The Swiss Archives of Neurology and Psychiatry were founded in 1917 by Constantin von Monakow. The main motivation was the growing need for an independent Swiss journal. Previously Swiss neurologists had had to submit their scientific contributions to German or French journals, with the result that they were often insufficiently informed of their own compatriots’ work. For the Swiss Neurological Society, established in 1908, the foundation of the Swiss Archives was also a milestone in its battle to be accepted as an independent medical faculty. While the papers were mainly Swiss authored, the Swiss Archives also occupied a significant international position from the outset, as witness the regular contributions from well-known European neurologists. Constantin von Monakow remained editor-inchief until his death in 1930. Most of the famous (neuro-)scientific-philosophical works written during the last 15 years of his life were published in the Swiss Archives. Certainly the most outstanding volume was No 13 (1923), which contains 52 articles by the most renowned neurologists, psychiatrists, neuroanatomists and physiologists of that time as a festschrift for Constantin von Monakow’s 70th birthday. After Constantin von Monakow’s death, R. Bing (Basel) and M.Minkowski (Zurich) took over as editors of the neurology section. Both neurologists published a considerable number of papers in the Swiss Archives and contributed greatly to the quality of the journal, not least through their contacts with scientists from all over the world. It was in the Swiss Archives that M. Minkowski published his well-known work on the course of the optic nerve fibres (1920) and studies on the reflexes of the human foetus (1924, 1925). The 1st International Neurological Congress, which took place in Berne in 1931, was an important event in the history of Swiss neurology and formed the subject of a commentary in the Swiss Archives. At the end of the congress all the participants received a presentation copy of the most recent volume of the Swiss Archives. During the years preceding World War II the Swiss Archives played a remarkable international role by continuing to publish foreign papers, despite growing nationalism and racism in the surrounding countries of Europe.The journal also appeared regularly during the war. In 1959 the neurosurgeon H. Krayenbühl, who in 1941 had published his classic work on cerebral aneurysm in the Swiss Archives, became editor-inchief. During his editorship the journal’s name was expanded to Swiss Archives of Neurology, Neurosurgery and Psychiatry (until 1986). The most cited papers since 1945 have been those of R. Adams (on normopressive hydrocephalus) and B. Roth (on narcolepsy and hypersomnia). Several contributions have come from related specialities, e.g. the physiologist and Nobel Prize winner W. Hess wrote on the autonomic nervous system, the anatomist G. Töndury on foetopathies and the paediatrician G. Fanconi on poliomyelitis. Full article

Multiple sclerosis confronts patients, partners and their children with a wide array of challenges. In addition to physical symptoms, multiple sclerosis may result in cognitive dysfunction, depression or personality changes. Changes in family roles, loss of work, income and social status and caregiver burnout are common in these families. Thus multiple sclerosis represents a disease condition that has the potential to affect partner and children in a number of ways. Based on studies of 70 families and 110 of their offspring by means of semi-structured psychiatric interviews, questionnaires and psychological tests, we evaluated the coping process of the ill, the healthy parent and their children. We analysed disease variables, parental factors and family situations favouring the coping process of children or aggravating their burden, resulting in serious consequences for their psychosocial development. The results with respect to the coping of all family members show the mutual influences between the parental couple as well as between the parents and their children. The better patient or partner are able to cope with the disease, the higher are the values of the coping capacity of the partner and the patient respectively. Children’s coping abilities are influenced by their parents’ coping behaviour, and even stronger by the coping competence of the healthy parent. With regard to disease variables there are significant correlations between the severity of the disease, cognitive impairment and the degree of depression. Additional cognitive dysfunction of the patient is associated with depressive symptoms in his or her partner. Psychological distress affects not only the chronically ill patient but also the caregiver. A severe parental disease is considered as a potential risk factor for psychological trauma in children. If multiple psychosocial distress factors come together, the impact on the child’s psychological development may be traumatising. Main factors aggravating children’s burden and impeding their coping process are: parental depression, single parenthood and social isolation of the family, parental communication incapacity and unresolved traumatic experience in the parents’ own past history. The latter may be associated with the parent’s feelings of victimisation and a tendency to focus his or her life around the disease. Such an attitude interferes with his or her capacity to perceive and be available to the child’s developmental and affective needs, leading eventually to her or his emotional or in rare cases physical abuse. In this context, social isolation may operate as a more or less important factor. As to symptom formation in children, somatisation and hypochondrial fears are more frequent manifestations – evolving through identification with the ill parent – in children of somatically ill parents than in children of healthy parents. Important factors contributing to children’s mental health in a situation of a multiple sclerosis disease are: fulfilling children’s developmental and emotional needs and respecting their plays, interests and activities; parents’ perception of the illness effects on children and open communication within the family about the disease situation; assuring children that their parents are adequately supported by a social network and opportunity for children to engage in alternative relationships. Against the background of the disease course, family dynamics and life phase, the coping process of each family and its individual members varies greatly. There is clearly no right or wrong way of coping; to understand the very particular meaning each family attributes to the disease is of fundamental importance to the helping professional. The importance of the family of the ill parent, the function and competence of the parental couple and the welfare of the children have to be analysed most carefully before the possibility of preventive/ therapeutic intervention can be introduced. Fatigue affects 75–95% of all multiple sclerosis (MS) patients. It belongs to one of the most disabling symptoms in multiple sclerosis causing social and occupational problems and affecting quality of life in a markedly negative way. Fatigue can be present at all stages of the disease including disease onset and often persists throughout the disease course. The pathophysiology is still unknown although several reasonable suggestions have been made. In this context peripheral and central abnormalities as well as neuroendocrine alterations were discussed as causal factors for fatigue. Even though these factors might contribute in part to the pathology they are not able to explain the entire complexity of the symptom. The current understanding of fatigue as a multicausal symptom complicates a reliable assessment.At present, methods to assess fatigue in an objective way are still missing.The knowledge about the pathology itself therefore mostly depends on subjective measures such as self-reports from patients.Without quantitative measures, however, it is difficult to define fatigue and to differentiate it from non-pathological tiredness. By developing different fatigue scales one has aimed at least at standardising the fatigue symptom and quantifying its dimension. However, these scales often suffer from insufficient discriminatory power between patients and controls, they often do not differentiate between cognitive and motor fatigue, and they were not validated according to methodological standards. At present, therefore, no reliable self-report instrumentis available to diagnose patients’ fatigue in clinical routine. Objective measures as might be provided by electrophysiological methods are employed even considerably less frequently but might be an interesting methodological approach for the future.The definition of fatigue being difficult and the pathophysiology still unknown, the development of specific therapeutic standards consequently is problematic as well. Various drugs that are supposed to raise the level of neuronal activity have been investigated for the management of MS fatigue. Significant benefits were described for some of the drugs in some subgroups of patients while in others adverse reactions were observed before a benefit could be noticed. It is recommended to combine non-pharmacological treatments and pharmacological approaches in treating MS fatigue. However, one has to be aware that a specific fatigue treatment is still lacking and that the options available today cannot be regarded as a standard treatment for fatigue. The present article will introduce the complexity of MS fatigue by discussing the major issues, assessment and treatment options. Full article

Bladder dysfunctions, particularly voiding disorders, are commonly seen in multiple sclerosis. 96% of those suffering from multiple sclerosis for more than ten years show urological findings. The impact on quality of life is high. Complications like hydronephrosis, stone formation, vesicourethral reflux, infections and renal failure can affect the upper urinary tract. Symptoms such as urgency, frequency, urinary incontinence and signs of obstruction, e.g. urinary retention, can, but need not be secondary to multiple sclerosis and may even present as primary clinical manifestation of the ailment. As multiple sclerosis is a dynamic disease with exacerbations and remissions, lower urinary tract symptoms and urodynamic findings change. Due to the poor correlation between symptoms, origin and bladder function, an initial urodynamic evaluation is advised. Therapy should be individualised, adapted to the stages of multiple sclerosis and planned in an interdisciplinary setting. Oral anticholinergics and intermittent self-catheterisation are important and effective treatments. Measurement of post-void residual bladder volume by ultrasound or catheterisation helps control therapy and bladder function. Cystoscopic botulinum-A toxin injection may be a promising new treatment option for many different urological dysfunctions. 10% of patients with bladder dysfunction, however, will still require surgery such as suprapubic cystostomy, augmentation cystoplasty, incontinent ileovesicostomy and sphincterotomy when conservative treatment fails Full article
