The evidence for photobiomodulation in reducing postoperative pain after endodontic instrumentation is classified as low or very low certainty, indicating a need for further research. Longitudinal pain assessments over 24 h are crucial, and studies should explore these pain periods. Background/Objectives: This double-blind, randomized controlled clinical trial evaluated the effect of PBM on pain following single-visit endodontic treatment of maxillary molars at 4, 8, 12, and 24 h. Primary outcomes included pain at 24 h; secondary outcomes included pain at 4, 8, and 12 h, pain during palpation/percussion, OHIP-14 analysis, and frequencies of pain. Methods: Approved by the Research Ethics Committee (5.598.290) and registered in Clinical Trials (NCT06253767), the study recruited adults (21–70 years) requiring endodontic treatment in maxillary molars. Fifty-eight molars were randomly assigned to two groups: the PBM Group (n = 29), receiving conventional endodontic treatment with PBM (100 mW, 333 mW/cm2, 9 J distributed at 3 points near root apices), and the control group (n = 29), receiving conventional treatment with PBM simulation. Pain was assessed using the Visual Analog Scale. Results: Statistical analyses used chi-square and Mann–Whitney tests, with explained variance (η2). Ten participants were excluded, leaving 48 patients for analysis. No significant differences were observed in postoperative pain at 24, 4, 8, or 12 h, or in palpation/percussion or OHIP-14 scores. Pain frequencies ranged from 12.5% to 25%. Conclusions: PBM does not influence post-treatment pain in maxillary molars under these conditions. These results emphasize the importance of relying on well-designed clinical trials to guide treatment decisions, and future research should focus on personalized dosimetry adapted to the anatomical characteristics of the treated dental region to enhance the accuracy and efficacy of therapeutic protocols. Background: Crystalline glucosamine sulfate (cGS) claims to be a stabilized form of glucosamine sulfate with a defined crystalline structure intended to enhance chemical stability. It is proposed to offer pharmacokinetic advantages over regular glucosamine sulfate (rGS) which is stabilized with potassium or sodium chloride. However, comparative human bioavailability data are limited. Since both forms dissociate in gastric fluid into constituent ions, the impact of cGS formulation on absorption remains uncertain. This pilot study aimed to compare the bioavailability of cGS and rGS using a randomized, double-blind, crossover design. Methods: Ten healthy adults received a single 1500 mg oral dose of either cGS or rGS with a 7-day washout between interventions. Capillary blood samples were collected over 24 h. Glucosamine and its metabolite concentrations were quantified by Liquid Chromatography-High Resolution Mass Spectrometry (LC-HRMS), and pharmacokinetic parameters—including maximum concentration (Cmax), time to reach Cmax (Tmax), and area under the curve (AUC)—were calculated. Results: Mean AUC0–24, Cmax, Tmax, and T½ values for glucosamine and glucosamine-6-sulfate (GlcN-6-S) were comparable between cGS and rGS. Although the AUC0–24 for glucosamine was modestly higher with rGS (18,300 ng·h/mL) than with cGS (12,900 ng·h/mL), the difference was not statistically significant (p = 0.136). GlcN-6-S exposure was also similar between formulations (rGS: 50,700 ng·h/mL; cGS: 50,600 ng·h/mL), with a geometric mean ratio of 1.39, a delayed Tmax (6–8 h) and longer half-life, consistent with its role as a downstream metabolite. N-acetylglucosamine levels remained stable, indicating potential homeostatic regulation. Conclusions: This pilot study found no significant pharmacokinetic advantage of cGS over rGS. These preliminary findings challenge claims of cGS’ pharmacokinetic superiority, although the small sample size limits definitive conclusions. Larger, adequately powered studies are needed to confirm these results. Full article

Fungal communities in high plateau ecosystems remain understudied despite their crucial roles in soil ecosystems, and yeasts inhabiting extreme regions have potential for industrial and biotechnological applications. We studied the fungal diversity in soils across 14 Chilean Altiplano sites using amplicon-based metagenomics and isolation of yeasts to assess their growth under various conditions and hydrolytic enzyme secretion. Using the metagenomic approach, the Ascomycota and Basidiomycota phyla were found to be the most abundant (85% and 8%, respectively). Unclassified families and genera prevailed at six and ten sites, respectively. At the other sites, the most abundant families included Cladosporiaceae, Teratosphaeriaceae, and Sporormiaceae, and the genera Oleoguttula, Coniochaeta, and Peziza. Biodiversity indices did not correlate with the soil’s geographic origin, organic matter content, humidity, or pH. Most isolated yeasts belong to the Naganishia, Holtermanniella, and Vishniacozyma genera, growing at temperatures ranging from 4 °C to 26 °C. Most isolates could use glucose, sucrose, and maltose as carbon sources and exhibited amylase, esterase, pectinase, and protease activities at 30 °C and below. Our results indicate that the evaluated soil physicochemical parameters do not explain the fungal distribution in the Altiplano and highlight the region as a reservoir of unknown fungi, including yeasts with industrially relevant enzymes. Helicobacter pylori, a spiral-shaped bacterium found in the stomach, is associated with various gastrointestinal and systemic health conditions. Effective suppression of H. pylori is therefore critical for managing gastrointestinal diseases. In a search for natural products with anti-H. pylori activity, the extract of Atractylodes macrocephala rhizoma showed significant inhibitory effects. Chromatographic purification of A. macrocephala extract yielded thirteen compounds, which were identified as ten sesquiterpenes and three polyacetylenes by spectroscopic analysis. The sesquiterpene compounds belong to the eudesmane or eudesmane lactone types and exhibited structure-dependent efficacy. The major eudesmane lactone sesquiterpene, atractylenolide I (1), showed strong inhibitory activity comparable to metronidazole, a positive control, and atractylenolide III (3) also showed good efficacy. However, structural modification such as hydroxylation, methylation, or acetylation of the sesquiterpenes led to reduced activity. In contrast, polyacetylene derivatives displayed only mild inhibitory effects. Further evaluation of the active compounds against three H. pylori strains such as 51, 43504, and 26695 showed that atractylenolide I (1) had potent inhibitory effects against all three strains, with MIC50 values of ranging from 27.3 to 48.6 μM and MIC90 values from 45.4 to 87.2 μM. Atractylenolide III (3) exhibited selective activity against strain 51 with MIC50 value of 89.9 μM. Both compounds also exhibited anti-inflammatory activity with IC90 values of 23.3 and 31.1 μM, respectively, although they showed little effect on urease. This is the first report on the anti-H. pylori efficacy of various constituents of A. macrocephala and comparative analysis of inhibitory effects against several strains, which will provide scientific evidence supporting its potential as therapeutic agent for H. pylori-related infection. Mango is highly sensitive to heat stress, which directly affects the yield and quality. The extreme heat waves of 2024, with temperatures reaching 41–47 °C over 25 days, caused significant impacts on sensitive cultivars. The impact of heat waves on ten commercial cultivars from subtropical regions viz.,‘Dashehari’, ‘Langra’, ‘Chausa’, ‘Bombay Green’, ‘Himsagar’, ‘Amrapali’, ‘Mallika’, ‘Sharda Bhog’, ‘Kesar’, and ‘Rataul’, and thirteen selected elite hybrids H-4208, H-3680, H-4505, H-3833, H-4504, H-1739, H-3623, H-1084, H-4264, HS-01, H-949, H-4065, and H-2805, is reported. The predominant effects that were observed include the following: burning symptoms or blackened tips, surrounded by a yellow halo, with premature ripening in affected parts and, in severe cases, tissue mummification. Among commercial cultivars, viz., ‘Amrapali’ (25%), ‘Mallika’ (30%), ‘Langra’ (30%), ‘Dashehari’ (50%), and ‘Himsagar’ and ‘Bombay Green’ had severe impacts, with ~80% of fruits being affected, followed by ‘Sharda Bhog’. In contrast, mid-maturing cultivars like ‘Kesar’, ‘Rataul’, and late-maturing elite hybrids, which were immature during the stress period, showed no symptoms, indicating they are tolerant. Biochemical analyses revealed significantly elevated total soluble solids (TSS > 25 °B) in affected areas of sensitive genotypes compared to non-affected tissues and tolerant genotypes. Aroma profiling indicated variations in compounds such as caryophyllene and humulene between affected and unaffected parts. The study envisages that the phenological maturity scales are indicators for the selection of climate-resilient mango varieties/hybrids and shows potential for future breeding programs. Full article

In Vivo Antibiotic Elution and Inflammatory Response During Two-Stage Total Knee Arthroplasty Revision: A Microdialysis Pilot Study

Introduction: Two-stage revision with an antibiotic-loaded, temporary static cement spacer is a common treatment for periprosthetic joint infection (PJI) of the knee. However, limited data exists on in vivo antibiotic elution kinetics after spacer implantation. This pilot study uses the technique of microdialysis (MD) to collect intra-articular knee samples. The aim was to evaluate MD as an intra-articular sampling method to detect spacer-eluted antibiotics within 72 h after surgery and to determine whether they show specific elution kinetics. Methods: Ten patients (six male, four female; age median 71.5 years) undergoing two-stage revision for knee PJI were included. A MD catheter was inserted into the joint during explantation of the infected inlying implant and implantation of a custom-made static spacer coated with COPAL cement (0.5 g gentamicin (G) and 2 g vancomycin (V)). Over 72 h postoperatively, samples were collected and analyzed for spacer-eluted antibiotics, intravenously administered antibiotics (e.g., cefazolin and cefuroxime), metabolic markers (glucose and lactate), and Interleukin-6 (IL-6). Local and systemic levels were compared. Results: All catheters were positioned successfully and well tolerated for 72 h. Antibiotic concentrations in MD samples peaked within the first 24 h (G: median 9.55 µg/mL [95% CI: 0.4–17.36]; V: 37.57 µg/mL [95% CI: 3.26–81.6]) and decreased significantly over 72 h (for both p < 0.05, G: 4.27 µg/mL [95% CI: 2.26–7.2]; V: 9.69 µg/mL [95% CI: 3.86–24]). MD concentrations consistently exceeded blood levels (p < 0.05), while intravenously administered antibiotics showed higher blood concentrations. Glucose in MD samples decreased from 17.71 mg/dL to 0.89 mg/dL (p < 0.05). IL-6 and lactate concentrations showed no difference between MD and blood samples. Conclusions: Monitoring antibiotics eluted by a static spacer with intra-articular MD for 72 h is feasible. Gentamicin and vancomycin levels remained above the minimal inhibitory concentration. Differentiating infection from surgical response using metabolic and immunological markers remains challenging. Prolonged in vivo studies with MD are required to evaluate extended antibiotic release in two-stage exchanges. Full article

Plant secondary metabolites regulate plant growth and serve as valuable pharmaceutical resources. Napier grass (Pennisetum purpureum Schumach.), a Poaceae species, shows potential as a functional food. In this study, we employed high-resolution mass spectrometry combined with a data-independent acquisition (DIA) strategy for the untargeted detection of anthocyanins, a group of secondary metabolites, in napier grass. Clear MS2 fragmentation patterns were observed for anthocyanins, characterized by diagnostic aglycone signals and sequential losses of hexosyl (C6H10O5), deoxyhexosyl (C6H10O4), pentosyl (C5H8O4), and p-coumaroyl groups (C9H8O3). Based on matching with authentic standards and an in-house database, ten anthocyanins were identified, seven of which were newly reported in napier grass. In a single-laboratory validation analysis, both absolute and semi-quantitative results reliably reflected the specific distribution of metabolites across different cultivars and plant organs. The purple cultivar (TS5) exhibited the highest anthocyanin content, with the cyanidin 3-O-glucoside content reaching 5.0 ± 0.5 mg/g, whereas the green cultivar (TS2), despite its less pigmented appearance, contained substantial amounts of malvidin 3-O-arabinoside (0.7 ± <0.1 mg/g). Flavonoid profiling revealed that monoglycosylated anthocyanins were the dominant forms in floral tissues. These findings shed light on napier grass metabolism and support future Poaceae breeding and functional food development. Full article

Chronic lung infection is the main predictor of morbidity and mortality in cystic fibrosis (CF), and current pharmacological alternatives are ineffective against Pseudomonas aeruginosa infections. We developed ciprofloxacin (CIP) for inhalation, aiming at improving its solubility through the formation of an amorphous solid dispersion (ASD) using gelatin (GA). CIP and GA were dissolved in varying ratios and then spray-dried, obtaining CIP-GA microspheres in a single step. The dissolution rate, size distribution, morphology, and aerodynamic properties of CIP-GA microspheres were studied, as well as their antimicrobial activity on P. aeruginosa biofilms. Microspheres formulated with a higher GA ratio increased the dissolution of CIP ten-fold at 6 h compared to gelatin-free CIP. Formulations with 75% GA or more could form ASDs and improve CIP’s dissolution rate. CIP-GA microspheres outperformed CIP in eradicating P. aeruginosa biofilm at 24 h. The spray-drying process produced CIP-GA microspheres with good aerodynamic properties, as indicated by a fine particle fraction (FPF) of 67%, a D50 of 3.52 μm, and encapsulation efficiencies above 70%. Overall, this study demonstrates the potential of gelatin to enhance the solubility of poorly soluble drugs by forming ASDs. As an FDA-approved excipient for lung delivery, these findings are valuable for particle engineering and facilitating the rapid translation of technologies to the market. Full article

The effect of replacing wheat with corn on the color distribution across various locations in the carcasses of broilers was assessed. One thousand two hundred ROSS 308 one-day-old male chicks were reared in an experimental farm, with ten pens per treatment, based on the primary cereal ingredient during both the starter (1–14 d) and grower (15–41 d) phases: corn and wheat. At 41 days old, slaughtering was performed. At 24 h post slaughter, color measurements were taken in 15 carcasses per treatment using a MINOLTA CM600d reflectance spectrocolorimeter in the CIEL*a*b* space. Twelve locations were measured: drumette, apterial latero-pectoral area, neck, the back at the seventh thoracic vertebra, saddle, thigh, ham, hock and around the vent, shanks, and the surface and interior of the breast. The results indicate that the color distribution in the broiler carcass is not homogeneous. Very small color differences can be found between the surface and the interior of the breast, but they differ greatly from areas where there is skin. Corn produces a darker color than wheat, though the intact skin homogenizes lightness* across the entire carcass, reducing differences between regions. Corn increased mainly yellowness, influencing color saturation. The pronounced color differences between the shank and other locations almost disappear when corn is excluded from the feed. Hue° could serve as a potential indicator of color variations in the breast resulting from differences in dietary ingredients. Full article

Biographies of Max Wilson and Gunnar Jungner were published in 2017 and 2020. An in-depth appreciation of the Wilson and Jungner principles, and the publication they were presented in, ‘Principles and Practice of Screening for Disease’, published as nr. 34 in the Public Health Paper-series of the World Health Organisation (W.H.O), called PHP-34 hereafter, was not published as yet. Here an analysis is given of PHP-34 and the ten screening principles, focusing on three subjects. First, by careful analysis of PHP-34, the literature published in the peer reviewed scientific literature, and other sources, the historical background and origin of the ten principles is determined. Second, the precise composition of PHP-34 is described, as parts of the monograph were derived from other seminal works published between roughly 1950 and 1965. Third, it is determined what the contributions of both authors of the monograph were. Results together are discussed in relation to the time PHP-34 was conceptualized and the importance of PHP-34 and the ten principles in the current era. Results show that in the 15 years preceding the publication of PHP-34, many principles of screening were published by authors in the United States of America, a selection of which ended up in PHP-34. Secondly, about 33% of the 145 pages of PHP-34 are drawn from other publications and studies on screening. Thirdly, the case can be made that the actual writing of PHP-34 was done (almost) entirely by Wilson. Regardless, Wilson and Jungner to this day should be applauded for their work. It is a testimony to the value of PHP-34 that we are still reflecting upon, discussing and seeking to intelligently apply the screening principles almost 60 years after their original publication. Hypothesis: Structural optimization of crosslinkers within a reactive glycerol-based hexagonal lyotropic liquid crystal (HLLC) system is proposed to enhance the interfacial stability of hexagonal mesophases and improve the hexagonal structure retention during polymerization. This targeted modification is anticipated to significantly improve the water filtration efficiency of HLLC-templated nanofiltration. Experiments: The effect of crosslinkers on the interfacial stability of glycerol-based hexagonal mesophases was studied by evaluating their concentration accommodation within the mesophases using 13C solid NMR, FTIR and SAXS. Findings: A hydrophilic crosslinker consisting of ten ethylene glycol units shows less interference with the interfacial stability of hexagonal mesophases, therefore contributing to a higher concentration accommodation compared to the one with three ethylene glycol units. This long-chain crosslinker, despite having a low content of reactive groups, effectively connects the cylinders and better retains the hexagonal structures during polymerization than the hydrophobic crosslinker with shorter ethylene glycol units but a higher content of reactive groups. The retained hexagonal nanofiltration membranes show a remarkable pure water permeability of 40 L m−2 h−1 bar−1 µm, resulting from the strong hygroscopic effect of glycerol and the crumpled surface of membranes due to the flexible nature of the system plasticized by glycerol. Objective: This study aimed to investigate the efficacy of far-infrared (FIR) garments in enhancing recovery following resistance exercise in recreationally active individuals. Methods: Ten recreationally active adults (six females, four males; aged 20.7 ± 3.2 years) completed a resistance exercise protocol and were randomly selected to wear either FIR (n = 5) or placebo (n = 5) tights post-exercise. The FIR garments incorporated Celliant-based fibers emitting wavelengths in the 2.5–20 µm range. The participants’ recovery was assessed using countermovement jump (CMJ) metrics, including their jump height, takeoff velocity, and modified reactive strength index (mRSI), along with their fatigue biomarkers and subjective recovery perceptions. The CMJ performance was tested immediately post-exercise and at 24 and 48 h. Results: The FIR garments led to significant improvements in neuromuscular recovery, with greater increases in the jump height, takeoff velocity, and mRSI observed at 48 h post-exercise (p < 0.05). Notably, the mRSI showed earlier improvements at 24 h. The fatigue biomarkers did not differ between the groups (p > 0.05), suggesting localized rather than systemic recovery effects. The participants in the FIR group reported faster subjective recovery, with a readiness to resume activity perceived within 48 h, compared to slower recovery in the placebo group. Conclusions: FIR garments may enhance neuromuscular recovery and subjective recovery perceptions following resistance exercise, likely by improving the peripheral blood flow, metabolic clearance, and tissue oxygenation. These findings suggest that FIR garments may be effective in enhancing both neuromuscular and perceived recovery following resistance exercise, supporting their potential use as a post-exercise recovery tool. Background and Objectives: The perioperative use of beta-blockers remains controversial due to conflicting evidence of their risks and benefits. The aim of this study was to evaluate the association between chronic beta-blocker (bb) therapy and perioperative cardiac events in non-cardiac surgeries using 24 h continuous Holter monitoring. Materials and Methods: A prospective observational study was conducted on patients undergoing elective or emergency non-cardiac surgery at a Romanian tertiary care hospital. The patients were divided into two groups: G1 (not receiving Bb) and G2 (on chronic Bb). The incidences of perioperative cardiac events, such as severe bradycardia (<40 b/min), new-onset atrial fibrillation (AF), extrasystolic arrhythmia (Ex), and sustained ventricular tachycardia (sVT) and arterial hypotension, were compared between the two groups using clinical, electrocardiography (ECG), and Holter ECG data. Beta-blocker indications, complications, and outcomes were analyzed using chi-squared tests and logistic regression. Results: A total of 100 consecutive patients (63% men, mean age of 53.7 years) were enrolled in the study. G2 included 30% (n = 30) of patients on chronic beta-blocker therapy. The indications included atrial fibrillation (46.7%, n = 14), arterial hypertension (36.7%, n = 11), extrasystolic arrhythmias (10%, n = 3), and chronic coronary syndrome (6.6%, n = 2). Beta-blocker use was significantly associated with severe bradycardia (n = 6; p < 0.001) in G2, whereas one patient in G1 had bradycardia, and 15 and 1 patients had hypotension (p < 0.001) in G1 and G2, respectively. The bradycardia and arterial hypotension cases were promptly treated and did not influence the patients’ prognoses. The 14 patients with AF in G2 had a 15-fold higher odds of requiring beta-blockers (p < 0.001, odds ratio (OR) = 15.145). No significant associations were found between beta-blocker use and the surgery duration (p = 0.155) or sustained ventricular tachycardia (p = 0.857). Ten patients developed paroxysmal postoperative atrial fibrillation (AF), which was related to longer surgery durations (165 (150–180) vs. 120 (90–150) minutes; p = 0.002) and postoperative anemia [hemoglobin (Hg): 10.4 (9.37–12.6) vs. 12.1 (11–13.2) g/dL; p = 0.041]. Conclusions: Patients under chronic beta-blocker therapy undergoing non-cardiac surgery have a higher risk of perioperative bradycardia and hypotension. Continuous Holter monitoring proved effective in detecting transient arrhythmic events, emphasizing the need for careful perioperative surveillance of these patients, especially the elderly, in order to prevent cardiovascular complications These findings emphasize the necessity of tailored perioperative beta-blocker strategies and support further large-scale investigations to optimize risk stratification and management protocols. Dapagliflozin, a sodium-glucose cotransporter 2 inhibitor, treats type 2 diabetes by blocking renal glucose reabsorption and promoting urinary glucose excretion. This mechanism lowers blood glucose concentrations independently of insulin. The resulting caloric loss also contributes to weight reduction. Although these effects are well documented in patients with diabetes, their magnitude and underlying mechanisms in healthy individuals remain poorly understood. Background/Objectives: We investigated metabolic alterations after a single 10 mg dose of dapagliflozin in healthy adults with normal body-mass indices (BMIs) using untargeted metabolomics. Methods: Thirteen healthy volunteers completed this study. Plasma was collected before and 24 h after dosing. Untargeted metabolic profiling was performed with ultra-high-performance liquid chromatography–quadrupole time-of-flight/mass spectrometry. Results: Twenty-five endogenous metabolites were annotated; ten were putatively identified. Eight metabolites increased significantly, whereas two decreased. Up-regulated metabolites included phosphatidylcholine (PC) species (PC O-36:5, PC 36:3), phosphatidylserine (PS) species (PS 40:2, PS 40:3, PS 36:1, PS 40:4), lysophosphatidylserine 22:1, and uridine. Dehydroepiandrosterone sulfate and bilirubin were down-regulated. According to the Human Metabolome Database, these metabolites participate in glycerophospholipid, branched-chain amino acid, pyrimidine, and steroid-hormone metabolism. Conclusions: Dapagliflozin may affect pathways related to energy metabolism and homeostasis beyond glucose regulation. These data provide a reference for future investigations into energy balance and metabolic flexibility in metabolic disorders. Full article

Honey is prized for its nutritional and healing properties, but its quality can be affected by contamination with toxic elements. This study evaluates the nutritional value and health risks of fifteen honey samples from different agro-climatic regions of Pakistan. Physicochemical properties such as color, pH, electrical conductivity, moisture, ash, and solids content were within acceptable ranges. ICP-OES analysis was used to assess six essential minerals and ten toxic metals. Except for slightly elevated boron levels (up to 0.18 mg/kg), all elements were within safe limits, with potassium reaching up to 1018 mg/kg. Human health risk assessments—including Average Daily Dose of Ingestion, Total Hazard Quotient, and Carcinogenic Risk—indicated no carcinogenic threats for adults or children, despite some elevated metal levels. Antioxidant activity, measured through total phenolic content (TPC) and DPPH radical scavenging assays, showed that darker honeys had stronger antioxidant properties. While the overall quality of honey samples was satisfactory, significant variations (p ≤ 0.05) were observed across different regions. These differences are attributed to diverse agro-climatic conditions and production sources. The findings highlight the need for continued monitoring to ensure honey safety and nutritional quality.
