Tetralogy of Fallot is the most common cyanotic heart defect. This congenital defect depicts one of the most remarkable success stories of congenital heart surgery. The outlook for untreated children with tetralogy of Fallot is bleak. A third of these children will die within one year, less than half will life to age three, less than ten percent to age 30. The advent of surgical palliation by creating systemic-to-pulmonary artery connections, followed by an attempt of staged corrective surgery and finally the concept of primary repair of this defect at any age (usually neonatal repair) improved impressively survival rates of affected children. Nowadays, >90 percent of newborns with tetralogy of Fallot will celebrate their 30th anniversary. Despite surgical successes, we have learned that with these patients, however, truly long-term follow-up is necessary. The late adverse sequelae after surgical repair are now well described and include pulmonary regurgitation, right ventricular dilatation and dysfunction, progressive exercise intolerance, and arrhythmias as well as sudden death. So, while many patients with tetralogy of Fallot have made their journey from “the blue into the pink”, we should not forget: “Adults with tetralogy of Fallot – repaired yes, cured no”. Full article
