Long-Term Outcomes of First-Line Anti-TNF Therapy for Chronic Inflammatory Pouch Conditions: A Multi-Centre Multi-National Study

Background/Objectives: Anti-tumour necrosis factor (anti-TNF) medications were historically commonly prescribed as the first-line biologic treatment for chronic inflammatory pouch conditions. However, their use in these conditions is mainly based on retrospective studies of relatively small numbers of patients with short follow up periods. We aimed to describe the long-term outcomes of first-line anti-TNF therapy in a large, multi-centre, multi-national patient cohort with chronic inflammatory pouch conditions. Methods: This was an observational, retrospective, multi-centre, multi-national study. We included patients with chronic inflammatory pouch conditions initially treated with anti-TNF drugs infliximab (IFX) or adalimumab (ADA), who had a follow up of at least 1 year. The primary outcome was anti-TNF treatment persistence, defined as continuation of anti-TNF throughout the study period. The secondary outcome was pouch failure, defined by the need for a defunctioning ileostomy or pouch excision. Results: We recruited 98 patients with chronic inflammatory pouch conditions initially treated with anti-TNF medications—63 (64.3%) treated with IFX and 35 (35.7%) treated with ADA. Average follow up length was 94.2 months (±54.5). At the end of the study period only 22/98 (22.4%) patients were still on anti-TNF treatment. In those in whom the first-line anti-TNF was discontinued, the median time to discontinuation was 12.2 months (range 5.1–26.9 months). The most common cause for anti-TNF discontinuation was lack of efficacy despite adequate serum drug levels and absence of anti-drug antibody formation (30 patients, 30.6%). Loss of response due to anti-drug antibody formation was the cause for discontinuation in 18 patients (18.4%), while 12 patients (12.2%) stopped treatment because of adverse events or safety concerns. Out of the 76 patients discontinuing anti-TNF treatment, 34 (34.7% of the cohort) developed pouch failure, and 42 (42.8% of the cohort) are currently treated with a different medical therapy. Conclusions: First-line anti-TNF therapy for chronic pouch inflammatory conditions is associated with low long-term persistence rates. This is due to a combination of lack of efficacy and adverse events. A significant percentage of patients initially treated with anti-TNF therapy develop pouch failure. Full article

With a potentially increasing share of the electricity grid relying on wind to provide generating capacity and energy, there is an expanding global need for historically accurate, spatiotemporally continuous, high-resolution wind data. Conventional downscaling methods for generating these data based on numerical weather prediction have a high computational burden and require extensive tuning for historical accuracy. In this work, we present a novel deep learning-based spatiotemporal downscaling method using generative adversarial networks (GANs) for generating historically accurate high-resolution wind resource data from the European Centre for Medium-Range Weather Forecasting Reanalysis version 5 data (ERA5). In contrast to previous approaches, which used coarsened high-resolution data as low-resolution training data, we use true low-resolution simulation outputs. We show that by training a GAN model with ERA5 as the low-resolution input and Wind Integration National Dataset Toolkit (WTK) data as the high-resolution target, we achieved results comparable in historical accuracy and spatiotemporal variability to conventional dynamical downscaling. This GAN-based downscaling method additionally reduces computational costs over dynamical downscaling by two orders of magnitude. We applied this approach to downscale 30 km, hourly ERA5 data to 2 km, 5 min wind data for January 2000 through December 2023 at multiple hub heights over Ukraine, Moldova, and part of Romania. With WTK coverage limited to North America from 2007–2013, this is a significant spatiotemporal generalization. The geographic extent centered on Ukraine was motivated by stakeholders and energy-planning needs to rebuild the Ukrainian power grid in a decentralized manner. This 24-year data record is the first member of the super-resolution for renewable energy resource data with wind from the reanalysis data dataset (Sup3rWind). Full article

Background and Objectives. Umbilical hernia is particularly common among patients with liver cirrhosis, affecting about 20% of this group, compared to 3–8.5% in healthy individuals. This increased prevalence is mainly due to weakened abdominal fascia, elevated intra-abdominal pressure, and malnutrition. The rapid progression of umbilical hernias often leads to complications such as skin necrosis, perforation, and strangulation. Historically, patients with liver cirrhosis and complicated umbilical hernia have faced high morbidity and mortality rates. However, recent advancements in perioperative management, especially in controlling ascites, have improved outcomes in elective treatments. Despite these advancements, managing patients with decompensated liver cirrhosis and complicated umbilical hernia in emergency settings remain a significant surgical challenge. Materials and Methods: We conducted a retrospective review of patients treated for complicated umbilical hernia at the University Medical Centre Ljubljana from 2015 to 2024, using prospectively collected data. This analysis involved implementing hernioplasty combined with incisional negative pressure wound therapy (iNPWT) as part of the surgical protocol. The primary endpoint of our study was the rate of local complications, while the secondary endpoints included the rate of systemic complications and 90-day mortality. Results: We treated 28 consecutive patients with complicated umbilical hernia and liver cirrhosis. Local wound complications were observed in three (10.7%) patients. Systemic complications developed in 10 patients (35.7%). The median duration of hospitalization was 8 days (range: 5–29), and no readmissions were recorded within the 30-day period. Two (7.1%) patients died within 90 days. Conclusions: Our experience indicates that iNPWT, when combined with surgical repair, can be safely utilized, yielding outcomes comparable to elective hernia repairs, even in emergency contexts. Further randomized controlled trials are necessary to validate these findings and optimize treatment protocols. Full article

Urban food markets are increasingly being recognized not only as centres of cultural identity and tourism but also as pivotal spaces for promoting urban sustainability. This study explores the role of urban markets in advancing sustainable cultural tourism, using Obor Market in Bucharest as a case study. As a historic marketplace and cultural landmark, Obor Market embodies Bucharest’s traditional commercial practices and community-oriented values. Through a mixed-methods approach, combining Geographic Information Systems (GIS) mapping and systematic surveys, we evaluate the location of markets, cultural authenticity, and visitor satisfaction. The quantitative findings reveal meaningful correlations between demographic factors (particularly nationality and age) and visitor perceptions, highlighting the market’s appeal through its authentic culinary offerings and immersive sensory experience. However, this study also identifies shortcomings in current promotional strategies employed by local stakeholders. The results suggest that urban food markets can serve as sustainable urban assets, fostering cross-cultural integration, supporting local economies, and encouraging environmentally conscious tourism behaviours. We argue for data-informed urban cultural policies that enhance the visibility, accessibility, and sustainability of such spaces, reinforcing their dual role as economic drivers and cultural touchstones. This research is focused on housing stock rehabilitation and construction in Urban Rehabilitation Areas located in diverse contexts in the Portuguese territory. The main objective of this research is to show how the local actors have managed the ARUs’ opportunities to restore and develop the housing in these areas in the Portuguese territory. An analytical national legal framework is made to show that the diffuse criteria at national and regional levels are reflected in the limited effectiveness of the ARUs’ flexible criteria in local implementation. A national legislative and regulatory framework in Portugal, focusing on urban rehabilitation and housing promotion themes, is discussed to emphasize the potential role of Urban Rehabilitation Area (ARU) particularities and housing provision and preservation in diverse contexts in Portugal. A comparative analysis is conducted of five ARUs—Belmonte, Soure, Penacova, Vila Real, and Devesas—located in Portugal, in the North and Center regions, to highlight the particularities/diversity of urban contexts, including towns, small to medium-sized cities, and historic centres. The analysis assesses the effectiveness of ARU urban rehabilitation strategy implementation over time. The analysis of five ARUs will discuss the following: (i) ARU physical characteristics; (ii) ARU population profile; (iii) ARU urban rehabilitation strategies progress (initial, intermediate, and final); and (iv) ARU alignment with PDM priorities in urban rehabilitation. The findings underscore the pivotal role that ARUs and their actors can have in housing rehabilitation provision and preservation on different scales and contexts within the territory. The outcomes show different strategies that each ARU has used to prioritize building rehabilitation. Full article

In response to growing concerns about overtourism and the need for sustainable heritage tourism, this study investigates how external environmental stimuli affect tourists’ perceived value and behavioral intentions in historic urban environments. Using the Stimulus–Organism–Response (SOR) model as the theoretical framework, and based on 275 valid questionnaires collected from five major sites in the Historic Center of Macau, this study explores the direct and indirect relationships among four types of environmental stimuli (physical factors, social activities, environmental atmosphere, and information and services), perceived value, and behavioral intentions. The results of structural equation modeling (SEM) indicate that all four stimuli significantly influence perceived value and behavioral intentions. Among them, physical factors exert the strongest influence on perceived value (β = 0.291, p < 0.001), while social activities are the most influential predictor of behavioral intentions (β = 0.225, p < 0.01). Perceived value plays a significant mediating role in all relationships, with the largest mediation effect found in the path from physical factors to behavioral intentions (27.99%), followed by environmental atmosphere (24.80%), information and services (22.62%), and social activities (11.66%). These findings validate the applicability of the SOR model in heritage tourism contexts and highlight the central role of perceived value in shaping tourist behavior. Theoretically, this study advances our understanding of how multidimensional environmental stimuli contribute to value-based decision-making in tourism. Practically, it provides actionable insights for urban planners and heritage managers to design environments that promote deeper engagement and foster sustainable tourist behavior in high-density historic destinations like Macau. Various platforms, such as Google Maps, provide information about the services of public transport companies worldwide. Operators publish the planned (static) timetable using the General Transit Feed Specification (GTFS) format, while the GTFS Realtime (GTFS-RT) specification provides live (dynamic) information about the services. In this paper, we present our dataset that was built by retrieving and pre-processing the data sources of the open data platform of BKK Futár, hosted by the Centre for Budapest Transport Company (BKK). The paper contains a well-detailed description of our methods for retrieving and pre-processing the data among statistical features. The dataset covers a one-year period in which the data collection mechanism used for realtime data was continuously improved from collecting only live vehicle positions to covering all the available feeds and increasing the query frequency. We merged the static data with the vehicle positions to filter them, yielding a clean set of tracked trips. As a result, more than 90% of the daily planned trips could be reconstructed from the responses. We provide an interactive web-based visualization for the analysis of the GTFS schedule’s, and the GTFS-RT Vehicle Positions feed’s, geospatial features. The dataset and also our methodology can serve as input for various research studies to investigate the common characteristics of delays and disruptions or predict real departure times based on the current vehicle positions and historical data. Historic centres are extremely complex parts of contemporary cities, particularly from morphological, architectural, and cultural points of view, where a significant proportion of the land area may be occupied by built heritage sites that require protection and conservation. These urban contexts are also characterised by scarce green and public open spaces endowment, a high proportion of private property, and high levels of natural risk exposure. From a climate change adaptation perspective, Nature-based Solutions (NbS) have emerged as measures to manage urban ecosystems to address environmental and societal challenges. To overcome the conflicting objectives of climate change adaptation and built heritage preservation, this study proposes a three-step methodology applied to the historic centre of Catania (Italy): (i) Land-Use/Landownership and Land Cover/Maintenance and Quality analyses; (ii) Land Transformability Assessment; (iii) Land Transformation Scenarios Assessment. According to this methodology, five Land Transformation Scenarios have been drawn up: (1) NbS full installation; (2) NbS installation with some limitations; (3) NbS installation after re-arrangement; (4) NbS installation strongly limited; (5) NbS installation not viable. This approach allowed us to identify the most feasible and suitable buildings and open spaces, while distinguishing public and private properties, to implement a more comprehensive integration of NbS and built heritage preservation in historic cities for mutual benefits. Between the late-nineteenth and the early-twentieth centuries, Beatrice and Laura, as literary characters and beloved women of Dante and Petrarch, were at the centre of a vigorous scholarly debate, which gained traction in Romagna’s literary circles. Offering a comparative analysis of two key case studies—La vita di Beatrice Portinari (1904) by Giovanni Federzoni, and Laura’s biographical profile from Femminismo storico (1901) by Sfinge (Eugenia Codronchi Argeli)—this article reconstructs the popularising turn of this debate and its effect on medieval female characters’ reception as poetic inspirers. While Federzoni is motivated by didactic aims, seeking to facilitate readers’ access to the Commedia by deconstructing Beatrice’s abstraction, Sfinge elevates Laura as a model for contemporary women. Through an accessible structure and a hybrid methodology blending historical inquiry with literary imagination, both authors challenge allegorical readings and reclaim Beatrice and Laura as historically grounded figures. Large-scale windstorms represent an important atmospheric hazard in the Northeastern US (NE) and are associated with substantial socioeconomic losses. Regional simulations performed with the Weather Research and Forecasting (WRF) model using lateral boundary conditions from three Earth System Models (ESMs: Geophysical Fluid Dynamics Laboratory (GFDL), Hadley Centre Global Environment Model (HadGEM) and Max Planck Institute (MPI)) are used to quantify possible future changes in windstorm characteristics and/or changes in the parent cyclone types responsible for windstorms. WRF nested within MPI ESM best represents important aspects of historical windstorms and the cyclone types responsible for generating windstorms compared with a reference simulation performed with the ERA-Interim reanalysis for the historical climate. The spatial scale and frequency of the largest windstorms in each simulation defined using the greatest extent of exceedance of local 99.9th percentile wind speeds (U > U999) plus 50-year return period wind speeds (U50,RP) do not exhibit secular trends. Projections of extreme wind speeds and windstorm intensity/frequency/geolocation and dominant parent cyclone type associated with windstorms vary markedly across the simulations. Only the MPI nested simulations indicate statistically significant differences in windstorm spatial scale, frequency and intensity over the NE in the future and historical periods. This model chain, which also exhibits the highest fidelity in the historical climate, yields evidence of future increases in 99.9th percentile 10 m height wind speeds, the frequency of simultaneous U > U999 over a substantial fraction (5–25%) of the NE and the frequency of maximum wind speeds above 22.5 ms−1. These geophysical changes, coupled with a projected doubling of population, leads to a projected tripling of a socioeconomic loss index, and hence risk to human systems, from future windstorms. In many European cities, old urban centres, particularly historical centres, reveal significant limitations to the intervention of civil protection agents—namely, firefighters—in terms of their mobility and action in emergencies, thus conditioning their action. This article analyses the case of a Portuguese city (the city of Ílhavo), proposing possible solutions applicable to territories with these specificities. This study was developed with the support of a literature review and field work that accompanied the actions of the local fire department. The proposals include new technical solutions (such as underground dry pipeworks), measures regarding traffic restrictions, the adequacy of signage, and recommendations for training the population living in these areas to intervene in fire situations. This study was developed through monitoring several interventions by the local fire department in real emergencies, allowing for the identification of some existing limitations in its activity resulting from the specific characteristics of the old urban centre. Measures already adopted in other Portuguese cities to improve mobility and reduce the negative impact on firefighters’ work in historical centres are also mentioned. It is also recommended that all civil protection agents be closely involved in planning and designing these urban rehabilitation interventions when carrying them out in these areas. Full article

Architecture in each era is profoundly shaped by its prevailing scientific and technological context, just as architects’ intellectual frameworks are influenced by contemporaneous philosophies of science. Since the 19th century, rapid scientific and technological advances have coincided with the parallel development of scientific–philosophical thought, giving rise to the modern and postmodern architectural movements. This paper reviews a century’s philosophy of science development and traces its impact on architectural thought, mapping the co-evolution of “philosophy of science—architecture” within a historical timeline and highlighting key paradigm shifts. It then investigates how technology and philosophies of science leverage architecture as a mediator to advance their own evolution. Through a comparative case study of Sou Fujimoto’s design theories and projects, this paper elucidates the mechanisms, constraints, and logical progression linking scientific philosophy and architectural practice. A Deep Learning Method for Photovoltaic Power Generation Forecasting Based on a Time-Series Dense Encoder

Deep learning has become a widely used approach in photovoltaic (PV) power generation forecasting due to its strong self-learning and parameter optimization capabilities. In this study, we apply a deep learning algorithm, known as the time-series dense encoder (TiDE), which is an MLP-based encoder–decoder model, to forecast PV power generation. TiDE compresses historical time series and covariates into latent representations via residual connections and reconstructs future values through a temporal decoder, capturing both long- and short-term dependencies. We trained the model using data from 2020 to 2022 from Australia’s Desert Knowledge Australia Solar Centre (DKASC), with 2023 data used for testing. Forecast accuracy was evaluated using the R2 coefficient of determination, mean absolute error (MAE), and root mean square error (RMSE). In the 5 min ahead forecasting test, TiDE demonstrated high short-term accuracy with an R2 of 0.952, MAE of 0.150, and RMSE of 0.349, though performance declines for longer horizons, such as the 1 h ahead forecast, compared to other algorithms. For one-day-ahead forecasts, it achieved an R2 of 0.712, an MAE of 0.507, and an RMSE of 0.856, effectively capturing medium-term weather trends but showing limited responsiveness to sudden weather changes. Further analysis indicated improved performance in cloudy and rainy weather, and seasonal analysis reveals higher accuracy in spring and autumn, with reduced accuracy in summer and winter due to extreme conditions. Additionally, we explore the TiDE model’s sensitivity to input environmental variables, algorithmic versatility, and the implications of forecasting errors on PV grid integration. These findings highlight TiDE’s superior forecasting accuracy and robust adaptability across weather conditions, while also revealing its limitations under abrupt changes. Full article

This study presents a comprehensive bibliometric analysis of scientific publications on electrochemical etching and electrochemical deposition from 1970 to 2023. Using the Science Citation Index Expanded (SCIE) database, we analysed 5166 publications on electrochemical etching and, 30,759 publications on electrochemical deposition. The analysis reveals distinct yet interconnected research landscapes for these two techniques. Electrochemical etching research has focused on themes such as porous silicon, photoluminescence, and applications in photonics, while electrochemical deposition research has centred on energy storage, catalysis, and biosensing applications. Keyword co-occurrence analysis illustrates the progression from fundamental studies to specialised applications in both fields. This study highlights the importance of international collaboration and provides insights into the historical and contemporary advancements in electrochemical methods for nanomaterial synthesis. The findings underscore the complementary nature of electrochemical etching and deposition, driving innovation and offering new opportunities in materials science and technology. Full article

Palynology is an invaluable tool for reconstructing past biodiversity in agrarian and cultural landscapes and for understanding present-day environmental assets. By analysing past evidence, rooted in botanical knowledge, we can foresee future environmental trends. Italy, at the centre of the Mediterranean, is one of the richest countries in terms of pollen analyses from archaeological sites and therefore is particularly suited to reconstructing human–environment relationships and anthropogenic impacts on flora over time. We selected data filled in the database BRAIN. This paper presents new elaboration on pollen data from 14 published and unpublished archaeological sites, showing past plant diversity and land management in prehistorical and historical contexts of southern Italy. Overall, the research demonstrates that the floristic palaeodiversity, as revealed through the group-equalised indicator species analysis, supports and validates the palynological data on the flora of Campania, Basilicata, and Sicily. The study highlights the presence of ubiquitous pollen taxa in anthropogenic environments and explores the connection between past and present plant diversity. Full article
