Background/Objectives: Thirdhand smoke (THS), residual tobacco pollutants persisting on surfaces, dust, and fabrics, poses specific risks to infants and children, yet its implications for nursing remain underexplored. This scoping review mapped existing evidence on THS in neonatal and pediatric contexts and synthesized nursing implications, focusing on nurses’ knowledge, unintentional environmental contamination, and educational roles. Methods: Following JBI methodology and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, a three-step search was performed across MEDLINE, CINAHL, Scopus, Web of Science, Cochrane Library, Google Scholar, and OpenGrey. Studies were included if they addressed (1) nurses’ knowledge, beliefs, and attitudes toward THS-related risks in infants and children; (2) nurses’ contribution to unintentional environmental THS contamination; or (3) nurse-led educational or preventive interventions targeting parents or communities. Results: Among 563 records, 8 met inclusion criteria. Four investigated nurses’ awareness and perceptions, revealing limited understanding of THS despite recognition of its harmfulness. One study examined contamination, detecting nicotine residues on nurses’ fingers, suggesting possible in-hospital transmission. No nurse-led interventions specifically targeting THS were found, though broader smoke-exposure education programs showed benefits when supported by nursing staff. Conclusions: Evidence is scarce but underscores significant gaps in nurses’ knowledge, clinical guidance, and educational initiatives concerning THS. Strengthening nursing education and research is essential to mitigate THS exposure in neonatal and pediatric settings and enhance nurses’ preventive and advocacy roles. Full article

Cigarette smoking is acknowledged as the most preventable risk factor for thrombogenesis-associated cardiovascular disease. Mice prenatally exposed to the thirdhand smoke (THS) form of tobacco exhibited a higher tendency to develop occlusive thrombosis, along with enhancement of several platelet functional responses. Our objective was to investigate whether prenatal (in utero) THS exposure impacts the platelet transcriptome, resulting in enhanced platelet functional responses, thereby underlying THS-associated thrombogenicity. Blood samples obtained from twenty male mice prenatally exposed to THS, along with an equal number of age-matched male mice exposed to clean air (CA) as a control, were divided into pools of five animals and used to prepare leukocyte and red blood cell-depleted platelets. RNA sequencing for mRNA and microRNA (miRNA) was utilized to analyze and compare the platelet expression profiles of the two exposure groups. RNA seq analyses revealed distinct changes in both gene expression and miRNA profiles, with 448 coding genes and 18 miRNAs significantly altered between the two groups. miRNA–mRNA interaction analysis highlighted 14 differentially expressed miRNAs that potentially target 120 of the differentially expressed genes in our data set. Interestingly, altered genes in miRNA–mRNA pairs were functionally enriched into pathways associated with platelet physiology, including platelet activation, signaling and aggregation, and cellular response to chemical stimuli. Our findings establish—for the first time—that prenatal exposure to THS modifies the platelet transcriptome, thereby rendering platelets hypersensitive to stimuli and more prone to thrombogenicity. Additionally, we illuminate the coordinated function of platelet miRNA and mRNA targets in mediating this response. Full article

Background: Previous studies have demonstrated that involuntary smoking (e.g., secondhand smoke [SHS] and thirdhand smoke [THS]) is not only associated with an increased risk of several physical health problems, such as cardiovascular disease and cancer, but also impacts mental health, including depression and anxiety. However, the relationships between SHS and THS exposure and non-suicidal self-injury (NSSI), suicidal ideation (SI), and suicide attempts (SAs) remain unclear. Methods: Participants were recruited at a Chinese vocational college via voluntary online surveys conducted on campus. Self-reported SHS exposure was determined by the frequency of contact with smokers or detecting tobacco odors in living environments, while THS was assessed through regular contact with smoker-contaminated surfaces (e.g., clothing, furniture, textiles). Logistic regression analysis was performed to evaluate the associations of SHS and THS exposure with the prevalence of NSSI, SI, and SAs in never-smoking participants. Results: The study included 5716 participants (mean age = 19.3 years; females, 85.4%). The prevalence of SHS and THS exposure was 87.6% and 77.4%, with 8.8% reporting ≥15 min of SHS exposure on at least one day per week. After controlling for potential covariates, exposure to SHS (≥15 min on at least one day per week) was significantly associated with the odds of SAs (OR [95%CI] = 1.85 [1.17–2.91]). Additionally, daily THS exposure was significantly associated with increased past-year NSSI prevalence (2.35 [1.29–4.28]) compared to those without THS exposure, with similar associations observed for SI (2.11 [1.28–3.48]) and SAs (2.40 [1.23–4.69]). Conclusions: Exposure to SHS and THS was significantly associated with increased likelihood of NSSI, SI, and SAs among young adults at a Chinese vocational college. Further studies are needed to validate these associations across more diverse populations. Full article

(1) Background: This narrative review examines in-vehicle tobacco smoke exposure among private, occupational, and commercial drivers, focusing on prevalence, nicotine biomarkers, and health consequences. (2) Methods: A comprehensive search on the PubMed, Scopus, and Embase databases was used to identify peer-reviewed, full-text, and English articles published between 2014 and 2024. Search terms were related to motor vehicles, tobacco smoke exposure, and drivers. Articles were selected for inclusion based on their relevance to in-vehicle smoking and second- or third-hand smoke exposure attributable to tobacco cigarettes through article title, abstract, and full-text screening. (3) Results: This review highlights the dangers of in-vehicle second- or third-hand smoke exposure, evidenced by the 17 articles included. Significant second-hand smoke exposure and biomarkers were revealed mostly among adolescents and children. However, a gap exists in addressing tobacco smoke exposure among occupational/commercial drivers, specifically, long-haul truck drivers (LHTDs), who have heightened exposure due to their work environment—the truck cabin—which may increase their lung cancer risk. (4) Conclusions: There is a significant literature gap regarding in-vehicle tobacco smoke exposure in occupational/commercial drivers. Future research should include nicotine biomarker usage to quantify nicotine exposures and smoking cessation intervention development tailored to LHTDs. Full article

Introduction: Thirdhand smoke (THS) was first identified by Graham and colleagues in 1953, and nicotine was detected in household dust from smokers in 1991. Thirdhand smoke (THS) consists of toxic nicotine residues that persist on surfaces long after tobacco use, posing a significant public health concern. Individuals can be exposed to thirdhand smoke through skin contact or inhalation, particularly affecting children and infants who are most vulnerable to tobacco contaminants. This review aims to assess the effectiveness of different methods for measuring nicotine THS residues to evaluate their accuracy across various age groups. Methods: Relevant literature was sourced from databases including ProQuest (Ovid), Medline (Ovid), Embase (Ovid), Scopus, and the Cochrane Library. The timeframe for included studies ranged the last 25 years, from 1999 to 2024. Eligible participants consisted of human populations exposed to thirdhand smoke residue. For this review, the animal studies were excluded. There were no restrictions regarding age, sex, ethnicity, or nationality for participant selection. For data management and screening, the Covidence systematic tool was utilized. Data extraction was performed independently by two reviewers. This protocol was registered with PROSPERO (CRD42024574140). Results: A total of 394 studies were retrieved from 5 databases for the initial screening. A total of 67 studies included in full-text screening, and ultimately, 36 studies were selected for full review. The studies were classified into four categories based on assessment methods: (1) analysis of human secretions, including salivary or urinary tests; (2) cellular analysis utilizing cellulose substrates or paper-based materials; (3) environmental assessments, which examined outdoor surfaces, vehicles, residential spaces, and fabrics; and (4) epidemiological assessments, employing surveys or questionnaires. Non-invasive matrices such as saliva and urine were frequently utilized for biomarker analysis. The studies collectively investigated nicotine and its metabolites in human biological samples, environmental surface contamination, and thirdhand smoke (THS) exposure. They employed a diverse range of assessment tools including surveys, machine learning technique, and cellulose-based substrates. Conclusions: This review identified various selective testing methods for detecting thirdhand smoke (THS) from nicotine. These assessment methods have advantages and disadvantages and underscores the need for further research. Improving these techniques for assessment of THS could significantly improve our understanding of the impact THS has on human health. Full article

Tobacco smoke is an important pollutant that causes over 8 million deaths each year, of which, 1.3 million deaths are attributed to second-hand smoke. Third-hand smoke refers to the chemical emitted from smoking that remains in the air, dust, and on the surfaces after smoking has stopped. These substances, which are deposited or adsorbed on indoor surfaces and dust and can be re-emitted into the indoor air continually, leading to human exposure over an extended period. The properties of the third-hand smoke chemicals and indoor surfaces are key factors influencing their indoor behaviors and human exposure. Additionally, the substances on surfaces can react with atmospheric oxidants to form secondary pollutants. For instance, nicotine in third-hand smoke reacts with atmospheric oxidants (ozone, nitrous acid, and hydroxyl radicals) to produce other toxic, carcinogenic substances, which may be more toxic, further increasing the risk to human health. This review aims to address three key questions: (1) What are the components of third-hand smoke? (2) How does third-hand smoke adsorb and desorb on/from indoor surfaces, and undergo chemical transformation? (3) How is exposure to third-hand smoke related to human health effects? Therefore, we conducted a comprehensive review of the chemical composition of third-hand smoke, its adsorption and desorption on indoor surfaces, chemical transformations indoors, and health effects, The chemical composition of third-hand smoke is complex, containing various toxic substances, carcinogens, and heavy metals. Rationale: Vapes are increasingly popular, however, their clouds leave a residue on surfaces, referred to as thirdhand smoke. Recent studies have reported the detrimental cellular impacts of thirdhand smoke. However, research on thirdhand vape residue exposure is relatively new and understudied. Objectives: This review aims to evaluate the current literature associated with the impact of thirdhand vape residue from nicotine and non-nicotine vapes on cells, compared to unexposed controls. Methods: A systematic search was performed in PubMed (Medline), Embase, Web of Science, Scopus, and Google databases to identify relevant studies. Two independent reviewers screened articles using the inclusion criteria of controlled experimental studies on human and animal in vitro and in vivo models which investigate thirdhand vape residue as the exposure variable and cell concepts. Studies were assessed for bias through tools specialised for animal studies. Data were extracted and synthesised in accordance with PRISMA guidelines. Results: Of 139 articles retrieved, three are included in this review, focusing on mice cell models only, one of which investigates non-nicotine vapes. No studies on human cell models that fit the criteria were found. Mice were directly exposed to vape-infused materials from which their cells were extracted and evaluated, finding that exposure to thirdhand nicotine vape residue damages mice cells. The effects of non-nicotine vapes are inconclusive. Conclusions: Thirdhand nicotine vape residue contributes to changes in some cells in mouse models but not others. Data available to date provide no convincing evidence of likely significant harm to humans. Further investigation is warranted to confirm or deny this impression. Full article

Background: The human microbiome, comprising trillions of microorganisms, significantly influences human health and disease. During critical periods like the perinatal phase, the microbiome undergoes significant changes, impacting lifelong health. Tobacco smoke, a known environmental pollutant, has adverse effects on health, particularly during pregnancy. Despite this, its association with the perinatal microbiome remains understudied. Methods: We conducted a systematic review to integrate findings on perinatal tobacco smoke exposure and its association with the maternal and neonatal microbiomes. We conducted a comprehensive literature search in the PubMed, Scopus, and Web of Science databases from January 2000 to February 2024. We selected studies that met predefined inclusion criteria and performed data extraction. Results: The review included eight studies that revealed diverse associations of perinatal tobacco exposure with the maternal and neonatal microbiome. Active smoking during pregnancy was linked to alterations in microbiome composition and diversity in children. Maternal smoking correlated with increased Firmicutes abundance and decreased Akkermansia muciniphila abundance in offspring. Additionally, exposure to thirdhand smoke in neonatal intensive care units was related to infant microbiome diversity. Infants exposed to tobacco smoke showed various microbial changes, suggesting potential implications for childhood health outcomes, including obesity risk. Conclusions: Perinatal exposure to tobacco smoke exerts significant influence on the maternal and neonatal microbiomes, with potential implications for long-term health outcomes. Addressing socioeconomic and psychological barriers to smoking cessation, implementing stricter smoking regulations, and promoting public health campaigns are essential steps towards reducing tobacco-related harm during the perinatal period. Further longitudinal studies and standardized assessment methods are needed to validate these findings and guide the development of effective preventive measures. Full article

Families play a primary role in protecting children. Thirdhand smoke (THS) is an underestimated public health issue, and health effects correlated to its exposition are emerging. The aim of this systematic review was to analyze papers focusing on knowledge, beliefs, and behaviors regarding THS among parents, families, and caregivers, published until May 2022 on MEDLINE, CINAHL, EMBASE, and the Cochrane Library. Critical appraisal of the included studies was conducted by two independent reviewers using the Joanna Briggs Institute Critical Appraisal tool. Out of the 97 articles, 8 were included in this review (two from the USA, one from Europe, and five from Asia). Although there were no restrictions on the date of publication, all the articles were published in the last 10 years, underlining that THS is an emerging concept. They were characterized by relevant heterogeneity in the study design and assessment tools. The results showed that percentages of parents who believed that THS is harmful ranged from 42.4% to 91%, but parental awareness was not always associated with the adoption of a home- and car-smoking ban or healthy behaviors. Further research is needed to understand the resistance and problems faced by parents who are aware of THS risks but unable to improve good practices. Full article

(1) Objectives: To examine the associations of child tobacco smoke exposure (TSE) with home quality and housing instability. (2) Methods. A secondary analysis of 32,066 U.S. 0–11-year-old children from the 2018–2019 National Survey of Children’s Health was conducted. Child home TSE status was defined as: no TSE: child lived with no smokers; thirdhand smoke (THS) exposure only: child lived with a smoker(s) who did not smoke indoors; and secondhand smoke (SHS) and THS exposure: child lived with a smoker(s) who smoked indoors. Home quality was assessed by the presence of mold or pesticide use and housing instability was assessed by home ownership, frequency of moves, and number of household members. We conducted weighted multivariable logistic regression and linear regression models while adjusting for important child covariates. (3) Results: In total, 12.3% had home THS exposure only and 1.6% had home SHS and THS exposure. Compared to children with no home TSE, children with home SHS and THS exposure were 2.60 times more likely (95%CI = 1.73, 3.92) to have mold inside their homes; 1.57 times more likely (95%CI = 1.09, 2.26) to live in homes where pesticides were used ≥1 time during the past 12-months; and more likely to have more frequent moves (β = 1.06, 95%CI = 0.62, 1.50). (4) Conclusions: Children with home TSE have higher rates of mold, pesticide use, frequent moves, and household members compared to children with no home TSE. Children with TSE should also be screened for home quality and housing instability and provided with appropriate interventions and resources. Full article

Objectives: Smokers with financial and food insecurity may find it difficult to quit smoking and reduce their children’s tobacco smoke exposure (TSE). The objective was to examine the associations between child TSE and financial and food insecurity among U.S. school-aged children. Methods: We examined the 2018–2019 National Survey of Children’s Health data on 17,484 children 6–11 years old. Children were categorized into TSE groups: (1) No TSE: did not live with a smoker; (2) thirdhand smoke (THS) exposure alone: lived with a smoker who did not smoke inside the home; or (3) secondhand smoke (SHS) and THS exposure: lived with a smoker who smoked inside the home. We conducted weighted logistic, ordinal, and linear regression analyses to assess the relationships between child TSE status and financial and food insecurity, adjusting for covariates. Results: Overall, 13.1% and 1.8% of children had THS exposure alone and SHS and THS exposure, respectively. Compared to children with no TSE, children with THS exposure alone were at 2.17 increased odds (95% CI = 1.83, 2.58, p < 0.001) and children with SHS and THS exposure were at 2.24 increased odds (95% CI = 1.57, 3.19, p < 0.001) of having financial insecurity. Children with THS exposure alone were at 1.92 increased odds (95% CI = 1.58, 2.33, p < 0.001) and children with SHS and THS exposure were at 2.14 increased odds (95% CI = 1.45, 3.16, p < 0.001) of having food insecurity. Conclusions: Children with TSE are at increased risk of experiencing financial and food insecurity. Smoking has been recognized by the World Health Organization (WHO) as the fifth highest threat to humanity. Smoking, a leading disease promoter, is a major risk factor for non-communicable diseases (NCDs) such as cancer, cardiovascular disease, diabetes, and chronic respiratory diseases. NCDs account for 63% of all deaths worldwide. Passive smoking is also a health risk. Globally, more than a third of all people are regularly exposed to harmful smoke. Air pollution is a common global problem in which pollutants emitted into the atmosphere undergo a series of physical or chemical reactions to produce various oxidation products, which are often referred to as secondary pollutants. Secondary pollutants include ozone (O3), sulfur trioxide (SO3), nitrogen dioxide (NO2), and respirable particulate matter (PM). It is worth mentioning that third-hand smoke (THS), formed by the reaction of nicotine with second-hand smoke (SHS) caused by indoor O3 or nitrous acid (HONO), is a major indoor secondary pollutant that cannot be ignored. As a form of indoor air pollution that is relatively difficult to avoid, THS exists in any corner of the environment where smokers live. In this paper, we summarize the important research progress on the main components, detection, and toxicity of THS and look forward to future research directions. Scientific understanding of THS and its hazards will facilitate smoking bans in indoor and public places and raise public concern for how to prevent and remove THS. Full article

We aimed to evaluate the degree of thirdhand smoke (THS) caused by short-term exposure to smoking-related substances. To this end, we evaluated the change in concentration of a smoking-related urine biomarker in volunteers before and after visiting public spaces where there is likely THS exposure. We hypothesized that a visit to such public spaces would result in an increase in such biomarkers. Participants visited one of the predetermined facilities (noraebang, PC café) and revisited the same facility after 24 h, spending around 2 h per visit. We selected creatinine-corrected urine cotinine (CUC) as a biomarker to evaluate THS. In addition, we collected nicotine-derived nitrosamine ketone (NNK) from surface dust at each site with cotton swabs (diameter of 2.5 cm). We examined whether CUC concentration significantly changed across three time points (baseline, first visit, and second visit) via repeated-measures analysis of variance (RM-ANOVA). Moreover, we analyzed the interaction to determine whether cigarette smell affects the CUC concentration. Finally, CUC and dust NNK were analyzed with Pearson’s correlation. The CUC concentration did not increase from baseline to the first visit, but increased from the baseline to the second visit (Diff = Ln [0.565] ng/mg, P < 0.01). Further, the CUC concentration increased from the first to the second visit (Diff = Ln [0.393] ng/mg, p < 0.01). In the case of the interaction effect, there were statistically significant differences in CUC concentration depending on the smell of smoke in the facility (Diff = Ln [0.325], F value = 4.438, p value = 0.041). The change in CUC concentration from baseline to the second visit (r = 0.562, p < 0.001) and from the first to the second visit (r = 0.544, p < 0.001) were correlated with NNK concentration. We evaluated whether a short stay in a facility with smoke-related substances that adhere to the surrounding environment would expose individuals to THS even if they do not smell or are directly exposed to cigarette smoke. We confirmed that even two relatively short stays (approximately 2 h each) in a facility in which people had previously smoked can lead to THS exposure. Full article

There is scarce evidence on the knowledge and opinions about third-hand smoke (THS) of health care professionals. The main aim of this study was to explore the knowledge and opinions of health care professionals about THS and, secondarily, to explore the factors that are associated with this knowledge. Cross-sectional study using a snowball sample of multi-national health care professionals (n = 233). Data were obtained from an exploratory, online questionnaire. The health care professionals’ knowledge and opinions on THS were described with absolute frequency and percentage. Chi-square and Fisher-Freeman-Halton exact tests, and simple logistic regression models, were used to explore the bivariate association between the knowledge of the concept THS and sex, continent of birth, educational level, occupation, years of experience, and attitude towards smoking. Finally, a multivariable logistic regression model incorporating all the above variables was fitted. A total of 65.2% of the participants were unaware of the term THS before the study began. In the bivariate analysis, an association was found between prior knowledge of the term THS and continent of birth (p-value = 0.030) and occupation (p-value = 0.014). In the multivariable logistic regression model, a significant association was observed between prior knowledge of the concept THS and sex (p-value = 0.005), continent of birth (p-value = 0.012), and occupation (p-value = 0.001). Almost two out of three health care professionals who participated in our study did not know what THS was. Educational activities on this topic should be implemented. Full article

Toxins present in cigarette and e-cigarette smoke constitute a significant cause of illnesses and are known to have fatal health impacts. Specific mechanisms by which toxins present in smoke impair cell repair are still being researched and are of prime interest for developing more effective treatments. Current literature suggests toxins present in cigarette smoke and aerosolized e-vapor trigger abnormal intercellular responses, damage mitochondrial function, and consequently disrupt the homeostasis of the organelle’s biochemical processes by increasing reactive oxidative species. Increased oxidative stress sets off a cascade of molecular events, disrupting optimal mitochondrial morphology and homeostasis. Furthermore, smoking-induced oxidative stress may also amalgamate with other health factors to contribute to various pathophysiological processes. An increasing number of studies show that toxins may affect mitochondria even through exposure to secondhand or thirdhand smoke. This review assesses the impact of toxins present in tobacco smoke and e-vapor on mitochondrial health, networking, and critical structural processes, including mitochondria fission, fusion, hyper-fusion, fragmentation, and mitophagy. The efforts are focused on discussing current evidence linking toxins present in first, second, and thirdhand smoke to mitochondrial dysfunction. Full article
