Progression of Structural Lung Disease in Different Aspergillus fumigatus Disease Phenotypes in Children with CF

Aspergillus fumigatus (Asp) is frequently cultured from airways of children with Cystic Fibrosis (CwCF), but the impact on structural lung disease (SLD) remains unknown. In this retrospective study of 125 CwCF with a positive Asp airway culture (Asp+) at Sophia Children’s Hospital between 1988 and 2021, four Asp disease phenotypes were defined based on serum Asp-specific IgE (IgEAsp) and IgG (IgGAsp): colonisation, sensitisation, bronchitis, and allergic bronchitis. SLD was assessed on biennial chest CTs (n = 382) using the PRAGMA-CF score. Annual progression of SLD was modelled for the Asp disease phenotypes, adjusting for Pseudomonas aeruginosa and Allergic Bronchopulmonary Aspergillosis (ABPA). Annual SLD progression was high in all phenotypes but was higher in Asp sensitisation and bronchitis compared to colonisation. The proportion of air trapping was high in the full study population (mean 57%), but no differences were found in annual progression between the different Asp disease phenotypes. CwCF with Asp allergic bronchitis had a 10-fold higher risk to develop ABPA during the study follow-up than those with Asp colonisation. The four Asp disease phenotypes, colonisation, sensitisation, bronchitis, and allergic bronchitis, that were defined based on IgEAsp and IgGAsp show different rates of progression of SLD and different risks of ABPA development. Several studies have reported the incidence of fungi and mycotoxins in coffee beans; however, there are few reports related to occupational exposure to these agents at coffee dry milling industries. The aim of this review was to identify and to analyze studies assessing occupational exposure to fungi and mycotoxins in coffee industries. Therefore, a systematic literature search was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology and focusing on the assessment of occupational exposure to fungi and mycotoxins in the coffee industry. In these papers, different environmental matrices were considered in evaluating occupational exposure, but the most used matrix was airborne dust (four of the five studies). Airborne fungi were sampled using active (four of the five studies) and passive sampling. Only the most recent of the studies (2022) identified microorganisms by their genera and species, and only two groups of mycotoxins were analyzed in the studies considered, namely, Ochratoxin A and Aflatoxins. None of the studies reported data on both fungi and mycotoxins. The fungal genera identified in these occupational environments included Cladosporium, Paecilomyces, Aspergillus, Penicillium, and other genera. Among the mycotoxins, only aflatoxins and ochratoxin A were investigated. Occupational exposure to these biological agents may lead to several health effects. Fungal spores and fragments can cause respiratory diseases such as asthma, allergic rhinitis, bronchitis, and hypersensitivity pneumonitis. Additionally, the mycotoxins studied—particularly Aflatoxins and Ochratoxin A—are associated with serious toxicological effects. Coexposure to both fungi and mycotoxins may enhance health risks and should be carefully considered in occupational risk assessments. Considering the possible effects related to exposure to fungi and mycotoxins and the number of workers involved in this type of industry in the world, more studies should be developed. This is the first review to systematically consolidate data on occupational exposure to both fungi and mycotoxins specifically within the coffee industry, highlighting existing knowledge gaps and the need for targeted risk assessments in coffee-producing settings. Cystic fibrosis is a severe, inherited, life-limiting disorder, and over half of those living with CF are children. Persistent airway infection and inflammation, resulting in progressive lung function decline, is the hallmark of this disorder. Aspergillus colonization and infection is a well-known complication in people with CF and can evolve in a range of Aspergillus disease phenotypes, including Aspergillus bronchitis, fungal sensitization, and allergic bronchopulmonary aspergillosis (ABPA). Management strategies for children with CF are primarily aimed at preventing lung damage and lung function decline caused by bacterial infections. The role of Aspergillus infections is less understood, especially during childhood, and therefore evidence-based diagnostic and treatment guidelines are lacking. This narrative review summarizes our current understanding of the impact of Aspergillus on the airways of children and young people with CF. Full article

Background. The clinical relevance of Aspergillus fumigatus (Af) in cystic fibrosis (CF) is controversial. The aims of the study were to assess the prevalence of Af disease in our cohort of CF patients and evaluate whether allergic bronchopulmonary aspergillosis (ABPA) and sensitization to Af affected lung function, body mass index (BMI) and exacerbations. Methods. Clinical data and lung function of CF patients aged 6–18 years followed at the CF Centre of Parma (Italy) were recorded. Patients were classified as: patients with no signs of Af, patients sensitized or colonized by Af, patients with ABPA or patients with Aspergillus bronchitis (Ab). Results. Of 38 CF patients (14.2 years (6.2–18.8) M 23), 8 (21%) showed Af sensitization, 7 (18.4%) showed ABPA, 1 (2.6%) showed Af colonization and 1 (2.6%) showed Ab. Compared to non-ABPA, patients with ABPA had lower BMI (15.9 ± 1.6 vs. 19.7 ± 3.4, p < 0.005), lower lung function (FEV1 61.5 ± 25.9% vs. 92.3 ± 19.3%, p < 0.001) and more exacerbations/year (4.43 ± 2.44 vs. 1.74 ± 2.33, p < 0.005). Patients with Af sensitization showed more exacerbations/year than non-Af patients (3.5 ± 3.2 vs. 0.9 ± 1.2, p < 0.005). ABPA and sensitized patients had more abnormalities on chest CT scans. Conclusion. This study showed the relevant clinical impact of ABPA and Af sensitization in terms of exacerbations and lung structural damage. Full article

Fungi have many potential roles in paediatric asthma, predominantly by being a source of allergens (severe asthma with fungal sensitization, SAFS), and also directly damaging the epithelial barrier and underlying tissue by releasing proteolytic enzymes (fungal bronchitis). The umbrella term ‘fungal asthma’ is proposed for these manifestations. Allergic bronchopulmonary aspergillosis (ABPA) is not a feature of childhood asthma, for unclear reasons. Diagnostic criteria for SAFS are based on sensitivity to fungal allergen(s) demonstrated either by skin prick test or specific IgE. In children, there are no exclusion criteria on total IgE levels or IgG precipitins because of the rarity of ABPA. Diagnostic criteria for fungal bronchitis are much less well established. Data in adults and children suggest SAFS is associated with worse asthma control and greater susceptibility to asthma attacks than non-sensitized patients. The data on whether anti-fungal therapy is beneficial are conflicting. The pathophysiology of SAFS is unclear, but the epithelial alarmin interleukin-33 is implicated. However, whether individual fungi have different pathobiologies is unclear. There are many unanswered questions needing further research, including how fungi interact with other allergens, bacteria, and viruses, and what optimal therapy should be, including whether anti-neutrophilic strategies, such as macrolides, should be used. Considerable further research is needed to unravel the complex roles of different fungi in severe asthma. Full article

Citrullus colocynthis L. Schrader is an annual plant belonging to the Cucurbitaceae family, widely distributed in the desert areas of the Mediterranean basin. Many pharmacological properties (anti-inflammatory, anti-diabetic, analgesic, anti-epileptic) are ascribed to different organs of this plant; extracts and derivatives of C. colocynthis are used in folk Berber medicine for the treatment of numerous diseases—such as rheumatism arthritis, hypertension bronchitis, mastitis, and even cancer. Clinical studies aimed at confirming the chemical and biological bases of pharmacological activity assigned to many plant/herb extracts used in folk medicine often rely on results obtained from laboratory preliminary tests. We investigated the biological activity of some C. colocynthis stem, leaf, and root extracts on the mycotoxigenic and phytopathogenic fungus Aspergillus flavus, testing a possible correlation between the inhibitory effect on aflatoxin biosynthesis, the phytochemical composition of extracts, and their in vitro antioxidant capacities. The Aspergillus terreus species complex is found in a wide variety of habitats, and the spectrum of diseases caused covers allergic bronchopulmonary aspergillosis, Aspergillus bronchitis and/or tracheobronchitis, and invasive and disseminated aspergillosis. Invasive infections are a significant cause of morbidity and mortality mainly in patients with hematological malignancy. The section Terrei covers a total of 16 accepted species of which most are amphotericin B resistant. Triazoles are the preferred agents for treatment and prevention of invasive aspergillosis. Poor prognosis in patients with invasive A. terreus infections seems to be independent of anti-Aspergillus azole-based treatment. Full article
