Low WT1 Expression Identifies a Subset of Acute Myeloid Leukemia with a Distinct Genotype

Background: Wilms’ tumor gene 1 (WT1) is a critical player in acute myeloid leukemia (AML), often serving as a biomarker for measurable residual disease (MRD). The WT1 gene is overexpressed in the majority of AML cases at diagnosis, with apparently no correlation with prognosis, and in the meantime, its role in patients with low-level expression is still undefined. This study investigates the mutational landscape and clinical outcomes of AML patients with low WT1 expression at diagnosis. Methods: We analyzed 34 AML patients with low WT1 expression (WT1/ABL1 < 250) diagnosed and treated from 2013 to 2017 at three institutions. Next-generation sequencing (NGS) was employed to investigate the mutational status of 32 genes commonly mutated in AML. The presence of specific mutations, as well as clinical outcomes, was compared to the general AML population. Results: Patients with low WT1 expression showed a significantly higher mutational burden, with a median of 3.4 mutations per patient, compared to the general AML population. Notably, clonal hematopoiesis (CHIP) or myelodysplasia-related (MR) mutations, particularly in ASXL1, TET2, and SRSF2, were present in most patients with low WT1 expression. All but one case of NPM1- or FLT3-mutant AML in the low-WT1 cohort harbored more CHIP or MR mutations. Patients with low WT1 expression had an overall survival (OS) that was superimposable to the OS expected in MR AML. Conclusions: Low WT1 expression in AML is associated with a distinct and complex mutational profile, marked by frequent CHIP and MR mutations. Full article

A syndrome called “Kiwifruit Decline Syndrome” (KiDS) affects kiwifruit in several Mediterranean areas, causing growth arrest and wilt that rapidly progress to desiccation, scarce root growth, absence of fibrous roots, brown soft-rotting areas, and cortical detachment from the central cylinder. The origin is considered multifactorial, and a correlation with hydraulic conductance impairment caused by a high vapor pressure deficit (VPD) and temperature was detected. In this work, over-tree micro-sprinkler irrigation and shading nets were tested to protect leaves from overheating and locally decrease VPD. Leaf gas exchanges, leaf temperature, stem water potential, stem growth, root starch content, root xylem vessel diameter, density, and vulnerability to cavitation were assessed. A positive effect of over-tree irrigation associated with shading was observed: lower leaf temperature, higher stem water potential, stomatal conductance, and photosynthesis were detected; moreover, root starch content was higher in the summer. Narrow xylem vessel diameters were observed, indicating a long-term adaptation to rising VPD for lower vulnerability to cavitation, in all plants, but higher diameter, lower density, and higher vulnerability index indicated lower plant water stress under over-tree irrigation associated with shading. These results indicate that microclimate control by proper agronomic management can protect kiwifruit from climate stress, decreasing the risk of KiDS onset. Full article

At its earliest appearance, atrial fibrillation (AF) is often unnoticed, asymptomatic, and/or merely device-detected. Widespread use of heart-rate monitoring technologies has facilitated such “nascent atrial fibrillation (nAF)” recognition. Consequently, clinicians face a growing number of patients affected by new-onset AF in the absence of a definite indication for anticoagulation due to several counterarguments: (1) a CHA2DS2-VA score ≤ 1 in otherwise apparently healthy subjects; (2) an uncertain embolic/hemorrhagic benefit/risk ratio with anticoagulation; (3) EKG demonstration and confirmation of AF; and (4) existence of a pathogenic mechanism other than atrial hypercoagulability. In this frustrating limitation of pharmacological options, cardiologists may miss a complete comprehension of drugs with proven anti-ictal potential, whose administration may serve both as a bridge strategy toward future anticoagulation and as a consolidative strategy paralleling anticoagulation. This review aims to summarize and elucidate such therapeutic strategies and their preventative mechanisms. Full article

Background: Fatigue is a prevalent and complex condition with significant impacts on well-being. Existing fatigue assessments often lack comprehensiveness or practicality for general population studies. Methods: This study validated the REST Questionnaire, a novel fatigue assessment tool, in a sample of 268 adults. Psychometric properties, including internal consistency and construct validity, were evaluated. REST scores were correlated with WHO-5 well-being, BMI, self-rated health, and chronic conditions. Exploratory factor analysis identified underlying dimensions of fatigue. Results: The REST Questionnaire demonstrated excellent internal consistency (Cronbach’s alpha = 0.918) and construct validity. Higher fatigue scores were associated with lower well-being, female gender, and the presence of certain chronic conditions (cancer, kidney stones, gastric ulcers). Two distinct fatigue dimensions, “physical fatigue and functional impacts” and “emotional and social consequences”, were identified. Conclusions: The REST Questionnaire is a reliable and valid tool for assessing fatigue in the general population. Its multidimensional framework and sensitivity to comorbidities offer valuable insights for research and public health applications, with the potential to inform targeted interventions aimed at improving well-being. Dried fig production and trade are prominent in Mediterranean and Middle east countries, with Turkey as the major player. Concerns about the quality of Turkish supply in the EU market give chances to the Italian production to uphold quality standards and cope with the increasing demand. National consumption of dried figs is showcased in market reports and nutritional surveys, with insufficient insights on consumers’ preferences. This research combines qualitative and quantitative approaches to enrich the current knowledge and uncover consumers’ attitudes towards whole dried figs. A face-to-face survey was conducted to assess the relevance of selected intrinsic, extrinsic and credence attributes upon purchasing. A hierarchical cluster analysis detected four typologies of consumers and validate previous findings on taste being the prime purchasing driver. Price affects more the behaviour of low-income large families; the groups are featured by contrasting opinions on environmental and societal impacts of dried fig production, and its rootedness to the local context. This allowed to draft some considerations on possible marketing strategies tailored to different consumers. Full article

West Nile Virus, an arthropod-borne RNA virus, may result in severe neurological disease. West Nile neuroinvasive disease is characterized by meningitis, encephalitis, and possible acute flaccid paralysis. Here, we report a case of neuroinvasive WNV in a 65-year-old woman hospitalized for hyperpyrexia, chills, intense asthenia, and continuous vomiting. Within days, her clinical condition worsened with the onset of severe neurological symptoms, leading to her death within 10 days despite supportive therapies being administered. The diagnosis of West Nile disease was made through nucleic acid amplification testing (NAAT) on blood and cerebrospinal fluid. However, in the final stages of the illness, cerebrospinal fluid collection was not possible due to the patient’s critical condition, and a nasopharyngeal swab was used instead. The nasopharyngeal swab facilitated the collection of a sample, which was subsequently analyzed for the presence of the virus and allowed for sequencing, showing that it was a strain that had been circulating in Sardinia for some time and had demonstrated its pathogenicity by causing the death of a hawk in 2021. This case report highlights the rapid progression and severity of WNV infection, particularly in vulnerable individuals, and suggests the potential utility of nasopharyngeal swabs as a less invasive option for sample collection. It also underscores the potential for the zoonotic transmission of the virus from birds to humans through vectors, emphasizing the importance of monitoring and controlling WNV outbreaks, especially in regions where such circulation is observed. Background: Arrhythmias in pregnancy are complex to manage due to the teratogenic effects of many antiarrhythmic drugs and the common use of ionizing radiation during catheter ablation procedures. Furthermore, pregnant women are extremely vulnerable and difficult to treat because of the progressive physical and hormonal changes that occur during the nine months of pregnancy. Case Presentation: In this case report, we describe a complex clinical case of a 34-year-old pregnant woman who was affected by an incessant right atrial tachycardia, with signs and symptoms of initial hemodynamic instability. This tachycardia was refractory to antiarrhythmic drugs, so a zero-fluoroscopy ablation was performed. The first procedure was complicated by cardiac tamponade, quickly resolved without further complications for the mother or the fetus. In the following days, a deep venous thrombosis occurred at the femoral venous access. After a few days, the patient underwent a second procedure that was successful and resulted in the restoration of a sinus rhythm. Conclusions: The management of this clinical case was complex both from a procedural and a clinical (cardiological and gynecological) point of view. Finally, the integration of the various skills led to an excellent result. Full article

Risk stratification for malignant ventricular arrhythmias and sudden cardiac death is a daunting task for physicians in daily practice. Multiparametric mapping sequences obtained via cardiovascular magnetic resonance imaging can improve the risk stratification for malignant ventricular arrhythmias by unveiling the presence of pathophysiological pro-arrhythmogenic processes. However, their employment in clinical practice is still restricted. The present review explores the current evidence supporting the association between mapping abnormalities and the risk of ventricular arrhythmias in several cardiovascular diseases. The key message is that further clinical studies are needed to test the additional value of mapping techniques beyond conventional cardiovascular magnetic resonance imaging for selecting patients eligible for an implantable cardioverter defibrillator. Kiwifruit decline syndrome (KiDS) has affected kiwifruit orchards for more than ten years in the Mediterranean area, severely compromising productivity and causing extensive uprooting. The affected plants go through an irreversible and fast wilting process. The problem has not been solved yet, and a single cause has not been identified. In this work, we carried out a survey on ten five-year-old healthy kiwifruit cv. Hayward plants cultivated in an area strongly affected by KiDS and characterised by a rising temperature and vapor pressure deficit (VPD). Five plants were located in a KiDS-affected orchard. Our goal was to assess the hydraulic conductance of asymptomatic plants in a KiDS-affected area where rising climate change stress is underway. Our hypothesis was that a rising temperature and VPD could impair xylem functionality, leading the plants to develop strategies of tolerance, such as vessel narrowing, or stress symptoms, such as cavitation or implosion, inducing a higher risk of KiDS onset. Hydraulic conductance was investigated using a physiological and morphological approach to detect trunk sap flow, trunk growth and daily diameter variations, leaf gas exchanges and temperature, stem water potential, and the root xylem vessel diameter and vulnerability to cavitation. A strong xylem vessel narrowing was observed in all plants, with the highest frequency in the 30–45 µm diameter class, which is an indicator of long-term adaptation to a rising VPD. In some plants, cavitation and implosion were also observed, which are indicative of a short-term stress response; this behaviour was detected in the plants in the KiDS-affected orchard, where a high leaf temperature (>39 °C), low stomatal conductance (<0.20 mol H2O m−2 s−1) and transpiration (<3 mmol H2O m−2 s−1), low stem water potential (<−1 MPa), high vulnerability to cavitation (3.7 μm mm−2), low trunk sap flow and high daily stem diameter variation confirmed the water stress status. The concurrence of climate stress and agronomic management in predisposing conditions favourable to KiDS onset are discussed, evidencing the role of soil preparation, propagation material and previous crop. Lumpy skin disease (LSD) is a viral disease of cattle and water buffalo characterized by cutaneous nodules, biphasic fever, and lymphadenitis. LSD is endemic in Africa and the Middle East but has spread to different Asian countries in recent years. The disease is well characterized in cattle while little is known about the disease in buffaloes in which no experimental studies have been conducted. Six buffaloes and two cattle were inoculated with an Albanian LSD virus (LSDV) field strain and clinically monitored for 42 days. Only two buffaloes showed fever, skin nodules, and lymphadenitis. All samples collected (blood, swabs, biopsies, and organs) were tested in real-time PCR and were negative. Between day 39 and day 42 after inoculation, anti-LSDV antibodies were detected in three buffaloes by ELISA, but all sera were negative by virus neutralization test (VNT). Cattle showed severe clinical signs, viremia, virus shedding proven by positive real-time PCR results, and seroconversion confirmed by both ELISA and VNT. Clinical findings suggest that susceptibility in buffaloes is limited compared to in cattle once experimentally infected with LSDV. Virological results support the hypothesis of buffalo resistance to LSD and its role as an accidental non-adapted host. This study highlights that the sensitivity of ELISA and VNT may differ between animal species and further studies are needed to investigate the epidemiological role of water buffalo. Full article

West Nile virus (WNV) (Flaviviridae, Flavivirus) infection is a mosquito-borne zoonosis able of causing disease and death in humans and animals. Over the past decade, WNV infections have been a significant public health concern in Europe, and Italy has been among the most affected countries since 2008. The 2022 vector season has been characterized by an intense and early circulation of WNV. This report describes cases of co-circulation of WNV L1 and of WNV L2 occurring at the end of the 2022 vector season in Sicily and Tuscany, regions where no strains had ever been sequenced. The phylogenetic analysis of the detected strains confirmed the peculiar WNV scenario that has characterized the Italian West Nile disease (WND) epidemic since its appearance. The circulation observed in Tuscany was in fact a consequence of the spread of endemic strains to new areas while the Sicilian episodes were linked to new introductions of WNV L1 and L2 strains likely from other European countries. West Nile virus (WNV) is a globally significant mosquito-borne Flavivirus that causes West Nile disease (WND). In Libya, evidence of WNV circulation has been reported in humans but never in animals. The aim of this study was to determine the seroprevalence of WNV infection in horses and dogs in Libya. In total, 574 and 63 serum samples were collected from apparently healthy, unvaccinated horses and dogs, respectively, between 2016 and 2019. A commercially available competitive enzyme-linked immunosorbent assay (c-ELISA) kit was initially used to test the collected samples for the presence of WNV Ig-G antibodies. Positive and doubtful sera were also tested using a more specific virus neutralisation assay to confirm whether the ELISA-positive results were due to WNV or other Flavivirus antibodies. The seroprevalence of WNV IgG antibodies according to ELISA was 13.2% out of 574 of total horses’ samples and 30.2% out of 63 of total dogs’ samples. The virus neutralisation test (VNT) confirmed that 10.8% (62/574) and 27% (17/63) were positive for WNV-neutralising titres ranging from 1:10 to 1:640. Univariable analysis using chi-square tests was conducted to measure the statistical significance of the association between the hypothesized risk factors including city, sex, breed, and age group and were then analyzed using the subsequent multivariable logistic regression model for horse samples. Age group was found to be the only significant risk factor in this study. The results of the present study provide new evidence about WNV circulation in Libya. Full article

The role of migratory birds in the spread of ticks and tick-borne pathogens along their routes from Africa to Europe is increasingly emerging. Wild birds can host several tick species, often infected by bacteria responsible for zoonoses. The aim of the study is to assess the possible introduction of exotic ticks carried by migratory birds into Italy from Africa and to detect the presence of Rickettsia species and Coxiella burnetii they may harbor. During a two-year survey, we collected ticks from migratory birds captured during their short stop-over on Ventotene Island. Specimens were first identified by morphology or sequencing molecular targets when needed, and then tested by real-time PCR for the presence of selected pathogens. A total of 91% of the collection consisted of sub-Saharan ticks, more than 50% of which were infected by Rickettsia species belonging to the spotted fever group, mainly represented by R. aeschlimannii. In contrast, the suspected C. burnetii detected in two soft ticks were confirmed as Coxiella-like endosymbionts and not the pathogen. Although there are still gaps in the knowledge of this dispersal process, our findings confirm the role of migratory birds in the spread of ticks and tick-borne pathogens, suggesting the need for a continuous surveillance to monitor the potential emergence of new diseases in Europe. The lack of young people engaged in agriculture threatens future food security and social balance. The agricultural sector and rural areas are experiencing an aging demography. The data confirm the global historical trend of a shortage of young people deciding to pursue careers in agriculture and remaining in rural spaces. Understanding young people’s views on agriculture is the key to supporting their participation in the sector by examining the unattractive factors that influence their perceptions. This study aims to explore these factors through a literature review of the past 10 years, applying the PRISMA model. We used VOSviewer to identify the key elements linked in a co-word map and revealed four clusters that mainly influence young people’s perceptions. The reviewed papers are from developed and developing countries, as our approach seeks to transcend the national boundaries that often delimit analyses on generational renewal. The study brings out the common factors that affect youth’s choice in agriculture, highlighting their perceptions and aspirations. The discussions and conclusion aim to provide a different perspective to understand the decision-making process of young people toward an agricultural career. Full article

The increase in oil production from petroleum reservoirs has led to studies examining the effects of these activities on groundwater quality. Oily wastewater associated with oil production is often reinjected through abandoned wells into the unproductive portions of the reservoir to avoid its discharge on the surface. The reinjection process is designed to be environmentally friendly and to exclude direct interactions between injected fluids and the surrounding groundwater; nevertheless, the evaluation of the compatibility between this process and the protection of the surrounding environment is of utmost importance when oilfields are located within sensitive and protected areas. The present work aimed to evaluate the impact of the oily wastewater reinjection into a long-term and high-rate disposal well in the Val d’Agri oilfield (Southern Italy). Previous preliminary investigations carried out at the study site led researchers to hypothesize the possible hydrocarbon contamination of the shallower aquifer caused by reinjection well integrity issues. Our strategy is based on an integrated and multidisciplinary approach involving isotopic (stable isotopes 2H and 18O), chemical, and microbiological (characterization of bacterial and archaeal communities) analyses. After a comprehensive and meticulous examination of the research data, it has been ascertained that significant discrepancies exist between the shallow and reinjection water systems. This allowed us to clarify the area’s complex flow dynamics and exclude hydrocarbon contamination of spring waters caused by the reinjection process. Full article
