst a nutrition transition. Aim: This study assessed dietary diversity, nutritional status, and their associations among children aged 2–5 years attending funded Early Learning Centres (ELCs) in Johannesburg (Region C). Methods: Using systematic random sampling across 33 nutrition-funded ELCs in Region C, we assessed the nutritional status of children aged 2–5 years with WHO Anthro software (z-score cut-offs for undernutrition: stunting, underweight, thinness; overnutrition: overweight, obesity). Dietary diversity scores (DDSs) were derived from a 24 h recall of 16 food groups, classified by primary nutrient contributions (some groups spanning multiple classes), and categorized as low (≤8) or normal (≥9). Associations between DDS and nutritional indicators were analyzed using Poisson regression to estimate adjusted prevalence ratios (aPRs). Results: Despite structured feeding practices, all ELCs reported inadequate nutritional funding, prompting calls for dietitian support. While 27% sourced groceries from wholesalers, most relied on supermarkets and spaza shops; all had cooking infrastructure, but only 12% had food gardens, and 88% expressed interest in establishing them to improve dietary diversity. The mean DDS was 9.47 (±1.07), and 83% of children had a normal DDS (≥9), with common consumption of cereals (100%), vitamin A-rich vegetables (100%), oils (100%), and leafy greens (96%), but limited intake of protein-rich foods like eggs (7%), legumes (19%), and fish (37%). A dual burden of malnutrition was observed: 31% of children were stunted and 30% were overweight or obese. Multivariable analysis showed that boys had significantly lower odds of stunting compared to girls (aPR = 0.38; 95%CI: 0.01–0.74), while younger age (aPR = 0.61; 95%CI: 0.37–0.85) and low DDS (aPR = −0.15; 95%CI: −0.29–−0.06) were independently associated with increased risk of stunting. Age was positively associated with underweight (aPR = 1.27; 95%CI: 0.58–1.96), and thinness was strongly associated with boys (aPR = 17.00; 95%CI: 15.12–18.74), with a marginal association with age. Conclusions: Integrated nutrition strategies are critical to addressing the dual burden of stunting and being overweight in urban ELCs. Strengthening funding, professional dietetic support, and promoting food gardens can enhance dietary diversity and child health outcomes. Full article

Background: The COVID-19 pandemic has imposed significant changes on daily life. It negatively impacts children’s physical activity and lifestyle behaviors, which may cause accelerated weight gain during the COVID-19 pandemic. This study aims to evaluate children’s longitudinal age- and sex-adjusted body mass index (BMI) percentile changes during and after the COVID-19 pandemic. Methods: Height and weight data were obtained from electronic medical records for children (ages 2–18) visiting outpatient clinics during three periods: pre-COVID-19 lockdown, during COVID-19 lockdown, and post-COVID-19 lockdown. According to the availability of anthropometric information, three partially overlapping cohorts were formed: Cohort 1 (N = 934, pre- and during lockdown), Cohort 2 (N = 1129, during and post-lockdown), and Cohort 3 (N = 203, data from all three periods). Results: During the lockdown, the mean changes in percentiles of age- and sex-adjusted BMI were 6% ± 0.23, while after the lockdown, there were smaller mean changes in BMI percentiles (1% ± 0.19). There were significant associations of age and the baseline BMI categories with the change in the means of the adjusted BMI percentile of children during and after the COVID-19 pandemic (p-value < 0.001). In addition, there were significant associations of gender and the baseline BMI categories with the change in the means of the adjusted BMI percentile of children over a three-time series (p-value: <0.04, <0.001, respectively). Conclusions: In conclusion, children experienced increased BMI during and after the COVID-19 lockdown. This result highlights the importance of structured interventions to mitigate the consequences during challenging times on children’s health. Full article

Background/Objectives: Childhood obesity remains a critical public health concern in Greece, particularly among socioeconomically vulnerable groups. This study conducted a secondary analysis of five large-scale epidemiological datasets to examine the association between energy balance-related behaviours (EBRBs) and obesity in children in need. Methods: Data were compiled from five nationally or regionally representative studies (Genesis, ToyBox, Healthy Growth, ENERGY, and Feel4Diabetes) involving children aged 1–12 years. Stratified and subgroup analyses were performed to examine associations between weight status and EBRBs, including dietary habits, physical activity, and sedentary behaviour. Determinants of EBRBs were also analysed using the socio-ecological model framework. Results: Children in need demonstrated a higher prevalence of overweight and obesity compared to the general child population. Key risk factors for EBRBs included frequent consumption of sugar-sweetened beverages, sweet snacks, and high screen time. Protective behaviours associated with lower obesity risk included regular breakfast consumption, adequate sleep duration, and physical activity. Determinants of high-risk EBRBs were primarily interpersonal and, to a lesser extent, individual and community-level factors. Conclusions: These findings highlight the disproportionate burden of childhood obesity among vulnerable populations and identify modifiable behaviours and determinants that can inform targeted interventions. These results provide a robust evidence base to guide national public health policies, including the development of school- and community-based obesity prevention programmes aligned with the goals of Greece’s National Action Against Childhood Obesity. Prioritising children in need in such initiatives is essential to reduce health inequities and improve long-term health outcomes. Full article

Maternal overweight and obesity have reached global epidemic levels, altering metabolic adaptations during pregnancy and lactation. Beyond their well-known impact on gestational outcomes, elevated BMI profoundly influences the secretion of adipokines—hormones derived from adipose tissue that circulate in maternal blood and are secreted into breast milk—thereby directly linking maternal metabolism to offspring development. In this state-of-the-art narrative review, we synthesize current evidence on how maternal overweight and obesity shape concentrations of key adipokines (leptin, adiponectin, ghrelin, obestatin, and resistin) in serum, cord blood and breast milk. Excess maternal weight robustly increases leptin, while effects on adiponectin, ghrelin, obestatin, and resistin remain uncertain. To our knowledge, this is the first review to focus specifically on the impact of maternal overweight and obesity on adipokine alterations across both pregnancy and lactation. Future studies should apply standardized sampling and analytical protocols and use longitudinal designs including body composition assessments to clarify their role in maternal and child metabolic health. Background/Objective: Childhood obesity and mental health disorders in preschool-aged children represent critical public health challenges with a rising global prevalence, closely linked to lifestyle behaviors and the family environment. This cross-sectional study aims to investigate the combined influence of maternal sociodemographic, socioeconomic, anthropometric characteristics, and lifestyle factors on the physical and mental health status of preschool-aged children. Methods: Validated questionnaires were administered to assess dietary habits, psychosocial parameters (depression, anxiety, stress), and sociodemographic, socioeconomic, and anthropometric variables among 200 preschool-aged children and their mothers, who served as the primary informants. Results: Maternal obesity was associated with a higher prevalence of childhood overweight/obesity (36.7% vs. 18.5% in children of non-obese mothers, p = 0.009). Maternal psychological factors, specifically depressive symptoms (B = 0.998, OR = 2.712, 95% CI: 1.222–6.020, p = 0.014) and anxiety (B = 1.676, OR = 5.346, 95% CI: 2.471–11.565, p < 0.001), were independently associated with an increased likelihood of child anxiety. Anthropometric measures, including waist circumference (p = 0.032) and hip circumference (p = 0.031), primarily influenced children’s physical health, whereas maternal psychological factors predominantly affected their emotional well-being. Conclusions: The findings underscore the necessity for targeted interventions focusing on enhancing maternal nutrition and mental health literacy, aiming to promote healthy dietary patterns, physical activity, and lifestyle behaviors. Such interventions are pivotal for preventing childhood obesity and fostering overall well-being at the population level. Full article

Background: Undernutrition and overnutrition are considered a rising challenge among adolescents in low- and middle-income countries, including Lebanon, where overlapping economic, political, and public health crises have worsened food insecurity. Food and nutrition literacy in adolescents may serve as protective factors against food insecurity and its nutritional consequences. This study aims to evaluate the associations between adolescent and parental food and nutrition literacy with household and adolescent food insecurity, and explores their relationship with stunting and overweight/obesity. Methodology: A cross-sectional survey was conducted between March and July 2022 among 442 Lebanese adolescents (10–18 years) and one parent/caregiver per household, recruited via snowball sampling from all eight governorates. Validated tools assessed adolescent food and nutrition literacy, parental food literacy, household/adolescent food insecurity, and anthropometric status. Chi-square, t-tests, and multivariable logistic regressions identified factors associated with food insecurity, stunting, and overweight/obesity. Results: Higher adolescent food and nutrition literacy was significantly associated with lower odds of severe food insecurity (aOR = 0.43, 95% CI: 0.26–0.70). Higher parental food literacy scores were linked to reduced odds of severe household food insecurity (aOR = 0.94, 95% CI: 0.90–0.98). Severe food insecurity was more likely in households in Akkar and among adolescents not attending school or with poor food and nutrition literacy. Overweight/obesity was positively associated with attending private school and higher parental body mass index, but inversely associated with higher child food security and household crowding index. No significant association was found between food insecurity and stunting. Conclusions: Both adolescent and parental food and nutrition literacy are protective against severe food insecurity, highlighting the value of literacy-focused interventions alongside economic support measures. Addressing both educational and structural determinants may help break the cycle of malnutrition in crisis-affected Lebanese youth. Full article
