This study investigates the benefits of innovative full-contact insoles, crafted using polyester fabric sheets of thermoplastic extruded materials, for individuals with hallux valgus-induced plantar pain. Thirty-five individuals with hallux valgus-induced foot pain were randomly allocated to either the experimental group, wearing innovative full-contact insoles 6 h daily, or the control group, using regular foot insoles, over a period of 12 weeks. Data collection occurred at baseline, and at 1 week, 2 months, and 3 months after the initial fitting. Results indicated that the innovative full-contact insoles significantly reduced anteroposterior displacement by an average of 0.9 cm (p = 0.025) and displacement area by 0.79 cm2 (p = 0.012). Gait improvements included an increase in the swing phase (36.46%, p = 0.008) and a reduction in stance phase duration (63.54%, p = 0.019). Pressure peaks at critical foot regions like the second metatarsal and medial heel were reduced by up to 39.45 kPa (p = 0.016) and 104.07 kPa (p = 0.031), while contact areas in the midfoot increased by 6.17 cm2 (p = 0.039). Foot pain decreased by an average score of 1.28 points on a 10-point scale across various measures (p = 0.041). These findings suggest that the innovative full-contact insoles effectively enhance pressure distribution and gait dynamics in patients with hallux valgus, providing a viable option for managing foot discomfort. Background: Different exercises have different effects upon physical fitness and cognitive domains. In this context, physical fitness behaviors have been identified as a contributing factor to cognitive decline in patients with very mild dementia. The present study aimed to further determine baseline senior fitness behaviors in patients with very mild dementia and possible factors related to rapid cognitive decline. Methods: This prospective cohort study was performed in a medical center in Taiwan, involving 132 patients with very mild dementia who were followed-up over 1 year. Assessments included the Senior Fitness Test (physical function), Mini-Mental State Examination (MMSE), and Clinical Dementia Rating (CDR) Scale. Patients with a decline in MMSE of at least 2 points within 1 year were defined as having rapid cognitive decline. Results: Age, sex, years of education, and baseline MMSE did not differ significantly between the groups (p > 0.05). At 1 year of follow-up, dietary habits and comorbidities did not differ between the rapid decline and not-rapid decline groups. At 1 year, performance on the right back scratch test was significantly better in the not-rapid decline group compared with the rapid decline group (−14 [−39–37.5] cm vs. −17 [−57–7] cm; p = 0.038). In a multiple regression analysis, the only factor that was significantly associated with rapid cognitive decline was the right back scratch test (p = 0.022). Conclusions: Despite the similarity in the status of dementia, the right hand back scratch test appears to serve an important function in detecting cognitive decline in patients with very mild dementia. Jujube (Ziziphus jujuba Mill.) is the first tree species in China, with a long history and abundant yield. However, fresh jujubes have a short shelf-life and are not resistant to storage. Therefore, more and more processed jujube products are being studied. These processed products can extend the shelf-life of jujubes and attract widespread attention for their rich functional nutrients. This review summarized changes in nutrients of fresh jujube and processed products and the research progress of different preparation methods of jujubes. Meanwhile, the pharmacological effects of bioactive components in jujube-based products were concluded. Jujube and its processed products contain rich polysaccharides, vitamin C, and other functional nutrients, which are beneficial to humans. As the initial processing method for jujubes, vacuum freezing or microwave drying have become the most commonly used and efficient drying methods. Additionally, processed jujube products cannot be separated from the maximum retention of nutrients and innovation of flavor. Fermentation is the main deep-processing method with broad development potential. In the future, chemical components and toxicological evaluation need to be combined with research to bring consumers higher quality functional jujube products and ensure the sustainable development of the jujube industry. The presence of spread through air spaces (STASs) in early-stage lung adenocarcinoma is a significant prognostic factor associated with disease recurrence and poor outcomes. Although current STAS detection methods rely on pathological examinations, the advent of artificial intelligence (AI) offers opportunities for automated histopathological image analysis. This study developed a deep learning (DL) model for STAS prediction and investigated the correlation between the prediction results and patient outcomes. To develop the DL-based STAS prediction model, 1053 digital pathology whole-slide images (WSIs) from the competition dataset were enrolled in the training set, and 227 WSIs from the National Taiwan University Hospital were enrolled for external validation. A YOLOv5-based framework comprising preprocessing, candidate detection, false-positive reduction, and patient-based prediction was proposed for STAS prediction. The model achieved an area under the curve (AUC) of 0.83 in predicting STAS presence, with 72% accuracy, 81% sensitivity, and 63% specificity. Additionally, the DL model demonstrated a prognostic value in disease-free survival compared to that of pathological evaluation. These findings suggest that DL-based STAS prediction could serve as an adjunctive screening tool and facilitate clinical decision-making in patients with early-stage lung adenocarcinoma. Full article

Introduction: Concomitant nerve injuries with musculoskeletal injuries present a challenging problem. The goals of nerve reconstruction for the shoulder include shoulder abduction and external rotation. When patients fail to achieve acceptable shoulder external rotation and shoulder abduction, tendon transfers such as trapezius transfer offer a reliable option in the subsequent stage. Case Presentation: A 32-year-old male presented with weak external rotation in his left shoulder, after previous axillary nerve reconstruction. He received the ipsilateral lower trapezius transfer with the aim of improving the external rotation. Discussion: The lower trapezius restores a better joint reaction force in both the compressive–distractive and anterior–posterior balancing and provides a centering force through the restoration of the anterior–posterior force couple. Conclusion: We believe that the ipsilateral lower trapezius transfer to the infraspinatus is a good outcome and is effective in improving overall shoulder stability and the shoulder external rotation moment arm or at least maintaining in neutral position with the arm fully adducted in patients with post axillary nerve injuries post unsatisfactory nerve reconstruction to increase the quality of life and activities of daily living. Objective: This meta-analysis aimed to ascertain the efficacy of non-invasive brain stimulation (NIBS)—comprising repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS)—for depression in traumatic brain injury (TBI) patients. Methods: Comprehensive searches were conducted in PubMed, Cochrane Database of Systematic Reviews, and the Cochrane Central Register of Controlled Trials up to 28 January 2023. Random-effects models assessed the treatment effects, and heterogeneity was evaluated through I2 statistics and funnel plot inspection. Results: From 10 trials (234 participants; 8 rTMS, 2 tDCS), NIBS was found significantly more effective than sham in alleviating depressive symptoms (SMD: 0.588, 95% CI: 0.264–0.912; p < 0.001). rTMS, specifically, showed higher efficacy (SMD: 0.707, 95% CI: 0.306–1.108; p = 0.001) compared to sham, whereas tDCS outcomes were inconclusive (SMD: 0.271, 95% CI: −0.230 to 0.771; p = 0.289). Meta-regression found no correlation with the number of sessions, treatment intensity, or total dose. Notably, while post-treatment effects were significant, they diminished 1–2 months post intervention. Adverse events associated with NIBS were minimal, with no severe outcomes like seizures and suicide reported. Conclusions: rTMS emerged as a potent short-term intervention for depression in TBI patients, while tDCS findings remained equivocal. The long-term efficacy of NIBS is yet to be established, warranting further studies. The low adverse event rate reaffirms NIBS’s potential safety. Full article

This case report describes a new approach to segmental meniscal reconstruction using a peroneal longus autograft in a patient with recurrent traumatic medial meniscus tear and anterior cruciate ligament reconstruction (ACLR) failure. While allograft meniscal transplantation is the preferred method for treating meniscal deficiency, its high cost and various legal regulations have limited its widespread use. Autologous tendon grafts have been proposed as a substitute for allograft meniscus transplantation, but their initial results were poor, leading to little progress in this area. However, recent animal experiments and clinical studies have demonstrated promising results in using autologous tendon grafts for meniscal transplantation, including improvements in pain and quality of life for patients. Further research is needed to evaluate the effectiveness of segmental meniscal reconstruction using autologous tendon grafts, but it could potentially lead to more accessible and cost-effective treatment options for patients with meniscal deficiency. (1) Purpose: To investigate the efficacy of myopia treatment in children using atropine 0.125% once every two nights (QON) compared with atropine 0.125% once every night (HS). (2) Methods: This retrospective cohort study reviewed the medical records of two groups of children with myopia. Group 1 comprised children treated with atropine 0.125% QON, while group 2 included children treated with atropine 0.125% HS. The first 6 months of data of outcome measurements were subtracted as washout periods in those children undergoing both atropine QON and HS treatment. The independent t-test and Pearson’s chi-square test were used to compare the baseline clinical characteristics between the two groups. A generalized estimating equations (GEE) model was used to determine the factors that influence treatment effects. (3) Results: The average baseline ages of group 1 (38 eyes from 19 patients) and group 2 (130 eyes from 65 patients) were 10.6 and 10.2 years, respectively. There were no significant differences in axial length (AL) or cycloplegic spherical equivalent (SEq) at baseline or changes of them after 16.9 months of follow-up. GEE showed that the frequency of atropine 0.125% use has no association with annual AL (QON vs. HS: 0.16 ± 0.10 vs. 0.18 ± 0.12) and SEq (QON vs. HS: −0.29 ± 0.44 vs. −0.34 ± 0.36) changes in all children with myopia. It also showed that older baseline age (B = −0.020, p < 0.001) was associated with lesser AL elongation. (4) Conclusion: The treatment effects of atropine 0.125% HS and QON were similar in this pilot study. The use of atropine 0.125% QON may be an alternative strategy for children who cannot tolerate the side effects of atropine 0.125% HS. In the digital era, new financial technologies and big data are accelerating the development of financial transactions. With the rise of e-commerce transactions, the financial industry has come to recognize that banking as a service can be seamlessly integrated into any scenario, thanks to disruptive innovation driven by electronic and third-party payments. This study aims to examine the consumer acceptance of third-party payment systems offered by electronic payment platforms for e-commerce, as well as their continued usage in the context of digital transformation in finance. This study employed the questionnaire survey method, and it distributed questionnaires to consumers who have used third-party payment systems. A total of 332 valid questionnaires were collected. The results indicate that user acceptance of innovative technologies and various external variables (e.g., the user’s external environment, internal characteristics, and information system quality) were significantly positively correlated with perceived usefulness, perceived ease of use, and behavioral intention regarding the electronic payment behaviors of consumers. Based on the empirical results, this study proposes important managerial implications for the financial industry and e-commerce platforms in promoting electronic payment innovation. Full article

This study aimed to examine whether different agricultural work patterns may cause MSDs in different body areas and assess their severity. In previous studies, musculoskeletal disorders (MSDs) have been recognized as an occupational disease in agricultural workers. However, not all MSDs resulting from agriculture occur in the same body areas or have similar severity. This study conducted a questionnaire survey to inquire about the body areas and severity of MSDs among fruit farmers. A total of 212 valid answers were collected. Subsequently, their main harvesting postures were classified into three groups to examine the differences in MSD occurrence and severity among those groups. The results indicate that the harvesting posture did result in different severities of MSDs in certain body areas. Raising hands above the shoulders caused significantly higher MSD severity in the neck, shoulder, upper back, and elbows despite the same standing posture. Additionally, those who squatted, bent, or kneeled had a significantly higher severity of MSDs in the shoulders compared to those who stood but did not raise their hands above their shoulders. This study confirmed that the different harvesting heights of fruits can affect farmers’ posture, leading to differences in the body areas affected by, and severity of, MSDs. Full article

Although radiotherapy is one of the most important curative treatments for cancer, its clinical application is associated with undesired therapeutic effects on normal or healthy tissues. The use of targeted agents that can simultaneously achieve therapeutic and imaging functions could constitute a potential solution. Herein, we developed 2-deoxy-d-glucose (2DG)-labeled poly(ethylene glycol) (PEG) gold nanodots (2DG-PEG-AuD) as a tumor-targeted computed tomography (CT) contrast agent and radiosensitizer. The key advantages of the design are its biocompatibility and targeted AuD with excellent sensitivity in tumor detection via avid glucose metabolism. As a consequence, CT imaging with enhanced sensitivity and remarkable radiotherapeutic efficacy could be attained. Our synthesized AuD displayed linear enhancement of CT contrast as a function of its concentration. In addition, 2DG-PEG-AuD successfully demonstrated significant augmentation of CT contrast in both in vitro cell studies and in vivo tumor-bearing mouse models. In tumor-bearing mice, 2DG-PEG-AuD showed excellent radiosensitizing functions after intravenous injection. Results from this work indicate that 2DG-PEG-AuD could greatly potentiate theranostic capabilities by providing high-resolution anatomical and functional images in a single CT scan and therapeutic capability. Studies of the neurobiological causes of anxiety disorders have suggested that the γ-aminobutyric acid (GABA) system increases synaptic concentrations and enhances the affinity of GABAA (type A) receptors for benzodiazepine ligands. Flumazenil antagonizes the benzodiazepine-binding site of the GABA/benzodiazepine receptor (BZR) complex in the central nervous system (CNS). The investigation of flumazenil metabolites using liquid chromatography (LC)-tandem mass spectrometry will provide a complete understanding of the in vivo metabolism of flumazenil and accelerate radiopharmaceutical inspection and registration. The main goal of this study was to investigate the use of reversed-phase high performance liquid chromatography (PR-HPLC), coupled with electrospray ionization triple-quadrupole tandem mass spectrometry (ESI-QqQ MS), to identify flumazenil and its metabolites in the hepatic matrix. Carrier-free nucleophilic fluorination with an automatic synthesizer for [18F]flumazenil, combined with nano-positron emission tomography (NanoPET)/computed tomography (CT) imaging, was used to predict the biodistribution in normal rats. The study showed that 50% of the flumazenil was biotransformed by the rat liver homogenate in 60 min, whereas one metabolite (M1) was a methyl transesterification product of flumazenil. In the rat liver microsomal system, two metabolites were identified (M2 and M3), as their carboxylic acid and hydroxylated ethyl ester forms between 10 and 120 min, respectively. A total of 10–30 min post-injection of [18F]flumazenil showed an immediate decreased in the distribution ratio observed in the plasma. Nevertheless, a higher ratio of the complete [18F]flumazenil compound could be used for subsequent animal studies. [18F] According to in vivo nanoPET/CT imaging and ex vivo biodistribution assays, flumazenil also showed significant effects on GABAA receptor availability in the amygdala, prefrontal cortex, cortex, and hippocampus in the rat brain, indicating the formation of metabolites. We reported the completion of the biotransformation of flumazenil by the hepatic system, as well as [18F]flumazenil’s potential as an ideal ligand and PET agent for the determination of the GABAA/BZR complex for multiplex neurological syndromes at the clinical stage. Background: Children with congenital heart disease (CHD) have impaired pulmonary function both before and after surgery; therefore, pulmonary function assessments are important and should be performed both before and after open-heart surgery. This study aimed to compare pulmonary function between variant pediatric CHD types after open-heart surgery via spirometry. Methods: In this retrospective study, the data for forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), and the ratio between FEV1 and FVC (FEV1/FVC) were collected from patients with CHD who underwent conventional spirometry between 2015 and 2017. Results: A total of 86 patients (55 males and 31 females, with a mean age of 13.24 ± 3.32 years) were enrolled in our study. The diagnosis of CHD included 27.9% with atrial septal defects, 19.8% with ventricular septal defects, 26.7% with tetralogy of Fallot, 7.0% with transposition of the great arteries, and 46.5% with other diagnoses. Abnormal lung function was identified by spirometry assessments after surgery. Spirometry was abnormal in 54.70% of patients: obstructive type in 29.06% of patients, restrictive type in 19.76% of patients, and mixed type in 5.81% of patients. More abnormal findings were found in patients who received the Fontan procedure (80.00% vs. 35.80%, p = 0.048). Conclusions: Developing novel therapies to optimize pulmonary function will be critical for improving clinical outcomes. Full article

Urinary tract infections (UTIs) are the most frequent bacterial infections in the clinical setting. Even without underlying anatomic or functional abnormalities, more than 40% of women experience at least one UTI in their lifetime, of which 30% develop recurrent UTIs (rUTIs) within 6 months. Conventional management with antibiotics for rUTIs may eventually lead to the development of multidrug-resistant uropathogens. Targeting of the pathogenicity of rUTIs, the evolution of uropathogenic Escherichia coli (UPEC), and inadequate host defenses by immune responses should be explored to provide non-antibiotic solutions for the management of rUTIs. The adaptive evolution of UPEC has been observed in several aspects, including colonization, attachment, invasion, and intracellular replication to invade the urothelium and survive intracellularly. Focusing on the antivirulence of UPEC and modulating the immunity of susceptible persons, researchers have provided potential alternative solutions in four categories: antiadhesive treatments (i.e., cranberries and D-mannose), immunomodulation therapies, vaccines, and prophylaxis with topical estrogen therapy and probiotics (e.g., Lactobacillus species). Combination therapies targeting multiple pathogenic mechanisms are expected to be a future trend in UTI management, although some of these treatment options have not been well established in terms of their long-term efficacy. Additional clinical trials are warranted to validate the therapeutic efficacy and durability of these techniques. Full article

Inorganic/organic hybrids of ZnO nanorods (NRs)/bisindolo quinoxaline (BIQ) were fabricated for broadband photosensing applications. Multiple material characterizations revealed the BIQ was self-assembled in a regular form of rod-like domain and an irregular form of amorphous aggregation that were distributed on the ZnO NRs. Optical measurements showed that BIQ can absorb visible light with a wavelength up to 630 nm and effectively generate photoelectrons. Moreover, clustering of BIQ can be observed via the 3D optical microscopy. ZnO/BIQ hybrids were promising for future UV and visible light environmental monitoring applications. Full article
