Cover Story: Firearm violence in the United States has risen over 40% in the last 15 years, with nearly half of gunshot wound (GSW) injuries involving the extremities. However, initial antibiotic prophylaxis management of extremity GSWs is highly variable. This study evaluated adherence to a single-dose cephalosporin prophylaxis protocol for uncomplicated extremity GSWs and the impact on infection rates. The rate of adherence increased from 15% to 39% after the protocol was implemented. Infection rates were not different before and after protocol implementation or between patients who did and did not follow the protocol. Implementation of a single-dose cephalosporin antibiotic protocol increased adherence without increasing infection rates for extremity GSWs which could decrease healthcare costs without compromising infection risk. View this paper
