Bridging the Care Gap: Integrating Family Caregiver Partnerships into Healthcare Provider Education

Background: Family caregivers are a vital yet often under-recognized part of the healthcare system. They provide essential emotional, physical, and logistical support to individuals with illness, disability, or frailty, and their contributions improve continuity of care and reduce system strain. However, many healthcare and social service providers are not equipped to meaningfully engage caregivers as partners. In Alberta, stakeholders validated the Caregiver-Centered Care Competency Framework and identified the need for a three-tiered education model—Foundational, Advanced, and Champion—to help providers recognize, include, and support family caregivers across care settings. This paper focuses on the development and early evaluation of the Advanced Caregiver-Centered Care Education modules, designed to enhance the knowledge and skills of providers with more experience working with family caregivers. The modules emphasize how partnering with caregivers benefits not only the person receiving care but also improves provider effectiveness and supports better system outcomes. Methods: The modules were co-designed with a 154-member interdisciplinary team and grounded in the competency framework. Evaluation used the first three levels of the Kirkpatrick–Barr health workforce education model. We analyzed pre- and post-surveys from the first 50 learners in each module using paired t-tests and examined qualitative feedback and SMART goals through inductive content analysis. Results: Learners reported a high level of satisfaction with the education delivery and the knowledge and skill acquisition. Statistically significant improvements were observed in 53 of 54 pre-post items. SMART goals reflected intended practice changes across all six competency domains, indicating learners saw value in engaging caregivers as partners. Conclusions: The Advanced Caregiver-Centered Care education improved providers’ confidence, knowledge, and skills to work in partnership with family caregivers. Future research will explore whether these improvements translate into real-world practice changes and better caregiver experiences in care planning, communication, and navigation. Full article

This study focuses on the current service/care difficulties and challenges that social institutions in Hungary are facing during their daily operations; how they can react to them utilizing their internal resources, mechanisms, and capacities; and what concrete, tangible needs and demands are emerging in terms of methodological professional support, potential forms, interventions, and direction for professional development. A total of 24 general and 55 specific service and operational problems were identified and assessed in eight different service areas (family and child welfare services, family and child welfare centers, respite care for children, care for the homeless, addiction intervention, care for people with disabilities, care for psychiatric patients, specialized care for the elderly, and basic services for the elderly). The empirical base of the study uses a database of 201 online questionnaires completed by a professional target group working for social service providers in two counties (Győr-Moson-Sopron and Veszprém), representing 166 social service providers. The questionnaires were completed between November and December of 2022. The findings will be used to develop a professional support and development problem map. Social institutions face complex and serious service/care difficulties and challenges in their daily operations. Three distinctive basic problems clearly stand out in both severity and significance from the complex set of factors assessed. The biggest problem in the social care system is clearly the complex challenge of low wages, followed by the administrative burdens in the ranking of operational difficulties, and the third key factor was the psycho-mental workload of staff. Much U.S. research supports the effectiveness of parenting education delivered via the home visiting method. Home visitors are essential to reaching fathers in this context, but not all have favorable attitudes toward father engagement or feel confident working with fathers. Given that father involvement is important for a wide range of child and adolescent outcomes and that fathers benefit from parenting education, it is important to better understand the forces that shape home visitors’ attitudes toward fathers, and thus their subsequent efforts to include them in publicly funded programming. Using survey data from 95 home visitors in Tennessee, this study explores whether home visitors’ beliefs about fathers and attitudes toward father engagement vary as a function of home visitor or agency characteristics. Results suggest training in social work, reporting father-friendly organizational attitudes and behaviors at one’s agency, and reporting supervisor support specifically for father engagement relate to various favorable fathering attitudes. Home visitors’ strategies to engage fathers in home visiting are presented, including strategies for before, during, and after the home visit. Overall, family service agency administrators are in key positions to make decisions that can improve agency father-friendliness, home visitor attitudes toward fathers, and subsequent outcomes for fathers, mothers, and children. The process of youth leaving residential care, for those who grew up in care, remains a global challenge. In South Africa, leaving care is complicated by the almost complete absence of aftercare support services, and a general lack of preparation for leaving care. This study aims to describe South African care-leavers’ views on how well they think they were prepared for care-leaving and what they believe could improve preparation for leaving care. This study is framed in resilience theory, to identify the interactional processes that facilitate better-than-expected outcomes. One-on-one qualitative interviews were conducted with 17 care-leavers, from both state-run and non-governmental residential care facilities. Grounded theory methods of data analysis were used. Participants identified both enablers of care-leaving (e.g., supportive relationships and early workplace exposure) and hinderances to their leaving care (e.g., overprotection and lack of family reunification services). This study concludes that more purposeful preparation for leaving care is required, with an emphasis on building youths’ capacity for interdependence, completion of education before leaving care, and ensuring family reunification work is done. Social work has long operated at the intersection of care and control—nowhere is this more apparent than in its treatment of sex-working parents. This article draws on participatory research with thirteen sex-working parents in California to examine how the child welfare system, family court, and public benefit infrastructures extend punitive surveillance under the guise of support. Utilizing the framework of prison industrial complex abolition, the analysis identifies three key findings: first, family policing systems often mirror the coercive dynamics of abusive relationships that sex work helped participants to escape; second, access to social services is contingent on the performance of respectability, with compliance met not with care but with suspicion and deprivation; and third, sex-working parents enact abolitionist praxis by creating new systems of safety and stability through mutual aid when state systems fail. As social work reckons with its complicity in the carceral state, the everyday practices of sex-working parents offer a powerful blueprint for care rooted in trust, unconditional positive regard, and self-determination. School social work practice in the South African context is a growing field; however, there is limited research regarding the roles and responsibilities of school social workers, particularly in the Gauteng province. This province is unique in that school social workers are employed by multiple institutions, including individual schools and the education and social development departments. This study aimed to explore and describe the roles and responsibilities of school social workers in the Gauteng province, recognizing them as critical specialists in addressing learners’ psychosocial needs within school settings. An explorative qualitative design was used in this study. Data were collected from 22 purposively selected participants, comprising school social workers, supervisors, and provincial managers of school social work programs. Semi-structured interviews were conducted to collect data, and thematic analysis was employed to identify themes. The findings revealed context-specific roles of school social workers, including the creation of conducive teaching and learning environments, advocacy for social justice and child protection, conducting interviews and psychosocial assessments, providing counseling and trauma debriefing, conducting home visits and offering family services, removing abused learners from harmful environments, including their respective homes, and providing parental skills training and support. As a conclusion, this study highlights the need for standardized national and provincial guidelines to formalize and support school social work practice. It is recommended that the identified roles be incorporated into future practice frameworks. Furthermore, it is suggested that a uniform assessment tool be developed to promote consistency and guide school social workers in the initial evaluation processes. Full article

The widespread adoption of mobile work, driven by advancements in information and communication technology, has increasingly blurred the boundaries between work and personal life. This phenomenon can increase job stress, potentially leading to sleep deprivation, which affects not only employees’ health and well-being but also organizational performance. Grounded in Conservation of Resources theory, this study examines the pathway through which mWork contributes to sleep deprivation, focusing on the mediating role of job stress, and investigates the moderating effects of gender and number of children on this relationship. Data were collected using a stratified random sampling method across three waves with 4-week intervals, involving 325 employees in South Korea engaged in diverse occupations, including the administrative, technical, service, and sales sectors. The findings reveal that mWork increases sleep deprivation through the mediation of job stress. Furthermore, the relationship between mWork and job stress was found to vary depending on gender and number of children, with stronger moderating effects observed among women and employees with children. This study underscores the need for organizations to develop tailored management strategies that address the unique challenges posed by mWork, taking particular note of employees’ gender and family responsibilities. By mitigating the negative effects of mWork on job stress and sleep deprivation, organizations can enhance employee well-being and promote sustainable long-term performance. Full article

Rights-Based Priorities for Children with SEND in the Post-COVID-19 Era: A Multi-Method, Multi-Phased, Multi-Stakeholder Consensus Approach

Background: The provision of education, health, and social care for children with special educational needs and disabilities (SEND) in England has long been criticised for its inequities and chronic underfunding. These systemic issues were further exacerbated by the onset of the COVID-19 pandemic and the accompanying restrictions, which disrupted essential services and resulted in widespread unmet needs and infringements on the rights of many children with SEND. This study aimed to use a three-phase consensus-building approach with 1353 participants across five stakeholder groups to collaboratively develop evidence-informed priorities for policy and practice. The priorities sought to help address the longstanding disparities and respond to the intensified challenges brought about by the pandemic. Methods: A total of 55 children with SEND (aged 5–16), 893 parents/carers, and 307 professionals working in SEND-related services participated in the first phase through online surveys. This was followed by semi-structured interviews with four children and young people, ten parents/carers, and 15 professionals, allowing for deeper exploration of lived experiences and priorities. The data were analysed, synthesised, and structured into five overarching areas of priority. These were subsequently discussed and refined in a series of activity-based group workshops involving 20 children with SEND, 11 parents/carers, and 38 professionals. Results and Conclusions: The consensus-building process led to the identification of key priorities for both pandemic response and longer-term recovery, highlighting the responsibilities of central Government and statutory services to consider and meet the needs of children with SEND. These priorities are framed within a children’s rights context and considered against the rights and duties set out in the United Nations Convention on the Rights of the Child (1989). Priorities include protecting and promoting children with SEND’s rights to (1) play, socialise, and be part of a community, (2) receive support for their social and emotional wellbeing and mental health, (3) feel safe, belong, and learn in school, (4) “access health and social care services and therapies”, and (5) receive support for their parents/carers and families. Together, they highlight the urgent need for structural reform to ensure that children with SEND receive the support they are entitled to—not only in times of crisis but as a matter of routine practice and policy. Parents provide most of the support needed for children with cerebral palsy (CP) to increase the child’s participation and independence. Understanding the experiences of parents caring for children with CP is essential for developing effective family programs and services. The current knowledge about parents’ experiences in CP is based on studies in Western countries, with little known about this phenomenon in Arab countries like Saudi Arabia. This study aimed to understand the unique experiences and support needs of Saudi parents caring for children with CP from a social-ecological perspective. We conducted a qualitative, exploratory, descriptive study involving 12 semi-structured interviews with mothers and fathers of children with different types of CP. We analyzed the data using a reflexive thematic approach, following six distinct phases. Participants’ narratives revealed a complex caregiving journey marked by both challenges and rewards. Support from Saudi nuclear and extended family members was considered important; however, many parents expressed a need for additional physical and financial assistance from their families. Parents reported feeling stressed and experiencing challenges in accessing and navigating educational and healthcare services. Our findings highlight that Islamic values play a crucial role in the experiences of Saudi parents. These values foster a sense of collectivism, highlighting the importance of family support and community involvement, which can affect the Saudi caregiving environment. Parents remain an essential yet often invisible part of the Saudi caregiving system. Without adequate support, parents are at risk of experiencing social, financial, academic, physical, and mental health challenges, which may affect their overall family well-being. Future work may need to consider spiritual and gender roles when developing programs or services to support Saudi parents of children with CP. Full article

Flourishing in children is an indicator of positive development in the areas of emotional, social, and cognitive development. Using a recent dataset from the US National Survey of Children’s Health, this study investigates the association between access to a quality healthcare system and flourishing indicators in school-aged children with autism. The outcome variable describes the proportion of children aged 6–17 with autism meeting the flourishing criteria, which include measures related to learning, resilience, and self-regulation. The main independent variable includes six core indicators for school-aged children and adolescents, which assess whether the family feels like a partner in their child’s care, the child has a medical home, receives regular medical and dental preventive care, has adequate insurance, has no unmet needs or barriers to accessing services, and has prepared for transition to adult healthcare. Multivariable logistic regression models were used to examine the relationships between various independent variables and the outcome of interest. The results show that children with autism who receive comprehensive and coordinated care are more likely to flourish compared to those without such care for five of these six indicators, while controlling for sex, race, parental education, household income, self-reported autism severity, general health status, and the number of adverse childhood experiences. Children with autism are more likely to flourish when their families and healthcare providers work together effectively. Addressing gaps in the quality care system is essential for developing holistic approaches that empower children with autism to thrive and reach their full potential. Supportive health policies and effective coordination between families and healthcare providers are crucial for fostering the flourishing of children with autism, ensuring comprehensive, individualized, and continuous care. This study explores the effects of husbands’ post-infidelity behaviors on wives’ forgiveness from a gender perspective. The study employs a longitudinal research design and hermeneutic phenomenology to investigate the wives’ forgiveness potential paths/experiences after their husband’s infidelity. It involves 15 years of in-depth interviews with five wives who had encountered their husbands’ infidelity, with three to six interviews per participant. The findings reveal that husbands’ post-infidelity behaviors are associated with power dynamics in the marriage. At the same time, pressures from culture, gender roles, and social expectations lead wives to adopt “pseudo-forgiveness.” The study proposes two pathways to “genuine forgiveness” for wives. The path includes phases of “Her Rethinking,” leading to the “Balance Marital Relationship and Non-Self-Sacrifice stage.” For low-power-in-relationship wives, the path comprises stages such as “Her Awakening,” “Challenge Women’s Roles in Social Expectations,” and “Take Actions to Enhance Her Power/Ability,” ending in “Balance Marital Roles and Self-Realization.” Both pathways emphasize that forgiveness is a personal decision-making process and that empowerment and enhanced wives’ ability are essential for achieving “genuine forgiveness.” These findings can contribute to marriage and family work and welfare services, helping wives and professionals understand the types and processes of forgiveness and better navigate complex challenges related to marital infidelity. The High Andean region of Peru, characterized by a complex orography, has unique and highly biodiverse ecosystems. This region has several headwater basins that play a critical role in the hydrological cycle, providing diverse ecosystem services essential to sustain biodiversity and supply water to human communities. Despite the importance of this region, it faces significant human intervention, particularly mining activities, which affect basin headwaters and jeopardize water security. This study aimed to calibrate the Biological Monitoring Working Party (BMWP) index to evaluate water quality in High Andean rivers in Peru affected by mining activities, using aquatic macroinvertebrates as bioindicators. We used a 15-year dataset (2008 to 2023) from three headwater basins in the High Andean region; this dataset included physicochemical water quality parameters, trace metals, and aquatic macroinvertebrates. The BMWP was calibrated for the High Andean region of Peru with this dataset (BMWP/PeIAZIM); afterward, it was validated to assess water quality in an area influenced by mining activities in this region. The results allowed us to differentiate between aquatic macroinvertebrate families tolerant to mining pollution and highly sensitive families. The sites heavily affected by mining activity returned very low BMWP/PeIAZIM scores; sites with no mining impact had the highest scores. These findings indicate that the calibrated index can be used for water resource management in the High Andean region, contributing to the conservation of its ecosystems. Full article

Grasslands, particularly permanent grasslands, provide vital ecosystem services and, therefore, focus a number of management challenges. Grassland management revolves around organizing how livestock graze in both space and time, using various grazing methods. However, international research describes these grazing methods using diverse and sometimes inconsistent terminologies. This lack of standardization may create ambiguity and hinder comparative research on grazing methods. Here, to address this issue, we conducted a literature review aiming to identify common patterns of grazing methods based on shared grazing management criteria. Through multivariate analysis, we analyzed 249 experimental datapoints derived from 102 studies. We ran principal component analysis followed by hierarchical clustering on principal components on seven management criteria. This review identified 4 broad families of grazing methods: continuous grazing, conventional rotational grazing, deferred rotational grazing, adaptative multi-paddock grazing. This work distinguishes rotational from continuous grazing methods, as commonly described in the literature. Furthermore, it identifies adaptative multi-paddock grazing as a distinct and innovative group of rotational grazing. The approach developed here could serve as support to characterize and compare different grazing methods. Full article

A Collaborative Response to Addressing Family Violence with Racialized and Diverse Communities During Pandemic Recovery in Peel Region

Family violence is a social issue that impacts families and communities every day in Canada and around the world. As family violence rates continue to increase there is an urgent need for cross-sectoral collaboration to codesign social work and social service systems, in partnership with those experiencing family violence. This article will share learnings from a two-year community-based participatory research study that worked alongside survivors and witnesses of family violence, community partners from diverse social service agencies, and researchers, to understand experiences of family violence in racialized communities in Peel region, Ontario, Canada. An intersectional-trauma-informed approach guided the work that included establishing a community advisory board, hiring peer research assistants, Photovoice, and holding a knowledge exchange event (KEE) with survivors and witnesses of family violence, researchers, and community partners to rapidly generate ideas for intervention areas through a 25/10 crowdsourcing activity and codesign preliminary solutions through a mini hackathon. Key findings from the photovoice highlighted systemic failures and gaps experienced by those facing family violence. As we shifted into ideation, this preliminary focus on systems solidified and top ideas identified included barrier-free, culturally aware provision of services ranging from mental health supports, safe housing, financial independence, and accessing wrap-around services. Our work concluded with the collaborative development of preliminary solutions to these ideas and emphasized the need for cross-sectoral partnerships and lived experience engagement to change systems. Centering the voices of those who have experienced FV in system-level change and advocacy is necessary to ensure services and supports meet the needs of service users. Full article

Background/Objectives: This study aims to fill gaps in the existing literature through a qualitative evaluation of stakeholders involved in Food Is Medicine (FIM) programs. The primary objective was to examine the structural components, implementation process, and perceived impact of the University of California Los Angeles (UCLA) and Venice Family Clinic (VFC) Emergency Prepared Meal Program (UCLA-VFC Food Program), developed as a collaboration between a university and Federally Qualified Health Center (FQHC) during a period of community crisis. A secondary objective was to compare this program with three other FIM initiatives, identifying convergences and divergences in design and effectiveness. Methods: The methodology involved semi-structured interviews conducted with stakeholders across all four programs. Participants were recruited based on their direct involvement with program ideation, design, or implementation. Interviews were conducted online between July and September 2022, with 11 stakeholders. A thematic analysis was applied to the transcribed responses using an inductive thematic analysis. Results: Key findings highlighted four recurrent themes: (1) the critical role of leadership and a collaborative culture; (2) the importance of community partnerships and health education; (3) challenges related to logistics, funding, and sustainability; and (4) the need for assessment and evaluation. These findings provide valuable insight into the design of future FIM interventions, particularly those embedded in healthcare settings. Conclusions: In conclusion, this study offers preliminary evidence supporting the unique potential of university–community partnerships to address food insecurity. Unlike previous research that emphasized clinical outcomes, our findings provide a contextualized understanding of programmatic implementation. While further quantitative evaluation is necessary, this work lays the groundwork for a collaborative model between various entities including universities, healthcare systems, clinics, and community health/food services aimed at addressing social determinants of health.
