Global Trends and Cross-Country Differences in Authorship by Women in Academic Anaesthesiology Since 1996: A Repeated Cross-Sectional Analysis

Background/Objectives: Despite an increase in the number of women medical graduates, gender disparities persist in academic anaesthesiology. Women in medical science face challenges in publications, research funding, editorial board membership, and peer review, and they remain under-represented, particularly in senior authorship and leadership positions. Methods: This repeated cross-sectional bibliometric analysis examined global trends and cross-country differences in the representation of women as first, co-, and senior authors of peer-reviewed articles published in five high-impact anaesthesiology journals over three decades, with a focus on developments in recent years. Gender was assigned to authors’ first names algorithmically in two steps (Gender API and NamSor). Results: A total of 7571 publications were analysed, comprising 37,738 authors. Women constituted 11,732 (31.09%) authorships in total, and men consistently accounted for a substantial majority among authors (p < 0.001). Despite a significant overall increase in authorship by women—peaking in 2022 at 590 (36.88%) first authors, 2245 (37.85%) co-authors, and 402 (28.05%) senior authors (all p < 0.001)—stagnation was observed in recent years, with no significant changes for first and co-authors after 2016 (p > 0.05). Country-level analysis revealed few significant differences, with Japan consistently reporting the lowest percentages of women authors. Conclusions: This study underscores persistent gender disparities in academic anaesthesiology, despite a moderate increase in authorship by women over three decades. Gender disparity remains a global issue, and the recent stagnation highlights the necessity for more comprehensive efforts and innovative strategies to foster a more inclusive research community in academic anaesthesiology. Background: While Asian American individuals are well represented in medical schools in the United States, their advancement to senior positions within the field of orthopaedic surgery is disproportionately low. This underrepresentation not only limits diversity in leadership but also constrains the development of people-centred systems that reflect the needs of an increasingly diverse patient population. Objectives: This study systematically examines Asian American representation across the orthopaedic surgery professional pipeline, focusing on disparities between training-level representation and advancement into both faculty and leadership positions., and framing these gaps as a health equity concern. Methods: A comprehensive literature search for peer-reviewed original research articles was conducted via PubMed, EBSCO Open Research, Wiley Online Library, Google Scholar, and ScienceDirect. The potential articles were screened against prespecified eligibility criteria, and risk of bias was assessed using the Newcastle–Ottawa Scale (NOS). Data were then systematically extracted and analysed. Results: This analysis included 20 research articles investigating Asian American representation in orthopaedic surgery. The results demonstrated an underrepresentation of Asian Americans in orthopaedic leadership positions despite improvements in training programme representation with subspecialty clustering in adult reconstruction and spine. Asian American surgeons were less likely to occupy academic and leadership roles than their non-Asian American peers. Across studies, underrepresentation was consistently observed, with effect size estimates indicating a substantial disparity (e.g., pooled risk difference = 0.19; 95% CI [0.12, 0.28]) in those studies reporting comparative outcomes. Similarly, while Asian Americans in residency programmes increased, this growth did not translate proportionally to faculty advancement. In contrast, Asian women face compounded barriers, particularly in subspecialties like spine surgery. These inequities undermine workforce inclusivity and may reduce cultural and linguistic concordance with patients. Conclusions: Despite having strong representation in orthopaedic training programmes, Asian Americans are disproportionately absent from leadership positions. This poses a challenge to equity in surgical education and patient-centred care. To promote equity in leadership, focused mentorship, clear promotion processes, and institutional reform are necessary to address structural barriers to career advancement, this will reflect the diversity of both the workforce and populations served. This convergent mixed-methods study explores the lived experiences of Black women DEI leaders at predominantly white institutions within the context of an increasingly contentious national discourse surrounding diversity, equity, and inclusion (DEI) in higher education. Conducted prior to the 2024 election, a period marked by escalating resistance to DEI efforts, this research explores how America’s racial reckoning influenced institutional DEI initiatives and shaped the realities of those leading this work. Data were collected through a climate survey of 20 DEI administrators and semi-structured interviews with three senior-level Black women DEI leaders. The survey findings suggest that institutional commitments to DEI were largely reactive, emerging as crisis responses to national calls for racial justice. These efforts resulted in the short-term elevation of Black women into leadership roles, often without sustained structural support. The interview data revealed that Black women senior DEI leaders routinely encounter discrimination, marginalization, and the paradox of hypervisibility and invisibility within their roles. This study concludes with implications and suggestions for institutional policy and structural reform aimed at fostering more equitable and sustainable DEI leadership environments. Full article

Agrarian women are at the forefront of rural livelihoods increasingly affected by the frequency and severity of climate change impacts. However, their household livelihood resilience (HLR) remains limited due to gender-blind policies, scarce sex-disaggregated data, and inadequate consideration of gender-specific needs in resilience-building efforts. Grounded in participatory feminist research, this study employed a multi-method qualitative approach, including semi-structured interviews and oral history narratives, with 60 women in two climate-vulnerable provinces. Data were analyzed through thematic coding, CATWOE (Customers, Actors, Transformation, Worldview, Owners, Environmental Constraints) analysis, and descriptive statistics. The findings identify nine major climate-related events disrupting livelihoods and reveal a limited understanding of HLR as a long-term, transformative concept. Adaptation strategies remain short-term and focused on immediate survival. Barriers to HLR include financial constraints, limited access to agricultural resources and technology, and entrenched gender norms restricting women’s leadership and decision-making. While local governments, women’s associations, and community networks provide some support, gaps in accessibility and adequacy persist. Participants expressed the need for financial assistance, vocational training, agricultural technologies, and stronger peer networks. Strengthening HLR among agrarian women requires gender-sensitive policies, investment in local support systems, and community-led initiatives. Empowering agrarian women as agents of change is critical for fostering resilient rural livelihoods and achieving inclusive, sustainable development. Full article

Whilst there has been an increasing trend of women holding academic positions in European Higher Tertiary Institutions (HTIs), leadership positions are held predominantly by men. The study draws on radical feminism theory with which its methodology is aligned by investigating the perceptions of both genders. To that end, the study categorizes the impediments holding women back from breaking the glass ceiling into endogenous and exogenous factors. By doing so, the authors are in a better position to recommend the implementation of policies and procedures to address this inequality and navigate towards achieving sustainable gender equality. The research was conducted using an online survey questionnaire administered among all academic and administrative staff of universities in the Republic of Cyprus, the country with the highest glass ceiling in the EU. The authors found that the binary genders differ in their perceptions of what keeps women from breaking the glass ceiling and that this is attributable to exogenous factors, namely, (a) the walls created by male leaders, reinforcing a feeling of marginalization and mansplaining; and (b) family obligations enhancing women’s experiencing a lack of time and burnout. Furthermore, the exogenous factors and the extremely gendered higher echelons of HTIs underpin the endogenous factor of self-sabotage, making women feel they would rather avoid the toxic leadership environment with its lack of professional credit, a view supported by radical feminism theory. The authors suggest practical policy implications to rectify the gender imbalance in leadership in HTIs and suggest directions for future research. Full article

This article explores the role of the European Union (EU) as a normative gender actor promoting women’s participation in STEM (Science, Technology, Engineering, and Mathematics) within the Eastern Partnership (EaP) region. In a context marked by global inequality and overlapping international efforts, this paper assesses the extent to which EU-driven Europeanisation influences national gender policies in non-EU states. Using a postfunctionalist lens, this research draws on a qualitative analysis of EU-funded programmes, strategic documents, and a detailed case study encompassing Armenia, Georgia, Moldova, Ukraine, Belarus, and Azerbaijan. This study highlights both the opportunities created by EU initiatives such as Horizon Europe, Erasmus+, and regional programmes like EU4Digital and the challenges presented by political resistance, institutional inertia, and socio-cultural norms. The findings reveal that although EU interventions have fostered significant progress, structural barriers and limited national commitment hinder the long-term sustainability of gender equality in STEM. Moreover, the withdrawal of other global actors increases pressure on the EU to maintain leadership in this area. This paper concludes that without stronger national alignment and global cooperation, EU gender policies risk becoming symbolic rather than transformative. Full article

Despite decades of ongoing efforts to address gender equality, female physicians, particularly women of color, continue to face significant barriers in medicine, which are exacerbated by microaggressions. In this qualitative study, 133 female physicians recalled experiences with microaggressions and the impact of these experiences on their professional identities and career progression. Thematic analysis revealed four major themes: (1) disregard for professional status; (2) undermining contributions; (3) intersectionality; (4) impact on career mobility and professional confidence. Further, there was one emerging theme: leadership and culture. The results underscore the compounding effects of microaggressions for female physicians with intersecting identities. Highlighting the application of social identity theory, intersectionality, and organizational justice, this study provides a comprehensive view of the impact of microaggressions on female physicians, offering new perspectives on the intersectional nature of discrimination and its effects on professional identity and career satisfaction. Gender inequality remains a persistent issue in healthcare, especially in ophthalmology, where women face systemic barriers such as pay gaps, limited surgical opportunities, harassment, and unequal family expectations. Despite increasing entry into the field, women remain underrepresented in leadership, affecting career advancement and patient care. This study examines how virtual platforms, and co-led initiatives can address gender disparities in eye health. In 2021, Women in Ophthalmology, Seva Foundation, and Orbis International launched the Women’s Leaders in Eye Health (WLEH) initiative—a global community promoting mentorship, networking, and leadership development. Starting with virtual webinars and informal networking, the initiative expanded to in-person events in 2023 due to strong global engagement and demand. Early virtual programming, including webinars and “Coffee Hour” sessions, proved effective and laid the groundwork for broader offerings such as mentorship and professional development grants. WLEH’s success underscores the power of collaboration in promoting gender equity. By fostering connections and leadership pathways, WLEH offers a scalable model to break down gender challenges and uplift the next generation of women leaders to deliver more accessible eyecare globally. This inquiry articulates a conceptually cohesive framework to explore the interplay between ethical leadership–affective commitment, particularly in settings characterized by socio-environmental volatility. Central to the analysis is the examination of how ethical climate functions as a psychological conduit and how psychological empowerment modulates this pathway. Empirical validation was carried out with a sample of 448 employees (175 women, 273 men) from Colombia’s electricity sector, a context where institutional fragility elevates the salience of ethical practices. The study employed structural equation modeling to test the model’s robustness. Results underscore that ethical leadership cultivates a benevolent ethical climate, which in turn reinforces affective commitment. Importantly, this sequence is not fixed but is contingent upon psychological empowerment. The influence of ethical leadership on ethical climate and especially on affective commitment is amplified when empowerment is high; likewise, the ethical climate–affective commitment link becomes more salient under elevated empowerment conditions. These findings highlight the influence of ethical dynamics in organizations. Beyond model validation, this research contributes to broader conversations on social sustainability. Ethical leadership is shown to foster organizational climates rooted in fairness, stakeholder sensitivity, and moral coherence—factors essential for long-term institutional legitimacy. In environments such as Colombia’s electricity industry, where governance infrastructures are evolving, such leadership emerges as a necessary condition for rebuilding trust and promoting shared ethical standards. Accordingly, this study advocates for the systematic cultivation of ethical leadership as a means to enhance organizational loyalty and public credibility. The theoretical model presented here offers fertile ground for cross-cultural replication and further inquiry across industries in emerging economies. Full article

Malnutrition and Osteosarcopenia in Elderly Women with Rheumatoid Arthritis: A Dual Clinical Perspective

Background/Objectives: Rheumatoid arthritis (RA) is a chronic inflammatory disease frequently accompanied by comorbid conditions that contribute to disability and worsen long-term outcomes. Among these, malnutrition and osteosarcopenia remain under-recognised. This cross-sectional study aimed to assess the prevalence of malnutrition and osteosarcopenia among elderly women with RA and explore the clinical impact of these conditions. Methods: Sixty-five women over 65 years with RA were evaluated using Global Leadership Initiative on Malnutrition (GLIM) criteria for malnutrition and EWGSOP2-based assessments for sarcopenia; bone status was measured by dual-energy X-ray absorptiometry (DXA), trabecular bone score (TBS), and three-dimensional DXA (3D-DXA). Results: Malnutrition was identified in 49.2% and osteosarcopenia in 52.3% of participants. A significant bidirectional association was observed: malnourished patients had higher rates of osteosarcopenia (65.6% vs. 34.4%; p < 0.05), and osteosarcopenic patients were more frequently malnourished (61.8% vs. 39.1%; p < 0.05). Both conditions were associated with older age, lower body mass index (BMI), impaired muscle parameters, and reduced bone mineral density. Malnourished and osteosarcopenic patients reported worse fatigue and lower physical quality of life, despite similar inflammatory activity. Significant correlations were found between muscle mass indices and bone quality metrics assessed by 3D-DXA. These findings highlight a substantial burden of malnutrition and osteosarcopenia in elderly women with RA, even with well-controlled disease despite similar inflammatory activity (mean Disease Activity Score 28: 2.8 ± 1.0; 43.1% in remission. Conclusions: There is a substantial burden of malnutrition and osteosarcopenia in elderly women with RA that support the integration of systematic nutritional and musculoskeletal screening into routine care. Future studies should evaluate age- and disease-specific mechanisms and assess the benefit of multidisciplinary strategies to prevent frailty and improve long-term outcomes. Full article

This research investigates the pivotal role of women’s entrepreneurship in urban Ireland. Utilizing a demand–constraint–choice framework for analysis, it explores the diverse motivations behind Irish women’s entrepreneurial ventures, emphasizing the impact of time- and place-specific factors. The study also assesses gender equality and disparities. This qualitative research presents a comparative analysis of motivations, perceptions, and experiences conducted through semi-structured interviews in Ireland’s diverse cultural and economic contexts. The findings highlight the influence of cultural factors on women’s entrepreneurial choices and the significance of government support systems. They underscore the necessity of proactive government intervention for gender-inclusive economic development, offering insights applicable beyond Ireland. Valuable for policymakers, researchers, and entrepreneurs, the findings enhance understanding of women’s entrepreneurial leadership, contributing to global strategies for empowering women in entrepreneurship. Around the world, many post-conflict societies face the pressing challenge of reintegrating former combatants, especially women, into community life in ways that promote peace and sustainable development. Educational strategies that foster Social Innovation, Entrepreneurship, and Complex Thinking are increasingly recognized as essential tools in this process, helping to rebuild social cohesion and empower local leadership. This study evaluates the impact of the SEL4C methodology on strengthening key competencies in Social Entrepreneurship, Innovation, and Complex Thinking among 49 women signatories of the Peace Agreement in Colombia, who currently live in the Mariana Páez Territorial Training and Reincorporation Space (ETCR). Through a pre- and post-intervention design using validated instruments and statistical analyses (paired t-tests and effect size calculations), the study found significant improvements in areas such as Social Awareness, Social Innovation, and Scientific Thinking. Although the gains in Complex Thinking were moderate, the results show that the methodology effectively enhanced leadership skills and the ability to address systemic challenges—both crucial for successful reintegration. In practical terms, the findings support the use of SEL4C by organizations working in peacebuilding, educational institutions, and public programs that seek to empower women in vulnerable contexts. The methodology’s adaptability and evidence-based structure make it a valuable resource for designing participatory training processes focused on innovation, sustainability, and community transformation. Travestis and transgender women in Brazil face a disproportionate burden of mental health conditions, exacerbated by structural discrimination, violence, and social exclusion. This narrative review synthesizes evidence on the prevalence of depression, anxiety, suicidality, and substance use among travestis and transgender women in Brazil, and examines intersecting social and health disparities. We searched PubMed, Embase, and PsycINFO in April 2025, identifying peer-reviewed studies in English or Portuguese reporting mental health outcomes or associated social determinants of health in this population. Thirty-one studies across twelve different cities (n = 7683) were included and grouped into two thematic domains. Reported prevalence ranged from 16–70.1% for depression, 24.8–26.5% for anxiety, and 25–47.3% for suicidality. Substance use was also highly prevalent, with studies reporting high rates of alcohol (21.5–72.7%), tobacco (56.6–61.6%), cannabis (19–68.9%), and cocaine/crack (6–59.8%) use. Discrimination, violence, economic hardship, and HIV were consistently associated with psychological distress and barriers to care. These findings underscore the urgent need to integrate mental health, gender-affirming care, and HIV services into Brazil’s Unified Health System (Sistema Único de Saúde–SUS), strengthen anti-discrimination and violence-prevention policies, and adopt inclusive public health strategies that prioritize the leadership and lived experiences of transgender, nonbinary, and gender diverse people, particularly amid rising political threats to gender-affirming care. The Relationship Between Spirituality, Stress, and Depression Among Health Professionals in Greece

Background: Spirituality has emerged as a potential protective factor that may promote mental well-being and resilience among healthcare workers. Aim: This study aims to examine the relationship between spirituality, stress, and depression among healthcare professionals in Greece. Methods: This cross-sectional study surveyed 412 employees at the Corinth General Hospital in Greece, including medical, nursing, and administrative personnel. The data were collected using the Functional Assessment of Chronic Illness Therapy—Spiritual Well-Being 12 (FACIT-SP12), the Perceived Stress Questionnaire (PSQ), and the Center for Epidemiologic Studies Depression Scale (CES-D). The statistical analyses included non-parametric tests, correlation coefficients, and multiple regression. Results: The mean spirituality score was 34.6 (±6.83), while the stress and depression scores were 74.6 (±14.87) and 14.7 (±10.20), respectively. Spirituality was significantly and negatively correlated with both stress (r = −0.479, p < 0.001) and depression (r = −0.452, p < 0.001). Gender, years of service, educational level, and marital status were also associated with variations in stress, depression, and spirituality levels. Women and those with lower education reported significantly higher levels of stress and depression. Additionally, some demographic variables such as age and sector of employment did not show significant associations with spirituality or depression. The regression analysis confirmed spirituality as an independent predictor of lower stress levels (B = −1.158, p < 0.001). Conclusions: Spirituality is a significant predictor in mitigating stress and depression among healthcare workers. Promoting a supportive spiritual climate and incorporating elements of spiritual leadership in healthcare settings may enhance employee well-being and resilience. Future research should expand on these findings across diverse institutional and cultural contexts. Full article

This study highlights the pivotal role of women in corporate governance and their potential influence on achieving sustainable goals, particularly in the context of emerging countries. Using the two-step System-Generalized Method of Moments (GMM) with the dynamic short panel data of 351 nonfinancial listed companies in Vietnam from 2010 to 2022, this research examines the dynamics between earnings management and tax avoidance, focusing on the moderating role of women on the board of directors. The results confirm that both accrual-based and real earnings management are positively associated with corporate tax avoidance. However, there is a significant negative relationship between female representation on the board and tax avoidance, as well as a significant moderation of the relationship between earnings management and tax avoidance. This study reinforces that female leadership contributes to reducing earnings management and tax avoidance through improved monitoring and governance of corporate ethical activities, emphasizing the importance of strategically empowering women in leadership roles. The implications of this study are given to minimize harmful financial practices and align corporate strategies with ethical practices. Full article
