Dear Colleagues,
Rehabilitation services have undergone a profound transformation, driven by the need to expand access, integrate the use of technologies, promote continuity of care, and strengthen the connections between different levels of healthcare. The traditional model, focused exclusively on rehabilitation in a clinical setting, has gradually evolved to consider strategies that value interdisciplinary work, personalized care, and the active integration of individuals into their social and home contexts.
Integrating rehabilitation services requires an understanding of the therapeutic process as a continuum in which the patient transitions between different care settings (hospital, outpatient services, community, and home) in a coordinated, safe, and effective manner. This integrated approach requires innovation in public policies, the reorganization of care flows, the use of digital health technologies (such as telerehabilitation), and the strengthening of collaborative networks between professionals, patients, families, and communities. This Special Issue will bring together scientific evidence, practical experiences, and theoretical–scientific reflections that contribute to the advancement of this integrated rehabilitation model.
We welcome studies addressing assessment and intervention strategies, service organization, professional training, assistive technologies, evidence-based practices, and community initiatives that promote continuity and effective care. We invite researchers, clinicians, managers, and health and rehabilitation professionals to submit scientific articles for consideration for this Special Issue. We will publish works that strengthen the dialogue between science, clinical practice, and public policy, promoting more accessible, humane, and integrated rehabilitation—from the clinic to the community, including at home.

Manuscripts should be submitted online at www.mdpi.com by registering and logging in to this website. Once you are registered, click here to go to the submission form. Manuscripts can be submitted until the deadline. All submissions that pass pre-check are peer-reviewed. Accepted papers will be published continuously in the journal (as soon as accepted) and will be listed together on the special issue website. Research articles, review articles as well as short communications are invited. For planned papers, a title and short abstract (about 100 words) can be sent to the Editorial Office for announcement on this website.
Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-blind peer-review process. Healthcare is an international peer-reviewed open access semimonthly journal published by MDPI.
