Background/Objectives: The Achilles tendon is the largest tendon in the human body; it is frequently ruptured during sports and other dynamic physical activities. The purpose of this study was to compare recovery patterns over time between injuries to the dominant (DMT) and non-dominant (NDMT) limbs, and to examine differences in the limb symmetry index (LSI) for the uninjured side. Methods: This study includes a retrospective analysis of individuals who completed a standard rehabilitation program for 12 months and had regular checkups every three months. The study sample comprised 17 patients with DMT injuries and 17 patients with NDMT injuries, all active male recreational participants who underwent surgical repair of an Achilles tendon rupture. Outcome measures included dorsiflexion range of motion (ROM), calf circumference, plantarflexion strength, hop test performance, and the Y-Balance Test (YBT). Results: Both groups demonstrated continuous, progressive improvement in ROM, plantarflexion strength, hop test distance, and YBT scores, showing a significant main effect of time (p < 0.05). Although DMT showed greater strength than NDMT at 6 and 9 months, this difference was no longer significant at 12 months (p > 0.05). In all three YBT directions, the DMT group achieved greater reach distances than the NDMT group at 12 months (p < 0.05). At the final follow-up, both groups exceeded 90% LSI in ROM, calf circumference, plantarflexion strength, and hop performance (p < 0.05). However, in the YBT, only the DMT group surpassed 90% LSI, whereas the NDMT group showed poorer recovery. Conclusions: ROM, calf circumference, and muscle strength ultimately showed no significant differences between groups, but dynamic balance recovery was superior in the DMT group. These findings suggest that clinicians and rehabilitation specialists should consider leg dominance when designing rehabilitation programs. Full article

Background: A sedentary lifestyle contributes to chronic disease risk in adults and may predict unfavourable body composition in adolescents. Declining physical activity and rising sedentary behaviour are linked to increasing global obesity rates. Given the scarcity of longitudinal studies examining how participation in organized sports and screen device use relate to body composition in early adolescence, this study aims to address this gap by analyzing temporal trends in both sexes. Methods: A sample of 158 Italian students, 38% of whom were female, was followed longitudinally from ages 11 to 13. Annual anthropometric assessments were conducted, and self-reported data on screen time and organised sports participation were collected. Fat mass (FM), fat-free mass (FFM), fat mass index (FMI), fat-free mass index (FFMI), body mass index (BMI), and waist-to-height ratio (WHtR) were subsequently calculated, along with annual increments. Repeated-measures ANOVA assessed age and sex effects, while multiple regression models evaluated associations between behavioural variables or sex and body composition indices. Results: Significant differences in %F, FM, FFM and its increment, WHtR and its increment, FMI, and FFMI (all p < 0.01) were observed by age and sex interaction. At age 13, weekly sports participation was negatively associated with annual increments in %F (β = −0.204, p = 0.04) and FMI (β = −0.227, p = 0.03). Female sex was associated with greater increments in %F (β = 0.188, p < 0.05) and WHtR (β = 0.323, p < 0.01), and with smaller increments in FFM (β = −0.421, p < 0.01). No significant associations were found for screen time (p > 0.05). Conclusions: Sporting during early adolescence seems to have positive effects on body composition changes, while sex-specific patterns warrant further attention. A deeper understanding of how early adolescent lifestyle factors, such as physical activity and sedentary behaviour, shape body composition is essential for promoting long-term health. Full article

The present study proposes to identify information from health, educational and sports science studies that used Bronfenbrenner’s theory of human development to verify the complex relationship between factors associated with physical activity (PA) and sedentary behavior (SB) in children and adolescents. The scoping review will be developed across seven databases (PubMed, Scopus, SPORTDiscus, Web of Science, PsycINFO, ERIC, and Scielo). The inclusion criteria were formulated based on the PCC (Population, Concept, Context) framework: (a) children and adolescents (5–17 years); (b) studies on PA and/or SB that used Bronfenbrenner’s theory; (c) any context. Only peer-reviewed journal articles published in English, Spanish, or Portuguese will be included; grey literature will not be included. Finally, two reviewers will screen studies using Rayyan. A standardized charting form will be used to extract data on study characteristics and the factors mapped considering Bronfenbrenner’s theory components. This study is expected to show how Bronfenbrenner’s theory has been applied to explain PA and SB in children and adolescents, as well as to map the methodological tools used in this area, identifying gaps and providing a clear framework for future research on the complex and multilevel determinants of PA and SB in children and adolescents. Cervical spine (c-spine) injuries are a prominent concern in sporting activities, and dynamic axial (i.e., head-first) impacts are associated with a high risk of c-spine trauma. This methodology study implanted pressure sensors in post-mortem human subject (PMHS) cervical intervertebral discs (CIVDs) to assess biomechanical response and disc pressure changes during dynamic injurious axial impacts. Two fresh frozen male head–neck PMHS (cephalus with complete c-spine) were instrumented with miniature pressure sensors (Model 060S, Precision Measurement Company, Ann Arbor, MI, USA) at three CIVD levels (upper, middle, and lower c-spine). Experiments replicated the Nightingale et al. studies, simulating a rigid unconstrained head vertex (0°) axial impact. PMHS were raised to a drop height of 0.53 m to reach the desired impact velocity of ~3.2 m/s and were allowed to drop vertically. Results showed characteristic c-spine deformations/buckling motion patterns and marked CIVD pressure differences between CIVD levels. The more cranial (C2–C4) and caudal (C6–T1) CIVD exhibited greater and more comparable pressure values than those of the mid-spine (C4–C6), and the pressure in upper/lower levels was at least ~four to six times higher than that of the middle. This study establishes the feasibility and assesses the potential of CIVD pressure as a biomechanical metric for assessing injurious axial loading and contributes a novel experimental framework for future injury tolerance research and model validation. Background: Patient-reported outcome measures (PROMs) provide important insights into recovery after anterior cruciate ligament reconstruction (ACLR). Previous research suggests that males and females recover differently after ACLR, with females reporting greater pain, slower functional gains, and lower psychological readiness at later stages of rehabilitation. However, it is unknown if patient-reported outcomes differ by sex early after ACLR. To address this gap, we conducted a cross-sectional analysis comparing patient-reported outcome measures between sexes three months after ACLR. We hypothesized that females would report worse PROMs compared to males. Methods: This cross-sectional analysis used data from a prospectively maintained ACL reconstruction cohort. Fifty-six individuals (female: 23 and male: 33) with primary, unilateral ACLR completed PROMs three months after surgery. These PROMs included the Knee Injury and Osteoarthritis Outcome Score (KOOS; Symptoms, Pain, Activities of Daily Living, Sport and Recreation, Quality of Life), International Knee Documentation Committee (IKDC) subjective score, Knee Outcome Survey–Activities of Daily Living Scale (KOS-ADLS), Anterior Cruciate Ligament–Return to Sport After Injury (ACL-RSI), and the Tampa Scale of Kinesiophobia (TSK). All outcomes were expressed on a 0 to 100 percent scale, with higher scores indicating better outcomes, except for TSK, where lower scores indicated better outcomes. Normality was assessed within sex, using the Shapiro–Wilk test. Two-tailed independent-samples t-tests with Welch correction were used for approximately normal variables; otherwise, Mann–Whitney U tests were utilized (α = 0.05). Several outcomes had limited statistical power to detect MCID-sized differences, and findings for these measures should be interpreted cautiously. Results: No significant differences between sexes were found for any of the PROMs. Males trended towards having better KOOS Sport and Recreation and IKDC, but these were not statistically significant, and the effect sizes were small-to-moderate. Conclusions: No statistically significant sex-based differences were detected in PROMs at approximately 3 months after ACLR, indicating that any sex-related divergences between these measures may not occur until later in recovery. Background: Low peak bone mass (PBM) is a major contributor to later osteoporosis risk. This study sought to understand young people’s knowledge of factors associated with bone health. Methods: Young people in Aotearoa New Zealand were approached. Eight focus groups (26 participants in total, aged 11 to 17 years) were conducted using a semi-structured approach with open-ended questions and prompts. Transcripts were thematically coded using an inductive content analysis approach. Results: Knowledge of factors associated with good bone health was limited. There was a general awareness of the positive and negative impacts of many lifestyle behaviours on health generally, but not specifically PBM. Dairy intake was commonly mentioned as being beneficial for bone health. Some participants reported potential benefits of sport, but most did not know that weight bearing activity specifically was beneficial. Conclusions: Knowledge of osteoporosis and lifestyle factors that impact PBM was limited. Educational interventions involving promotion of bone health knowledge and supporting weight bearing physical activity in adolescents may be an important contributor to public health strategies. This paper addresses the problem of imbalance between coastal and inland areas and recognises the reuse of abandoned buildings as an evolutionary regeneration strategy which, through specific interventions linked by a system of routes for tourism and sport, can gradually trigger sustainable development on a regional scale. It presents research conducted in recent years on behalf of local administrations and continued in national and European projects. The reference context is the Abruzzo region, where coastal, hilly and mountainous areas are a short distance apart and include both densely built-up and populated urban centres and small depopulated towns surrounded by landscapes of high environmental value. The objective is to define, through the responsible use of built resources, viable and sustainable strategies for regeneration and rebalancing oriented towards the concept of transformative resilience. The methodology adopted is divided into phases and includes both theoretical developments and case study applications according to an approach that networks building restoration and reuse interventions in the region. The key results consist of defining a reuse logic that considers the regional territory as a whole, linking different resources, functions and environments. This logic, which envisages the organisation of new functions on a regional scale, emphasises the capacity of building reuse to produce positive effects on the territory and trigger socio-economic development dynamics. This research forms part of the experience underlying a project of significant national interest (PRIN 2022 TRIALs), which will provide guidelines for activating the transformative resilience capacities of inland areas of central Italy. Background/Objectives: Intense physical activity can cause gastrointestinal symptoms, negatively impacting athletic performance. A low-FODMAP diet has the potential to reduce these symptoms and is increasingly being considered by physically active individuals. The aim of this review is to present the current knowledge on the importance of FODMAPs in sports nutrition. Methods: A narrative review was conducted in PubMed, Web of Science, ScienceDirect, and Google Scholar, covering publications published up to October 2025. Original studies, reviews, and meta-analyses addressing the relationship between FODMAP intake and gastrointestinal symptoms during physical activity were included. Selected articles were assessed for specific criteria, and the results were grouped thematically to present the current state of knowledge. Results: FODMAP consumption increases the risk of intestinal symptoms. Short-term FODMAP restriction, especially before and during exercise, reduced the severity of symptoms in most of the analyzed studies. Data on the long-term effects of a low FODMAP diet on the health, nutrition, and gut microbiota of athletes remain limited. Conclusions: A strategy of short-term FODMAP restriction in athletes’ diets shows potential for reducing gastrointestinal symptoms. An optimal approach requires individualization. Further research is needed to monitor potential side effects and long-term outcomes. Full article

Muscular strength plays a crucial role in sports performance and is often evaluated using vertical jump tests such as the Squat Jump (SJ) and Countermovement Jump (CMJ). Measurements based on flight time (FT) assume that takeoff and landing postures are identical, yet differences in ankle position can introduce systematic errors. This study examined whether dorsiflexion (DF) or plantarflexion (PF) of the ankle during the flight phase affects jump height. Forty-three active university students completed four repetitions each of SJ and CMJ under DF and PF across two sessions. Jump heights were recorded using a Chronojump-Boscosystem platform. No significant difference was observed in SJ between DF and PF, while CMJ heights were consistently higher under DF (DF: 28.29 cm ± 7.7 cm vs. PF: 27.08 cm ± 7.03 cm, p = 0.001; d = 0.16). Notably, the effect of DF appeared more pronounced in CMJ, suggesting that higher jumps are more sensitive to postural variations. These findings could suggest that DF can artificially increase jump heights as measured on a jump platform, without reflecting true improvements in force production. Coaches and practitioners should interpret FT-derived data with caution, particularly for higher jumps. Future research combining precise motion capture with force platforms could directly track center-of-mass changes and validate this mechanism. Full article

Background: Arthroscopic anterior cruciate ligament (ACL) reconstruction is widely recognised as the primary treatment for ACL injuries. However, with the increasing incidence of sports-related injuries and growing demand for rehabilitation services, conventional rehabilitation models—largely reliant on therapists’ experience and subjective assessment—are increasingly insufficient to meet the clinical need for precise and individualised rehabilitation programmes. This study aimed to evaluate the effectiveness of a rehabilitation protocol incorporating an artificial intelligence (AI)-based assessment and correction system on functional recovery following ACL reconstruction. Methods: Using convenience sampling, 80 patients undergoing ACL reconstruction between June to December 2024 were recruited for this randomised controlled trial. Participants were randomly assigned to either a control group (n = 40), which received conventional functional exercise training, or a trial group (n = 40), which received rehabilitation intervention guided by an AI-based assessment and correction system. Knee function scores (Lysholm score, IKDC score), Berg Balance Scale (BBS) scores, joint range of motion (ROM), and rehabilitation exercise compliance scores were collected and analysed 1, 2, 3, and 4 months postoperatively. Results: Compared with the control group, the trial group demonstrated significantly greater improvements in Lysholm score, IKDC score, BBS score, and active knee joint ROM (p < 0.05) at postoperative assessment points. Additionally, rehabilitation exercise adherence was significantly higher in the trial group compared to the control group (p < 0.05). Conclusions: Rehabilitation protocols integrating AI-based assessment and correction systems effectively enhance knee function recovery, joint mobility and balance ability following ACL reconstruction. Moreover, these protocols significantly improve rehabilitation exercise adherence, demonstrating superior efficacy compared to conventional rehabilitation approaches. This digital rehabilitation model represents an efficient and promising intervention for postoperative ACL rehabilitation. Full article

Background. Arm wrestling is a complex strength sport requiring detailed biomechanical analysis. This study investigated elbow functionality in medial epicondylitis using kinematic and electromyographic (EMG) approach. Methods. Hook technique specialists underwent a 10-session rehabilitation program (manual therapy and high-power laser). Outcomes were assessed via the NRS and QuickDASH. Functional analysis utilized surface EMG (Biceps Brachii, Pronator Teres, Brachioradialis, Extensor muscle) and an inertial sensor measuring Mean Jerk (MJ) for movement fluidity. Results. Data analysis for the eleven male athletes (mean age: 22.4 years) revealed substantial improvements following the intervention. NRS decreased from 5.1 to 1.5, and QuickDASH dropped from 25.2 ± 5.3 to 5.5 ± 1.0, while mean jerk remained stable (3.37 to 3.22). Pronator Teres activation markedly increased in the concentric phase (30.14 µV to 127.3 µV), indicating better coordination. Biceps Brachii (BB): Assumed a more pronounced concentric role, likely a compensatory adaptation after pain reduction; and lastly, Common Finger Extensor increased activation suggested increased extensor loading during the push phase. Conclusions. The combined kinematic and EMG data were crucial for identifying underlying musculoskeletal dysfunctions. The findings support an integrated approach for elbow health in arm wrestlers, providing objective data for targeted rehabilitation and prevention programs focusing on both pain and neuromuscular coordination. Studies have rarely examined the effects of changes in legal anti-doping knowledge (LADK) on doping tendencies in athletes. This study aimed to evaluate the effectiveness of a structured educational intervention focused on LADK and to analyze how LADK changes affect elite athletes’ doping tendency. The participants were athletes (n = 310; 156 females; 24.1 ± 4.2 years of age), all actively competing at the senior national or international level in either individual (N = 119) or team sports (N = 191), tested on sociodemographic-, sport-, doping-factors (including doping tendency—DT), and LADK. Participants were randomly divided into an experimental group (E: N = 140) and a control group (C: N = 170). The E group participated in a structured educational program on LADK. A pre- and posttest design was used to evaluate changes in LADK (dependent variable). Logistic regression was calculated to evaluate the association between LADK and binarized DT (negative vs. neutral/positive DT). Factorial ANOVA for repeated measurements revealed significant improvement in LADK in the E group, with significant ANOVA effects for time (F test = 35.8, p < 0.05) and time × group interaction (F test = 12.27, p < 0.05). The logistic regression did not reveal significant correlations between LADK and DT. Further studies exploring younger athletes, as well as long-term, multidimensional interventions, are warranted. Leg dominance has been linked to an increased risk of lower-limb injuries in sports. This study examined bilateral asymmetry in muscle synergy patterns during one-leg stance on stable and multiaxial unstable surfaces. Twenty-five active young adults (25.6 ± 3.9 years) performed unipedal stance tasks on their dominant and non-dominant legs while surface electromyography (EMG) was recorded from seven lower-limb muscles per leg. Muscle synergies were extracted using non-negative matrix factorization (NMF), and structural similarity was assessed via cosine similarity with the Hungarian matching algorithm. Four consistent synergies were identified under both surface conditions, accounting for 88% of the total variance. On the stable surface, significant asymmetry in muscle weightings was observed in the rectus femoris (p = 0.030) for Synergy 1 and in the rectus femoris (p = 0.042), tibialis anterior (p = 0.024), peroneus longus (p = 0.023), and soleus (p = 0.006) for Synergy 2. On the unstable surface, asymmetry was evident in the biceps femoris (p = 0.048) for Synergy 2 and the rectus femoris (p = 0.045) for Synergy 3. Overall, dominance-related asymmetry was more pronounced under stable conditions and became more subtle as postural demand increased, revealing bilateral asymmetry in neuromuscular coordination during unipedal stance. Background, Clinical Significance: Anterior cruciate ligament (ACL) injuries are common, however the occurrence of concurrent gastrocnemius muscle tears is exceptionally rare. Given this, the diagnosis and management of this pattern of injury is poorly characterised, with lack of current clinically relevant classification systems and evidence-based guidelines to guide treatment. Early recognition is essential. with advanced imaging critical to guiding the diagnosis and management of patients with this pattern of injury. Case presentation: A 39 year old man presented with acute right knee swelling, pain and difficulty weightbearing following a sports-related fall. Clinical examination was suspicious for an ACL injury. Magnetic Resonance Imaging (MRI) of the knee demonstrated the disrupted and displaced ACL fibres, with extensive peri-cruciate oedema around the expected position of the ACL. It was associated with partial avulsion of the medial gastrocnemius origin and incomplete avulsion of the lateral gastrocnemius origin. The patient was referred for an urgent orthopaedics review and is currently on trial of conservative management. Conclusions: In this case report and review of the literature, we evaluate the current understanding of the complexities of combined musculoskeletal injuries and limitations of existing classifications in providing accurate diagnosis and management strategies. Given the rarity of this presentation, the case underscores the lack of evidence-based recommendations for early management, particularly in young, active individuals who are at risk of significant long-term functional impact.
