Steady Quiet Asthma Without Biologics: One-Year Outcomes of Single-Inhaler Triple Therapy for Severe Asthma with Small Airway Dysfunction

Background: Small airway dysfunction (SAD) plays a critical role in the management of severe asthma, particularly in patients at risk of requiring biological therapies (BTs). Short-term studies have shown that switching to single-inhaler triple therapy (SITT) with extrafine beclomethasone–formoterol–glycopyrronium improves outcomes and helps achieve quiet asthma, a state marked by symptom control, no exacerbations or oral steroids, reduced inflammation, and better small airway function. This study investigated whether, over one year, patients could maintain this state as Steady Quiet Asthma (SQA) and whether baseline measures could predict this sustained response. Methods: Twenty-six patients with severe asthma and SAD were transitioned from open triple-inhaler therapy to a closed, single-inhaler triple therapy containing extrafine beclomethasone–formoterol–glycopyrronium. Assessments at baseline (T0) and at one-year follow-up (T12) included clinical evaluations, spirometry, and impulse oscillometry, with a focus on Fres as a predictor for the need for BT. When prescribed, biologic therapies included mepolizumab, benralizumab, and dupilumab. Results: Of the 26 patients, 9 (34.6%) achieved SQA and did not require biologic therapy at the one-year follow-up, while 17 patients (65.4%) initiated biologic treatment. At T0, patients who required biologics had significantly higher median Fres (21 (19.47; 24.58) vs. 17.61 (15.82; 20.63); p = 0.049) compared to those who remained biologic-free. They also exhibited higher residual volume to total lung capacity ratio (%RV/TLC) values and lower forced expiratory volume in one second/forced vital capacity ratios (FEV1/FVC). At T12, patients spared from BT showed significant reductions in Fres (p = 0.014) and improvements in small airway function (difference in airway resistance between 5 Hz and 20 Hz (R5–20), forced expiratory flow between 25% and 75% of FVC (%FEF25–75), and better asthma control (ACT). In contrast, patients on BT demonstrated less favorable changes in these parameters. Conclusions: Baseline Fres, FEV1/FVC ratio, and %FEV25–75 are valuable predictors of achieving Steady Quiet Asthma (SQA) and sparing biologic therapy. These findings support the use of SITT in severe asthma and highlight the importance of early functional assessments to guide personalized management. Full article

This study focuses on realistic modeling of forming load and microstructural evolution during hot metal extrusion, emphasizing the effects of friction models and hydraulic press behavior. Rather than merely predicting load magnitudes, the objective is to replicate actual press operation by integrating a load limit response into finite element modeling (FEM). By applying Coulomb and shear friction models under both constant and hydraulically controlled press conditions, the resulting impact on grain size evolution during deformation is examined. The hydraulic press simulation features a maximum load threshold that dynamically reduces die velocity once the limit is reached, unlike constant presses that sustain velocity regardless of load. P91 steel is used as the material system, and the predicted grain size is validated against experimentally measured data. Incorporating hydraulic control into FEM improves the representativeness of simulation results for industrial-scale extrusion, enhancing microstructural prediction accuracy, and ensuring forming process reliability. Digitalization is rapidly reshaping the global construction industry; however, its adoption in developing countries, such as Pakistan, remains limited and uneven. Hence, this study investigates and evaluates the current status of digital technology integration in Pakistan’s construction industry, with a primary focus on key tools, implementation challenges, and necessary policy interventions. Using a three-phase mixed-method approach involving a literature review, expert interviews, and a nationwide survey, this research identifies Building Information Modeling, Geographic Information Systems, and E-Procurement as essential technologies with strong potential to improve transparency, efficiency, and collaboration. However, adoption is hindered by a lack of awareness, limited technical expertise, and the absence of a cohesive national policy. This study also highlights that the private sector shows greater readiness compared to the public sector; however, systemic barriers persist across both sectors. Based on stakeholder insights, a three-part policy strategy was also proposed. This includes establishing a national regulatory framework, investing in capacity-building programs, and providing financial or institutional incentives to encourage the adoption of these measures. The findings emphasize that digitalization is not just a technical upgrade; it represents a pathway to improved governance and more efficient infrastructure delivery. With timely and coordinated policy action, the construction industry in Pakistan can align itself with global innovation trends and move toward a more sustainable and digitally empowered future. This study examines the adverse environmental impacts of solar photovoltaic parks located in established protected areas, aiming to determine the level of landscape fragmentation through the calculation of relevant landscape metrics. For this purpose, a case study was carried out in a Mediterranean Natura 2000 Special Protection Area (SPA), and landscape metrics were calculated using Geographic Information System spatial analysis tools. The analysis of metrics showed that the installation of renewable energy parks within the designated protected area negatively affect landscape fragmentation and the absence of carefully defined and evidence-based mitigation measures. The land cover categories that are significantly affected are those considered critical habitats of bird species that have been designated as SPAs. The results of this study highlight the need to integrate, in the National Renewable Energy Spatial Plans, specific biodiversity objectives, such as conservation objectives and the suspension of the installation of photovoltaic parks in certain areas that are important for conservation of biodiversity, in order to ensure the overall sustainability of renewable energy production. Our contemporary society is powered by fossil fuels, which results in environmental catastrophes. The combustion of these materials results in the release of CO2, which accelerates the progression of climate change and its catastrophic consequences. The environmental repercussions of fossil fuel extraction have been highlighted through research into alternative energy sources. This inquiry uses the Tapio-Z decoupling approach to assess energy inputs and emissions. Furthermore, the fuzzy logic model is used to inspect the economic growth of the USA and China, as well as the impact of environmental factors, energy sources, and utilization, through decoupling effects from 1994 to 2023. The findings are substantiated by the individual perspectives of the environmental factors regarding decoupling, which ultimately lead to the acquisition of valuable results. We anticipate a substantial reduction in the total volume of CO2 emissions in both the USA and China. Compared to China, the USA shows a significant increase in CO2 emissions due to its reliance on fossil fuels. It is evident that a comprehensive transition to renewable resources and a broad range of technology is required to mitigate CO2 emissions in high-energy zones. In their pursuit of sustainability, these two nations are making remarkable strides. The percentage change in CO2 emissions indicates that effective changes in economic growth, energy input, and energy utilization, particularly sustainable energy, transmute energy output, as does the sustained implementation of robust environmental protection policies. The percentage change in CO2 emissions indicates a remarkable transformation in energy input, energy consumption, and economic growth. This transition has been most visible in the areas of energy transformation, sustainability, and the maintenance of strong environmental protection measures. Full article

Water is essential to human civilization and development, yet its quality is increasingly threatened by climate change, pollution, and resource mismanagement. This work introduces an empirical, non-invasive framework for assessing water potability using electrical impedance spectroscopy (EIS) combined with a novel equivalent circuit model. A customized sensor holder was designed to reduce impedance magnitude and enhance phase sensitivity, improving detection accuracy. Various water samples, including seawater, groundwater, and commercially bottled water, were analyzed. The proposed method achieved a 100% classification accuracy in distinguishing among water types, as validated by extracted circuit parameters and verified by inductively coupled plasma (ICP) measurements. Sensitivity analysis demonstrated the ability to detect compositional changes as small as 10%, highlighting a strong potential for fine discrimination of ionic contents. The extracted parameters, such as resistance, capacitance, and inductance, showed clear correlations with ionic composition, enabling reliable potability classification in accordance with WHO guidelines. The approach is rapid, label-free, and suitable for field applications, offering a promising tool for real-time water quality monitoring and supporting sustainable water resource management. As a semi-pelagic fish with commercial value, Mediterranean horse mackerel, Trachurus mediterraneus (Steindachner, 1868), is highly important both for the ecosystem, as a link between trophic levels, and for fisheries and local livelihoods. This study investigates the T. mediterraneus stock along the Romanian coast for more than 10 years (2014–2024), reporting the following data on its bio-ecological characteristics and stock status: size, age, sex ratio, and estimated biomass. Horse mackerel at the Romanian coast revealed an initially slower growth rate followed by acceleration in later years, which may reflect local ecological influences such as resource availability, environmental conditions, or selective pressure. The spatial distribution of the species along the Romanian shelf indicates a clear pattern of coastal aggregation, highlighting the need for targeted and precautionary fisheries management measures, aiming to ensure a sustainable stock. This paper provides geospatial and correlation analysis of heavy metal distribution in the soil cover of the city of Temirtau and its industrial zones. Based on 25 soil samples taken in 2024, concentrations of nine heavy metals (As, Pb, Zn, Cu, Ni, Co, Mn, Cr, Ba) were determined using X-ray fluorescence analysis. Spatial data interpolation was performed using the Kriging method in the ArcGIS Pro environment. The results showed the presence of localized extreme pollution zones, primarily near the Qarmet JSC metallurgical plant. The most significant exceedances of maximum permissible concentrations (MPC), up to 348× MPC for Cr, 160× MPC for Zn, and 72× MPC for As, were recorded at individual locations. Correlation analysis revealed a moderate positive relationship between several elements, particularly Mn and Cu (r = 0.64). Comparison of the spatial distribution of pollution with population data allowed for the assessment of potential environmental risks. This research emphasizes the need to implement systematic monitoring, sustainable land management practices, ecological maps, and preventive measures to reduce the long-term impact of heavy metals on ecosystems and public health, and to promote environmental sustainability in industrial regions. In the digital era, secure data sharing has become a core requirement for enabling cross-domain collaboration, cloud computing, and Internet of Things (IoT) applications, as well as a critical measure for safeguarding privacy and defending against malicious attacks. In light of the risks of data leakage and misuse in open environments, achieving efficient, controllable, and privacy-preserving data sharing has emerged as a key research focus. This paper first provides a systematic review of the prevailing secure data sharing technologies, including proxy re-encryption, searchable encryption, key agreement and distribution, and attribute-based encryption, summarizing their design principles and application features. Subsequently, game-theoretic modeling based on incentive theory is introduced to construct a strategic interaction framework between data owners and data users, aiming to analyze and optimize benefit allocation mechanisms. Furthermore, the paper explores the integration of game theory with secure sharing mechanisms to enhance the sustainability and stability of the data sharing ecosystem. Finally, it outlines the critical challenges currently faced in secure data sharing and discusses future research directions, offering theoretical insights and technical references for building a more comprehensive data sharing framework. The Impact of Regional Policies on Chinese Business Growth: A Bibliometric Approach

In the context of both domestic and international economic landscapes, regional policy has emerged as an increasingly influential factor shaping the developmental trajectories of Chinese enterprises. Despite its growing significance, the extant literature lacks a comprehensive and systematically visualized synthesis that encapsulates the scope and trends of research in this domain. This study addresses this critical gap by conducting an integrative bibliometric and qualitative review of the academic output related to regional policy and Chinese firm growth. Drawing on a final dataset comprising 3428 validated academic publications—selected from an initial pool of 3604 records retrieved from the Web of Science Core Collection between 1991 and 2022, the research employs a two-stage methodological framework. In the first phase, advanced bibliometric tools, and software applications, including RStudio, Bibliometrix, VOSviewer, and CitNetExplorer, are utilized to implement techniques such as keyword co-occurrence analysis, thematic clustering, and the tracing of thematic evolution over time. These methods facilitate rigorous data cleansing, breakpoint identification, and the visualization of intellectual structures and emerging research patterns. In the second phase, a targeted qualitative review is conducted to evaluate the influence of regional policies on Chinese firms across three critical stages of business development: start-up, expansion, and maturity. The findings reveal that regional policy interventions generally exert a positive influence on firm performance throughout all stages of development. Notably, a significant concentration of citation activity occurred prior to 2017; however, post-2017, the volume of scholarly publications, journal-level impact (as measured by h-index), and author-level influence experienced a marked increase. Among the 3428 analyzed publications, a substantial portion—2259 articles—originated from Chinese academic institutions, highlighting the strong domestic research interest in the subject. Furthermore, since 2015, there has been a discernible shift in keyword co-occurrence trends, with increasing scholarly attention directed towards sustainable development issues, particularly those related to carbon dioxide emissions and green innovation, reflecting evolving policy priorities and environmental imperatives. The intra-articular application of drugs has gained considerable interest with regard to formulations for advanced drug delivery systems. It has been identified as a potential route for local drug delivery. A drug agent is usually incorporated into the hydrogel to prolong and control the drug release. This study aimed to design and evaluate an intra-articular hydrogel based sodium hyaluronate, which was modified with an additional polymer to enable the sustained release of the incorporated anti-inflammatory agent, diclofenac sodium (NaDic). Viscosity studies, drug release tests and FTIR−ATR measurements, as well as DSC analysis, were carried out to evaluate the obtained formulations. The viscosity measurements were performed using a rotational viscometer. The drug release was carried out by employing the apparatus paddle over the disk. The concentration of the released drug was obtained spectrophotometrically. The results revealed that the addition of the second polymer to the matrix influenced the dynamic viscosity of the hydrogels. The highest viscosity of (25.33 ± 0.55) × 103 cP was observed when polyacrylic acid (PA) was doped in the formulation. This was due to the hydrogen bond formation between both polymers. The FTIR−ATR investigations and DSC study revealed the hydrogen bond formation between the drug and both polymers. The drug was released the slowest from hydrogel doped with PA and 17.2 ± 3.7% of NaDic was transported to the acceptor fluid within 8 h. The hydrogel based on hyaluronan sodium doped with PA and containing NaDic is a promising formulation for the prolonged and controlled intra-articulate drug delivery of anti-inflammatory agents. The intensification of anthropogenic pressures, particularly those related to agriculture driven by increasing demands for food and cash crops, generates negative environmental externalities. Assessing these externalities is essential to better identify and implement measures that promote the environmental sustainability of rural landscapes. This study aims to develop a multi-criteria assessment method of the negative environmental externalities of rural landscapes in the northern Benin agricultural basin, based on satellite-derived data. Starting from a 12-class land cover map produced through satellite image classification, the evaluation was conducted in three steps. First, the 12 land cover classes were reclassified into Human Disturbance Coefficients (HDCs) via a weighted sum model multi-criteria analysis based on nine criteria related to the negative environmental externalities of anthropogenic activities. Second, the HDC classes were spatially aggregated using a regular grid of 1 km2 landscape cells to produce the Landscape Environmental Sustainability Index (LESI). Finally, various discretization methods were applied to the LESI for cartographic representation, enhancing spatial interpretation. Results indicate that most areas exhibit moderate environmental externalities (HDC and LESI values between 2.5 and 3.5), covering 63–75% (HDC) and 83–94% (LESI) of the respective sites. Areas of low environmental externalities (values between 1.5 and 2.5) account for 20–24% (HDC) and 5–13% (LESI). The LESI, derived from accessible and cost-effective satellite data, offers a scalable, reproducible, and spatially explicit tool for monitoring landscape sustainability. It holds potential for guiding territorial governance and supporting transitions towards more sustainable land management practices. Future improvements may include, among others, refining the evaluation criteria and introducing variable criteria weighting schemes depending on land cover or region. Full article

This article aims to explore and analyze current issues and features of environmental protection in managing local tourism destinations based on the principles of sustainable development through the relevant literature and thus to provide an insight into major environmental measures and activities that should be implemented in practice, emphasizing the importance of environmental sustainability as a key factor in the development and success of local tourist destinations in today’s business environment. Qualitative methods were used, with the literature review based on content analysis by keywords. This particularly affects the business process efficiency and the participation of destination stakeholders and in many cases leads to a low level of environmentally sustainable destination practices. In addition to this theoretical approach, this study also has direct managerial implications for destination environmental business operations. An attractive and well-preserved environment is the primary factor of tourism and local tourism destination development and its success, as well as an integrated part of the tourism product. This study addresses a critical gap in the existing literature on environmental sustainability at local destinations, where prior work has often overlooked the integration of actionable, practice-oriented frameworks tailored for both researchers and practitioners. While theoretical insights into sustainable practices abound, there remains a scarcity of holistic analyses that bridge scholarly understanding with implementable strategies for on-the-ground application. To fill this void, our research provides a comprehensive overview and systematic analysis of current practices, with targeted emphasis on co-developing scalable frameworks for improving environmentally sustainable practices at local destinations. This study evaluates the effectiveness of the B Impact Assessment (BIA) as a catalyst for integrating sustainability into industrial firms through a qualitative case study of LC Paper, the first B Corp-certified tissue manufacturer globally and a pioneer in applying BIA in the pulp and paper sector. Based on semi-structured interviews, organizational documents, and direct observation, this study examines how BIA influences corporate governance, environmental practices, and stakeholder engagement. The findings show that BIA fosters structured goal setting and the implementation of measurable actions aligned with environmental stewardship, social responsibility, and economic resilience. Tangible outcomes include improved stakeholder trust, internal transparency, and employee development, while implementation challenges such as resource allocation and procedural complexity are also reported. Although the single-case design limits generalizability, this study identifies mechanisms transferable to other firms, particularly those in environmentally intensive sectors. The case studied also illustrates how leadership commitment, participatory governance, and data-driven tools facilitate the operationalization of sustainability. By integrating stakeholder and institutional theory, this study contributes conceptually to understanding certification frameworks as tools for embedding sustainability. This research offers both theoretical and practical insights into how firms can align strategy and impact, expanding the application of BIA beyond early adopters and into traditional industrial contexts. Development and Validation of a Self-Assessment Tool for Convergence Competencies in Humanities, Arts, and Social Sciences for Sustainable Futures in the South Korean Context

Addressing global challenges such as climate change and inequality requires convergence competencies that enable learners to devise sustainable solutions. Such competencies have been emphasized in Science, Technology, Engineering, Mathematics (STEM) fields, but empirical research and assessment tools tailored to Humanities, Arts, and Social Sciences (HASS) remain scarce. This study aimed to develop and validate a self-assessment tool to measure convergence competencies among HASS learners. A three-round Delphi survey with domain experts was conducted to evaluate and refine an initial pool of items. Items with insufficient content validity were revised or deleted, and all retained items achieved a Content Validity Ratio (CVR) of ≥0.800, with most scoring 1.000. The validated instrument was administered to 455 undergraduates participating in a convergence education program. Exploratory factor analysis identified five key dimensions: Convergent Commitment, Future Problem Awareness, Future Efficacy, Convergent Learning, and Multidisciplinary Inclusiveness, explaining 69.72% of the variance. Confirmatory factor analysis supported the model’s goodness-of-fit (χ2 (160) = 378.786, RMSEA = 0.054, CFI = 0.952), and the instrument demonstrated high internal consistency (Cronbach’s α = 0.919). The results confirm that the tool is both reliable and valid for diagnosing convergence competencies in HASS contexts, providing a practical framework for interdisciplinary learning and reflective engagement toward sustainable futures.
