Special Issue Information

Dear Colleagues,

Surgical practice has become increasingly complex over the last few decades. The ageing and frail patient populations presenting for surgery with multiple comorbidities represent higher risks for adverse postoperative outcomes. In addition, advances in surgical technologies nowadays offer a variety of novel surgical options for previously untreatable and advanced conditions. These translate to up to 3% of perioperative mortality after non-cardiac surgery. It also adversely affects approximately 15% of patients who suffer significant postoperative complications, including renal and respiratory impairment, myocardial injury or severe infections. Such postoperative morbidity also has implications to health status after leaving the hospital as it leads to reduced long-term survival and reduced health-related quality of life.

Considering the prevalence and impact of perioperative morbidity and mortality associated with technically successful major surgeries, improved molecular monitoring of crucial aspect of surgical insults on homeostasis is required. As inflammation and metabolic dysregulation  are major common pathways linking patient co-morbidities, risk profiles and major postoperative complications, there is an urgent need to advance  our diagnostic and monitoring capabilities to better understand such basic determinants of the perioperative complications.

This Special Issue of Metabolites is therefore open but not limited to translational science addressing the linkage between inflammation and metabolic alterations. Original patient-centred articles providing new molecular information on these issues, review articles on the major paradigms and novel technologies addressing either phenotypes or candidate targets are all welcome. Such strategies especially integration of metabolic and inflammatory datasets have already been useful in stratifying critically ill patients. Similarly, comprehensive, integrated analysis of molecular measurements should allow unbiased identification and monitoring pathological signals in the perioperative setting. Together with hypothesis-driven molecular characterization, this Special Issue is expected to highlight the state of art of “molecular perioperative medicine”.
