Long COVID and Reduced Thrombosis in Antihistamine-Treated Patients: An Observational Study in the Metropolitan Area of Barcelona

Background: Early evidence from a nursing home in Yepes (Toledo, Spain) indicated that antihistamines combined with azithromycin prevented deaths and hospitalizations during the first COVID-19 wave. Subsequent data from the Consorci Sanitari de Terrassa (CST) showed that patients chronically taking antihistamines had significantly reduced hospital admissions and mortality. However, a concerning rise in long COVID incidence (2–5%) after the third infection and a doubling of thrombosis rates in patients over 60 were observed. Objective: This study aimed to determine whether chronic antihistamine prescription is associated with a reduction in long COVID syndrome and thrombotic events. Methods: We analyzed anonymized data from the CST population (n = 192,651 as of March 2025). Variables included age, gender, chronic antihistamine use, number of chronic treatments (nT), COVID-19 vaccination status, SARS-CoV-2 infection history, long COVID (LC) incidence, and aggregated thrombotic events. Odds ratios (OR) were calculated using chi-square tests. Results: The prevalence of LC increased progressively with successive infections in the non-antihistamine group. No significant differences were found with the antihistamine group, which presented no LC cases among the 52 patients with three documented infections. Thrombotic events were significantly less frequent in antihistamine users with at least one chronic prescription (p < 0.0001). Conclusions: Results suggest a protective effect of antihistamines against thrombotic events. While confirmation via multicenter, randomized trials is needed, a pragmatic approach using antihistamines could be considered for symptomatic patients in the early stage of infection. Full article

Urban coastal areas along the Mediterranean are exposed to short-duration convective rainfall, producing infrastructure disruptions and flood-related impacts. This study analyzes 45 rainfall episodes in the Metropolitan Area of Barcelona between 2014 and 2022, combining radar products, rain gauge observations, and urban-scale impact datasets. Storm radar tracking enabled the identification of key spatiotemporal features and assessment of short-term forecasting performance. Convective cells were typically short-lived, lasting less than 30 min in most cases. The main goal of the research has been the comparison between VIL density (DVIL) radar field and short-duration rainfall intensity provided by rain gauges. This is the first study comparing both data types, being a pioneer in this field. We have found a linear relationship between both data types, with weaker values for larger values. More persistent cells had higher DVIL values, observing a difference in behavior with a break point at 2 g/m3. The tracking and nowcasting system were evaluated based on its ability to anticipate convective precipitation. It achieved good scores values (POD of 0.73 and FAR of 0.33), considering the difficulties of tracking this type of convective system. Finally, false alarms associated with elevated DVIL values suggested the difficulty of capturing storm severity by surface-based precipitation measurements. Full article

Urban environments play an important influence in influencing healthy lifestyles and reducing sedentary behaviour (SB), particularly as facilitators of physical activity (PA). Urban spaces often do not support healthy lifestyles. A Community of Practice (CoP) could be a valuable strategy for co-designing proposals to enhance healthy and active urban environments. We aimed to develop strategies through a CoP to promote PA and reduce SB in the urban setting of a multicultural population based in the Barcelona Metropolitan Area, with a particular focus on people with chronic diseases. A three-session CoP involving 25 participants (community members with chronic conditions, health professionals, urban planners, and local authorities) was implemented as a participatory research approach to identify barriers and facilitators to PA and co-design feasible urban improvement proposals. Ethical approval was obtained from Bellvitge University Hospital’s Ethics Committee. Participants provided informed consent and image release forms. Participants highlighted the importance of accessible, adaptable, and interconnected urban spaces to address barriers and leverage facilitators to PA. Proposed interventions targeted four selected areas of the neighbourhood and included expanding shaded areas, creating pedestrian-friendly routes and enhancing green spaces. This study highlights the effectiveness of a CoP in identifying and addressing barriers to PA within urban environments for people with chronic diseases. Findings emphasise the impact of neighbourhood design and accessibility on reducing SB and promoting active lifestyles. The participatory approach offers a replicable model for other urban settings aiming to foster health, although its qualitative and local nature limits generalisability. Full article

To mitigate the impacts of urbanisation and the attendant surface sealing, appropriate measures are required when adapting urban spaces and drainage infrastructure. In this context, the deployment of Sustainable Drainage Systems (SuDSs) has emerged as a viable alternative, delivering highly positive outcomes by enhancing hydrological, hydraulic and landscape performance while restoring ecosystem services to the community. This study evaluates the relative performance of five SuDS typologies, green roofs, bioretention cells, infiltration trenches, permeable pavements, and rain barrels, implemented in a 64 ha subbasin of the metropolitan area of Barcelona, Spain. Using Giswater integrated with the SWMM, the stormwater drainage network was modelled under multiple rainfall scenarios. Performance was assessed using two qualitative indicators, the junction index (Ij) and the conduit index (Ic), which measure surcharge levels in manholes and pipes, respectively. The results show that SuDS implementation affecting 42.8% of the drained area can enhance network performance by 35.6% and reduce flooded junctions by 67%. Among the typologies, rain barrels and bioretention cells were the most effective. The study concludes that SuDS construction, supported by open-source tools and performance-based indicators, constitutes a replicable and technically robust strategy for mitigating the effects of surface sealing and increasing urban resilience. Background/Objectives: Breastfeeding is the best source of food for newborns. Leading health organizations recommend exclusive breastfeeding for the first 6 months of life, followed by the gradual introduction of complementary foods. Evidence shows that breastfeeding offers numerous benefits for newborns, mothers, society as a whole, and the environment. Current breastfeeding rates fall below the established recommendations. This study aims to describe breastfeeding rates at hospital discharge and one month postpartum, analyze fluctuations in feeding types during this period, and identify any characteristics, preventive factors, and barriers to breastfeeding and mixed feeding. Methods: This is a multicenter observational study in the North Metropolitan area of Barcelona, with 411 participants surveyed by midwives. Results: In total, 79% of women were exclusively breastfeeding, 14% practiced mixed breastfeeding, and 7% used formula feeding at hospital discharge. At one month postpartum, these rates fluctuated to 64%, 23%, and 13%, respectively. Factors such as older age, having a university education, having 16 weeks of parental leave, and having a foreign-born status were positively associated with exclusive breastfeeding, while mental illness was associated with a lower prevalence. The use of breastfeeding accessories was not significantly associated with fluctuations in feeding type, but it was associated with maintaining mixed feeding. Conclusions: It is essential to implement individual and community interventions, educate healthcare professionals on factors that hinder breastfeeding, and promote workplace policies that support breastfeeding. Urban boundary delineation is essential for understanding spatial structure, monitoring urbanization, and guiding sustainable land management. Nighttime light (NTL) data effectively capture urban dynamics across multiple spatial scales. This study integrates NTL data with spectral indices to delineate the urban boundaries of the Barcelona Metropolitan Region (BMR) from 2006 to 2018. Through multivariate regression analysis, the normalized difference vegetation index (NDVI) and normalized difference built-up index (NDBI) are identified as key indicators of urban spatial heterogeneity. These indices are combined with brightness thresholds derived from three NTL datasets, DMSP-OLS, Black Marble, and VIIRS, to delineate urban areas more accurately. Results indicate that VIIRS achieved the highest precision in identifying construction land and urbanized areas, with an overall accuracy exceeding 90% and consistency with population density and GDP distribution. A strong spatial correlation between urban distribution and the NDVI–NDBI relationship is confirmed in the BMR. The coupling of multisource remote sensing data improves the accuracy, stability, and reliability of urban boundary delineation, overcoming single-source limitations. This integrated method supports urban planning and sustainable land management through consistent, objective urban mapping and offers a practical reference for applying remote sensing technologies to monitor urbanization dynamics across broader spatial and temporal contexts. Background: Long COVID can persist for years, but little is known about its prevalence in relation to the number of infections. This study examines the prevalence of long COVID in association with the number of infections and vaccination status. Methods: We analyzed anonymized data on long COVID cases, thrombotic events and polypharmacy from March 2020, provided by the Data Analysis Control Department for the population assigned to the CST (192,651 at March 2025). Additionally, we analyzed responses to a long COVID symptom-specific survey distributed in March 2024 to individuals aged 18 to 75 years from the CST population diagnosed with COVID-19 as of December 2023 (n = 43,398; 3227 respondents). Symptomatic patients suspected of having long COVID underwent blood tests to exclude alternative diagnoses. Results: The overall detected prevalence of long COVID was 2.4‰, with higher frequency among women aged 30–59 years (p < 0.001). The survey, combined with specific blood tests, improved detection rates by 26.3%. Long COVID prevalence was 3–10 times higher in individuals with three or more infections than in those with only one recorded infection (based on survey/CST data, respectively). The absolute number of thrombotic events among individuals aged >60 doubled from 2020 to 2024, occurring in both vaccinated and unvaccinated individuals, as well as in those with or without prior documented COVID-19 infection, including in patients without chronic treatments. Conclusions: We found a link between SARS-CoV-2 reinfection and long COVID, and a post-pandemic rise in thrombotic events across all populations, regardless of vaccination or prior infection. Findings support continued COVID-19 diagnosis in suspected cases and mask use by healthcare workers treating respiratory patients. Full article

The Barcelona Metropolitan Area (AMB) has implemented various policies to reduce car use and promote more sustainable mobility. Initiatives such as superblocks, Low Emission Zones (LEZs), and the Bicivia network aim to transform the urban model in response to environmental and congestion challenges. However, the high reliance on private vehicles for intermunicipal travel, uneven infrastructure, and social resistance to certain changes remain significant issues. This study examines the evolution of mobility patterns and assesses the effectiveness of the above policies in fostering real and sustainable change. A mixed-methods approach was adopted, which combined an exploratory factor analysis (EFA) of 2011–2024 data, trend linear regression, and a comparative international analysis. The EFA identified four key structural dimensions: traditional transport infrastructure, active mobility and bus lines, public bicycles and mixed use, and transport efficiency and punctuality. The findings reveal a clear reduction in private car use and an increase in sustainable modes of transport. This indicates that there are prospects for future transformation. Nonetheless, challenges persist in intermunicipal mobility and the public acceptance of the measures. This study provides empirical and comparative evidence and emphasizes the need for integrated metropolitan governance to achieve a resilient and sustainable urban model. Full article

Contemporary urban planning faces the ongoing challenge of developing Green Infrastructure capable of providing vital ecosystem services. Within this framework, the Barcelona metropolitan area has advanced a network of parks that, while serving local neighborhoods, also aim for metropolitan relevance. This study offers a forward-looking analysis of selected parks in the southern Llobregat River basin—an area shaped by historic villages and working-class settlements—to evaluate their contribution to both Green Infrastructure and the region’s polycentric structure. Building on previous landmark studies and multidisciplinary perspectives, the research examines eight parks through four spatial and scalar lenses, assessing their territorial role and accessibility, ecological connectivity, urban integration and permeability, and landscape design with both qualitative and quantitative data. Using a comparative framework alongside research-by-design methods tested in urban design studios, the research links analytical insights to design-based strategies. The outcome is a set of actionable guidelines aimed at enhancing local park performance, with broader implications for over 50 ‘Metropolitan Parks’ spread in more than 30 municipalities. These insights contribute to shaping a more integrated, livable, and resilient metropolitan region. Full article

Background: Breastfeeding mothers have an increased demand for nutrients, including increased intake of certain nutrients, and are recommended to consume a theoretical 500 extra kilocalories (kcal), follow a varied diet, and increase protein, carbohydrate, omega-3, iron, and vitamin D intake. Objectives: We sought to analyze mothers’ energy and nutrient intake and food habits during the postpartum period 6 weeks after delivery and to identify whether there are any differences between breastfeeding and non-breastfeeding mothers. Methods: This was a cross-sectional, multicenter observational study at seven sexual and reproductive healthcare centers in the Metropolitan North area of Barcelona (Spain). The sample comprised 393 women who responded to an infant feeding questionnaire and 24 h diet recall (24 HR). We used frequencies and medians for descriptive analysis as well as the chi-squared and Kruskal–Wallis tests for the bivariate analysis. Results: Mean energy intake was lower than the recommendations in 57% of participants. Mothers who exclusively breastfed consumed a median of 201 kcal more than non-breastfeeding mothers, although this was not significant. The intake of fatty acids and micronutrients, such as iron, calcium, magnesium, and especially vitamin D, was insufficient. Breastfeeding mothers consumed significantly more polyunsaturated fatty acids (PUFAs) (p = 0.0297): 15.4 g vs. 12.7 g per day. Conclusions: There are no significant differences between the diet of breastfeeding and non-breastfeeding women, except for PUFA intake. A general insufficient intake of the analyzed micronutrients was observed. Educational campaigns and dietary guidance from health professionals are a priority. Full article

Assessing water demand is essential for urban planning, aligning with socio-economic and climatic needs. The territorial water footprint identifies water requirements across sectors and detects sources of consumption. This aids in mitigating impacts and evaluating alternative water sources like reclaimed water. In Gavà, water impacts were assessed for residential, commercial, municipal, tourism, industrial, agricultural, and livestock sectors. The total water footprint is 6,458,118 m3, comprising 3,293,589 m3 from blue water, 2,250,849 m3 from green water, and 913,680 m3 from grey water. Agriculture (54.2%), residential (30.9%), and industrial activities (5.8%) are the main water footprint contributors. A key methodological advancement of this study is the refinement of grey water footprint calculations for livestock facilities within the RWF framework, allowing for a more precise assessment of their environmental impact. Integrating geographic information systems with land use mapping helps localize impacts, detect hotspots, and identify infrastructure improvement opportunities. Full article

The scientific literature on residential segregation in large metropolitan areas highlights various explanatory factors, including economic, social, political, landscape, and cultural elements related to both migrant and local populations. This paper contrasts the impact of these factors individually, such as the immigrant rate and neighborhood segregation. To achieve this, a machine learning analysis was conducted on a sample of neighborhoods in the main Spanish metropolitan areas (Madrid and Barcelona), using a database created from a combination of official statistical sources and textual sources, such as Wikipedia. These texts were transformed into indexes using Natural Language Processing (NLP) and other artificial intelligence algorithms capable of interpreting images and converting them into indexes. The results indicate that the factors influencing immigrant concentration and segregation differ significantly, with crucial roles played by the urban landscape, population size, and geographic origin. While land prices showed a relationship with immigrant concentration, their effect on segregation was mediated by factors such as overcrowding, social support networks, and landscape degradation. The novel application of AI and big data, particularly through ChatGPT and Google Street View, has enhanced model predictability, contributing to the scientific literature on segregated spaces. This paper presents a novel study of the Nature-based Solutions (NbSs) approach to analyze and propose mitigation measures for extreme floods. The study area is the Llobregat River in Catalonia, which crosses urban areas. We have selected one section in the final stretch of 4.5 km in the Barcelona Metropolitan Area. The section has suffered several damages in the last floods (e.g., 2016, 2018 and 2019), and we propose measures to reduce flood risk. Therefore, we proposed the following three specific objectives: (a) the identification of critical areas in the river stretches; (b) the identification of NbS opportunities and utilities; and (c) the mitigation measures in concrete areas from NbSs. The effectiveness of a NbS is based on the 2D simulation of the Gloria flood event (20–21 January 2020) with HEC-RAS software (version 6.0) for the better management of stormwater, and it is influenced by design and placement aspects; however, the better use of NbSs can improve flood mitigation and enhance urban resilience. Transport systems are essential for the path toward sustainable urbanisation and the transition to more sustainable living. Recently, European cities have undergone substantial changes, and suburbanisation is posing new challenges. Suburban areas are often more affordable in terms of housing, but these neighbourhoods tend to be car-oriented. This leads to higher commuter costs, immobility, transport and time poverty, pollution, higher accident rates and a lack of social interactions. To offer sustainable mobility options to citizens, we must comprehensively understand, together with their individual characteristics, their specific mobility practices and the built environment where they live. This study is centred on the Barcelona Metropolitan Region, which has a public transport network that covers its entire area. The aim of this study is to examine the relationships between travel behaviour, transport mode use, individual characteristics and built environment characteristics in the place of residence using detailed information sources. Herein, we used data from the 2018 to 2021 annual travel survey conducted in the Barcelona region, together with land use and sociodemographic information. Our findings suggest that transport policies have encouraged sustainable mobility practices, particularly in the centre of Barcelona. Despite the positive results, considerable disparities exist between the inner and outer city, with a notable decline in sustainable mobility practices in the latter, due to the uneven distribution of basic services and uneven provision of public transport, together with lower density areas. Our results demonstrate that this uneven distribution reduces the available sequence profiles of inhabitants. In conclusion, the promotion of sustainable mobility policies necessitates further advances in transport, city and land-use planning that consider equity, gender, the socioeconomic profiles of citizens and mixed urban planning. The concept of mixed-use urban planning is gaining recognition as a crucial element in the development of sustainable and vibrant urban environments. In contrast, many 20th-century cities were designed with segregated land uses and monofunctional zones, following the principles set out in the 1933 Athens Charter. Over time, this approach has been widely criticized, and in the present era, mixed-use environments are praised for fostering social interaction, generating economic synergies, and reducing environmental impacts. This article explores the complex relationship between urban activities, morphology, and planning, with a particular focus on the Barcelona metropolitan area. Utilizing GIS mapping and morphological drawings, this research offers innovative perspectives by analyzing a series of selected urban fragments, highlighting the differences and similarities among various urban fabrics. After a review of the evolution of mixed-use planning regulations and plans since the mid-20th century, a threefold analysis was conducted: examining planning standards and codes, assessing the ground floor activities in promoting urban mixticity, and defining the characteristics of urban patterns’ vitality. Through mapping and indexes, the research offers both qualitative and quantitative evaluations, uncovering new tools to better understand functional mix as a critical element in addressing the challenges of contemporary urbanization.
