The name Eugen Bleuler is almost exclusively linked with matters concerning the nosology of schizophrenia and the relationship of clinical psychiatry and psychoanalysis. His bibliography lists different works which deal with contemporary topics concerning legislation, the fight against alcoholism and for the most part, from the later years, topics concerning general psychology. In this article subject matters from texts by Bleuler, including some which have been inaccessible up to now, will be reviewed, which either allow interpretative conclusions on his conception of the world or which explicitly have this as a topic.With explicit references to his different publications, often dealing with basic problems of medical psychology and a scientifically based ethic, Bleulers anti-religious and anti-philosophical position will be shown. An effort towards integration of seemingly incompatible, side-by-side standing, unrelated biological and psychological concepts, which probably is most likely to be explained by the contemporary background of ideas, is recognisable. In this context Eugen Bleuler refers to an already (especially by Richard Semon) systematically developed theory called “Mnemism” that he interpreted and applied to the psychological circumstances mentioned above. The theory of “mnemism”, which can be most adequately described as a biogenetic-vitalistic theory, is assuming that all organic life – independent of the possibility of a self-reflecting consciousness – is able to learn from experiences made by analysis of environment and to pass it on to following generations. Patterns of stimulus reactions are in the sense of this theory memorised and reactivated under similar situational circumstances by the psychological mode of association. It represents the benchmark for Bleuler’s standpoints, for example the question of determination of human acting.The assumption of the efficacy of specific, not necessarily consciously remembered “engrams” of memory suggests the hypothesis of the existence of unconsciousness. Objective: In Switzerland, like in some other European countries, medical doctors may breach patient confidentiality and report to police authorities any patient who seems prone to automobile accidents or traffic violations. The aim of this study was to see if those patients reported to authorities actually represent a higher risk. Design: This is a study designed after a casecontrol study comparing the characteristics of a group of psychiatric patients who were reported to authorities for preventive purposes with the characteristics of another group of people having disorders that were noticed at the time of an accident or traffic violation. Results: The results show that medical doctors reported more male patients, patients having a low level of education, or patients with a severe psychiatric background.The subjects of the control group that had often been involved in accidents or committed traffic violations in their past did not possess these characteristics. Conclusions: Patients reported to authorities do not appear to represent a higher risk. The breach of medical confidentiality by doctors in reporting to authorities patients who are allegedly at risk is ethically problematical as long as the evaluation of driving performance does not rely on objective bases. A now growing number of studies suggest that delayed intervention in schizophrenic psychosis has detrimental effects on several parameters of illness outcome. Duration of untreated psychosis (= DUP) is decreased by early recognition. However, early recognition is often hampered by the clinically unspecific symptoms and different definitions of the prodromal phase by the widely used classification systems. Help-seeking behaviour is influenced by negative symptoms, prejudice, fear of stigmatisation as well as sometimes inappropriate supplies of help. We suggest that intervention should occur in the least traumatising way, including separate treatment programs and low-dose medication.Young, drug-naive patients have lower receptor thresholds and suffer more often from neuroleptic side effects. Non-compliance and thus a long duration of untreated psychosis are inherent risks of unadapted treatment practices. Full article

The paper draws attention to an emergent pantheoretical model in child and adolescent mental health intervention, which gets its inspiration from a combination of biological, systems, cognitivebehavioural and psychodynamic perspectives, and which is at least for the moment, mostly implicit, to be inferred from attitudes rather than explicitly declared. It is most evident in the “how” of interventions rather than the “what”. A characterization of this implicit systemic model is offered, and its manifestations on treatment approaches taken as a whole are described. Full article
