Long-Term Radiographic Stability of Tibial Plateau Angle Following TPLO in Small-Breed Dogs: A Retrospective Study

Cranial cruciate ligament disease (CCLD) is among the most common orthopedic conditions in dogs, and tibial plateau leveling osteotomy (TPLO) is the current standard of care; however, long-term radiographic data on tibial plateau angle (TPA) stability specific to small-breed dogs (<10 kg) remain limited. This retrospective study evaluated radiographic correction and long-term stability of TPA following TPLO in small-breed dogs and assessed whether observed TPA changes fell within the range of measurement error. Thirty stifles from 25 small-breed dogs (body weight 2.56–9.46 kg) that underwent TPLO between 2022 and 2025 were reviewed, with a mean follow-up of 12.6 ± 6.8 months. TPA was measured preoperatively, immediately postoperatively, at 1–2 months, and at ≥6 months using digital orthopedic planning software. Preoperative TPA averaged 31.23 ± 4.60° and decreased to 4.68 ± 1.84° immediately after surgery; at ≥6 months, mean TPA was 5.19 ± 1.83°. The mean change (ΔTPA) was 0.52 ± 0.64° (95% CI: 0.28–0.75°). Although statistically significant (p < 0.001), this fell below the 1.5° intraobserver measurement error previously reported. Body weight showed no correlation with ΔTPA (r = −0.059, p = 0.756). These findings were robust in a sensitivity analysis restricted to one stifle per dog (n = 25; mean ΔTPA 0.56 ± 0.67°) and in the subgroup without concurrent medial patellar luxation (n = 19; mean ΔTPA 0.40 ± 0.49°), both remaining within the reported measurement error. TPLO achieved effective and radiographically stable TPA correction in small-breed dogs. Full article

A comparative study on hemicircular tibial plateau leveling osteotomy (hTPLO) and TPLO for correcting cranial cruciate ligament (CCL) ruptures at excessive tibial plateau angles (eTPAs) was performed with a specially fabricated hTPLO saw blade (hemicircular) and TPLO saw blade (quartercircular), respectively. Ten 3D-printed bone models with eTPAs (TPA > 40°), were resized to accommodate the respective saw blade shape for each method. Cranial closing wedge osteotomy (CCWO) combined with TPLO was also performed. Distances of segment rotation below the patellar tendon insertion (PTI) and differences between anatomical and mechanical axes after correction were measured. The TPAs of the bone models were 46.06 ± 4.40° (40.0–50.7°). Postoperative TPAs of hTPLO, TPLO, and TPLO combined with CCWO were 4.93 ± 1.025°, 4.52 ± 0.085°, and 4.17 ± 1.128°, respectively. Rotation distances from the PTI were 0.46 ± 2.153 mm, 8.67 ± 2.318 mm, and 1.13 ± 1.796 mm, respectively. hTPLO showed significantly less movement of the segment below the PTI than TPLO with or without CCWO (p < 0.01). Differences between anatomical and mechanical axes after hTPLO and TPLO with or without CCWO were 5.74 ± 2.231°, 6.43 ± 2.105°, and 5.16 ± 2.465°, respectively. Within the limitations of this model-based study, hTPLO demonstrated geometric feasibility for correction of excessive tibial plateau angles and was associated with less rotational displacement of the proximal tibial segment relative to the patellar tendon insertion than conventional TPLO and TPLO combined with CCWO. Further cadaveric, biomechanical, and clinical studies are required to evaluate its safety and clinical applicability. Full article

Cranial cruciate ligament (CrCL) rupture in cats is less common than in dogs, and its optimal treatment remains a subject of debate. This study aimed to evaluate the application of cranial tibial wedge osteotomy (CTWO) as a dynamic stabilization technique in cats with CrCL rupture, describing the technical aspects and clinical outcomes obtained. Five cases with a confirmed diagnosis of CrCL rupture between 2020 and 2024 were included in this study. All patients were treated with CTWO using specific osteosynthesis locking plates designed for use in dogs and a complementary cerclage wire. Radiographic rechecks were performed at 8 and 12 weeks postoperatively, and clinical evaluations were performed 24 h, 8 weeks, 12 weeks, and 6 months postoperatively in every patient. Successful and complete bone healing of the tibial osteotomy was observed in every case. No intraoperative or postoperative complications related to implants or soft tissues were recorded. All cats achieved complete functional recovery without lameness at the last recheck six months after surgery. The technique was performed without significant technical difficulties, providing adequate stability and favorable clinical outcomes in all cases. These preliminary results support the use of CTWO as an effective surgical alternative for the treatment of CrCL rupture in cats. However, further studies with a larger number of cases and a longer follow-up are required to better evaluate its clinical application, outcomes, and influence on osteoarthritis progression in the long term. Abnormal tarsal posture in dogs may be associated with structural lesions such as common calcaneal tendon complex rupture, plantar-supporting structure injury, tarsocrural instability, or neurologic dysfunction; however, common calcaneal tendon rupture is typically associated with a plantigrade posture rather than the weight-bearing-dependent tarsal hyperextension pattern described in this case. A 3-year-old intact male Pomeranian developed bilateral weight-bearing-dependent tarsal hyperextension immediately after bilateral medial patellar luxation (MPL) surgery performed at another hospital. At our hospital, complete cranial cruciate ligament rupture of the right stifle and recurrent grade II MPL of the left stifle were identified. Right lateral fabellotibial suture stabilization using a LigaFiba® 75 lateral suture and crimp system, with supportive reapplication of a 0.8 mm Kirschner wire at the previous tibial tuberosity transposition site, and left trochlear block recession with capsular resection and imbrication were performed. No direct surgical treatment of either tarsus was undertaken. During follow-up after stifle stabilization, bilateral tarsal hyperextension progressively decreased and was no longer clinically observed by 5 months. This case suggests that bilateral weight-bearing-dependent tarsal hyperextension may occur secondary to proximal stifle pathology even in the absence of major structural tarsal instability, and that hyperextension may resolve clinically following stifle stabilization. Background: Anterior cruciate ligament (ACL) injuries are increasing in young athletes and many are related to non-contact, spontaneous mechanical fatigue-related ruptures. The objective of this narrative review is to identify and synthesize the anatomical, histological, physiological, and biomechanical basis of extracellular matrix (ECM) factors that contribute to ACL injuries and suggest ways to decrease their occurrence. Methods: The primary investigator searched PubMed, Web of Science, and Google Scholar database titles and abstracts using search phrases with Boolean operators: “anterior cruciate ligament” OR “ACL”, OR “cranial cruciate ligament” AND “disease”; “anterior cruciate ligament” OR “ACL”, OR “cranial cruciate ligament” AND “spontaneous rupture” OR “non-contact injury”; and “anterior cruciate ligament” OR ACL, OR cranial cruciate ligament” AND “crosslink”, “collagen” OR “extracellular matrix”; and “anterior cruciate ligament” OR “ACL”, OR “cranial cruciate ligament” AND “microtrauma”, OR “sudden” OR “fatigue failure”. The primary investigator and a sports orthopedic surgeon reviewed titles and abstracts of diverse evidence sources. From these identified sources, the study team performed full text reviews, selected contributing articles, performed Strength of Recommendation Taxonomy (SORT) grading, and synthesized the following themes: A Hostile Environment, ACL Strain, and Poor Nutrient Delivery; Accumulative ACL Microtrauma and Mechanical Failure; The ACL Differs From Other Ligaments; Collagen, the ECM, and ACL Mechanobiology; Crimps and ACL ECM Stretch; Crosslinks Improve ECM Mechanical Properties; The Delicate Collagen Synthesis and Degradation Balance; Exercise Training and the ACL; Can Nutraceuticals Help Restore the Balance?; Training Induced ACL Hypoxia; Estrogen and the Female Athlete; Counting Pitches or Counting Collagen Fiber Ruptures; and Restoring A Positive Anabolic–Catabolic Collagen Balance. Results: Regular exercise training within a physiologically safe loading range is vital to ACL ECM health. However, low or moderate evidence suggested that poor blood supply, slow metabolism, and a hypoxic environment may unbalance anabolic and catabolic homeostasis. Active rest and recovery concepts that prevent youth baseball shoulder and elbow injuries may help prevent non-contact ACL injuries. Conclusions: More prescriptive active rest and recovery intervals and neuromuscular control training may restore the anabolic–catabolic balance that increases mature crosslink density and improves ACL ECM strength. Confirmatory studies are needed to better establish therapeutic intervention mode(s), timing, dosage, and frequency optimization. Full article

Gonadectomy is performed in bitches to prevent unwanted reproduction and reduce the risk of sex hormone-related conditions. However, growing evidence suggests that the timing of spaying may influence long-term susceptibility to non-reproductive diseases. This retrospective case–control study (2013–2023) evaluated the association between timing of spaying and the development of orthopedic and neoplastic disorders in a Belgian referral-hospital population. Cases included bitches diagnosed with cranial cruciate ligament rupture, hip dysplasia, elbow dysplasia, lymphoma, mast cell tumor, osteosarcoma, or hemangiosarcoma, while disease-free bitches served as controls. Associations between disease occurrence and spaying status were assessed using multivariable logistic regression adjusted for age, weight category, and body condition score. Age at gonadectomy (<12 vs. ≥12 months) and timing relative to the first estrus were evaluated in separate models. Spaying <12 months of age was associated with increased odds of all conditions compared with intact females. Spaying ≥12 months of age was associated with lower odds of several orthopedic and neoplastic outcomes compared with early spaying, although odds were not always comparable to those in intact females. Large body size and higher body condition score were independently associated with increased odds of orthopedic outcomes. These findings support individualized spaying strategies rather than a universal age threshold. Full article

Cranial cruciate ligament rupture is a common orthopedic condition in dogs, and tibial tuberosity advancement is a well-established surgical treatment. The aim of this retrospective study was to evaluate the short-term clinical and radiographic outcomes of a cageless tibial tuberosity advancement technique in small-breed dogs. Medical records of 63 dogs (77 stifles) treated using this technique were reviewed. Fixation was achieved using three construct types: a screw–pin construct (the majority of cases), a screw-only construct, or a screw combined with two pins. Due to small subgroup sizes, fixation-type outcomes were primarily analyzed descriptively. Clinical and radiographic evaluations were performed immediately after surgery and at eight weeks postoperatively. Clinical outcomes were graded based on limb function, and radiographic bone healing was scored using a standardized scale. Postoperative complications were recorded and analyzed in relation to patient and procedural variables. No intraoperative complications were observed, while postoperative complications occurred in 27% of dogs and were predominantly minor and implant-related. Lameness scores improved significantly over the follow-up period. All treated stifles demonstrated stable implants, maintained advancement, and satisfactory bone healing. The use of bone graft material appeared to be associated with fewer complications and more favorable clinical outcomes; however, this observation should be interpreted with caution given the retrospective and non-randomized design of the study. In this retrospective case series, cageless tibial tuberosity advancement using screw-based fixation (predominantly screw–pin constructs) was associated with favorable short-term clinical and radiographic outcomes. These findings should be considered preliminary and limited to short-term evaluation, given the retrospective design, absence of a control group, and relatively short follow-up period. Further prospective studies with larger populations, standardized outcome measures, and longer follow-up are warranted to confirm these findings. Full article

Congenital patellar luxation (PL) and cranial cruciate ligament rupture (CCLR) are among the most prevalent hereditary musculoskeletal disorders in dogs. Despite their frequency, the microstructural alterations in the stifle joint capsule associated with these conditions remain poorly understood. This study aimed to characterize histomorphological changes in the joint capsule of dogs affected by PL and/or CCLR. Capsule samples were collected from dogs undergoing corrective surgery for PL or CCLR, while control tissue was obtained from dogs with unrelated orthopedic conditions. Histological evaluation assessed capsule thickness, the organization of its layers (stratum fibrosum, stratum subsynoviale, and stratum synoviale), synovial cell number, and villous or plicae formation. Associations with disease type, chronicity, sex, age, breed, and weight were analyzed. Dogs with PL and CCLR exhibited increased superficial synovial cell layers compared to controls, whereas chronic cases showed reduced villous formation (SEF = 2.46). Absence or marked reduction of the stratum subsynoviale correlated with PL and prolonged lameness. In PL cases, the stratum synoviale was frequently absent, whereas CCLR cases exhibited an increase in overall capsular thickness. These findings reveal distinct histomorphological remodeling in PL and CCLR, suggesting that chronic instability drives degenerative changes. Understanding these alterations may aid in early diagnosis, improve treatment strategies, and inform breeding programs targeting joint stability in predisposed breeds. Full article

Ehlers–Danlos syndrome (EDS) is a congenital disorder affecting connective tissue. Patients diagnosed with EDS may present with joint instability, and in human medicine, an increased risk of cranial cruciate rupture has been described. A few therapeutic options for these patients have been described, with no evident superiority of one technique due to small study groups. Cranial cruciate rupture has never been described in a dog with EDS. This is a case report of an 11-month-old Maltese diagnosed with bilateral cruciate ligament rupture and classical EDS with a previously undescribed heterozygous COL5A1 missense variant that underwent stifle stabilization. This study aims to evaluate the morpho-functional changes in the patellar ligament in dogs undergoing tibial plateau leveling osteotomy (TPLO) during the first six months of the postoperative follow-up and correlate the radiographic and conventional ultrasonography changes in the patellar ligament with its elastic properties. Dogs with a unilateral cranial cruciate ligament (CCL) rupture who underwent TPLO were enrolled. The patients were subjected to radiographic, ultrasonographic and elastosonographic evaluations of the patellar ligament before surgery (T0) and 1 month (T1), 2 months (T2) and 6 months (T3) after surgery. The radiographic and ultrasound thicknesses of the patellar ligament were recorded at three points: proximal, central and distal. A color map of strain elastosonography with superimposed B-mode images was used to evaluate the elasticity (hardness and softness) of the ligament. Twenty-two dogs completed the trial. At T0, the patellar ligament showed statistically reduced radiographic and ultrasonographic thicknesses compared to those at T1, T2 and T3. Additionally, the patellar ligament in the distal portion was statistically thicker in comparison to the proximal and central points at T2 and T3. At T1, T2 and T3, there was an increase in hardness and a reduction in softness of the patellar ligament compared to that at T0. The elastosonographic analysis and the radiographic and ultrasonographic thicknesses of the patellar ligament showed no correlation. The patellar ligament showed structural and mechanical alterations in the dogs undergoing TPLO, even six months after surgery. Full article

Background: Canine cranial cruciate ligament (CCL) rupture is a common orthopedic condition leading to stifle joint dysfunction, discomfort, and reduced mobility. Diagnosis typically involves radiography, computed tomography (CT), and magnetic resonance imaging (MRI). In this study, we conducted a retrospective analysis to evaluate the use of infrared thermography in assessing local temperature and thermal patterns in dogs with acute-onset lameness due to CCL rupture compared to those with intact ligaments. Methods: The study involved 12 dogs with cranial cruciate ligament rupture and nine dogs with intact ligaments. The stifle area of all dogs was clipped and scanned using a FLIR E50 thermographic camera. Two regions of interest (ROI), designated El1 and Bx1, were analyzed with FLIR Tools software 5.X by comparing the average of the maximum and of the mean temperature values between the two groups. Results: Thermal imaging revealed differences between the two groups of dogs, which were further supported by significantly higher temperatures in the El1 (lateral aspect of the stifle joint) and Bx1 (cranial aspect of the stifle joint) areas in the study group compared to the control group using a comparative analysis—two-sample t-test. In the El1 area, the study group showed a temperature increase of 1.8 °C compared to the control group, while in the Bx1 area, the difference was 1.76 °C. Conclusions: Infrared thermography shows potential to differentiate dogs with acute-onset lameness due to CCL rupture from dogs with intact ligaments, but further studies are needed to assess its accuracy in distinguishing it from other stifle pathologies. Cranial cruciate ligament (CrCL) rupture is a common orthopedic disorder in dogs, leading to stifle joint instability and progressive osteoarthritis. This study aimed to develop and biomechanically evaluate a novel intra-articular reconstruction system designed to mimic the natural ligament and restore joint stability following CrCL excision. The system consisted of a 3D-printed thermoplastic polyurethane (TPU) graft, cerclage wire, and H-button fixation. Fourteen pelvic limbs from mature mixed-breed cadaveric dogs were used. The inclination angle, dimensions, volume, tensile strength, and elongation of the native CrCL were measured. Seven CrCL-deficient stifles were reconstructed using the proposed system and tested biomechanically. The native CrCL showed a significantly higher tensile strength than the TPU graft; however, the TPU demonstrated a greater flexibility. The reconstruction system successfully stabilized the joint and provided repeatable fixation. Significant correlations were found between CrCL volume and both age and body weight. These findings support the mechanical suitability of the proposed system for ex vivo stifle stabilization and highlight the potential of 3D-printed TPU in ligament reconstruction. Further in vivo studies are recommended to assess long-term performance, including implant integration, tissue remodeling, and clinical outcomes. Full article

A 4-year-old neutered female rottweiler underwent three separate anaesthetic events for the diagnosis and treatment of cranial cruciate ligament rupture. The patient was otherwise healthy, with consistently normal pre-anaesthetic electrolyte values. However, during the second anaesthetic procedure, the patient developed severe hyperkalaemia with life-threatening cardiovascular compromise. Prompt treatment led to rapid normalisation of plasma potassium levels and resolution of cardiac abnormalities. This case highlights the need for further research to elucidate the mechanisms underlying this rare but serious complication. Clinicians should remain vigilant, as intra-anaesthetic hyperkalaemia, though uncommon, can be fatal. While the condition has been historically associated with greyhounds, recently reported cases in rottweilers may warrant similar concern. Full article

Cranial cruciate ligament rupture is the most common orthopedic condition in dogs, causing lameness, joint instability, and discomfort due to its role in knee stability. Surgical treatment, particularly osteotomies in the proximal tibia, is the preferred approach, with tibial tuberosity advancement (TTA), tibial plateau leveling osteotomy (TPLO), and the modified Maquet technique (MMT) being widely used. A common postoperative complication is patellar desmopathy—thickening of the ligament that is detectable on radiographs and can cause further discomfort. However, studies on this complication in TPLO are limited, and there are no direct comparisons of ligament thickening between these surgical techniques. This study aimed to determine the prevalence of ligament desmopathy in TPLO, examine any associations with age, sex, or weight, and compare findings with those from other techniques. A retrospective analysis was conducted, with ligament thickness measured at three points during postoperative follow-up, which had a mean of approximately 2.5 months. Results showed an 86.7% prevalence of desmopathy, with average thicknesses of 3.90 mm proximally, 4.69 mm at mid-ligament, and 5.98 mm distally. These findings align with previous TPLO studies and suggest that TPLO may cause greater ligament thickening than TTA or MMT, particularly in the distal portion. Full article

This study explored the complications and prognosis of modified tibial plateau leveling osteotomy with tibial tuberosity transposition (mTPLO-TTT) for simultaneously correcting high-grade medial patellar luxation (MPL) and cranial cruciate ligament disease (CCLD) in small-breed dogs. This retrospective study evaluated patient data, lameness scores, radiographic outcomes, and complications over a median follow-up period of 10 weeks. Additionally, an owner interview was conducted 6 months postoperatively. Nine stifles from seven dogs were included in this study. All cases showed satisfactory patellar alignment and stability after surgery, with no major complications or reluxations. The lameness scores improved, and radiographic assessments confirmed implant stability and appropriate bone healing. Owner-reported outcomes at 6 months were also favorable. These findings suggest that simultaneous mTPLO-TTT is an effective surgical option for small-breed dogs with concurrent CCLD and Grade IV MPL. Full article
