Diet Diversity and Adherence to a Mediterranean Diet Pattern in Pregnancy Is Protective Against the Development of Early-Childhood Atopic Dermatitis

Background/Objectives: The role of maternal diet in atopic dermatitis (AD) requires better understanding, as AD often manifests early in life and precedes other allergic diseases. We evaluated the association between maternal diet and AD up to 2 years of age. Methods: A total of 116 mother–child dyads from the CARE birth cohort study were included. Maternal diet during pregnancy was assessed with a validated self-administered 97-item food frequency questionnaire, and dietary scores were calculated. AD was evaluated at ages 4 months, 1 year, and 2 years. The associations between maternal dietary patterns and AD were examined by logistic regression analysis adjusting for total energy intake, gender of the child, maternal antibiotic therapy during pregnancy, and history of atopic disease among both parents. Results: Of the 116 children, 27 (23.3%) developed AD by 2 years, 11 of whom (40.7%) had persistent AD within the first 2 years. AD risk was reduced with a higher Mediterranean diet score during pregnancy (upper median > 3 points versus lower median: adjusted OR 0.24, 95% CI 0.08–0.69, p = 0.009) and with greater dietary diversity, as measured by the number of items consumed (upper median > 53 items versus lower median: OR 0.19, 95% CI 0.06–0.58, p = 0.005). No association was found with macronutrients and micronutrients. Red meat consumption showed a positive association with the persistent AD phenotype (adjusted OR 5.04, 95% CI 1.47 to 31.36, p = 0.034). Conclusions: Adherence to a Mediterranean diet and a diverse diet during pregnancy may decrease the risk of developing early childhood AD. This highlights the synergistic role of nutrients in dietary patterns as they modulate immune development and disease susceptibility. Full article

Background: Adolescents with a chronological age of less than 15 years or a gynecological age of less than 2 years may have a higher probability of complications because they are more likely to conceive within 1 to 2 years of menarche and, therefore, are still growing and maturing. This could impair their ability to adapt to the physiological demands of pregnancy. Objective: To evaluate the relationship between chronological age and gynecological age with low birth weight and small for gestational age among adolescent mothers in Mexico City. Methods: A retrospective cohort design of adolescent mother–child dyads was conducted. The study followed 1242 adolescents under 19 years of age and their children, collecting data on physical, socioeconomic, and clinical characteristics, including hemoglobin levels. Low birth weight was assessed using the Intergrowth-21st project standards and categorized as above or below 2500 g. The mothers were grouped by chronological age (<15 years and ≥15 years) and gynecological age (<3 years and ≥3 years). Adjusted odds ratios were calculated using binary logistic regression models. The outcome variables were low birth weight and small for gestational age. The independent variables included gynecological age, chronological age, age at menarche, hemoglobin concentration, and gestational weight gain, among others. All independent variables were converted to dummy variables for analysis. Calculations were adjusted for the following variables: marital status, maternal education, occupation, educational lag, family structure, socioeconomic level, pre-pregnancy body mass index, and initiation of prenatal care. Results: The average age of the participants was 15.7 ± 1 years. The frequency of small for gestational age and low birth weight was 20% and 15.3%, respectively. Factors associated with small for gestational age included gynecological age < 3 years [aOR = 2.462, CI 95%; 1.081–5.605 (p = 0.032)], hemoglobin < 11.5 g/dL [aOR = 2.164, CI 95%; 1.08–605 (p = 0.019)], insufficient gestational weight gain [aOR = 1.858, CI 95%; 1.059–3.260 (p = 0.031)], preterm birth [aOR = 1.689, CI 95%; 1.133–2.519 p = 0.01], and living more than 50 km from the care center [aOR = 2.256, CI 95%; 1.263–4.031 (p = 0.006)]. An early age of menarche [aOR = 0.367, CI 95%; 0.182–0.744 (p = 0.005)] showed a protective effect against small for gestational age. Factors associated with low birth weight included gynecological age < 3 years [aOR = 3.799, CI 95%; 1.458–9.725 (p = 0.006)], maternal age < 15 years [aOR = 5.740, CI 95%; 1.343–26.369 (p = 0.019)], preterm birth [aOR = 54.401, CI 95%; 33.887–87.335, p = 0.001], living more than 50 km from the care center [aOR = 1.930, CI 95%; 1.053–3.536 (p = 0.033)], and early age of menarche [aOR = 0.382, CI 95%; 0.173–0.841 (p = 0.017), which demonstrated a protective effect, respectively. Conclusions: The study concludes that biological immaturity, particularly early gynecological age, significantly contributes to adverse birth outcomes during adolescent pregnancies. Interestingly, early menarche appeared to have a protective effect, whereas chronological age was not a significant predictor of small for gestational age. Chronological age has an even greater impact: women younger than 15 years are 5.7 times more likely to have low birth weight infants. However, chronological age did not increase the likelihood of having an SGA newborn. Full article

Objective: To comprehensively evaluate the interaction between diet quality and multivitamin intake during pregnancy on offspring neurodevelopment. Methods: This analysis was grounded in mother-child dyads from the 3D Cohort Study in Quebec, Canada. Among the 2366 participants initially enrolled in the 3D study, 1535 women successfully completed the 3-day food record during 20–24 weeks of gestation. A Canadian adaptation of the Healthy Eating Index (HEI-C) 2010 was used to quantify diet quality. The total HEI-C score was dichotomized into low and high diet quality by median split. Cognitive and motor development in childhood were assessed using the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley-III). Language abilities were measured using the toddler short-form version of the MacArthur–Bates Communicative Development Inventories (MCDI) questionnaire, administered in either English or French. After excluding participants with missing covariate data, cognitive, motor, and language development scores at 2 years of age were available for 1066, 1040, and 981 children, respectively. Multiple linear regression models were employed to calculate adjusted effect estimates. The interaction on an additive scale was assessed by incorporating a product term into the linear regression model. Results: Statistically significant interactions were detected between diet quality and multivitamin intake in relation to the cognitive and language development outcomes of the offspring (interaction p-values were 0.018 and 0.023, respectively). The lowest cognitive and language scores were observed in the group of women who neither took multivitamins nor maintained a high-quality diet. Among women not taking multivitamins, a high-quality diet was associated with improved offspring cognitive and language scores (mean difference [95% CI] = 4.2 [0.1, 8.2], p = 0.04; and 11.3 [3.1, 19.5], p = 0.01, respectively). However, among women taking multivitamins, no such associations were identified. Conversely, in participants with a low-quality diet, multivitamin intake was associated with a 3.0-point increase in cognitive composite scores (95% CI: 0.3, 5.8, p = 0.03), but this was not the case for those with a high-quality diet. No statistically significant interactions were observed between maternal diet quality and multivitamin intake for motor development outcomes. Conclusions: Adequate nutritional supply during pregnancy, achieved either through a high-quality diet or multivitamin supplementation, is fundamental for the neurodevelopment of children. Research suggests that maternal stress is related to aspects of child development. Positive and stimulating interactions, such as shared reading, may act as protective factors, mitigating the negative effects of maternal stress on children’s development and behavior. This cohort study investigated the predictive relationship between maternal stress and children’s milestones and behavioral problems, with maternal interactive style during shared reading as a moderator. A total of 91 mother–child dyads participated. During the shared reading session, conducted in a private room at a research center, each mother and child interacted freely while reading a book, without specific instructions. The sessions were video-recorded and later analyzed by trained researchers using an established method. Children’s data were assessed using questionnaires completed by the mother on the same day. Descriptive, bivariate, and multivariate analyses were performed to build regression models with moderation analysis. Results revealed that maternal stress significantly predicted children’s internalizing and externalizing symptoms. Notably, more affectionate interactions during shared reading can moderate the effect of stress on children’s externalizing difficulties. These findings suggest that fostering positive and engaging interactions between mothers and children, such as shared reading, may have a beneficial impact on children’s behavioral development, even in the presence of maternal stress. Background: Neurobehavioral developmental disorders significantly affect children’s future well-being and contribute to the global disease burden. While prenatal micronutrient supplementation is crucial for fetal neural development, their individual and combined effects on subsequent neurobehavioral outcomes in childhood remain poorly understood. This study aimed to examine the individual and combined effects of prenatal micronutrient supplementation on neurobehavioral developmental disorders in preschool children, and to explore their effects across specific developmental domains. Methods: 15,636 mother-child dyads were recruited from the 2022 children’s survey in Shenzhen, China. Mothers provided information on prenatal supplementation of calcium, folic acid, iron, and multivitamins. Five domains of children’s neurobehavioral functioning were assessed using the Ages and Stages Questionnaire-Third Edition (communication, gross motor, fine motor, problem-solving, and personal-social status). Logistic regression models were used to estimate the effect of micronutrient supplementations on NDDs across crude, adjusted, and full-inclusion models. Combined effects were assessed by multiplicative and additive interactions calculated from crossover analysis. Results: 11.7% of preschool children were identified as at risk for neurobehavioral developmental disorders, with the highest prevalence in the gross motor domain. Prenatal multivitamin supplementation showed a protective effect against neurobehavioral developmental disorders (OR = 0.73, 95% CI = 0.66–0.81). Interaction analysis revealed that the combination of iron and multivitamins further enhanced this protection, with both multiplicative (IOR = 1.26, 95% CI = 1.02–1.57) and additive interactions (RERI = 0.18, 95% CI = 0.02–0.35). The problem-solving domain consistently showed the greatest benefit from the supplementation of these micronutrients individually and in combination. Conclusions: Prenatal multivitamin supplementation reduces the risk of neurobehavioral developmental disorders, especially when combined with iron supplementation. These findings highlight the potential benefits of prenatal co-supplementation strategies to improve neurobehavioral outcomes in offspring. Further studies are recommended to confirm these findings and explore underlying mechanisms. Full article

Adolescent externalizing problems are commonly linked to maternal psychological distress and family functioning, but these associations remain underexplored in the Greek sociocultural context. This study examined how maternal symptoms of depression, anxiety, and stress, along with adolescent-perceived family functioning, predict externalizing behaviors in Greek adolescents. A total of 563 adolescent–mother dyads (63.4% girls; M_age = 15.03 and SD = 0.83) participated. Mothers completed the Child Behavior Checklist (CBCL), the Depression Anxiety Stress Scales (DASS-21), and the Family Assessment Device (FAD–GF), while adolescents completed the Youth Self-Report (YSR) and FAD–GF. Hierarchical regression analysis showed that adolescent-perceived family functioning was the strongest predictor of externalizing behavior (β = 0.24 and p < 0.001), even after accounting for demographic and maternal mental health variables. The final model explained 18% of the variance in adolescent externalizing problems. Mediation analysis confirmed that family functioning partially mediated the relationship between maternal depression and adolescent externalizing problems, with a significant indirect effect (a × b = 0.088, Sobel z = 2.90, and p = 0.004). Gender differences were found for self-reported aggressive behavior (t = −2.40, p = 0.017, and d = 0.20), with girls scoring higher than boys. These findings highlight the indirect impact of maternal depression through family dynamics and underscore the importance of culturally sensitive, family-centered interventions to reduce adolescent externalizing problems. Although the adverse health effects of consuming ultra-processed foods (UPFs) are well-documented, limited evidence exists on their impact during lactation. This study examined the association between maternal UPF consumption, feeding practices, and malnutrition in breastfed infants. A cross-sectional analysis was conducted with 111 mother–child pairs up to 150 days postpartum. Infant feeding practices were assessed using WHO indicators, and malnutrition was evaluated by length-for-age and BMI-for-age. Maternal dietary intake was estimated using two 24 h recalls, and UPF consumption was classified by the Nova classification. Dyads were grouped based on the highest UPF consumption quartile, and adjusted binary logistic regression was applied. UPFs accounted for 26% of the maternal diet on average. While 71.2% of infants were exclusively breastfed, one-third were overweight, and 11.7% were stunted. High maternal UPF consumption (>32% of energy) was associated with increased odds of malnutrition by BMI-for-age (wasting or overweight) (OR 3.38; 95% CI: 1.29–8.83) and stunting (OR 3.89; 95% CI: 1.04–14.58). Feeding practices showed no association. These findings highlight that maternal UPF consumption is associated with malnutrition odds in breastfed infants in the population assessed, emphasizing the need for dietary guidance during lactation to improve infant health outcomes. Objective: Parental responsibilities for childcare remain unequally distributed between mothers and fathers. This study investigates whether such gender disparity also applies to night-time care, particularly when children experience sleep disorders. Methods: We conducted a cross-sectional survey study including 882 clinical files from sleep consultations for children aged 0 to 5 years, completed by one parent (98% mothers). To assess inter-rater reliability, 112 father–mother dyads outside the clinical setting were also surveyed. Additionally, 1409 mothers from the general population formed a control group. Results: In the clinical group, 60% of children were cared for exclusively by their mother at night, versus 9% by the father. In the control group, the figures were 64% and 6%, respectively. Gender disparities persisted even when both parents worked full-time or when the child was no longer breastfed. Inter-rater reliability was strong (r > 0.70). Factors such as number of night awakenings, child’s age, and maternal education influenced caregiving distribution. Greater maternal involvement was associated with increased psychological distress and lower relationship satisfaction. Conclusions: Mothers remain the primary caregivers at night, even in dual-earner families. This unequal distribution can affect maternal well-being and couple dynamics. Promoting paternal involvement may reduce maternal overload and improve child sleep outcomes. Full article

Background: The development of children is shaped by a complex interplay of biological, psychological, and social factors, yet the role of fathers remains underrepresented in research. This study contributes to addressing this gap by examining paternal involvement through the Parent–Child Relationship Scale (P-CRS), an observational tool previously validated for mother–child interactions. Methods: The sample included 204 father–child dyads, with children aged 6 months to 5 years (mean age: 43.3 months), encompassing both clinical and non-clinical groups. Experienced clinicians conducted in vivo observations across 4–5 sessions, scoring interactions along three dimensions: parent, child, and interaction. Results: Confirmatory factor analysis (CFA) validated the P-CRS’s applicability to father–child relationships, confirming its psychometric robustness and alignment with the factors observed in mother–child dyads. Conclusions: These findings highlight the unique and complementary role of fathers in shaping developmental trajectories and underscore the importance of incorporating fathers in parenting interventions and assessments. Additionally, they demonstrate the P-CRS’s effectiveness in capturing the nuanced dynamics of early parent–child relationships. Future research should investigate longitudinal differences in parental roles and expand the P-CRS’s application to diverse family structures. Full article

Background: Parent–child interactions during mealtime significantly influence social, emotional, and cognitive development in early childhood. Increasing parental use of digital technology has been linked to disruptions in these interactions, a phenomenon termed “technoference,” which is associated with relational conflicts and psychosocial difficulties in children. Feeding interactions are particularly important for fostering attachment and emotional regulation, making them a vital area for studying technology’s effects on parent–child dynamics. Aims: This study aimed to evaluate the impact of parental digital technology use during mealtime on the quality of feeding interactions and child dysregulation symptoms. Two groups were compared: mothers who used devices during mealtime (Technology Group, TG) and mothers who did not (Non-Technology Group, NTG). Methods: Participants included mother–child dyads (TG) and a matched control group (NTG) (Ntot = 174), selected from a broader research project. Mother–child pairs were observed during a 20 min midday meal using the validated Italian Scala di Valutazione dell’Interazione Alimentare (SVIA). The emotional and behavioral functioning of children was assessed with the Child Behavior Checklist (CBCL). Statistical analyses included ANOVAs and post hoc tests. Results: The TG group demonstrated significantly higher scores on all SVIA subscales, indicating greater parental affective challenges, conflict, food refusal behaviors, and dyadic distress. Additionally, children in the TG group exhibited more internalizing and externalizing problems, including dysregulation symptoms on the CBCL, compared to the NTG group. Conclusions: Parental technology use during mealtime negatively affects parent–child feeding interactions and increases dysregulation in children. These findings highlight the need for mindful parenting strategies to limit digital distractions and foster healthier family dynamics. Full article

A Mobile-Based Approach to Enhance Knowledge of Infant and Young Child Feeding Among Teenage Mothers in Nigeria: A Randomized Controlled Trial

Background/Objectives: The second highest prevalence of childhood malnutrition in the world is found in Nigeria. Infant and young child feeding practices (IYCF) are crucial predictors of the nutritional status of children. This study evaluated the effects of utilization of the BabyThrive app versus control on IYCF knowledge of Nigerian teenage mothers. Methods: A parallel, randomized controlled trial was conducted with 194 low-income teenage mother–child (0–2 years) dyads in Nigeria. Outcome measures included knowledge concerning food type for an infant, exclusive breastfeeding, complementary feeding, and total IYCF knowledge scores. Results: No significant differences in demographic characteristics or IYCF knowledge were found at baseline. Post-intervention, almost all BabyThrive participants vs. only 36.1% of controls were aware that expressed breast milk is the ideal food for an infant <6 months, when a mother resumes work (p < 0.05). As compared to the BabyThrive group, knowledge of how to maintain breast milk supply (5.2% vs. 83.5%), the duration of safety of breast milk at room temperature (15.4% vs. 85.6%), responsive feeding (58.8% vs. 96.9) and dietary diversity (15.5% vs. 99%) was substantially lower in controls (p < 0.05). Mean knowledge on exclusive breastfeeding (25.17 ± 1.37 vs. 19.87 ± 1.80), complementary feeding (15.68 ± 0.60, vs. 13.51 ± 1.21) and total knowledge scores (46.8 ± 1.79 vs. 38.65 ± 2.71) was higher in the BabyThrive group (p < 0.05). Conclusions: In sum, the BabyThrive app significantly increased maternal IYCF knowledge in comparison with controls. It will be a useful tool to improve maternal IYCF knowledge in resource-limited areas. Full article

Background/objective: Greater parent–infant synchrony is associated with improved child outcomes. Behavioral measures of synchrony are still developing in young infants; thus, researchers need tools to quantify synchrony between parents and their young infants. We examined parent–infant neural synchrony measured using dual EEG hyperscanning and associations between neural synchrony, infant behavioral measures of synchrony, and maternal bondedness and depression. Methods: Our prospective cohort study included mother–infant dyads at 2–4 months of age. We collected time-locked dual EEG recordings of mother and infant and simultaneous video-recordings during a scaffolded interaction where the mother sequentially layered sensory modalities to the interaction. Neural synchrony measured using EEG hyperscanning was analyzed using the circular correlation coefficient (CCorr), infant behavioral synchrony was measured using the validated Welch Emotional Connection Screen (WECS) scores, and maternal bondedness and depression were measured using standardized questionnaires. Results: Our study included n = 47 dyads. Dyadic CCorr increased across the interaction as the mother added tactile stimulation to visual stimulation. We also found associations between behavioral and neural measures of dyadic synchrony such that infants with higher scores on behavioral measures of emotional connection on the WECS showed greater increases in CCorr indicative of dyadic synchrony with their mother across this interaction. We found no associations between neural synchrony and maternal bondedness or depression. Conclusion: These findings support the construct validity of mother–infant dyadic neural synchrony measured using EEG hyperscanning and analyzed using CCorr. Opportunities for future research on quantification of neural synchrony between parents and young infants abound. Full article

Neonatal Circulating Amino Acids and Lipid Metabolites Mediate the Association of Maternal Gestational Diabetes Mellitus with Offspring Neurodevelopment at 1 Year

Background/Objectives: We aimed to identify neonatal circulating metabolic alterations associated with maternal gestational diabetes mellitus (GDM) and to explore whether these altered metabolites could mediate the association of GDM with offspring neurodevelopment. Additionally, we investigated whether neonatal circulating metabolites could improve the prediction of offspring neurodevelopmental disorders over traditional risk factors. Methods: The retrospective cohort study enrolled 1228 mother–child dyads in South China. GDM was diagnosed at 24–28 weeks of gestation. Neonatal circulating amino acids and lipid metabolites (carnitines) were measured from newborn heel blood 3–7 days postpartum. Offspring neurodevelopment was assessed at age 1 year using the Children Neuropsychological and Behavioral Examination Scale. Neurodevelopmental disorders were defined as developmental delay in any domain of the scale. Results: Twenty-one metabolites associated with GDM were identified, consisting of seven amino acids and fourteen carnitines. Among these metabolites, five (glycine, myristicylcarnitine, palmitoylcarnitine, octadecadienoylcarnitine, and 3-hydroxypalmitylcarnitine) mediated the negative association of GDM with offspring neurodevelopment at 1 year (mediation proportions: 3.91–10.66%). Furthermore, six metabolites (glycine, methionine, malonylcarnitine, isovalerylcarnitine, palmitoylcarnitine, and octadecadienoylcarnitine) significantly increased the predictive performance for offspring neurodevelopmental disorders at 1 year over five traditional risk factors including GDM, parity, infant sex, birth weight, and feeding patterns (area under curve: 0.762 vs. 0.718, p = 0.012). Conclusions: GDM was associated with a variety of amino acid and lipid metabolic alterations in neonatal circulation, among which certain metabolites mediated the association of GDM with adverse neurodevelopmental outcomes in offspring. Moreover, some neonatal circulating metabolites may serve as potential biomarkers that improved the prediction of offspring neurodevelopmental disorders over GDM and other traditional risk factors. Parental reactions to children’s negative emotions (PRCNEs), a type of emotion socialization, has been theorized to shape children’s socioemotional development. Few studies have examined cultural variations in PRCNE among Western/Minority World cultures. The present study used a cross-cultural, preschool-based sample of 101 parent–child dyads (child age = 36–79 months, 42% females; parent age = 19–58 years, 89% mothers) from two countries: U.S. (n = 33, 58% White/European American, 87% with college or above education) and France (n = 68, 61% French or other European origin, 40% with college or above education). Parents self-reported PRCNEs across 12 hypothetical scenarios. Children’s behavioral problems and prosocial behaviors were rated by parents and preschool teachers. Children’s emotion knowledge was measured by a behavioral task. We examined (1) cultural group variations in PRCNEs, and (2) the relations between PRCNE and children’s socioemotional outcomes. Controlling for demographics, French parents endorsed more non-supportive PRCNE than U.S. parents. Across both cultures, parents’ higher use of distress and punitive reactions were linked to lower parent-rated child prosocial behaviors. These findings revealed differences and similarities in PRCNEs between French and U.S. families and have implications for early childhood education practices promoting socioemotional competence. Background: The first year of life is the period of greatest brain plasticity. Postpartum depression can adversely affect the first interactions with the child and, consequently, their emotional, social, and cognitive development. Objectives: First, to describe the developmental profile of six-month-old infants of mothers suffering from severe postpartum depression, and, second, to compare the development of infants whose mothers suffer from depression with or without bipolar disorder. Methods: This is a retrospective descriptive study on 6-month-old babies hospitalized with their mothers at the Marseille Mother–Baby Unit (MBU) for maternal postpartum depression with or without bipolar disorder. Mothers were clinically diagnosed by a psychiatrist specialized in postpartum depression using the DSM-5; infant development was assessed at 6 months by an independent health professional using the revised Brunet–Lézine Scale, which allowed the calculation of global and partial developmental quotients (DQ). Results: We followed 40 mother–infant dyads. None of the 40 infants had a global developmental delay. However, maternal depression was significantly associated with poorer sociability (mean sociability DQ score of 94 ± 9.6, p < 0.001) and lower postural development (mean postural DQ score of 96.2 ± 8.9 *, p < 0.001) in the infants at 6 months of age. Postural development was significantly lower in children of bipolar mothers than in children of non-bipolar mothers (p = 0.03). Conclusions: Postpartum depression was associated with a weakness in sociability and posture at the age of 6 months, without relevant developmental delay. Screening infants at an early age with specific tools allows for earlier intervention, which would positively influence their developmental trajectory. Full article
