Special Issue Information

Dear Colleagues,

Interstitial lung diseases (ILDs) represent a heterogeneous group of disorders characterized by variable clinical behavior, therapeutic response, and prognosis. In recent years, the diagnostic and prognostic landscape of ILDs has undergone significant transformation, driven by the development of advanced imaging techniques, molecular profiling, and functional assessment tools. This Special Issue aims to explore the emerging role of innovative diagnostic modalities and their integration into personalized management strategies.

Particular attention will be paid to genomic classifiers, which are redefining diagnostic confidence in fibrotic ILDs, especially in cases where histopathology is unavailable or inconclusive. In parallel, advanced imaging technologies, such as photon-counting computed tomography, offer unprecedented spatial resolution and quantitative capabilities, enabling earlier detection of subtle parenchymal changes and more precise disease phenotyping. Additionally, novel applications of speckle-tracking transthoracic echocardiography may provide valuable insights into cardiopulmonary interactions, facilitating the early identification of pulmonary hypertension and improving risk stratification.

By integrating these tools into clinical practice, there is growing potential to move beyond traditional diagnostic algorithms toward a more individualized approach to ILD care. These innovations not only enhance diagnostic accuracy but also hold promise in predicting disease progression, guiding therapeutic decisions, and improving patient outcomes.

This Special Issue seeks to provide a comprehensive overview of these cutting-edge developments, highlighting their current evidence base, limitations, and future directions, with the ultimate goal of advancing precision medicine in the treatment of ILDs.
