Clinical Evaluation of Ripasudil for Corneal Edema: A Large-Scale Retrospective Cohort Study

Objectives: This study evaluated the therapeutic potential of topical Ripasudil hydrochloride hydrate in managing various forms of corneal edema. Methods: This retrospective study included 96 patients of 72.20 ± 10.52 years, with 53 females (55.2%) who were treated with Ripasudil for corneal edema, with a mean treatment duration of 5.2 ± 2.3 months, divided into four groups: post-cataract surgery (n = 32), Fuchs endothelial corneal dystrophy (FECD; n = 29), post-Descemet membrane endothelial keratoplasty (DMEK; n = 25), and post-penetrating keratoplasty (PKP; n = 10). All patients were treated with Ripasudil, typically administered three times daily in the first week and twice daily in the following months. Clinical efficacy outcomes were assessed using changes in best-corrected visual acuity (BCVA), central corneal thickness (CCT), and endothelial cell count (ECC) with specular microscopy, anterior segment optical coherence tomography (OCT), and slit-lamp examination, while intraocular pressure (IOP) was measured using the iCare tonometer. Results: Ripasudil treatment led to a reduction in CCT and improvement in visual acuity across most groups, with minimal changes in ECC. CCT decreased by 30.44 μm (p < 0.001), 25.56 μm (p < 0.001), 8.41 μm (p = 0.05), and 6.80 μm (p > 0.1); visual acuity improved by 0.27 (p = 0.001), 0.18 (p = 0.02), 0.17 (p = 0.025), and 0.07 logMAR units (p > 0.1); and ECC changed by +7.0 (p > 0.1), 15.4 (p > 0.1), −7.6 (p > 0.1), and 2.3 cells/mm2 (p > 0.1) in the post-cataract surgery, FECD, post-DMEK, and post-PKP groups, respectively. Conclusions: No adverse events or progression of edema were recorded during the follow-up period. These findings support the role of Ripasudil as a non-invasive pharmacological approach to managing corneal edema and delaying or possibly avoiding surgical interventions, such as corneal transplantation, in selected cases. Non-Descemet Stripping Automated Endothelial Keratoplasty (nDSAEK) for Late Endothelial Failure After Mushroom Keratoplasty: A Retrospective Analysis of Visual and Anatomical Outcomes

Background: Mushroom penetrating keratoplasty (MPK) is an alternative to traditional penetrating keratoplasty (PK) that offers improved graft survival and reduced immunological rejection. However, MPK grafts may still experience endothelial failure over time. This study evaluates the outcomes of non-Descemet Stripping Automated Endothelial Keratoplasty (nDSAEK) as a surgical approach for endothelial decompensation following MPK. Methods: A monocentric, retrospective study was conducted at the Ophthalmology Department of Sant’Orsola-Malpighi Hospital, including patients who underwent nDSAEK for endothelial failure after MPK between 2022 and 2024. Pre- and postoperative best-corrected visual acuity (BCVA), central corneal thickness (CCT), and endothelial cell density (ECD) were assessed. Results: Eighteen eyes from 18 patients (mean age: 39.94 years) were included. Primary MPK indications were post-keratitis leucoma (77.7%), traumatic scarring (16.7%), and keratoconus (5.6%). At one year, mean BCVA improved significantly from 1.40 ± 0.42 logMAR to 0.46 ± 0.19 logMAR (p < 0.05), and mean CCT decreased from 721 ± 70.12 µm to 616 ± 52.80 µm (p < 0.05). The mean postoperative ECD was 1748 ± 100 cells/mm2, with lower eye values requiring re-bubbling. No immunological rejection or graft failures were reported. Conclusions: nDSAEK is a promising treatment for MPK endothelial failure, demonstrating good visual and anatomical outcomes. Full article

The cornea, the transparent anterior window of the eye, critically refracts light and protects intraocular structures. Corneal pathologies, including trauma, infection, chemical injury, metabolic diseases, genetic conditions, and age-related degeneration, can lead to significant visual impairment. While penetrating keratoplasty or full-thickness corneal transplantation remains a standard and effective intervention for severe corneal dysfunction, limitations in donor tissue availability and the risk of immunogenic graft rejection necessitate alternative therapeutic strategies. Furthermore, for cases of isolated epithelial disfunction, a full-thickness cornea graft may not be required or effective. This review examines the potential of corneal epithelial constructs derived from autologous stem cells with functional barrier properties for corneal reconstruction and in vitro pharmacotoxicity testing. In this review, we delineate the current limitations of corneal transplantation, the advantages of stem cell-based approaches, and recent advances in generating engineered corneal epithelium. Finally, we address remaining technical challenges and propose future research directions aimed at clinical translation. Full article

Background/Objectives: Keratoconus (KC) is a bilateral asymmetric corneal ectasia characterized by progressive corneal thinning, irregular astigmatism, and impaired visual acuity. The National Eye Institute (NEI) developed the Visual Function Questionnaire (VFQ-25) to assess the impact of visual impairment on quality of life. This study aimed to evaluate the effect of penetrating keratoplasty (PKP) on quality of life and visual acuity in KC patients one year postoperatively. Methods: A retrospective study was conducted between January 2018 and December 2022 at the Ophthalmology Department of Saint Barbara Hospital, Trauma Center, Sosnowiec, Poland. A total of 71 patients (86 eyes) diagnosed with KC underwent PKP. The VFQ-25 questionnaire and visual acuity measurements were assessed preoperatively and one year postoperatively. Results: The study cohort included 71 patients (20 females, 28.17%; 51 males, 71.83%). Preoperative visual acuity ranged from less than 0.05 on the Snellen chart to 0.5. Postoperatively, visual acuity improved to a range of 0.1–1.0. A visual acuity of 1.0 was achieved in 21 eyes (24.42%; 5 females, 24%; 16 males, 76%), with a statistically significant improvement (p < 0.01). The mean VFQ-25 composite score increased from 57.96 (±17.58) preoperatively to 81.42 (±14.66) postoperatively (p < 0.001). Domains with the lowest preoperative scores were “role difficulties,” “general vision,” and “mental health,” while “color vision” scored highest. Conclusions: PKP significantly enhances both objective visual acuity and subjective quality of life in KC patients, as reflected in VFQ-25 questionnaire outcomes. Full article

This report presents two cases of herpes simplex keratitis recurrence after COVID-19 mRNA vaccination in patients on herpetic prophylaxis due to recurrent herpetic keratitis. A 58-year-old man with a history of a previous penetrating keratoplasty presented with blurred vision and evidence of corneal endothelitis 48 h after the first dose of the m-RNA vaccination, and a 24-year-old male student came with a dendritic ulcer 72 h post first vaccination dose. The original prophylactic treatment of 400 mg of acyclovir twice daily was increased to five times per day for a week for both patients. The grafted patient additionally received an increase in Dexamethasone 0.1% from twice daily to four times a day. Improvement was noted within two days and documented at the weekly review, during which both patients returned to their prophylactic antiviral regime without further recurrence. At the time of their second dose of vaccination, both patients followed the same regime with an increase in treatment as per the first dose of vaccination without recurrence. Our findings suggest that patients with recurrent herpetic disease receiving prophylactic treatment need close monitoring when experiencing even subtle symptoms of recurrence and may benefit from an increase in their dose to therapeutic levels during the first days after the COVID-19 mRNA vaccination. Purpose: Pellucid Marginal Degeneration (PMD) is a rare ectatic corneal disorder characterized by inferior peripheral thinning and significant irregular astigmatism. Despite its clinical similarities to keratoconus, PMD presents unique diagnostic and therapeutic challenges. This review aims to provide a comprehensive update on the pathophysiology, clinical features, diagnostic approaches, and management strategies for PMD, emphasizing the latest advancements in treatment options. Methods: A systematic literature search was performed in MEDLINE (via PubMed), Google Scholar, and Scopus up to February 2025 using the terms: “pellucid marginal degeneration,” “PMD,” “ectatic corneal disorders,” “keratoplasty in PMD,” “corneal cross-linking in PMD,” “ICRS in PMD,” “toric IOL PMD” and their Boolean combinations (AND/OR). The search was restricted to English-language studies involving human subjects, including case reports, case series, retrospective studies, clinical trials, and systematic reviews. A total of 76 studies met the inclusion criteria addressing treatment outcomes in PMD. Results: PMD is characterized by a crescent-shaped band of inferior corneal thinning, leading to high irregular astigmatism and reduced visual acuity. Diagnosis relies on advanced imaging techniques such as Scheimpflug-based corneal tomography, which reveals the characteristic “crab-claw” pattern. Conservative management includes rigid gas-permeable (RGP) lenses and scleral lenses, which provide effective visual rehabilitation in mild to moderate cases. Surgical options, such as CXL, ICRS, and toric IOLs, are reserved for advanced cases, with varying degrees of success. Newer techniques such as CAIRS, employing donor tissue instead of synthetic rings, show promising outcomes in corneal remodeling with potentially improved biocompatibility. Penetrating keratoplasty (PK) and deep anterior lamellar keratoplasty (DALK) remain definitive treatments for severe PMD, though they are associated with significant risks, including graft rejection and postoperative astigmatism. Conclusions: PMD is a complex and progressive corneal disorder that requires a tailored approach to management. Early diagnosis and intervention are critical to optimizing visual outcomes. While conservative measures are effective in mild cases, surgical interventions offer promising results for advanced disease. Further research is needed to refine treatment protocols and improve long-term outcomes for patients with PMD. Full article

Objectives: The aim was to present the complicated diagnostic and therapeutic process of atypical, painless keratitis caused by a cosmopolitan protozoan of the genus Acanthamoeba. Methods: This Case Report describes a medical case involving a 48-year-old woman who occasionally wears soft contact lenses and was referred to our hospital for treatment due to deteriorating visual acuity in her left eye. The diagnostic process included the isolation of amoebae from corneal scrapings and the morphological and molecular identification of the etiological agent of the infection. Results: After examination, painless atypical keratitis was diagnosed, initially considered recurrent herpetic keratitis. However, antiviral treatment did not bring about any improvement. Further observation revealed a dense, central, annular infiltrate on the periphery of the cornea. Despite treatment, the corneal infiltrate did not improve and the patient required therapeutic penetrating keratoplasty. Ultimately, the patient underwent combined surgery: corneal transplantation with cataract phacoemulsification and intraocular lens implantation. The postoperative course was uneventful. Conclusions: Acanthamoeba keratitis should be included in the differential diagnosis of keratitis, even in the absence of its characteristic feature of severe ocular pain, especially in contact lens wearers and patients who have had herpetic keratitis. Infection of the cornea with the Herpes simplex type 1 virus causes nerve degeneration, which probably translates into a painless course of Acanthamoeba castellanii infection. Full article

Background/Objectives: Post-keratoplasty astigmatism can limit visual recovery even after successful corneal transplantation. Femtosecond laser-assisted arcuate keratotomy (FSAK) has emerged as a method to reduce high residual astigmatism and enhance visual outcomes. This study aimed to evaluate the outcome of FSAK in treating astigmatism following keratoplasty. Methods: This retrospective study included 32 eyes from 31 patients who underwent FSAK after keratoplasty. Inclusion required complete suture removal, regular corneal topography, and the absence of additional ocular pathology or prior intraocular surgery. Data collected included uncorrected (UCVA) and best-spectacle-corrected visual acuity (BSCVA), manifest refraction, and tomographic parameters. The primary outcomes were changes in visual, refractive, and tomographic measures across the entire cohort, with further subgroup analysis between penetrating keratoplasty (PKP) and lamellar keratoplasty (LKP) eyes. Secondary outcomes were documentation of complications. Results: UCVA improved significantly from 0.92 ± 0.33 to 0.58 ± 0.39 LogMAR (p < 0.001). BSCVA showed a non-significant trend toward improvement from 0.32 ± 0.21 to 0.26 ± 0.22 LogMAR (p = 0.158). The manifest cylinder reduced significantly from −6.15 ± 2.75 D to −4.49 ± 2.92 D (p = 0.037). Corneal topography revealed significant postoperative steepening in keratometric values. While overall outcomes were comparable between the subgroups, LKP eyes demonstrated a greater myopic shift and a higher rate of overcorrection, whereas PKP eyes tended toward undercorrection. Conclusions: FSAK appears to be an effective approach for reducing post-keratoplasty astigmatism and improving uncorrected visual acuity. Given the biomechanical differences between graft types, individualized treatment planning based on graft characteristics may enhance surgical predictability and optimize outcomes. Full article

Background and Clinical Significance: Brittle cornea syndrome (BCS) is a rare, autosomal recessive connective tissue disorder characterized by extreme corneal thinning, high myopia, and increased risk of spontaneous or trauma-induced ocular rupture. It is primarily caused by mutations in the ZNF469 or PRDM5 genes, which regulate extracellular matrix integrity. Early recognition and diagnosis of BCS are crucial to prevent severe visual impairment. This report presents two genetically confirmed cases of BCS in Albanian siblings, emphasizing the diagnostic value of whole-exome sequencing and individualized surgical management strategies. Case Presentation: Two siblings—a 28-year-old male and a 25-year-old female—presented with progressive visual deterioration and marked corneal thinning (<200 µm). Both had a history of spontaneous ocular rupture following minor trauma in the contralateral eye. Detailed ophthalmologic evaluation revealed keratoglobus, high myopia, and irregular astigmatism. Genetic testing identified the homozygous pathogenic variant c.974delG (p.Cys325LeufsX2) in the PRDM5 gene in both cases. The male underwent penetrating keratoplasty (PKP), achieving a best-corrected visual acuity (BCVA) of 20/30. The female initially underwent deep anterior lamellar keratoplasty (DALK), which was converted to PKP intraoperatively due to central endothelial perforation, resulting in a BCVA of 20/25. Both patients remained complication-free over a 7-year follow-up period. Conclusions: These cases highlight the importance of early genetic diagnosis and a tailored surgical approach in managing BCS. Long-term monitoring and protective strategies are essential to prevent complications. Incorporating genetic testing into clinical practice can enhance diagnostic accuracy and guide personalized treatment plans in patients with hereditary corneal dystrophies. Full article

Background/Objectives: In this study, a mouse model of sutured endothelial keratoplasty was established and compared with a traditional penetrating keratoplasty (PKP) model in both syngeneic (BALB/c) and allogeneic (C57/BL6) patterns. Methods: For the endothelial keratoplasty (EK) model, chimeric donor tissues consisting of BALB/c epithelium-stroma combined with either syngeneic (BALB/c) or allogeneic (C57/BL6) stroma-endothelium were transplanted into BALB/c mice. Graft transparency, gene expression, and mRNA levels in the transplanted tissues were assessed using polymerase chain reaction (PCR) and quantitative real-time reverse transcription PCR (qRT-PCR) to evaluate inflammatory status. Results: Allogeneic PKP had a higher opacity score than syngeneic PKP. In contrast, syngeneic EK mice had similar opacity scores to those of allogeneic EK mice. Upregulation of CXCR3, the receptor for CXCL10, was demonstrated by qRT-PCR in allogeneic PKP mice but not in allogeneic EK mice. Conclusions: Comparison between the syngeneic and allogeneic PKP groups revealed differences in CXCR3 mRNA expression, suggesting that CXCR3 could be a potential biomarker for rejection in the PKP mouse model. Additionally, the EK model did not show CXCR3 upregulation despite the opaque cornea due to nonspecific inflammation. Purpose: This study aimed to refine the criteria for EndoART implantation regarding posterior corneal irregularity; to improve the selection of candidates for this synthetic alternative to endothelial keratoplasty. Methods: This study analyzed the impact of posterior corneal surface elevation differences; anterior chamber depth (ACD); and preoperative corneal pachymetry on the success of EndoART implant adhesion. Patients undergoing EndoART implantation at the Medical University of Vienna were assessed using OCT to measure corneal irregularities. Postoperative outcomes, including re-bubbling rates; implant adhesion; and visual acuity changes, were monitored. Results: EndoART successfully adhered in eyes with moderate posterior irregularities (elevation differences up to 204 µm). Severe irregularities (elevation differences > 200 µm) resulted in implant detachment. No significant correlation was found between corneal pachymetry or ACD and adhesion failure. Glaucoma devices and prior penetrating keratoplasty did not significantly affect adhesion. Some cases required re-bubbling, and patients reported pain reduction and moderate improvements in visual acuity. Conclusions: This study found that EndoART implantation can be successful despite posterior corneal irregularity. EndoART represents a viable solution for patients with poor biological graft survival prognosis, including those with glaucoma or prior surgeries, expanding its potential use and addressing the global donor cornea shortage. Background/Objectives: The aim of this study was to review the therapeutic and prognostic factors influencing Acanthamoeba Keratitis (AK) management. Methods: A systematic search was performed across MEDLINE® (via PubMed), Web of Science®, and Scopus®, following the PRISMA 2020 guidelines, and registered in PROSPERO (CRD420251010774). Studies reporting AK treatment regiments and prognostic factors were included. After extracting the data from the included articles, the relevant aspects of the treatment and the prognostic factors were compared and summarized. Results: Sixty-one articles were included: nine were prospective, including 3 randomized controlled trials (RCTs), and fifty-two were retrospective. The findings suggest that therapeutic epithelial debridement (TED), followed by an association with biguanides, diamidines, and an antibacterial agent, is a strong initial treatment option. An adjunctive medical treatment with topical voriconazole 1% or oral miltefosine may also be considered. Surgical approaches were also assessed when the pharmaceutical therapy failed, with Deep Anterior Lamellar Keratoplasty (DALK) playing an important role in the cases without a deep stroma involvement. Early Therapeutic Penetrating Keratoplasty (TPK) should be used as a salvage therapy and Optical Penetrating Keratoplasty (OPK) should be used for rehabilitation purposes. Key prognostic factors include older age, delayed diagnosis, corticosteroid use before prompt diagnosis, poor initial best corrected visual acuity (BCVA), and AK stage at presentation. Conclusions: The initial treatment with TED, biguanides, and diamidines remains the foundation of treatment. Surgical options can be considered in advanced cases. An early diagnosis, age, and initial BCVA are prognosis factors that should be considered. Future research may focus on improvement of protocols and searching for novel agents. Full article

While full-thickness penetrating keratoplasty (PK) has seen a decline in favor of partial-thickness techniques like endothelial keratoplasty (EK) and anterior lamellar keratoplasty (ALK), PK remains indicated for corneal disease involving the entire corneal thickness. Mushroom keratoplasty (MK) is a noteworthy modification of PK, designed to combine the refractive benefits of a large-diameter anterior lamellar graft with the graft survival advantage of limited replacement of the corneal endothelium. Leveraging the use of a microkeratome, the MK donor graft can be prepared by microkeratome dissection, thereby achieving a central interface compatible with 20/20 Snellen vision. This review explores the various surgical techniques, visual outcomes, graft survival rates, and complication rates associated with MK for various indications. Full article

Corneal Graft Dehiscence in Patients on Oral Angiotensin-Inhibiting Medications: Plausible Relationship and Review of the Literature

Wound dehiscence is a rare complication after penetrating keratoplasty (PK) that may occur with or without prior trauma. Multiple factors may influence corneal wound healing, including patient factors, corneal wound characteristics, and other external factors. There is also the possibility that systemic medications could impact corneal wound healing. Possible factors that may predispose a cornea to experience wound dehiscence are discussed. We propose a hypothesis that oral angiotensin-inhibiting medications could play a role in reduced corneal wound healing. A literature review was conducted to investigate the effect of angiotensin inhibitors on corneal wound healing. Five patients on systemic oral angiotensin-inhibiting medications at the time of PK developed dehiscence of the graft–host wound junction following removal of sutures. The dehiscence required resuturing in all cases and resulted in an expulsive choroidal hemorrhage and complete loss of vision in one eye. Age, diabetes, lack of corneal neovascularization, early suture removal, underlying epithelial basement membrane dystrophy, corneal oedema, slower tapering of topical corticosteroid dosage, and glaucoma medication with preservatives were possible predisposing factors for some of these instances of wound dehiscence. However, oral angiotensin-inhibiting medications were taken by all patients in this series, and the literature suggests that ACE inhibitors and ARBs can reduce corneal fibrosis, resulting in inadequate healing. Oral angiotensin-inhibiting medications could have played an anti-fibrotic role in these corneae and predisposed them to wound dehiscence with minimal trauma. Despite limited evidence, these medications warrant further investigation as potential modulators of corneal wound healing. Full article

Background: Managing post-keratoplasty astigmatism remains challenging. Even though graft viability is the main concern in keratoplasty, astigmatism might hinder vision recovery following a successful corneal transplant. Photorefractive keratectomy (PRK) and topography-guided PRK may be options for correcting refractive errors in patients who underwent keratoplasty. The aim of the study was to compare the results of PRK and topography-guided PRK in patients who had undergone corneal keratoplasty. Methods: This study was conducted at the Chair and Ophthalmology Clinic of the Medical University of Silesia, at the Railway Hospital Katowice, from 2023 to 2024. Patients who underwent penetrating keratoplasty due to keratoconus or corneal scar (post-traumatic and post-inflammatory) with a residual spherical myopic or mixed myopic astigmatic refractive defect were included in this study. The studied patients were divided into two groups: 15 patients underwent PRK and 15 patients underwent topography-guided PRK. Each participant underwent a preoperative examination, including uncorrected visual acuity (UCVA) and best spectacle-corrected visual acuity (BSCVA) measured on the Snellen chart (LCD panel for visual acuity testing Frey CP-400, Optotech Medical, Niepołomnice, Niepołomice, Poland), cycloplegic refraction, corneal pachymetry and topography (Schwind Sirius+, Pentacam), wavefront aberrations (Schwind Peramis, Pentacam), applanation tonometry, and anterior and posterior segment examinations, conducted at baseline and 1, 3, 6, and 12 months. Results: Keratoconus was the most common reason for keratoplasty (80% vs. 60%). Following PRK, the mean KI in group (1) decreased significantly to 43.88 ± 3.64 (p < 0.001), and in the group (2), the mean diopters decreased significantly after the intervention to 46.46 ± 2.80 (p < 0.001). The mean spherical refractive error in group (2) changed significantly after the intervention, reaching −2.72 ± 1.28 D (p < 0.001). The mean cylinder in group (1) changed significantly after PRK to have a mean of −2.75 ± 1.44 D (p < 0.001). Also, in group (2), it changed significantly after the intervention to have a mean of −2.95 ± 1.99 D (p < 0.001). There was a significant increase in both uncorrected visual acuity (UCVA) and best-corrected visual acuity (BCVA) after topography-guided PRK at all the follow-up points of 1, 3, 6, and 12 months (p < 0.001). The mean higher-order corneal aberrations (HOAs) after topography-guided PRK were 1.33 ± 0.50, ranging from 0.22 to 2.34 (p < 0.001). Conclusions: Topography-guided PRK is safer and more effective in correcting aberrations and refractive errors after corneal keratoplasty than regular PRK. Additionally, topography-guided PRK reduces high-order aberrations by utilizing advanced topographic mapping of the cornea, enabling precise customization of the treatment to address individual corneal surface irregularities.
