Introduction: Although lipomas are common benign tumors found in adults, lipomas of the bladder are extremely rare. Bladder lipomas are infrequently reported in the urologic literature, with only 19 cases published worldwide. These can present as a mass on cystoscopy and cause irritative voiding symptoms, depending on their location. Upon transurethral resection, seeing fat can be concerning for a perforation, as lipoma can be mistaken for extravesical fat. Hence, familiarity with this rare entity is of paramount importance for urologists to prevent unnecessary investigations and interventions that are needed in case of a true bladder perforation. Case presentation: This study presents a case of bladder lipoma in a 73-year-old male with end-stage renal disease who presented for pretransplant urologic evaluation due to microscopic hematuria and irritative lower urinary tract symptoms (LUTS). During cystoscopy, a bladder mass was seen, and a transurethral resection of the bladder tumor (TURBT) revealed bright yellow adipose tissue immediately underneath the bladder mucosa. Concerns about perforation were obviated when seeing intact detrusor muscle underneath, visually confirming the integrity of the bladder wall. The resection was completed, and the CT scan was re-read with the radiologist, which confirmed the presence of a lipoma that was missed pre-operatively due to patient’s oliguria and collapsed bladder. No catheter drainage or cystogram was performed based on these findings. Outcome: The patient healed without any complications. Histopathology confirmed the diagnosis of a mature lipoma. The patient was cleared for transplant from a urologic standpoint and had a successful renal transplantation without delay. Discussion: This case documents the anomalous occurrence of a lipoma within the bladder and supports maintaining a broad differential, including liposarcoma, angiomyolipoma, and other non-malignant fatty tumors during the evaluation of a bladder mass. Background: The International Frontal Sinus Anatomy Classification (IFAC) is a consensus created to simplify the classification of cells affecting frontal sinus drainage. Our study aims to determine the prevalence of the frontal cell variants using the IFAC and to identify their association with the development of FS in the Korean population. Methods: A total of 1060 computed tomography scans of paranasal sinuses (PNS CT) were reviewed. Patient demographics were recorded, and the presentation of types of IFAC cells and presence of frontal sinusitis (FS) were documented. Results: The mean age of the subjects’ scans is 49.8 ± 17, ranging from 16 to 94 years old. The frequency of cells presents from most common to least common are agger nasi cells (ANCs) at 97.1%, suprabullar cells (SBCs) at 73.8%, supraagger cells (SACs) at 38.1%, supraorbital ethmoid cells (SOECs) at 23.3%, frontal septal cells (FSCs) at 19.2%, suprabullar frontal cells (SBFCs) at 16.3% and supraagger frontal cells (SAFCs) at 10.1%. A total of 183 (17.7%) frontal sinuses had an infection, of which the majority were male 67.2%. The presence of SAFCs and/or SBFCs is significantly associated with the development of FS with ORSAFC = 1.646 and ORSBFC = 4.483, respectively. Conclusion: The presence of SAFCs and SBFCs statistically increased the probability of developing FS. Full article

The structural health monitoring of historic buildings represents one of the most significant challenges in contemporary structural analysis, particularly for large-scale structures with accumulated damage. Obtaining reliable diagnostics is crucial yet complex due to the inherent uncertainties in both geometric definition and material properties of historic constructions, especially when structural stability may be compromised. This study presents a comprehensive structural assessment of the Bullring of the Real Maestranza de Caballería de Ronda (Spain), an emblematic 18th-century structure, through an innovative multi-technique approach aimed at evaluating its structural stability. The methodology integrates various non-destructive techniques: 3D laser scanning for precise geometric documentation, operational modal analysis (OMA) for global dynamic characterisation, experimental modal analysis (EMA) for local assessment of critical structural elements, and sonic tests (ST) to determine the elastic moduli of the principal materials that define the historic construction. The research particularly focuses on the inner ring of sandstone columns, identified as the most vulnerable structural component through initial dynamic testing. A detailed finite-element (FE) model was developed based on high-precision laser-scanning data and calibrated using experimental dynamic properties. The model’s reliability was validated through the correlation between numerical predictions and experimental observations, enabling a thorough stability analysis of the structure. Results reveal concerning stability issues in specific columns of the inner ring, identifying elements at significant risk of collapse. This finding demonstrates the effectiveness of the proposed methodology in detecting critical structural vulnerabilities in historic buildings, providing crucial information for preservation strategies. Synchronous multiple parathyroid carcinoma is a rare condition within the already uncommon landscape of parathyroid malignancies, which comprise less than 1% of sporadic primary hyperparathyroidism cases. To date, only seven cases of synchronous multiple parathyroid carcinoma in sporadic primary hyperparathyroidism have been documented. This exceptional rarity complicates both the diagnostic process and therapeutic decision-making. Clinically, parathyroid carcinoma typically presents as a single mass determining severe symptoms. However, no single clinical, biochemical, or imaging feature allows for definitive preoperative diagnosis. Imaging modalities such as ultrasound and sestamibi scans exhibit variable sensitivity and may overlook multi-gland involvement. Histopathological examination remains the only reliable diagnostic method. Management strategies are also controversial: while some advocate for conservative surgery, en bloc resection is generally recommended for its association with improved local control and disease-free survival. Given the exceptional occurrence of synchronous multiple parathyroid carcinoma, there is a lack of standardized protocols for managing parathyroid carcinoma in cases of synchronous and multiple gland involvement. Early multidisciplinary evaluation and individualized treatment planning are therefore crucial. This review aims to synthesize the presently available knowledge about synchronous multiple parathyroid carcinoma, assist clinicians with the limited data available, and discuss the main challenges in the management of this rare entity. Hundreds of Japanese features of war (field positions, tunnels, and fortifications) were constructed in Hong Kong during World War II. However, most of them were poorly documented and were left unknown but still in relatively good condition because of their durable design, workmanship, and remoteness. These features of war form parts of Hong Kong’s brutal history. Conservation, at least in digital form, is worth considering. With the authors coming from multidisciplinary and varied backgrounds, this paper aims to explore these features using a scientific workflow. First, we reviewed the surviving archival sources of the Imperial Japanese Army and Navy. Second, airborne LiDAR data were used to form territory digital terrain models (DTM) based on the Red Relief Image Map (RRIM) for identifying suspected locations. Third, field expeditions of searching for features of war were conducted through guidance of Global Navigation Satellite System—Real-Time Kinetics (GNSS-RTK). Fourth, the found features were 3D-laser scanned to generate mesh models as a digital archive and validate the findings of DTM-RRIM. This study represents a reverse-engineering effort to reconstruct the planned Japanese defense tactics of guerilla fight and Kamikaze grottos that were never used in Hong Kong. Full article

This study presents a critical evaluation of image-based 3D reconstruction techniques for small archaeological artifacts, focusing on a quantitative comparison between Neural Radiance Fields (NeRF), its recent Gaussian Splatting (GS) variant, and traditional Structure-from-Motion (SfM) photogrammetry. The research targets artifacts smaller than 5 cm, characterized by complex geometries and reflective surfaces that pose challenges for conventional recording methods. To address the limitations of traditional methods without resorting to the high costs associated with laser scanning, this study explores NeRF and GS as cost-effective and efficient alternatives. A comprehensive experimental framework was established, incorporating ground-truth data obtained using a metrological articulated arm and a rigorous quantitative evaluation based on root mean square (RMS) error, Chamfer distance, and point cloud density. The results indicate that while NeRF outperforms GS in terms of geometric fidelity, both techniques still exhibit lower accuracy compared to SfM, particularly in preserving fine geometric details. Nonetheless, NeRF demonstrates strong potential for rapid, high-quality 3D documentation suitable for visualization and dissemination purposes in cultural heritage. These findings highlight both the current capabilities and limitations of neural rendering techniques for archaeological documentation and suggest promising future research directions combining AI-based models with traditional photogrammetric pipelines. Single-image super-resolution refers to the reconstruction of a high-resolution image from a single low-resolution observation. Although recent deep learning-based methods have demonstrated notable success on simulated datasets—with low-resolution images obtained by degrading and downsampling high-resolution ones—they frequently fail to generalize to real-world settings, such as document scans, which are affected by complex degradations and semantic variability. In this study, we introduce a task-driven, multi-task learning framework for training a super-resolution network specifically optimized for optical character recognition tasks. We propose to incorporate auxiliary loss functions derived from high-level vision tasks, including text detection using the connectionist text proposal network (CTPN), text recognition via a convolutional recurrent neural network (CRNN), keypoints localization using Key.Net, and hue consistency. To balance these diverse objectives, we employ a dynamic weight averaging (DWA) mechanism, which adaptively adjusts the relative importance of each loss term based on its convergence behavior. Experimental evaluation demonstrates that the proposed approach improves text detection, measured with intersection over union, by 1.09% for simulated and 1.94% for real-world datasets containing scanned documents, while preserving overall image fidelity. These improvements are statistically significant as confirmed by the Kruskal–Wallis H test and the post hoc Dunn test with Benjamini–Hochberg p-value correction. Our findings highlight the value of multi-objective optimization in super-resolution models for bridging the gap between simulated training regimes and practical deployment in real-world scenarios. Background: Chiari-like malformation (CM) and syringomyelia (SM) are commonly observed conditions in Pomeranian dogs. Affected dogs may develop clinical signs that significantly impact quality of life. Therefore, it is crucial to select only unaffected dogs for breeding. However, the progression of CM/SM has not been fully elucidated. Dogs that are unaffected or mildly affected may progress to severe SM over time. The primary aim of this study is to investigate the progression of CM/SM through repeated MRI scans. A secondary objective is to evaluate the effect of furosemide treatment on syrinx sizes, given its frequent prescription. Methods: Pomeranians that underwent two CM/SM screenings between 2015 and 2025 were included. CM/SM classifications were assessed, and quantitative syrinx measurements were conducted. Maximum syrinx diameter (MSD) and maximum syrinx-to-spinal cord diameter ratio (MSD/SCD-r) were measured and documented. Dogs were classified based on the progression of SM. Furosemide treatment was documented, and its effect on syrinx size was compared with that in dogs not receiving furosemide. Results: At the time of the second MRI, 39.6% of dogs either developed SM or showed substantial progression, whereas 12.5% demonstrated partial recovery. Of the dogs initially classified as free from SM, 20.7% had developed the condition. A significant increase was observed in both MSD (p = 0.0058) and MSD/SCD-r (p = 0.0038) between MRI1 and MRI2. Notably, the change in MSD between MRI1 and MRI2 was statistically significantly smaller in dogs treated with furosemide compared to untreated dogs (p = 0.030). Conclusions: These findings indicate that syrinx dimensions are dynamic and may fluctuate over time, although a general trend toward progression is observed. Furthermore, furosemide may mitigate the progression of SM. Full article

Three-dimensional (3D) reproduction of artworks has advanced significantly, offering valuable insights for conservation by documenting the objects’ conservative state at both macroscopic and microscopic scales. This paper presents the 3D survey of an earthquake-damaged panel painting, whose wooden support suffered severe deformation during a seismic event, posing unique restoration challenges. Our work focuses on quantifying how shape variations in the support—induced during restoration—affect the surface morphology of the pictorial layers. To this end, we conducted measurements before and after support consolidation using two complementary 3D techniques: structured-light projection to generate 3D models of the painting, tracking global shape changes in the panel, and laser-scanning microprofilometry to produce high-resolution models of localized areas, capturing surface morphology, superficial cracks, and pictorial detachments. By processing and cross-comparing 3D point cloud data from both techniques, we quantified shape variations and evaluated their impact on the pictorial layers. This approach demonstrates the utility of multi-scale 3D documentation in guiding complex restoration interventions. Full article

The Shenduntou Site, a significant Zhou Dynasty settlement in Anhui Province, provides rare insights into early Chinese woodcraft. This study examines exceptionally preserved wooden structures from Well J1, dating to the Western Zhou period (9th–8th c. BCE). Anatomical analysis identified the timber as Firmiana simplex (L.), indicating ancient selection of this locally available species for its water resistance and mechanical suitability in well construction. Comprehensive degradation assessment revealed severe structural deterioration: maximum water content (1100% ± 85% vs. modern 120% ± 8%) demonstrated extreme porosity from hydrolysis; X-ray diffraction (XRD) showed a 69.5% reduction in cellulose crystallinity (16.1% vs. modern 52.8%); Fourier Transform Infrared Spectroscopy (FTIR) spectroscopy confirmed near-total hemicellulose degradation, partial cellulose loss, and lignin enrichment due to chemical recalcitrance; Scanning Electron Microscopy (SEM) imaging documented multiscale damage including vessel thinning, pit membrane loss, and cell wall delamination from hydrolytic, microbial, and mineral degradation. These findings reflect Western Zhou inhabitants’ pragmatic resource utilisation while highlighting advanced material deterioration that poses significant conservation challenges, providing critical insights into Zhou-era woodcraft and human–environment interactions in the lower Yangtze region. Thanatosomes (hyaline globules or death bodies) are histologically observed in various non-neoplastic and neoplastic conditions. Some of these globules are associated with apoptotic cell death. Only six documented cases of thanatosomes have been reported in breast tumors. In this rare case involving a 64-year-old Japanese woman diagnosed as having rectal cancer, preoperative computed tomography scanning revealed breast cancer in her right breast. Following a right total mastectomy, a tumor characterized as apocrine carcinoma (carcinoma with apocrine differentiation) containing thanatosomes was discovered. These globules are PAS positive and diastase resistant, exhibit deep fuchsinophilic staining with Masson’s trichrome, stain dark blue with PTAH, and are negative for mucin by Alcian blue. The tumor cells tested positive for the androgen receptor, FOXA1, and GCDFP15. Human epidermal growth factor type 2 (HER2)/neu score was 3+/positive, and the Ki-67 labeling index was 60%. Thus, the tumor represented high-grade, HER2-enriched apocrine carcinoma. Thanatosomes are immunoreactive to cleaved caspase-3 and are histological markers of high cell turnover and apoptotic cell death. Therefore, in this nonspecific microscopic neoplastic condition, they are typically linked to high-grade tumors, as this case showed. This report presents a rare case of apocrine breast cancer featuring a limited number of thanatosomes. Full article

Among the craft recipes for artisans collected in the 4th-century Egyptian documents the Leyden and Stockholm papyri, there is one calling for adding animal milk to orchil for wool dyeing. To understand the rationale for this practice, wool yarns were dyed with and without goat milk added to orchil dyebaths, each made using lichens from three different sources. The results showed orchil containing milk dyed yarns a noticeably deeper red hue. The colorants extracted from the dyed yarns were analyzed by liquid chromatography-diode-array-detector-mass spectrometry to assess the relative amounts of nine identifiable orceins. The data showed that the yarns dyed with milk gave extracts exhibiting several fold more α-aminoorcein and α-hydroxyorcein, with only small differences in the other seven. Scanning electron microscopic analysis of a representative pair of dyed yarns showed that milk promoted surface changes in the fiber that may indicate increased cutaneous damage. Hypotheses for the milk’s effects on orchil dyeing were proposed that included the formation of milk–protein complexes with the two enriched orceins that possibly enhanced wool binding and/or better wool uptake of free and/or complexed orceins due to biodegradation of the wool’s surface cuticle caused by microbial growth promoted by the addition of milk. Radiologic and Clinical Correlates of Long-Term Post-COVID-19 Pulmonary Sequelae

Background/Objectives: The long-term sequelae of COVID-19 pneumonia, particularly the persistence of imaging abnormalities and their relationship to clinical symptoms, remain unclear. While the acute radiologic patterns are well-documented, the transition to chronic pulmonary changes—and their implications for long COVID symptoms—require systematic investigation. Methods: Our study included 93 patients with moderate to severe COVID-19 pneumonia who were admitted to Istanbul Medical Faculty Hospital, each having one follow-up CT scan over a ten-month period. Two thoracic radiologists independently calculated semi-quantitative initial chest CT scores to evaluate lung involvement in pneumonia (0–5 per lobe, total score 0–25). Two radiologists and one pulmonologist retrospectively examined the persistence of follow-up imaging findings, interpreting them alongside the relevant clinical and laboratory data. Additionally, in a subcohort (n = 46), mid-term (5–7 months) and long-term (≥10 months) scans were compared to assess temporal trajectories. Results: Among the 93 patients with long-term follow-up imaging, non-fibrotic changes persisted in 34 scans (36.6%), while fibrotic-like changes were observed in 70 scans (75.3%). The most common persistent non-fibrotic changes were heterogeneous attenuation (29%, n = 27) and ground-glass opacities (17.2%, n = 16), and the persistent fibrotic-like changes were pleuroparenchymal bands or linear atelectasis (58%, n = 54), fine reticulation (52.6%, n = 49), and subpleural curvilinear lines (34.4%, n = 32). Both persistent non-fibrotic and fibrotic-like changes were statistically correlated with the initial CT score (p < 0.001), LDH (p < 0.001), and ferritin levels (p = 0.008 and p = 0.003, respectively). Fatigue (p = 0.025) and chest pain (p < 0.001) were reported more frequently in patients with persistent non-fibrotic changes, while chest pain (p = 0.033) was reported more frequently among those with persistent fibrotic-like changes. Among the 46 patients who underwent both mid- and long-term follow-up imaging, 47.2% of those with non-fibrotic changes (17 out of 36) and 10% of those with fibrotic-like changes (4 out of 40) exhibited regression over the long term. Conclusions: Initial imaging and laboratory findings may indicate persistent imaging findings related to long-term sequelae of COVID-19 pneumonia. Many of these persistent imaging abnormalities, particularly non-fibrotic changes seen in the mid-term, tend to lessen over the long term. A correlation exists between persistent imaging findings and clinical outcomes of long COVID-19, underscoring the need for further research. Full article

Detection of Helicobacter pylori Infection in Histopathological Gastric Biopsies Using Deep Learning Models

Traditionally, Helicobacter pylori (HP) gastritis has been diagnosed by pathologists through the examination of gastric biopsies using optical microscopy with standard hematoxylin and eosin (H&E) staining. However, with the adoption of digital pathology, the identification of HP faces certain limitations, particularly due to insufficient resolution in some scanned images. Moreover, interobserver variability has been well documented in the traditional diagnostic approach, which may further complicate consistent interpretation. In this context, deep convolutional neural network (DCNN) models are showing promising results in the automated detection of this infection in whole-slide images (WSIs). The aim of the present article is to detect the presence of HP infection from our own institutional dataset of histopathological gastric biopsy samples using different pretrained and recognized DCNN and AutoML approaches. The dataset comprises 100 H&E-stained WSIs of gastric biopsies. HP infection was confirmed previously using immunohistochemical confirmation. A total of 45,795 patches were selected for model development. InceptionV3, Resnet50, and VGG16 achieved AUC (area under the curve) values of 1. However, InceptionV3 showed superior metrics such as accuracy (97%), recall (100%), F1 score (97%), and MCC (93%). BoostedNet and AutoKeras achieved accuracy, precision, recall, specificity, and F1 scores less than 85%. The InceptionV3 model was used for external validation, and the predictions across all patches yielded a global accuracy of 78%. In conclusion, DCNN models showed stronger potential for diagnosing HP in gastric biopsies compared with the auto ML approach. However, due to variability across pathology applications, no single model is universally optimal. A problem-specific approach is essential. With growing WSI adoption, DL can improve diagnostic accuracy, reduce variability, and streamline pathology workflows using automation. Full article

Generative artificial intelligence (AI) is rapidly transforming healthcare systems since the advent of OpenAI in 2022. It encompasses a class of machine learning techniques designed to create new content and is classified into large language models (LLMs) for text generation and image-generating models for creating or enhancing visual data. These generative AI models have shown widespread applications in clinical practice and research. Such applications range from medical documentation and diagnostics to patient communication and drug discovery. These models are capable of generating text messages, answering clinical questions, interpreting CT scan and MRI images, assisting in rare diagnoses, discovering new molecules, and providing medical education and training. Early studies have indicated that generative AI models can improve efficiency, reduce administrative burdens, and enhance patient engagement, although most findings are preliminary and require rigorous validation. However, the technology also raises serious concerns around accuracy, bias, privacy, ethical use, and clinical safety. Regulatory bodies, including the FDA and EMA, are beginning to define governance frameworks, while academic institutions and healthcare organizations emphasize the need for transparency, supervision, and evidence-based implementation. Generative AI is not a replacement for medical professionals but a potential partner—augmenting decision-making, streamlining communication, and supporting personalized care. Its responsible integration into healthcare could mark a paradigm shift toward more proactive, precise, and patient-centered systems. Full article
