Endotoxemia often leads to microcirculatory derangement. In six sevoflurane anaesthetized Beagle dogs, we investigated the effects of 1 mg/kg of Escherichia coli lipopolysaccharide endotoxin intravenous and blood pressure (mean arterial pressure of 65 mmHg versus 85 mmHg) on microcirculation assessed on buccal mucosa using side stream dark field microscopy. Dogs were afterwards resuscitated with fluids and noradrenaline. We investigated dose requirements of noradrenaline with or without dexmedetomidine. Microcirculatory parameters, and markers of sepsis (cardiac output, mixed venous oxygen saturation, carbon dioxide gap, and lactate) were analysed before endotoxemia, after endotoxemia, after a 30 mL/kg of Ringer’s acetate fluid bolus, and during noradrenaline +/− dexmedetomidine infusion, after a second fluid bolus, and a second time after vasopressor treatment in a cross-over fashion. Endotoxemia and mean arterial pressure had no statistically significant effect on microcirculation; however, endotoxemia resulted in a decrease in cardiac output. Dexmedetomidine neither improved microcirculation nor reduced noradrenaline requirements. Full article

Allergic rhinitis and asthma are significant public health concerns worldwide. While previous studies have explored how nasal and buccal bacteriotas influence these conditions, few have directly compared their bacteriomes within the same cohort. To bridge this gap, I analyzed 16S rRNA next-generation sequencing data from 347 individuals, including participants with allergic rhinitis, asthma and healthy controls. The nasal and buccal bacteriomes shared all dominant bacterial taxa but differed significantly in their phylum- and genus-level relative abundances. Alpha-diversity was significantly higher in the buccal cavity, while beta-diversity varied significantly across all indices and clinical groups. Over 80% of the predicted metabolic pathways were differentially regulated between the two cavities, yet these functional differences remained fairly consistent across clinical groups. Naso-buccal bacterial networks exhibited striking differences in structure, complexity and hub nodes. Notably, the network of healthy controls showed a clear segregation between nasal and buccal bacteria, with 93.5% of the interactions occurring within each respective cavity, and contained few pathogenic keystone taxa. In contrast, bacterial networks from diseased individuals exhibited reduced ecological specialization and more pathogenic keystone taxa linked to airway disease. These findings, thus, demonstrate that the naso-buccal bacteriome plays distinct yet interconnected roles in allergic rhinitis and asthma. Full article

Objective: A peri-implant gap or a “jumping gap” between an implant surface and the buccal bone can often complicate the successful integration of dental implants, impairing osseointegration and long-term implant stability. Although various techniques and materials are available for managing this gap, there is no consensus on the most effective approach. The current literature lacks standardized, evidence-based guidelines for selecting the optimal technique or material for managing peri-implant gaps, especially following immediate implant placement. This systematic review aims to evaluate the efficacy of various techniques and materials to manage the peri-implant gap to improve the implant stability, bone preservation, and esthetic outcomes using the PROSPERO registration number CRD42024508852. Methods: A comprehensive search of the MEDLINE, Embase, and Cochrane databases was conducted, and various studies were selected, including 11 randomized clinical trials that investigated different grafting materials and techniques for managing the gap between the implant and the buccal plate. The selected studies were assessed for the risk of bias, and the data were extracted based on primary outcomes such as implant stability, bone density, and esthetic parameters. Results: The findings indicate that xenografts and alloplastic grafts were superior in preserving bone volume compared to platelet-rich fibrin. Techniques like the socket shield and immediate provisional prothesis methods showed promise in maintaining soft tissue and bone integrity. However, heterogeneity across the studies limits definitive conclusions. Conclusions: Further high-quality research is needed to establish standardized guidelines for peri-implant gap management. The selection of techniques and materials should be tailored to individual patient needs. Full article

Background/Objectives: This study tested a method for evaluating the internal fit of indirect resin-based composite (RBC) restorations, as well as the influence of different combinations of impression and die materials on the reproducibility of the topography of teeth prepared for indirect RBC restoration. Methods: Bovine incisors received flattened and cavitated areas at the cervical and middle thirds of the buccal surface, respectively. The samples were randomly assigned to two groups according to the material used for impression taking (n = 5): irreversible hydrocolloid and polyvinyl siloxane (PVS). Die replicas were obtained with Type IV gypsum or elastomeric material. RBC restorations were fabricated through an indirect technique (test) and a direct-indirect technique as the control. The internal fit of restorations was assessed by measuring the cementation line thickness with a digital caliper (simulated cementation protocol with ultra-light PVS) and validated using scanning electron microscopy (SEM). Surface topography (Sa, Sq, and Sz) was analyzed via optical profilometry, and wettability was assessed through the water contact angle method. The data were analyzed using t-test, ANOVA, and Pearson correlation tests (α = 5%). Results: The simulated cementation resulted in internal gap values positively correlated to the values from SEM (R2 = 0.958; p = 0.0102). The internal gap of restorations was not significantly correlated with the discrepancies between the topography of the die and tooth substrate (p ≥ 0.067). The combination of irreversible hydrocolloid and gypsum resulted in restorations with the lowest cementation line thickness, although in terms of roughness, this combination was the only one that resulted in significant differences from the control (p ≤ 0.028). The internal mean gap values of restorations were significantly correlated to the cumulative wettability difference of materials used during impression taking, fabrication of die replica, and restoration build-up (R2 = 0.981; p = 0.003). Conclusions: The reproducibility of topographical characteristics of the tooth in the die replica did not affect the internal adaptation of indirect RBC restorations, whereas surface wettability of materials presented a more relevant effect on the overall gap formation. The simulated cementation technique tested in the study shows potential as a simpler, cost-effective, and non-destructive method for evaluating the adaptation of indirect RBC restorations. Background: Immediate post-extraction dental implants are increasingly popular, but ensuring primary stability and managing peri-implant tissues remain challenging. Implant macro-design significantly impacts stability and osseointegration. This study used Cone-beam Computed Tomography (CBCT) to evaluate changes in alveolar bone following immediate placement of two implant designs, System 2P and Dura-Vit 3P, which feature semi-conical microgeometry and apical self-tapping portions for improved stability and bone regeneration. Methods: With a 1:1 allocation ratio, the current investigation was a two-arm parallel group randomized clinical trial. Patients qualified if they required immediate dental replacements with adequate buccal bone support. Two types of implants were placed: System 2P (cylindrical shape) and Dura-Vit 3P (more conical shape, with a particular architecture of threads). Following the intervention, CBCT was performed both immediately (T1) and six months later (T2). Measurements of CBCT horizontal bone level at apical, medial, and bevel height on the palatal/lingual and vestibular sides as well as the buccal vertical gap were the primary results. Complications, implant stability quotient (ISQ), and torque insertion were evaluated. The Mann–Whitney test was used to determine time-based differences within each group, while the Wilcoxon test was used to estimate differences between groups. The impact of baseline marginal gap dimension and gingival biotype was estimated using multiple regressions. Results: Thirty patients were recruited and randomized to treatments, with two lost to follow-up. One System 2P implant failed and two patients of the Dura-Vit 3P group dropped out. At T1, the Dura-Vit 3P group exhibited a lower mean insertion torque and a higher ISQ than the System 2P group. Furthermore, the Dura-Vit 3P group showed lower bone reduction compared to System 2P at horizontal and vertical measurements with significant differences for the vestibular and palatal base and medial level (p-values < 0.05). Regression models indicated a positive effect of thick biotypes on gap filling and dimensional bone reduction. No complications were observed in both groups. Conclusions: The Dura-Vit 3P implant exhibits high primary stability when inserted in post-extraction sites. Furthermore, this kind of implant stimulates higher bone stability on both the palatal and buccal side when compared to the System 2P implant. The present findings support the evidence that the macro-design of the Dura-Vit 3P implant promotes increased primary stability and reduces bone loss. Full article

This study reports two new species of Ampharete and one new record of a species of Amphicteis (Ampharetidae) from the Korean subtidal zone, based on an integrative approach. Ampharete koreana sp. nov. is characterized by paleal chaetae several times thicker than notochaetae, with short filamentous tips. It more closely resembles A. finmarchica but differs in the shape of its paleae, interbranchial gap, thoracic uncini, and pygidial cirri and shows a 2% genetic divergence in histone H3 and in 18% of mtCOI genes. Ampharete namhaensis sp. nov. shares with A. petersenae the absence of paleae and the presence of 16 abdominal uncinigers, a rare combination of traits among known Ampharete species. However, A. petersenae differs in having a marked interbranchial gap, prominent eyes on the prostomium, double the number of abdominal uncini and buccal tentacles, and in its distribution (southern Korea for A. namhaensis sp. nov. vs. North Atlantic and Arctic-adjacent waters for A. petersenae). The Korean Amphicteis specimens examined in this study closely resemble A. glabra in key morphological features, including fine paleae tips, 14 thoracic and 15 abdominal uncinigers, and approximately eight paleae. However, given the need for a comprehensive revision of Amphicteis and the potential for cryptic diversity within the genus, these specimens are provisionally designated as Amphicteis sp. cf. Background and Objectives: Persistent urogenital sinus (PUGS) is a rare congenital anomaly resulting from disruptions in urogenital development during embryogenesis. Characterized by the confluence of the vaginal canal and urethra into a single channel, it poses diagnostic and management challenges, particularly in adult patients. Surgical correction remains the primary treatment at an early age, yet there are no universally accepted guidelines regarding treatment at later stages in life. We explored current evidence on the management of high urogenital sinus (HUGS) in adults, focusing on surgical techniques, outcomes, perioperative care, and follow-up. Materials and Methods: A scoping review was conducted using PubMed, Scopus, and Google Scholar, including studies published up to August 2024. Search terms included “high urogenital sinus”, “adult patients”, and related conditions. Articles were included if they addressed clinical, diagnostic, or therapeutic data on HUGS in adult populations. Data extraction was performed independently by two reviewers, and a PRISMA-ScR flow diagram was used to outline the study selection process. Results: Nine studies met inclusion criteria, with data on surgical techniques, perioperative care, and long-term outcomes extracted for analysis. The included studies highlighted diverse surgical approaches, such as feminizing genitoplasty, buccal mucosa vaginoplasty, posterior prone techniques, and robotic-assisted urogenital sinus mobilization. Complications like vaginal stenosis, urethrovaginal fistula, and urinary incontinence were reported. Long-term outcomes were under-reported, with limited data on sexual function and patient satisfaction. Hormonal therapies, including glucocorticoids, played critical roles in managing cases associated with congenital adrenal hyperplasia (CAH). Conclusions: The management of HUGS requires a multidisciplinary approach integrating advanced surgical techniques and individualized perioperative care. While short-term outcomes are generally favorable, long-term data on sexual function and quality of life remain scarce. Future research should prioritize longitudinal studies to address these gaps, aiming to optimize surgical outcomes, patient satisfaction, and psychosocial well-being. Full article

Objective: In everyday dentistry, lithium disilicate is a valid option for single-fix partial dentures, and this material crystallization process is available with two protocols: long and short. This study’s aim was to assess the effects of these two different crystallization protocols, long and short, on the marginal gap of lithium disilicate single crowns. Methods: A total of 24 abutment plastic teeth were scanned using an intra-oral scanner. For each plastic tooth, an identical pair of lithium disilicate crowns was milled (a total of 48 crowns) by a four-axis machine. Each paired sample was categorized into two groups: long crystallization (24 crowns) and short crystallization (24 crowns). To assess precision, each unit’s marginal gap (including abutments and crowns) was meticulously measured at four specified regions using a scanning electron microscope. A Kolmogorov–Smirnov test performed on the study variables indicated a normal distribution (p > 0.05), and it was followed by independent t-tests (α = 0.05). Results: For the long crystallization group, the mean total marginal gap values were 42.91 ± 9.67 μm, and for the short crystallization group, the values were 43.25 ± 8.14 μm, with no significant difference between the groups (p = 0.894). In addition, no significant differences were found between the groups regarding the mean marginal gap measurements for all four surfaces (distal (p = 0.310), mesial (p = 0.732), palatal (p = 0.655), and buccal (p = 0.535)). Conclusions: Both the long and short crystallization methods used for lithium disilicate single crowns demonstrated marginal gap values of less than 120 μm, which are within the clinically acceptable range, with no significant differences across any parameters between the two groups. Regarding the marginal gap value, it is recommended to use the short crystallization protocol as it is more time-efficient. Full article

Background/Objectives: Various bone substitutes have been recommended to augment the horizontal gap following immediate implantation. The purpose of this study was to compare the effectiveness of an autogenous mineralized dentin graft or a deproteinized bovine bone in horizontal gap augmentation following immediate implant placement in the maxillary anterior region. Methods: A total of 110 patients underwent tooth extraction followed by immediate implant placement. The patients were divided into two groups. The first group received an autogenous mineralized dentin graft (the test group) while the second group received a deproteinized bovine bone (the control group) to augment the horizontal gap. Preoperative (T0), immediate postoperative (T1), and 1-year postoperative (T2) cone beam computed tomography scans were taken from all the patients. Linear measurements were recorded 1 mm (R1) and 5 mm (R2) points apical to the implant platform at both T1 and T2 time intervals. Pink Esthetic Scores and prosthetic complications were evaluated as well. Results: There were 57 patients with a mean age of 45.42 ± 9.86 (range 24–63 years) selected as the test group and 53 patients with a mean age of 40.28 ± 11.69 (range 20–63 years) as the control group. The mean reduction in the buccal bone plate at R1 was 6.39 ± 3.78% in the test group and 6.99 ± 5.01% in the control group (p > 0.05). The mean reduction in the buccal bone plate at R2 was 5.46 ± 4.98% in the test group and 6.77 ± 7.60% in the control group (p < 0.05). The PES and prosthetic-related complications were shown to be negligible between the groups (p > 0.05). Conclusions: The efficiency of using an autogenous mineralized dentin graft for horizontal gap augmentation showed similar results in comparison to using a deproteinized bovine bone in relation to buccolingual socket reduction following immediate implantation. Background: This study compared the influence of crystallization on marginal gap adaptation by using computer-aided design and manufacturing (CAD-CAM) for producing monolithic zirconia-reinforced lithium silicate (ZLS) ceramic crowns. Methods: A total of 25 plastic teeth were scanned using a Primescan intra-oral scanner (IOS), and ZLS crowns were ground. For each unit (abutment and crown), the marginal gap was evaluated pre crystallization and post crystallization at four regions of interest through the use of a scanning electron microscope (SEM). To compare the marginal gap between the two groups, a Kolmogorov–Smirnov test performed on the study variables indicated a normal distribution (p > 0.05) followed by paired samples T-tests (α = 0.0005). Results: After crystallization, there were significantly higher circumferential marginal gaps (CMGs) for all four surfaces (distal (p = 0.0005), mesial (p = 0.0005), palatal (p = 0.0005), and buccal (p = 0.0005)). The total mean marginal gap (MMG) revealed a significantly higher result for the post-crystallization group (79.82 ± 7.86 μm) compared to the pre-crystallization group (24.25 ± 5.49 μm). Conclusions: The post-crystallization group showed a significantly higher marginal gap compared to the pre-crystallization group in all parameters, but both groups were in the clinically accepted threshold (<120 microns). In terms of the marginal gap, it is arguable whether to carry out post-crystallization for CELTRA® DUO crowns and achieve better mechanical properties but significantly increase the marginal gap. The presence of an adequate circumferential ferrule is of great importance for the prosthetic rehabilitation of endodontically treated teeth. However, there may not be an adequate circumferential ferrule effect. This study aimed to evaluate the fatigue performance of endocrown and post–core restorations applied to different configurations without an adequate ferrule effect using the finite element method and endeavors to offer a comprehensive perspective on the challenging rehabilitation of maxillary incisors with excessive coronal substance loss, addressing a notable gap in research and providing valuable insights for clinicians. The goal is to address this gap in research and contribute valuable insights that could be beneficial for practitioners. The maxillary central incisor was selected, and lithium disilicate (LS) and a polymer-infiltrated ceramic network (PICN) were used in post–core groups with no ferrule effect (PC0); a 2 mm ferrule effect on the buccal wall (PC1); a 2 mm ferrule effect on the buccal-mesial wall (PC2); a 2 mm ferrule effect on the buccal–mesial–palatal wall (PC3); and a 2 mm circumferential ferrule effect (PC4) In the endocrown groups, an external retention group with a circumferential ferrule (ECER) and an inner retention form group (ECIR) were prepared. Fatigue performance was examined by applying a 150 N oblique load. The evaluation of the fatigue performance of the restorative materials shows that LS always had more successful fatigue performance results, while the post–core groups were generally more successful in terms of dentin survival. In situations where there is insufficient circumferential ferrule, the application of endocrowns is likely to result in a less successful prognosis for survival. Full article

The objective of this study was to assess and compare the internal adaptation of various pulp-capping materials, namely TheraCal, Biodentine, and mineral trioxide aggregate (MTA), on the dentin of permanent teeth through the utilization of micro-computed tomography (MCT) and optical coherence tomography (OCT). Thirty permanent molars were divided into three groups using a random process: group A (TheraCal), group B (Biodentine), and group C (MTA, which served as the control group). On the buccal surface of each tooth, a class V cavity of a standardized cylindrical shape was prepared. Subsequently, the respective pulp-capping material was applied to the cavity based on the assigned group, followed by restoration with composite resin. Based on the MCT results, it was observed that group A had a considerably larger gap volume in comparison to groups B and C (p < 0.001). There was no significant difference in gap volume between groups B and C. Regarding the OCT findings, group A displayed a substantially higher level of light reflection than groups B and C (p < 0.001). Group C exhibited a significantly lower level of light reflection in comparison to group B (p < 0.001). Biodentine and MTA revealed similar outcomes in terms of how well they adhered to the dentinal surface in permanent teeth. Both materials exhibited superior performance in comparison to TheraCal. The utilization of OCT in clinical practice could be advantageous as it enables dentists to monitor and evaluate restorations during post-treatment follow-up. It is imperative to intensify efforts aimed at making OCT equipment more accessible and applicable, overcoming its current limitations, and allowing for its widespread utilization in clinical practice. Flapless immediate implant placement and provisionalization (FIIPP) is often associated with an increased risk of buccal soft-tissue recession. This study aims to assess the 3-year esthetic outcome. In 100 consecutive patients, one maxillary incisor, with or without a pre-extraction buccal bone defect (≤5 mm), was replaced by an implant installed in a maximal palatal position (buccal gap ≥2 mm). The created gaps were filled with bovine bone substitute. Patient satisfaction (PS), pink esthetic scores (PES/modPES), and white esthetic score (WES) were calculated at different time points. A multilevel regression analysis (MRA) was performed to analyze which factors may be associated with the esthetics. After three years, PS scored 8.9 ± 0.84 on a scale of 10 (n = 83), and the soft-tissue esthetics were high (PES = 12.2; modPES = 8.5), as was the WES (8.2), showing no decrease from one year. Buccal bone defect size and smoking could not be associated with the soft-tissue outcome; however, implant location, gap size, and emergence profiles could. Performing FIIPP, the final crown (WES) scored highest when it was cemented, the soft tissue (PES/modPES) in central-incisor positions, and all (WES/PES/modPES) with concave emergence profiles. Background: The aim of this study was to evaluate the adaptation of a resin-modified glass ionomer to the internal walls of dentin with different conditioning systems, with or without the application of silver diamine fluoride (SDF) and potassium iodide (KI). Methods: Cervical standardized cavities were prepared on the buccal and lingual sides of 15 extracted molars. Molars were then sectioned longitudinally in a buccolingual direction, obtaining two samples from each tooth with two cavities each (60 samples). Samples were randomly divided into three groups (n = 20). Each group was divided into two subgroups. One subgroup did not receive dentin conditioning, one was conditioned with 25% polyacrylic acid—10 s (PA), and one with 37% orthophosphoric acid—5 s (OPA). In the other subgroup, dentine was treated with SDF/KI and not conditioned or conditioned with PA or OPA. All cavities were filled with Riva Light Cure® (RLC). The adaptation of the RLC to the cavity walls was evaluated by SEM at 100× magnification. The area of maximum interfacial gap was magnified at 1000× to measure its size. Kruskal–Wallis and Mann–Whitney U test were used for comparison. A significance level of α = 0.05 was used. Results: No significant differences in the percentage of well-adapted samples were found in subgroups when SDF/KI was used, regardless of whether conditioner was used or not and whether PA or OPA was used (p > 0.05). Regardless of the gap size, dentin treatment with SDF/KI did not negatively influence ionomer adaptation to dentin walls significantly (p > 0.05), except for the subgroup conditioned with OPA (p < 0.05). Furthermore, it improved the adaptation in the axial wall of the subgroup without dentin conditioning (p < 0.05). Conclusion: According to the results of the present in vitro study, the use of SDF/KI did not affect RLC adaptation to the cavity walls. Subsequent use of a conditioner worsens the adaptation of the material to the cavity walls. Full article

Background and Objectives: Dentists play a very important part in the early identification of oral cancer lesions. This aspect of dental practice depends on the knowledge acquired during the faculty years. The aim of this study was to assess dental students’ and residents’ levels of awareness in terms of oral cancer. Materials and Methods: The cross-sectional study was conducted at the Faculty of Dental Medicine within the “Grigore T. Popa” University of Medicine and Pharmacy in Iasi, on a sample of 197 students in the fourth and the fifth years and first year residents in general dentistry. To assess their knowledge, a questionnaire was created containing 22 questions about the risk factors for oral cancer, with a focus on HPV infection. Results: Most participants correctly identified smoking, alcohol, and the HPV infection as risk factors and leukoplakia and erythroplasia as potentially malignant lesions. At the opposite site, aspects considered as unsatisfactory focused on the palpation of lymphatic nodules, a procedure largely carried out by 41.6% of the fourth year students, the counseling only of the patients at risk performed by 59.7% of residents, the lack of knowledge about the prevention of oral cancer through anti-HPV immunization found in 39.7% of the fifth year students. Other incorrect answers focused on other types of suspicious lesions, such as actinic cheilitis, as well as certain areas in the oral cavity subject to the frequent onset of oral cancer, such as buccal mucosa. Conclusions: Although the fifth year students and residents have better knowledge than the fourth year students, the gaps in terms of knowledge and practice encountered in all three categories of participants require a reevaluation of the academic curriculum and the focus on the building of the skills necessary for the correct screening of oral cancer.
