Age-Specific Responses to Immersive Virtual Reality During Pediatric Venipuncture: Evidence from Routine Clinical Practice

Background/Objectives: Virtual reality (VR) is increasingly used to reduce pain during pediatric needle procedures, but its effectiveness may vary by developmental stage and gender. This study evaluated whether immersive VR reduces venipuncture pain in children and adolescents and examined parent–patient agreement and gender-specific response patterns. Methods: A prospective nonrandomized clinical study was conducted within a hospital-based pediatric venipuncture service using an alternating 1:1 allocation sequence. Participants aged 4–18 years underwent venipuncture with either VR (n = 49) or standard care (n = 29). Procedural pain was measured using the Faces Pain Scale–Revised (FPS-R) with independent parent ratings. Analysis of covariance (ANCOVA) compared post-procedural FPS-R scores while adjusting for baseline pain. Exploratory age and gender-specific analyses were also performed. Results: VR led to a clear reduction in pain for children, even after adjusting for baseline scores (3.55 vs. 4.73; p = 0.003). Adolescents, however, reported similarly low pain in both groups (2.81 vs. 2.79; p = 0.60), and several mentioned that the PEGI 3 content felt too young for them, which likely limited how engaged they were. Among children, girls showed the most noticeable drop in pain, which matches the subgroup’s adjusted significance (p = 0.011). Parent–patient agreement was stronger in children (r ≈ 0.7–0.8) than in adolescents (r ≈ 0.4–0.5), and VR did not change this pattern. Most participants said they would choose VR again for future procedures. Conclusions: Immersive VR helped reduce venipuncture pain in children but had little effect in adolescents, underscoring the need for age-appropriate or more interactive VR content for older patients. Overall, these findings support using VR selectively as a distraction tool that fits the developmental needs of pediatric groups. Full article

Blood sampling by venipuncture is a routine procedure used in animal experiments and veterinary diagnostics. It can cause stress due to restraint and pain associated with penetration of the skin by a needle. When stressful procedures are repeated, stress can increase through anticipation after negative experiences or decrease due to habituation and/or positive reinforcement. The aim of this study was to describe the reaction to repeated blood sampling on the affective state of the animals. We wanted to observe how repeated blood sampling would affect stress parameters in goats that underwent an intensive cooperative vet care training protocol and were managed with a focus on gentle handling. Eight dairy goats underwent weekly blood sampling over a four-month period as part of a transfer study. Stress parameters were assessed using serum cortisol concentration, temperature changes in the inner corner of the eye measured by infrared thermography, and an ethogram showing voluntary and aversive behavior. Neither the serum cortisol concentration nor the eye temperature changed significantly over time during blood sampling. However, the ethogram showed that the goats approached the blood sampling position significantly faster and more voluntarily. In this study, we did not observe changes within the measured physiological stress parameters due to repeated blood sampling over three months. However, the voluntary behavior of the trained goats increased over time as a result of positive reinforcement training and habituation. Full article

Antibodies against the human papillomavirus type 16 (HPV16) E6 oncoprotein are promising biomarkers for the early detection of HPV-driven oropharyngeal squamous cell carcinoma (HPV+OPSCC). Standard serologic testing is challenging in underserved regions with high HPV+OPSCC incidence. Dried blood spot (DBS) cards offer a low-resource alternative but remain unevaluated for HPV antibody detection. A total of 25 OPSCC patients who provided paired serum (venipuncture) and DBS (finger-prick) samples were recruited from the University of Kentucky. HPV16 antibodies (L1, E1, E2, E4, E6, E7) were measured using multiplex serology, quantified as median fluorescence intensity (MFI) and dichotomized using established cutoffs. Correlation between serum and DBS MFI values was evaluated using linear regression and Bland–Altman plots, whereas sensitivity, specificity, and Cohen’s kappa assessed agreement. Mean MFI levels were lower in DBS than serum but were strongly correlated (R = 0.73 to 0.96; HPV16 E6 = 0.83). For HPV16 E6, DBS sensitivity was 90% (95% CI: 68–99) and specificity 100% (95% CI: 48–100), with kappa = 0.787. Specificity was 100% across all markers, while sensitivity varied from 0% (L1) to 100% (E2). DBS cards are accurate, inexpensive, and a scalable alternative for HPV16 E6 antibody detection, particularly in medically underserved regions, though further validation is needed. Full article

Exposure to heavy metals from mining activities has been consistently associated with disruptions in hematologic homeostasis, adversely affecting children’s overall development. We aimed to determine population-specific distributions of hematological markers and to compare anemic and nonanemic children in a mining-exposed highland community. A cross-sectional study was conducted with 156 children aged 3 to 7 years from the Peruvian highlands, using non-probability sampling and following CLSI C28-A3 guidelines for this population. Inclusion criteria were children with complete blood count results and residency in mining-contaminated areas. Blood samples were collected via venipuncture and analyzed with a 3-part Sysmex differential hematology analyzer. The mean WBC count was 10.42 ± 1.76 × 103/µL, with no significant differences between males and females (p = 0.770). Hematological indices, including RBC, hemoglobin, and hematocrit levels, were consistent between sexes. However, significant differences were noted between anemic and nonanemic 3–4-year-old children for RBC (5.56 ± 0.47 vs. 7.06 ± 0.96 × 106/µL) and HCT (33.97 ± 6.89 vs. 35.64 ± 5%) (each p < 0.00001), with lower values in anemic subjects. Also, anemic and nonanemic 5–7-year-old children had significant differences in RBC (5.87 ± 1.02 vs. 7.36 ± 0.79 × 106/µL) and HCT (31.13 ± 1.73 vs. 36.54 ± 4) (each p < 0.00001). Our findings reveal variations in hematological parameter distributions, emphasizing the importance of personalized blood assessments for mining-exposed populations. This approach could enable earlier diagnosis and intervention for anemia among vulnerable pediatric groups. Full article

Background/Objectives: This article discusses the clinical–surgical relevance of vascular anatomical variations, such as fenestrations—the division of a vessel into multiple channels that subsequently rejoin distally. Although rare in peripheral veins, these variations, which originate from the incomplete condensation of the embryonic capillary plexus, can predispose thrombosis and necessitate preoperative recognition to avert complications during routine procedures. This study aims to report a rare case of dual origin and double fenestration of the cephalic vein. Methods: During a cadaveric dissection, a variation of the cephalic vein was identified. Results: In this case, an origin of the cephalic vein was observed arising from the dorsal venous network of the hand. It exhibited a double fenestration in the forearm, where a branch of the medial cutaneous nerve of the forearm perforated it before draining into the brachial vein. The second, a proximal origin, arose from the convergence of two tributaries—one originating from the subcutaneous tissue lateral to the brachial muscle and the other from the biceps brachii muscle, forming a single trunk that drained into the subclavian vein. Conclusions: This rare variation of the cephalic vein (dual origin and fenestration) carries significant hemodynamic implications, including an increased risk of turbulence and thrombosis. The atypical anatomical relationship between the nerve and the fenestrated vein also heightens the potential for iatrogenic injuries. In-depth knowledge of such anomalies is crucial for healthcare professionals to minimize complications and optimize the success of procedures like venous access and arteriovenous fistulas, ultimately ensuring patient safety. This research proposed a feasibility study for a qualitative and semiquantitative analysis of sheep serum using Raman Spectroscopy analysis as an alternative to standard methodologies. Raman Spectroscopy was used to obtain information about molecular vibrations that can provide information about the behavior of specific variations in the protein network. This study was conducted during the traditional vernal shearing procedure in Sicilian sheep breeding. Twenty female sheep were randomly chosen from a one-hundred Comisana-bred flock habituated to the handling required for shearing and venipuncture. Animals had a mean body weight of 52.35 ± 4.55 kg, were aged between 2 and 3 years old, and were clinically healthy with no evidence of disease and free from internal and external parasites. All animals were shorn on the same day by hand using traditional shearing scissors in a 15 m × 10 m pen. The animals were released into an adjacent pen at the end of the shearing procedure. For each animal, blood samples were collected through jugular venipuncture into a vacutainer tube with a clot activator (Terumo Corporation, Japan) immediately before and 5 and 60 min after the end of the shearing procedure. On the obtained sera, ELISA and Raman spectroscopy analyses were performed to evaluate cortisol concentrations. The band area corresponding to the cortisol vibration mode was identified in the 1300–1366 cm−1 band. The Raman spectra obtained during the various protocol data points showed the same trend, with differences in the intensity of the band area 1300–1366 cm−1. A positive correlation was found between ELISA and Raman assessment in all experimental conditions. The obtained results demonstrate that Raman spectroscopy analysis could be a suitable tool for biomolecule identification. This study demonstrated that this technique provides useful information for understanding sheep responses to stress induced by management conditions. Full article

Ehrlichiosis and anaplasmosis are endemic to tropical and subtropical regions and pose significant zoonotic threats to both human and animal health. This study aimed to detect anti-Ehrlichia canis, anti-Borrelia burgdorferi, and anti-Anaplasma antibodies in dogs from the rural–urban area of Huamanga, Ayacucho. The cross-sectional survey was conducted at the Facultad de Ciencias Biológicas of the Universidad Nacional de San Cristóbal de Huamanga between May and August 2023. Samples were collected via venipuncture, and antibody detection was performed using the immunochromatographic assay Anigen Rapid CaniV-4 kit. Frequencies, percentages, and statistical analyses were conducted using the SPSS® software package. A total of 107 samples from dogs in the Covadonga Human Settlement were analyzed, comprising 64 (59.8%) males and 43 (40.2%) females. The majority (78.5%) were from mixed-breed dogs, while other dogs breed included Schnauzers, Pekingese, and Pitbulls. Thirty positive samples were identified, with antibodies against Ehrlichia canis (15.9%), Anaplasma phagocytophilum/Anaplasma platys (3.7%), mixed infections of Ehrlichia canis and Anaplasma phagocytophilum/Anaplasma platys (6.5%), and Ehrlichia canis/Borrelia burgdorferi (1.9%) detected, as well as an association between vector exposure and the presence of Ehrlichia canis antibodies. These findings underscore the urgent need for the implementation of integrated control strategies and enhanced surveillance programs targeting tick-borne diseases in high-risk areas, along with targeted educational campaigns to promote responsible pet ownership and preventive measures. Background: Venous blood collection in pediatric patients is a critical procedure for diagnostic and monitoring purposes, yet it remains considerably more challenging than in adults. Factors such as small vein size, limited cooperation, and heightened sensitivity to pain contribute to technical difficulties and increased error rates. Objectives: This mini-review aims to provide a concise synthesis of current clinical practices and emerging technologies that support safer, more efficient venipuncture in children. Results: Key findings include the anatomical and procedural considerations relevant to pediatric venipuncture, age-specific recommendations for technique and positioning, as well as evidence-based strategies to reduce pain and anxiety. Common preanalytical errors, particularly hemolysis and insufficient sample volumes, are also addressed, along with their implications for clinical outcomes. Recent advances in medical digitalization, including the use of venous ultrasound, near-infrared projection, and transillumination, offer valuable support in overcoming procedural challenges. These technologies are not meant to replace human expertise but to complement it, improving vein visualization and increasing first-attempt success rates when integrated into a child-centered approach. Conclusions: Venous blood collection in pediatric patients requires a delicate balance between technical proficiency and human-centered care. Emphasis is placed on the importance of a child-centered approach, combining technical skill with empathy and clear communication. Enhancing the quality and safety of venous sampling in children requires not only training and standardization, but also a deeper understanding of the psychological dimensions involved in pediatric care. Due to the feeding system (high-concentrate diet) during the early lactation stage, ruminal pH in dairy cows follows a diurnal pattern and can remain below the critical level of 5.5 for extended periods of the day. This study aimed to evaluate the effect of low ruminal pH on blood concentrations of certain macrominerals, trace minerals, and fat-soluble vitamins and on the oxidative status of dairy cows during the first half of lactation. Fifty-three randomly selected lactating Holstein cows were used; blood and ruminal fluid samples were collected from all cows on days 30, 90 and 150 of lactation. Blood samples were obtained via coccygeal venipuncture, while the ruminal fluid was obtained by rumenocentesis and the pH was measured immediately after collection. Using a threshold pH of 5.5, samples were classified as normal (pH > 5.5) or low pH (pH ≤ 5.5). Serum concentrations of Ca, Mg, K, Cr, Mn, Zn, Se, and vitamins A, D3, E, and K were not significantly affected by ruminal pH, either by days in milk or by their interaction (p > 0.05). Plasma malondialdehyde and reduced glutathione followed the same trend (p > 0.05). Copper concentration was significantly higher (p < 0.05), and Fe concentration tended to be higher in cows with low pH compared to those with normal pH (p = 0.052). On day 150 of lactation, Cu, Fe, and Co concentrations were significantly higher in low-pH cows compared to normal-pH cows (p < 0.05). Low ruminal pH is associated with significant changes in serum concentrations of copper, iron, and cobalt but has no significant effect on the oxidative status of the animals or on the serum concentrations of the macro elements and fat-soluble vitamins studied. Background: Reusable tourniquets are commonly used to aid venipuncture and blood collection. However, inadequate disinfection may lead to bacterial contamination and increase the risk of healthcare-associated infections (HAIs). Tourniquets can function as fomites, facilitating the spread of pathogenic bacteria. This study assessed microbial contamination of reusable tourniquets in the emergency department and operating theatre, focusing on clinically relevant HAI-associated pathogens. Methods: Tourniquets from four hospital departments (emergency: adult observation and resuscitation; surgical theatres: pediatric and adult general surgery) were sampled at three time points (n = 12). DNA was extracted and analyzed via 16S rRNA sequencing using NGS technology to identify microbial contamination and taxonomic composition. Results: Sequencing revealed 131 bacterial species across the 12 tourniquets, including clinically important pathogens. Among the top ten HAI-associated groups, Klebsiella spp. were detected in 5/12 samples, Enterococcus spp. in 9/12, Staphylococcus aureus in 1/12, Pseudomonas aeruginosa in 9/12, and Acinetobacter spp. in 10/12. No Escherichia coli, Clostridium difficile, coagulase-negative staphylococci, Proteus spp., or Enterobacter spp. were found. Emergency department tourniquets showed higher bacterial loads; operating theatres had greater species diversity. Conclusions: Reusable tourniquets harbor significant bacterial contamination. Considering disinfection challenges and the lack of guidelines, single-use tourniquets should be considered to reduce HAI risk. Full article

Background/Objectives: Venipuncture is a painful and distress-inducing procedure, especially in adolescents. However, the effect of stress on venipuncture pain remains unclear. This study investigated the relationships between stress (venipuncture-related and general stress) and venipuncture pain intensity and unpleasantness, hypothesizing that higher stress levels would be associated with greater pain levels. Methods: Forty-two adolescents (five boys, mean age 12.2 ± 1.4) participated in the study, which included completing questionnaires and a blood draw. General stress was assessed using the Perceived Stress Scale. Before the blood draw, participants were asked to rate their venipuncture-related stress level using a Visual Analog Scale (VAS). Following venipuncture, participants rated their pain intensity and pain unpleasantness using the VAS. Nineteen participants returned for a similar study visit after 1 year. Regression models were used to assess the relationships between pain and stress. In addition, correlations were used to examine the relationships between baseline and 1-year follow-up stress and pain levels. Results: Only baseline venipuncture stress, but not general stress, was related to venipuncture pain intensity (estimate (SE) = 0.185 (0.046), t-ratio = 4.00, p < 0.001) and pain unpleasantness (estimate (SE) = 0.378 (0.116), t-ratio = 3.27, p = 0.002). Baseline stress levels were related to stress levels at 1-year follow-up. However, this was not found for pain levels. In addition, stress at baseline did not impact pain levels at 1-year follow-up. Conclusions: General stress may be different from venipuncture stress, with the latter having a greater influence on venipuncture pain. Developing interventions focused on reducing stress related to venipuncture in adolescents could assist in reducing pain and increase willingness to undergo needle procedures. Full article

Tall fescue toxicosis negatively impacts blood flow, elevates body temperature, and reduces beef cattle’s average daily gain (ADG). In previous studies, isoflavones have diminished the symptoms of tall fescue toxicosis in ruminants. Therefore, this dataset determined the impact of low concentrations of isoflavone doses on animal vasculature, body temperature, ADG, and rumen microbial communities in beef cattle. A 21-day experiment with Angus cattle consisted of four isoflavone doses: 0 g, 2 g, 4 g, and 6 g, along with a control group. Isoflavones were mixed with 0.5 kg of dried distiller’s grains (DDGs). Daily individual rectal temperatures were recorded. Weekly blood serum was collected via coccygeal venipuncture, blood vasculature data were measured via color Doppler ultrasound, and body weight (BW) was recorded. Approximately 100 mL of rumen content was collected at the end of the trial. The pulsatility index (PI) decreased in the control group compared to the 2 g and 4 g groups (p = 0.01). Animals in the isoflavone treatment groups recorded a higher rectal temperature (p < 0.05). ADG was reduced in animals undergoing isoflavone treatments (p < 0.001). Finally, there was no impact on the rumen microbial communities (p > 0.05). Isoflavone supplementation may mitigate tall fescue toxicosis and improve animal performance at greater doses. Full article

This study explored the impact of licorice stem and leaf forage in the diet of meat sheep on their growth performance, physiological parameters, immunity, and gut microbiota. Conducted in the Hotan region of Xinjiang, the experiment involved 40 healthy local Hotan sheep (5–6 months old) that were randomly assigned to four groups, with both corn stalks and corn grain partially substituted at varying levels (30%, 40%, and 50%) by licorice stems and leaves for 80 days. Daily feed intake and refusal were recorded to assess feeding behavior and efficiency, while body weight was measured every 20 days to monitor growth performance. On day 80, blood samples were collected via venipuncture for biochemical and immune function analysis, followed by slaughter to obtain meat samples for quality assessment. Results showed that dietary licorice supplementation significantly improved growth performance. Sheep in the 40% replacement group had the highest final body weight and average daily gain (p < 0.05), indicating better feed efficiency. These findings suggest that replacing corn stalks and grain with licorice stems and leaves at a 40% ratio can significantly improve growth performance. Biochemical results showed improved protein metabolism and lipid profiles, with increased TP and ALB and decreased TC and TG levels. Immune indicators, including IgG, IgM, IgA, and IL-6, were also elevated in licorice-fed groups. In summary, licorice stems and leaves demonstrated clear nutritional and immunological benefits, especially at the 40% inclusion level. Gut microbiota analysis showed that licorice stems and leaves increased the relative abundance of Firmicutes while reducing Bacteroidota. At the genus level, UCG_005 and Bacteroides increased with higher licorice concentrations, whereas Treponema decreased. Pearson correlation analysis indicated that UCG_005 was positively correlated with TP, ALB, IgM, and IL-6 and negatively correlated with TG and TNF-α. These findings suggest that using licorice stems and leaves as a replacement for both corn stalks and corn grain modulates gut microbiota composition and is closely associated with immune and biochemical responses in sheep. Metabolomics has proven to be an effective tool for elucidating mechanisms and assessing the effectiveness of dietary interventions in canine and feline nutrition. In this context, the present study aimed to perform a metabolomic analysis of the serum of dogs supplemented with increasing levels of beta-1,3/1,6-glucans to generate evidence and gain a deeper understanding of the metabolic responses associated with this supplementation. Eight dogs were evenly assigned to two balanced 4 × 4 Latin squares. Four diets were tested, differing only in beta-glucan content (0.0%, 0.07%, 0.14%, and 0.28%), and the dogs were fed according to their individual maintenance energy requirements. Each experimental period lasted 35 days. On day 35, 5 mL of blood was collected via jugular venipuncture to obtain serum for metabolomic analysis. Nuclear magnetic resonance analysis identified 12 key serum metabolites. Hierarchical heat map analysis revealed differences in metabolite intensity between treatments (p < 0.05). Additionally, the most relevant metabolic pathways were phenylalanine, tyrosine, and tryptophan metabolism; alanine, aspartate, and glutamate metabolism; and glyoxylate and dicarboxylate metabolism. This study demonstrated that increasing levels of purified beta-1,3/1,6-glucans from Saccharomyces cerevisiae modulated key metabolic pathways in dogs, particularly those related to amino acid, lipid and energy metabolisms, and gut microbiota. These findings provide insights into the mechanisms by which beta-glucans influence canine health. Full article

High heterogeneity of breast cancer is due to a large variety of cancer cell characteristics at the genomic, epigenomic, transcriptome, and proteomic levels. One of the difficulties is the separation of molecular biological subtypes based on the expression of tumor markers. Another problem is the difficulty of venipuncture in cancer patients when taking blood at different stages of patient care. Objectives: To identify statistically significant changes in the level of salivary tumor markers depending on the molecular biological subtype of breast cancer in order to improve understanding of the individual properties of each of its subtypes, 140 volunteers (breast cancer—110; healthy control—30) took part in the case–control study. Saliva was collected strictly before the start of treatment, and the content of ten tumor markers was determined by ELISA: EGFR2, CA15-3, CA27.29, MCA, CEA, CA125, CA19-9, CYFRA 21-1, ferritin, and CRP. The content of MUC1 antigens (CA15-3, CA27.29, and MCA) statistically significantly decreased in the luminal B(+) subtype of breast cancer. The CA19-9 antigen showed high sensitivity to low HER2 expression. A reliable increase in the level of CYFRA 21-1 in saliva was shown in luminal A and luminal B(-) breast cancer. The work demonstrates the diagnostic capabilities of saliva in measuring tumor markers in patients with breast cancer. It was also found that there are reliable differences in the expression level and set of tumor markers in saliva depending on the molecular biological subtype of breast cancer. Thus, CYFRA 21-1 significantly increases with luminal A and luminal B(-), but CRP only increases with luminal A. CA15-3, CA27.29, MCA, and CA19-9 significantly decrease with luminal B(+) breast cancer. Full article
