Background: Current food systems contribute significantly to poor public health and environmental degradation. With increasing rates of chronic disease and undernutrition globally and in the United States of America (U.S.), transforming food systems toward sustainability is a critical public health priority. Objectives: This narrative review aimed to summarize U.S. policies from the past decade relevant to sustainable food systems, focusing on four domains—availability, accessibility, affordability, and desirability—proposed by the 22nd Annual Harvard Nutrition Obesity Symposium. Methods: Systematic searches were conducted using MEDLINE (PubMed), Scopus, U.S. Congress websites, and Google searches. Studies and policies published between 2013 and 2023 that addressed at least one of the four domains were included. Policies were reviewed according to their alignment with the policy lifecycle framework, encompassing formulation through implementation. Results: A review of the final 632 articles explained that, despite growing interest in sustainable food systems, there is a lack of comprehensive U.S. policies that address the four domains in an integrated manner. Most initiatives were limited in scope, often school-based, and not explicitly sustainability-focused. Food availability and accessibility policies exist but remain fragmented, while affordability and desirability domains are severely underrepresented. Few laws or bills have been enacted or evaluated for population-level or environmental outcomes. Conclusions: Transforming U.S. food systems requires more robust evidence-based policy development and evaluation. There is an urgent need for integrated multisectoral policy frameworks to ensure health, equity, and sustainability across all food system domains. Full article

Produce supply chains play a critical role in ensuring fruits and vegetables reach consumers efficiently, affordably, and at optimal freshness. In recent decades, hub-and-spoke network models have emerged as valuable tools for optimizing sustainable cold food supply chains. Traditional optimization efforts typically focus on removing inefficiencies, minimizing lead times, refining inventory management, strengthening supplier relationships, and leveraging technological advancements for better visibility and control. However, the majority of models rely on deterministic approaches that overlook the inherent uncertainties of crop yields, which are further intensified by climate variability. Rising atmospheric CO2 concentrations, along with shifting temperature patterns and extreme weather events, have a substantial effect on crop productivity and availability. Such uncertainties can prompt distributors to seek alternative sources, increasing costs due to supply chain reconfiguration. This research introduces a stochastic hub-and-spoke network optimization model specifically designed to minimize transportation expenses by determining optimal distribution routes that explicitly account for climate variability effects on crop yields. A use case involving a cold food supply chain (CFSC) was carried out using several weather scenarios based on climate models and real soil data for California. Strawberries were selected as a representative crop, given California’s leading role in strawberry production. Simulation results show that scenarios characterized by increased rainfall during growing seasons result in increased yields, allowing distributors to reduce transportation costs by sourcing from nearby farms. Conversely, scenarios with reduced rainfall and lower yields require sourcing from more distant locations, thereby increasing transportation costs. Nonetheless, supply chain configurations may vary depending on the choice of climate models or weather prediction sources, highlighting the importance of regularly updating scenario inputs to ensure robust planning. This tool aids decision-making by planning climate-resilient supply chains, enhancing preparedness and responsiveness to future climate-related disruptions. Full article

Despite growing recognition of the role that social determinants of health (SDOHs) and health-related social needs (HRSNs) play in chronic disease, limited research has examined their associations with Chronic Obstructive Pulmonary Disease (COPD) in population-based studies. This cross-sectional study analyzed 2022 Behavioral Risk Factor Surveillance System (BRFSS) data from 37 U.S. states and territories to determine how financial hardship, food insecurity, employment loss, healthcare access barriers, and psychosocial stressors influence the prevalence of COPD. Weighted logistic regression models were used to assess the associations between COPD and specific SDOHs and HRSNs. Several individual SDOH and HRSN factors were significantly associated with COPD prevalence, with financial strain emerging as a particularly strong predictor. In models examining specific SDOH factors, economic hardships like inability to afford medical care were strongly linked to higher COPD odds. Psychosocial HRSN risks, such as experiencing mental stress, also showed moderate associations with increased COPD prevalence. These findings suggest that addressing both structural and individual-level social risks may be critical for reducing the prevalence of COPD in populations experiencing financial challenges. Background/Objective: Food insecurity refers to social or economic challenges that limit or create uncertainty around access to enough food. Among college students, food security status is usually determined with the USDA 10-item Food Security Survey Module, which has not been validated for this population. Nutrition security refers to consistent access to food and beverages that promote well-being, prevent disease, and emphasize equitable access to healthy, safe, and affordable foods. Currently, there is no standardized measure that assesses food and nutrition security tailored to the unique experiences of college students. This study aims to evaluate the validity and reliability of a newly developed College Student Food and Nutrition Security Survey Module (CS-FNSSM). Methods: A mixed-methods approach that combined an online survey with semi-structured cognitive interviews. Participants were students aged 18 and older from U.S. public universities. Quantitative data were analyzed using RStudio (version 4.4.1), and interview transcripts were thematically analyzed. Results: Survey responses were collected from 953 participants, including a subset of 69 participants for reliability testing and 30 participants for cognitive interviews. Rasch analysis showed good item performance and structural validity. The CS-FNSSM demonstrated strong sensitivity (89.09%), specificity (76.2%), moderate test–retest reliability (0.59), and good internal consistency (Cronbach’s alpha = 0.79). Qualitative findings confirmed participant understanding of the items. Conclusions: The CS-FNSSM effectively identifies food and nutrition insecurity, with nutrition security emerging as a key issue. Addressing both is crucial for promoting the overall health and well-being of college students. Full article

This study analyzes artificial intelligence (AI)-based technologies for food waste reduction in restaurant management, particularly in the case of the Philippines. Using the multiple-stakeholder target-oriented robust-optimization (MS-TORO) approach, AI solutions are ranked based on cost, feasibility, infrastructure requirements, and effectiveness. The key findings highlight that Too Good To Go is the most practical AI solution due to its affordability and focus on surplus food redistribution, making it ideal for resource-limited settings. The study emphasizes the need for government support, financial incentives, and public–private partnerships to facilitate AI adoption. Additionally, integrating AI-driven waste reduction with food security initiatives and sustainability projects can enhance their impact. Addressing economic and infrastructural challenges is crucial for maximizing AI’s potential in food waste management in developing economies. Full article

Background: South Asian Americans (SAA) are one of the fastest-growing immigrant groups in the U.S. and face significant health disparities, particularly regarding chronic diseases like diabetes, hypertension, and cardiovascular disease. Dietary patterns play a crucial role in these disparities, with acculturation to Western diets linked to poorer health outcomes. Despite this, the impact of food insecurity on dietary habits among SAAs remains underexplored. This study aims to examine the availability, cost, and quality of ethnic food items and how food insecurity influences dietary practices in Southern California’s SAA population. Methods: The study was conducted in San Bernardino County, California, with field data collection focused on five South Asian ethnicity-specific grocery stores and three Western grocery stores. We assessed the availability and cost of key ingredients for commonly prepared SAA dishes. Additionally, focus group interviews were held with South Asian immigrants to understand food insecurity challenges and dietary adaptations. Results: The study found significant disparities in food availability and cost between SAA-ethnic grocery stores and Western stores. SAA stores were less accessible and more widely dispersed, with an average distance of 10 miles between them. While ingredients like ginger paste and cumin powder were available in both types of stores, items such as ghee, fenugreek seeds, and black gram were harder to find in Western stores. Focus group participants noted that ethnic foods, especially vegetarian ingredients, were more expensive than Western alternatives, leading many to substitute traditional meals with cheaper, less nutritious options. Participants also raised concerns about the poor quality of items in ethnic stores, such as expired produce, which further limited their food choices. Conclusions: Food insecurity, driven by limited availability, high cost, and poor quality of ethnic foods, poses significant challenges to the SAA community’s diet and health. Addressing these barriers could improve food security and health outcomes among SAA immigrants. Full article

Migraine is a neurovascular paroxysmal disorder characterized by neurogenic inflammation and has a remarkable impact on the quality of life. The Food and Drug Administration (FDA) approved onabotulinumtoxin A in 2010 for the prophylactic treatment of chronic migraine. Today, in its 4th decade, it is approved in 100 countries for 15 main indications. Its mechanism of action, based on the inhibition of neurotransmitter release from primary sensory neurons, is very complex: it affords antinociception, but it also has an analgesic effect on neuropathic pain conditions and reduces the need for rescue medications. Genetic variants have been investigated for their potential role in the pathogenesis and clinical expression of migraine and of the response to treatments. These studies primarily involved genes associated with vascular regulation and cardiovascular pathology, including those encoding angiotensin-converting enzyme (ACE) and methylenetetrahydrofolate reductase (MTHFR). However, epigenetics and, particularly, genetic and epigenetic modifications are still poorly studied in terms of understanding the mechanisms implicated in susceptibility to migraine, aura, chronification and response to symptomatic and preventive treatments. In particular, the aim of the present study is to gather evidence on the genetic variants and epigenetic modifications affecting the pathway of the calcitonin gene-related peptide (CGRP), the target of onabotulinumtoxin A and of all the novel monoclonal antibodies. Beef fat injection technology, used to enhance the perceived quality of lower-grade meat, often results in artificially marbled beef that mimics the visual traits of Wagyu, characterized by dense fat distribution. This practice, driven by the high cost of Wagyu and the affordability of fat-injected beef, has led to the proliferation of mislabeled “Wagyu-grade” products sold at premium prices, posing potential food safety risks such as allergen exposure or consumption of unverified additives, which can adversely affect consumer health. Addressing this, this study introduces a smart sensing system integrated with handheld mobile devices, enabling consumers to capture beef images during purchase for real-time health-focused assessment. The system analyzes surface texture and color, transmitting data to a server for classification to determine if the beef is artificially marbled, thus supporting informed dietary choices and reducing health risks. Images are processed by applying a region of interest (ROI) mask to remove background noise, followed by partitioning into grid blocks. Local binary pattern (LBP) texture features and RGB color features are extracted from these blocks to characterize surface properties of three beef types (Wagyu, regular, and fat-injected). A support vector machine (SVM) model classifies the blocks, with the final image classification determined via majority voting. Experimental results reveal that the system achieves a recall rate of 95.00% for fat-injected beef, a misjudgment rate of 1.67% for non-fat-injected beef, a correct classification rate (CR) of 93.89%, and an F1-score of 95.80%, demonstrating its potential as a human-centered healthcare tool for ensuring food safety and transparency. Due to soil nutrient depletion and rising food demand from an increasing global population, it is essential to find sustainable ways to boost crop yields, improve soil health, and address the environmental issues induced by agriculture. The most appropriate approach is to consider sustainable amendments, such as biochar and its derivatives, which are vital constituents of soil health due to their affordability, low reactivity, large surface area, and reduced carbon footprint. In this context, biochar and its derivatives in farming systems focus on improving soil structure, nutrient holding capacity, microbial activities, and the perpetuation of soil fertility. Despite its benefits, biochar, if it is used in high concentration, can sometimes become highly toxic, causing soil erosion due to reducing surface area, increasing pH levels, and altering soil properties. This review highlights the production methods and sources of feedstocks, emphasizing their important contribution to the soil’s physicochemical and biological properties. Furthermore, it critically evaluates the environmental applications and their impacts, providing data built upon the literature on contaminant removal from soil, economic factors, heavy metal immobilization, carbon sequestration, and climate resilience. This review emphasizes the main challenges and future prospects for biochar use in comparison to modified biochar (MB) to propose the best practices for sustainable farming systems. Full article

The study examines changes in household food security and identifies their key determinants in Zambia by comparing the pre-pandemic period to the COVID-19 pandemic period. Using nationally representative surveys from 2015 and 2021 and the coarsened exact matching (CEM) approach, 8650 households were matched for comparison. Two complementary food security measures are analysed using multinomial logit regression models: household expenditure share, representing economic vulnerability, and household dietary diversity score (HDDS), representing diet quality. The results show that household food expenditure share significantly increased from 53.8% to 61.4%, indicating increased economic vulnerability. Notably, household dietary diversity improved from 7.1 to 8.2 out of 12, indicating better dietary quality. Consistent determinants of food security—such as household size, education level, marital status, region, and employment—remained significant, but their protective effects weakened during the pandemic. Specifically, the protective effect of education declined, urban households became relatively more vulnerable, and wealthier households experienced minimal changes. The study recommends targeted interventions, including expanding social protection programmes for economically vulnerable households, supporting informal food markets, enhancing rural–urban food supply linkages, and promoting nutrition education to ensure diverse, affordable food access during crises. This paper presents an approach for integrating data between a production system and its digital twin, focusing on achieving proactivity in production control. Recognizing the unique nature of each production system, this research highlights that a universal, plug-and-play solution is only partially feasible, primarily through general guidelines. The study successfully applied and automated proposed data acquisition methods, resulting in a functional, simulation-based digital twin that adheres to the latest ISO standards. The developed solution incorporates multiple data acquisition strategies, including files containing comma-separated values, a permanent connection to the production control system database, open platform communications unified architecture, and external command files for scenario alteration. The main motivation behind the presented implementation is its application on the shop-floors of small and medium enterprises, where it could provide useful tools for keeping up with the ever-rising competition in the manufacturing sector. This integrated approach allows for affordable and accurate system representation within the proactive simulation concept. The methodology was empirically validated across two distinct production systems: a lab-scale food and beverage line focusing on product tracking, and a sub-assembly line with automated guided vehicle optimization. Despite system variability, the core data acquisition methods demonstrated remarkable adaptability. Full article

Global Drinking Water Standards Lack Clear Health-Based Limits for Sodium

Background/Objectives: High sodium consumption increases the risk of hypertension and cardiovascular disease. Although food remains the primary source of intake, elevated sodium levels in drinking water can further contribute to excessive intake, particularly in populations already exceeding recommendations. This review examines the extent to which national drinking water standards account for sodium-related health risks and aims to inform discussion on the need for enforceable, health-based sodium limits. Methods: National standards for unbottled drinking water in 197 countries were searched for using the WHO 2021 review of drinking water guidelines, the FAOLEX database, and targeted internet and AI searches. For each country, data were extracted for the document name, year, regulatory body, regulation type, sodium limit (if stated), and rationale. Socio-geographic data were sourced from World Bank Open Data. A descriptive analysis was conducted using Microsoft Excel. Results: Standards were identified for 164 countries. Of these, 20% (n = 32), representing 30% of the global population, had no sodium limit. Among the 132 countries with a sodium limit, 92% (n = 121) adopted the WHO’s palatability-based guideline of 200 mg/L. Upper limits ranged from 50 to 400 mg/L. Only twelve countries (9%) cited health as a rationale. Three countries—Australia, Canada, and the United States—provided a separate recommendation for at-risk populations to consume water with sodium levels below 20 mg/L. Conclusions: Globally, drinking water standards give inadequate attention to sodium’s health risks. Most either lack sodium limits or rely on palatability thresholds that are too high to protect health. Updating national and international standards to reflect current evidence is essential to support sodium reduction efforts. Health-based sodium limits would empower communities to better advocate for safe water. Amid rising water salinity, such reforms must be part of a broader global strategy to ensure universal and equitable access to safe, affordable drinking water as a basic human right. Full article

Neglecting preventive healthcare policies has contributed to the global surge in chronic diseases, increased hospitalizations, declining quality of care, and escalating costs. Non-communicable diseases (NCDs)—notably cardiovascular conditions, diabetes, and cancer—consume over 80% of healthcare expenditure and account for more than 60% of global deaths, which are projected to exceed 75% by 2030. Poor diets, sedentary lifestyles, regulatory loopholes, and underfunded public health initiatives are driving this crisis. Compounding the issue are flawed policies, congressional lobbying, and conflicts of interest that prioritize costly, hospital-based, symptom-driven care over identifying and treating to eliminate root causes and disease prevention. Regulatory agencies are failing to deliver their intended functions. For instance, the U.S. Food and Drug Administration’s (FDA) broad oversight across drugs, devices, food, and supplements has resulted in inefficiencies, reduced transparency, and public safety risks. This broad mandate has allowed the release of unsafe drugs, food additives, and supplements, contributing to the rising childhood diseases, the burden of chronic illness, and over-medicalization. The author proposes separating oversight responsibilities: transferring authority over food, supplements, and OTC products to a new Food and Nutraceutical Agency (FNA), allowing the FDA to be restructured as the Drug and Device Agency (DDA), to refocus on pharmaceuticals and medical devices. While complete reform requires Congressional action, interim policy shifts are urgently needed to improve public health. Broader structural changes—including overhauling the Affordable Care Act, eliminating waste and fraud, redesigning regulatory and insurance systems, and eliminating intermediaries are essential to reducing costs, improving care, and transforming national and global health outcomes. The information provided herein can serve as a White Paper to help reform health agencies and healthcare systems for greater efficiency and lower costs in the USA and globally. Full article

This study aimed to determine the intrinsic and extrinsic product cues affecting consumers’ perceived value of pork meat. A segmentation study of seventeen intrinsic (n = 103) and twenty-six extrinsic cues (n = 114) on a South African sample was implemented. Cluster analyses provided two consumer segments for the product cues: the sensory seekers and the indecisive consumers for the intrinsic cues, and the price seekers and the preparation inquirers for the extrinsic cues. The sensory seekers valued the visual appearance, odour, taste, and flavour of pork as moderately to very important. The preparation inquirers regarded all the extrinsic cues, except for the processing cues, as moderately to very important. Accentuating the importance of sensory and preparation cues in pork meat products may contribute to improving the quality of products. This paper highlights that more research is needed on how consumers could benefit from the cue-adjusted pork meat products that will influence their perceived value of this affordable and versatile food in the South African context. The global increase in early-onset cancers among adolescents and young adults has happened at the same time as the rise in the consumption of ultra-processed foods (UPFs). Far beyond their poor nutritional quality, UPFs are increasingly seen as Trojan horses, complex biological agents that interfere with many functions of the human organism. In this review, we utilise the Trojan horse model to explain the quiet and building health risks from UPFs as foods that seem harmless, convenient, and affordable while secretly delivering endocrine-disrupting chemicals (EDCs), causing chronic low-grade inflammation, altering the microbiome, and producing epigenetic alterations. We bring together new proof showing that UPFs mess up hormonal signals, harm the body’s ability to fight off harmful germs, lead to an imbalance of microbes, and cause detrimental changes linked to cancer. Important components, such as bisphenols and phthalates, can migrate from containers into food, while additional ingredients and effects from cooking disrupt the normal balance of cells. These exposures are especially harmful during vulnerable developmental periods and may lay the groundwork for disease many years later. The Trojan horse model illustrates the hidden nature of UPF-related damage, not through a sudden toxin but via chronic dysregulation of metabolic, hormonal, and genetic control. This model changes focus from usual diet worries to a bigger-picture view of UPFs as causes of life-disrupting damage. Ultimately, this review aims to identify gaps in current knowledge and epidemiological approaches and highlight the need for multi-omics, long-term studies and personalised nutrition plans to assess and reduce the cancer risk associated with UPFs. Recognising UPFs as a silent disruptor is crucial in shaping public health policies and cancer prevention programs targeting younger people. Full article
