Risk Assessment of Brucella Exposure Through Raw Milk Consumption in India: One Health Implications and Control Strategies

Brucellosis is a zoonotic disease with significant public health implications. Understanding the risks of consuming unpasteurized (raw) milk is critical for effective control measures. A quantitative risk assessment was conducted to estimate Brucella abortus contamination in milk from unregulated sources in Punjab, India, where 70% of milk is sold unpasteurized. Samples from lactating cattle and buffalo (N = 261) in ten villages were tested using the Rose Bengal plate test and indirect IgG ELISA. Modelled risk pathways estimated B. abortus shedding probabilities and colony-forming unit (CFU) concentrations in milk, with Sobol sensitivity analysis identifying influential parameters. Buffalo had a higher estimated shedding prevalence (0.04, 95% PI: 0.02–0.07) than cattle (6.3 × 10−3, 95% PI: 2.5 × 10−3–13.2 × 10−3). Mean contamination levels were 2843 CFU/100 mL (95% PI: 0–32,693 CFU/100 mL) for cattle, 17,963 CFU/100 mL (95% PI: 612–67,121 CFU/100 mL) for buffalo, and 7587 CFU/100 mL (95% PI: 82–39,038 CFU/100 mL) combined. High-shedding animals were the most influential factor (total effect sensitivity index of 0.86 [95% CI: 0.63–0.74]). Removing high-shedding animals reduced risk considerably for people who might drink raw milk once (absolute risk reduction of up to 54% in buffalo milk), but once-per-month consumption is still likely high risk. Effective risk mitigation requires a One Health approach, strengthening both public and animal health interventions, because animal health strategies alone will fail if milk from high-shedding animals reaches the unregulated milk market. Most mosquito surveillance programs rely on traps baited with carbon dioxide (CO2) to attract host-seeking mosquitoes. The source of CO2, traditionally dry ice or gas cylinders, poses operational challenges, especially in remote locations. CO2 production from citric acid and sodium bicarbonate (NaHCO3) using low-cost intravenous fluid bags (‘acid traps’) was evaluated in laboratory experiments. Field trials then compared the efficacy of these acid traps with dry ice traps. Using a 2 × 2 Latin square style design, trapping locations were established at two sites in Sydney, New South Wales, Australia (one urban and another peri-urban), to assess trap performance in diverse environments. The laboratory results showed CO2 production of sufficient amount and duration for overnight mosquito capture. Whilst field experiments showed that dry ice traps captured more mosquitoes (up to 59%) than acid traps, numbers were similar in the urban environment. At both sites, species composition was similar, with Culex quinquefasciatus, Aedes notoscriptus, and Aedes vigilax predominantly collected, and these are known species of pest and public health concern. Further modifications to the acid traps could further improve CO2 output and trap efficiency. Full article

Hendra virus disease (HeVD) is an emerging zoonosis in Australia, resulting from the transmission of Hendra virus (HeV) to horses from Pteropus bats. Vaccine uptake for horses is low despite the high case fatality rate of HeVD in both horses and people. We reviewed evidence-based communication interventions to promote and improve HeV vaccine uptake for horses by horse owners and conducted a preliminary evaluation of potential drivers for HeV vaccine uptake using the Behavioural and Social Drivers of Vaccination (BeSD) framework developed by the World Health Organization. Six records were eligible for review following a comprehensive search and review strategy of peer-reviewed literature, but evidence-based communication interventions to promote and improve HeV vaccine uptake for horses were lacking. An evaluation of potential drivers for HeV vaccine uptake using the BeSD framework indicated that horse owners’ perceptions, beliefs, social processes, and practical issues are similar to those experienced by parents making decisions about childhood vaccines, although the overall motivation to vaccinate is lower amongst horse owners. Some aspects of HeV vaccine uptake are not accounted for in the BeSD framework (for example, alternative mitigation strategies such as covered feeding stations or the zoonotic risk of HeV). Overall, problems associated with HeV vaccine uptake appear well-documented. We, therefore, propose to move from a problems-focused to a solutions-focused approach to reduce the risk of HeV for humans and horses. Following our findings, we suggest that the BeSD framework could be modified and used to develop and evaluate communication interventions to promote and improve HeV vaccine uptake by horse owners, which could have a global application to promote vaccine uptake for other zoonotic diseases in animals, such as rabies. Equine pregnancy loss is frustrating and costly for horse breeders. The reproductive efficiency of mares has significant implications for a breeding operation’s economic success, and widespread losses can have a trickle-down effect on those communities that rely on equine breeding operations. Understanding the causes and risks of equine pregnancy loss is essential for developing prevention and management strategies to reduce the occurrence and impact on the horse breeding industry. This PRISMA-guided scoping review identified 514 records on equine pregnancy loss and described the global spatiotemporal distribution of reported causes and syndromes. The multiple correspondence analysis identified seven clusters that grouped causes, syndromes, locations and pathology. Reasons for clustering should be the focus of future research as they might indicate undescribed risk factors associated with equine pregnancy loss. People engaged in the equine breeding industry work closely with horses and encounter equine bodily fluids, placental membranes, aborted foetuses, and stillborn foals. This close contact increases the risk of zoonotic disease transmission. Based on this review, research is required on equine abortion caused by zoonotic bacteria, including Chlamydia psittaci, Coxiella burnetii and Leptospira spp., because of the severe illness that can occur in people who become infected. The overpopulation of stray dogs is a serious public health and animal welfare concern in India. Neglected zoonotic diseases such as rabies and echinococcosis are transmitted at the stray–dog human interface, particularly in low to middle-income countries. The current study was designed to estimate the stray dog populations in Punjab to enhance the implementation of animal birth and disease (for example, rabies vaccination) control programs. This is the first systematic estimation of the stray dog population using a recommended method (mark–re-sight) in Punjab, India. The study was conducted from August 2016 to November 2017 in selected villages or wards in Punjab. For the rural areas, 22 sub-districts in each district were randomly selected, then one village from each of the 22 selected sub-districts was selected (by convenience sampling). For urban areas, 3 towns (less than 100,000 human population) and 2 large cities (more than or equal to 100,000 human population) were randomly selected, followed by convenience selection of two wards from each of the 5 selected towns/cities. To estimate the dog population size, we used a modified mark–re-sight procedure and analysed counts using two methods; the Lincoln–Petersen formula with Chapman’s correction, and an application of Good–Turing theory (SuperDuplicates method; estimated per km2 and per 1000 adult humans and were compared between localities (villages vs. towns), dog sex (male vs. female) and age group (young vs. adult) using linear mixed models with district as a random effect. The predicted mean (95% CI) count of the dogs per village or ward were extrapolated to estimate the number of stray dogs in Punjab based on (a) the number of villages and wards in the state; (b) the adult human population of the state and (c) the built-up area of the state. Median stray dog populations per village and per ward using the Lincoln–Petersen formula with Chapman’s correction were estimated to be 33 and 65 dogs, respectively. Higher estimates of 61 per village and 112 per ward are reported using the SuperDuplicates method. The number of males was significantly higher than the number of females and the number of adult dogs was about three times the number of young dogs. Based on different methods, estimates of the mean stray dog population in the state of Punjab ranged from 519,000 to 1,569,000. The current study revealed that there are a substantial number of stray dogs and a high number reside in rural (versus urban) areas in Punjab. The estimated stray dog numbers pose a potential public health hazard in Punjab. This impact requires assessment. The estimated stray dog numbers will help develop a dog population and rabies control program in which information about the logistics required as well as costs of implementing such programmes in Punjab can be incorporated. That dogs can live and breed as free-living animals contributes to public health risks including zoonotic transmission, dog bites, and compromising people’s sense of safety in public spaces. In Australia, free-living dog populations are comprised of domestic dogs, dingoes, and dog–dingo hybrids, and are described using various terms (for example, stray or community), depending on social or geographic context. Urban expansion and regional migration mean that risks associated with contact between humans and free-living dogs are increasing. Public health authorities, local governments, and community organisations have called for transdisciplinary partnerships to address dog-related health risks with a sustainable long-term approach. Values pluralism and a lack of sustained community engagement in affected areas have meant that the outcome of such efforts to date has been mixed. To identify ideas in public circulation about the impact of unrestrained and free-living dogs on human health and well-being, and understand the framework through which these animals are problematised and solutions are proposed in public discourse, we systematically examined coverage of these issues in print media. Our analyses indicate that reporting in Australian newspapers tends to frame the public health impacts of free-living dogs as problems of public order requiring direct government action to re-establish control. The public health impacts of free-living dog populations in Australia have complex causes that intersect at the nexus between human and canine behaviour, agricultural and land management practices, local bylaws, and efforts to conserve ecological systems. Placing responsibility on governments limits opportunities for greater community involvement in developing integrated One Health approaches. Better-quality evidence of the impacts of dog populations on community health and well-being, and broad community support are needed to reshape public debates on animal control, which, ultimately, will promote more effective approaches to mitigate dog-related public health risks at the human–animal–environment interface. Emerging infectious disease (EID) events have the potential to cause devastating impacts on human, animal and environmental health. A range of tools exist which can be applied to address EID event detection, preparedness and response. Here we use a case study of rabies in Southeast Asia and Oceania to illustrate, via nearly a decade of research activities, how such tools can be systematically integrated into a framework for EID preparedness. During the past three decades, canine rabies has spread to previously free areas of Southeast Asia, threatening the rabies-free status of countries such as Timor Leste, Papua New Guinea and Australia. The program of research to address rabies preparedness in the Oceanic region has included scanning and surveillance to define the emerging nature of canine rabies within the Southeast Asia region; field studies to collect information on potential reservoir species, their distribution and behaviour; participatory and sociological studies to identify priorities for disease response; and targeted risk assessment and disease modelling studies. Lessons learnt include the need to develop methods to collect data in remote regions, and the need to continuously evaluate and update requirements for preparedness in response to evolving drivers of emerging infectious disease. In Australia, the responsibility and associated costs for the control and prevention of Bovine Viral Diarrhoea Virus (BVDV) rest solely with producers. Veterinarians provide producers with farm-specific options for BVDV management and support BVDV control and elimination in their region. We surveyed veterinarians to determine their knowledge, attitudes and practices (KAP) associated with BVDV control in south-east Australia. We found that veterinarians’ recommendations do not always align with producers’ control measures. Veterinarians were uncertain about BVDV prevalence and the proportion of producers using BVDV control measures in their regions. Veterinarians generally promoted biosecurity and vaccination, and were concerned about the welfare and additional disease risks associated with persistently infected (PI) cattle. Veterinarians highlighted concerns about disease risks associated with a previously undocumented practice in which producers collect blood from PI cattle to administer to BVDV naïve cattle; termed “vampire vaccination” in this study. A greater understanding of the burden, impact and economics of BVDV is needed to align veterinarians’ and producers’ KAP to improve BVDV management on farms, and more appreciation of veterinarians’ and producers’ values is needed before BVDV control could be implemented at a regional or country level. Dingo classification and management is complicated by hybridisation with domestic dogs. Northern Australia is a relatively high-risk zone for a rabies incursion, and in the event of an incursion, Aboriginal and Torres Strait Islander peoples who reside in this region would prioritise the protection of dingoes. Therefore, the classification of dingoes in this context is important. Twelve pictures of canids with features associated with both dingoes and domestic dogs from camera traps in the Northern Peninsula Area (NPA), northern Queensland, were shown to Aboriginal and Torres Strait Islander rangers (n = 3), biosecurity officers (n = 2), environmental health workers (n = 2), and residents (n = 39) in the NPA. Nearly all pictures (10/12) were classified as dingo or domestic dog (none as hybrid) and two were inconclusive (no overall agreement). Dingoes were consistently identified as medium to large-framed dogs, with a long nose, pointed ears, narrow abdomen, a bushy or feathered tail, and smooth coats of a single base colour. Some hybrid features were acceptable, including sable coats, lack of white tail tip or feet, and curled tail. These findings are a preliminary guide for identifying canids in the NPA region for whom management might be controversial. Building on this approach via further consultation with residents is needed to inform rabies response policy. Our approach using locally acquired camera trap pictures could also be extended to other regions in which dingoes have value but their management is controversial. Full article

Australia is currently free of canine rabies. Spatio-ecological knowledge about dingoes in northern Australia is currently a gap that impedes the application of disease spread models and our understanding of the potential transmission of rabies, in the event of an incursion. We therefore conducted a one-year camera trap survey to monitor a dingo population in equatorial northern Australia. The population is contiguous with remote Indigenous communities containing free-roaming dogs, which potentially interact with dingoes. Based on the camera trap data, we derived dingo density and home range size estimates using maximum-likelihood, spatially explicit, mark–resight models, described dingo movements and evaluated spatial correlation and temporal overlap in activities between dingoes and community dogs. Dingo density estimates varied from 0.135 animals/km2 (95% CI = 0.127–0.144) during the dry season to 0.147 animals/km2 (95% CI = 0.135–0.159) during the wet season. The 95% bivariate Normal home range sizes were highly variable throughout the year (7.95–29.40 km2). Spatial use and daily activity patterns of dingoes and free-roaming community dogs, grouped over ~3 month periods, showed substantial temporal activity overlap and spatial correlation, highlighting the potential risk of disease transmission at the wild–domestic interface in an area of biosecurity risk in equatorial northern Australia. Our results have utility for improving preparedness against a potential rabies incursion. Full article
