Offline Factors Influencing the Online Safety of Adolescents with Family Vulnerabilities

Online safety guidance is frequently delivered as a specialist technology issue without considering adolescents’ home lives, offline vulnerabilities, or wellbeing. Yet, while the digital world offers connection, autonomy, and entertainment, vulnerable teens also encounter more violent content, sexual exploitation, and content concerning body image, self-harm or suicide than their non-vulnerable peers. Many struggle with social inclusion or less engaged and credible caregiver e-safety support, which may contribute to their negative experiences online. To improve their online safety and resilience, caregivers and educators might consider offline factors that can mediate exposure to online harms. This study compared the experiences of 213 adolescents with family vulnerabilities to 213 age- and gender-matched non-vulnerable adolescents. The contribution of (a) e-safety education, (b) close friendships, (c) a trusted adult at school, and (d) life-affecting worry was considered. No differences were found for exposure to, or engagement with, e-safety education. However, despite having received e-safety education, those with family vulnerabilities were more at risk of encountering severe online harms. This was mediated by life-affecting worry and parental e-safety guidance. These findings provide unique insights into the impact of family vulnerabilities on adolescents’ exposure to online harms and suggest a more holistic intervention framework for caregivers. Introduction: A limited March 2024 Google Scholar search regarding COVID-19 redeployment to emergency care in fourteen medical specialties found no oncologist returns. Identifying oncologist redeployment through a scoping review of peer-reviewed research from several databases investigates this anomaly. Method: Searched are Web of Science, Scopus, PubMed, OVID, Google Scholar, and the Cochrane COVID-19 Study Register with the keywords “burnout AND COVID-19 AND emergencies AND oncologists” concerning the emergency experienced, their burnout response, and patient outcome. Results: Following the PRISMA scoping review process, the assessment is of eight reports from 17,848 results. The finding is that there was a redeployment of oncologists to emergency care. It was defined in various ways and caused oncologist burnout for several internally and externally directed reasons. These reasons negatively affected patient outcomes, contributing to the adoption of different coping techniques by oncologists. Oncologists, uniquely among medical specialists, experienced burnout regarding empathy for the increased mortality risk of their patients and the diminished doctor/patient bond. They also lacked symptom-directed coping. Conclusion: The results of this study may reinforce to oncologists the importance of their doctor/patient dyad and of initiating coping strategies that include symptom-directed health improvement techniques when the redeployment of oncologists is again to emergency care. Ensuring mathematics education for all learners, including students with blindness learning in mainstream classrooms, is crucial. This exploratory research aims to clarify the characteristics of geometric learning among students with blindness and to identify the factors contributing to the challenges faced by this population. The Van Hiele theory of geometric thought served as a reference framework. Qualitative data were gathered through group interviews with specialists in the field of education for students with blindness and analyzed using inductive analysis. Participants affirmed that students with blindness progress through Van Hiele levels of geometric thought in a manner similar to sighted students, suggesting that much of the learning can take place alongside sighted peers in mainstream classrooms. However, they also highlighted the unique challenges these students face in reaching level 0, a level where students recognize shapes without a formal understanding of their properties or attributes. Among the reasons for these challenges were that for these particular students, subskills, such as bimanual exploration, hand coordination, and cognitive integration, are required to reach level 0. The study also identified the necessity for specialists in visual impairment education to guide students using appropriate tasks and learning materials that reflect the characteristics of haptic perception. Since level 0 serves as a gateway to both basic and advanced geometry, the findings underscore the importance of providing differentiated support that targets these subskills early in students’ schooling. To ensure meaningful geometry instruction, mainstream teachers are encouraged to collaborate with specialists in visual impairment education, who can guide the selection of appropriate learning tools and support the development of the subskills. Background/Objectives: Telemedicine has emerged as a transformative solution to healthcare access challenges in Sub-Saharan Africa, where many populations remain underserved. This systematic review focuses on the adoption, implementation, and technological prospects of telemedicine in South Africa, Kenya, and Nigeria, three countries leading the region in healthcare innovations. Methods: A systematic search of PubMed, Scopus, and Web of Science, guided by PRISMA protocols, identified 567 studies published between 2014 and 2024, of which 53 met the inclusion criteria with a focus on telemedicine adoption, implementation, and technological prospects in the selected countries. A structured critical appraisal was used to assess potential biases in the included studies’ design, selection criteria, and reporting, while findings were thematically analysed to provide actionable and comparative insights. Results: The findings reveal that South Africa has the highest adoption rate, focusing on specialist teleconsultations, chronic disease management, and mental health services. Kenya demonstrates strong mHealth integration and advanced mobile applications, particularly in maternal health, HIV care, and sexual and reproductive health. While facing infrastructural and regulatory constraints, Nigeria is advancing innovations for remote diagnosis and teleconsultation. Conclusions: By synthesising evidence from peer-reviewed literature, the review identifies adoption trends, enabling factors, and opportunities for scaling telemedicine in these contexts. Despite these advancements, challenges persist, including regulatory gaps, digital literacy limitations, and infrastructure constraints. Addressing these barriers requires targeted investments in broadband expansion, policy harmonisation, and healthcare workforce training to optimise telemedicine’s impact and ensure its sustainability as a healthcare delivery model in Sub-Saharan Africa. Background: Auditory Verbal Therapy (AVT) has spread globally in the past few years due to its proven positive effect as a rehabilitation method for children with hearing loss (HL). In 2023, a three-year hospital-based AV intervention was implemented in Denmark as a government-funded offer to all children with HL. In the present study, we introduce and study a protocol for facilitating this implementation with a specific focus on clinical applicability and quality assurance. Methods: A working group was set up to drive the implementation process and establish a base for clinical collaboration and coproduction. The final protocol included (1) regular workshops and meetings with all AV specialists, (2) the creation of a database to collect data on the effect of the intervention, and (3) mandatory testing of the auditory skills and receptive vocabulary of the children with HL and a screening of their behavioural and emotional status with the Strengths and Difficulties Questionnaire (SDQ). Results: Data from 53 children with a mean age of 6.6 years were analysed in this study. Results from the SDQ showed that the children scored like the Danish norm on both the SDQ total difficulty score and SDQ prosocial score. Conclusions: This study’s findings indicate that children with HL, who participated in three years of hospital-based AV intervention, demonstrate the same emotional and behavioural problems and the same social strengths as their hearing peers. We hope that this study can inspire and guide others who want to implement an AV intervention in a hospital setting. Full article

The increase in the provision of English language teaching programs in primary school contexts in Asia has resulted in a concomitant increase in non-specialist teachers, often generalist homeroom teachers, to address the shortage. However, non-specialist teachers often lack the pedagogical training necessary for effective English instruction, which often negatively impacts their self-efficacy. This study was designed to examine the effects of a professional development workshop, aimed at improving “classroom English” proficiency, on these teachers’ self-efficacy. It also explored sources influencing self-efficacy during the in-class implementation of what they learned in the workshop. Quantitative results revealed that the immediate impact of the training was substantial, leading to a rapid increase in self-efficacy. However, this effect appeared to diminish after three months of teaching. Qualitative findings indicated that non-specialist teachers’ perceptions of capability were significantly influenced by collegial assurances and support from individuals who evaluated their progress and achievements. Notably, support from specialist teachers emerged as a critical factor in sustaining or diminishing the enhanced self-efficacy of non-specialist teachers. These findings highlight the importance of providing needs-based, continuous professional development opportunities and establishing robust peer-support systems within schools to ensure the effective translation of training outcomes into classroom practices. The care needs of children and adolescents with type 1 diabetes and their families are frequently approached as if they were identical to those of adults, overlooking the distinct challenges young people may face. It has been stated that children and adolescents often find conventional conversations with diabetes specialists tiresome and unpleasant. The present study focuses on familial experiences of an advanced carbohydrate counting program tailored to children and adolescents. The data encompass semi-structured interviews with families who participated in a group-based child-centered advanced carbohydrate counting program. The analysis revealed five themes: (1) peer-to-peer interaction is an essential determinant of sharing and learning; (2) illness perception significantly influences dietary intake; (3) normalization of diabetes in everyday life eases the disease burden; (4) repetition of dietary knowledge is important for retention; and (5) creating a safe and playful learning environment is crucial to engaging children and adolescents in their own treatment. The present findings suggest that it would be beneficial to explore and consider alternative teaching approaches that are adapted to a more interactive and engaging learning environment that is specifically tailored to children and adolescents. This entails moving away from traditional individual approaches. Full article

Objective: People with variations in sex characteristics (VSCs) have been receiving inadequate care for many decades. The Chicago consensus statement in 2006 aimed to introduce improved comprehensive care, which would include the transition of care from pediatric to adult services organized by multidisciplinary teams. Yet, the evidence for transitional care is scarce. The aim of this paper is to outline the delivery of transition of care for adolescents and young adults with VSCs. Method: Seven focus groups were conducted with health care professionals and peer support groups by care teams in Central, Northern, and Western Europe. The data from the focus groups were examined using reflexive thematic analysis. Results: Even though the transition of care has been implemented in the last two decades, it remains inadequate. There are differences among countries, as the quality of care depends on available resources and variations in sex characteristics. Moreover, there are significant hurdles to adequate transition of care, as there is lack of time and funding. The lack of adult care providers and psychosocial support often leaves young adults with VSCs to navigate the health care system alone. Conclusion: The outcome of the study shows that the transition of care is organized through the department of pediatric endocrinology. The quality of care varies due to resources and variations in sex characteristics. A lack of adult specialists, and especially psychosocial support, represents the biggest obstacle for young adults and adults in navigating the health care system and for improvements in the provision of health care to adults. There is a risk of re-traumatization, as adolescents and young adults must often repeat their medical history and educate adult care providers who are insufficiently trained and knowledgeable. Background: Human-centric artificial intelligence (HCAI) aims to provide support systems that can act as peer companions to an expert in a specific domain, by simulating their way of thinking and decision-making in solving real-life problems. The gynaecological artificial intelligence diagnostics (GAID) assistant is such a system. Based on artificial intelligence (AI) argumentation technology, it was developed to incorporate, as much as possible, a complete representation of the medical knowledge in gynaecology and to become a real-life tool that will practically enhance the quality of healthcare services and reduce stress for the clinician. Our study aimed to evaluate GAIDS’ efficacy and accuracy in assisting the working expert gynaecologist during day-to-day clinical practice. Methods: Knowledge-based systems utilize a knowledge base (theory) which holds evidence-based rules (“IF-THEN” statements) that are used to prove whether a conclusion (such as a disease, medication or treatment) is possible or not, given a set of input data. This approach uses argumentation frameworks, where rules act as claims that support a specific decision (arguments) and argue for its dominance over others. The result is a set of admissible arguments which support the final decision and explain its cause. Results: Based on seven different subcategories of gynaecological presentations—bleeding, endocrinology, cancer, pelvic pain, urogynaecology, sexually transmitted infections and vulva pathology in fifty patients—GAID demonstrates an average overall closeness accuracy of zero point eighty-seven. Since the system provides explanations for supporting a diagnosis against other possible diseases, this evaluation process further allowed for a learning process of modular improvement in the system of the diagnostic discrepancies between the system and the specialist. Conclusions: GAID successfully demonstrates an average accuracy of zero point eighty-seven when measuring the closeness of the system’s diagnosis to that of the senior consultant. The system further provides meaningful and helpful explanations for its diagnoses that can help clinicians to develop an increasing level of trust towards the system. It also provides a practical database, which can be used as a structured history-taking assistant and a friendly, patient record-keeper, while improving precision by providing a full list of differential diagnoses. Importantly, the design and implementation of the system facilitates its continuous development with a set methodology that allows minimal revision of the system in the face of new information. Further large-scale studies are required to evaluate GAID more thoroughly and to identify its limiting boundaries. Full article

Background: Up to 80% of children with autism spectrum disorders (ASDs) have mental health issues—either emotional or behavioral problems. The underlying mechanisms are still unknown, even if emotional regulation (ER) is considered to play a major role in child and adolescent psychopathology. Several studies link the ability to regulate the intensity and quality of emotions with executive functioning. Therefore, we aimed to investigate the association between executive functions (EFs) and ER and affective problems in children with ASD. Methods: This meta-analysis is based on a literature search of peer-reviewed journals from the following databases: Scopus, ProQuest, Ebsco, Science Direct, Springer Link and Clarivate. We analyzed 15 studies that investigated the link between EF, ER or affective problems (APs) in children and adolescents with ASD aged between 2 and 18 y with ASD. To assess the effect size of the relationship between EF and ER, and EF and AP, 15 studies comprising 54 effect sizes were analyzed. Results: Our findings revealed a small effect size regarding the association between EF and ER, r = 0.331, p = 0.034, and a small effect size regarding the association between EF and AP, r = −0.213, p = 0.024. No significant moderators were found. The results are presented in regard to the two analyses developed, as well as a short review of the studies included in the meta-analysis. Conclusion: Even if there are several limitations of this study, especially considering the small number of studies included, the results suggest that it is worth considering EF as an underlying mechanism for the appearance of emotional or behavioral problems in children with ASD. These findings have important implications for the development of ASD intervention plans, as well as for increasing awareness among specialists about the importance of executive functions in school adjustment and social functioning. Prostate cancer is a common and life-altering condition among Canadian men, yet little is known about how follow-up care is provided to those who have completed treatment. Despite improving survival rates, survivors experience ongoing needs and are often not provided with support to manage them. This study sought to investigate the post-treatment experiences and needs of prostate cancer survivors and to determine if and how these needs are being met. Using a qualitative description design, prostate cancer survivors who had completed treatment took part in semi-structured interviews. The interviews were recorded and analyzed thematically. The participants experienced varying levels of satisfaction with their follow-up care. While primary care providers played significant roles, continuity of care and specialist involvement varied. Most participants felt unprepared to manage the long-term effects of their cancer due to a lack of information and resources from their healthcare providers. Instead, participants turned to their peers for support. Ongoing physical and psychosocial needs went unmet and had significant impacts on their daily lives. Participants felt that support for these issues should be automatically integrated into their follow-up care. In summary, this study revealed the importance of integrated, patient-centered follow-up care for prostate cancer in Atlantic Canada. Full article

Cyberbullying has emerged as a societal issue, and the majority of the research examining cyberbullying perpetration samples adolescent and/or emerging adult populations. A paucity of empirical attention has focused on young children (aged 8–10) regarding their cyberbullying frequency and predictors. The current study sampled 142 US youth aged 8–10 years and assessed their cyberbullying perpetration rate and cellular phone ownership. Results indicated that (a) older participants were more likely to cyberbully than their younger peers; (b) higher rates of cyberbullying were found for youth who already owned a cellular phone; and (c) an interaction between participant age and cellular phone ownership was found, suggesting that cyberbullying was highest for only the 10-year-old group who owned a cellular phone. These findings have implications for (a) parents, school administrators, health care providers, and anyone else interested in better understanding the predictors of cyberbullying perpetration; (b) intervention specialists focused on reducing cyberbullying in youth; and (c) a researcher interested in understanding the basic theoretical underpinnings of cyberbullying. Based on these findings, we recommend that (a) cyberbullying interventions be administered to youth as early as elementary school; (b) parents/guardians carefully consider the positive and negative consequences of youth cellular phone usage; and (c) increased communication between youth and parents/guardians concerning youth cellular phone activities. The authors prove that various forms of co-operative work such as problem-based learning, peer teaching and small group discussions are effective in training university students within further training programs in Special Translation. The study participants were students of the “Special Translation and Editing of Technical, Economic and Media Texts” further training program. The experiment was conducted within the module referred to as “The Translation of Culture Specific Realities”. The authors highlight the importance of comprehending cultural issues apart from extensive knowledge of translation techniques for specialists in translation, since intercultural misunderstanding can be a major obstacle in the way of effective communication. The conducted survey also revealed the importance of collaborative learning in increasing the motivation of the students to develop intercultural competencies and master communication strategies. Today, students are much more inquisitive than they were in the past because they are familiar with resources that make the learning process more interesting. Applying strategies of collaborative learning when students are working co-operatively in pairs or groups can significantly improve learning outcomes and boost motivation. Full article

Background: The PREVENTION e-platform was developed to provide accessible and evidence-based health information tailored to different Breast Cancer (BC) risk levels. The demonstration study objectives were to (1) assess the usability and perceived impact of PREVENTION on women with assigned hypothetical BC risk levels (i.e., near population, intermediate or high) and (2) explore perceptions and recommendations for e-platform improvement. Methods: Thirty women with no history of cancer were recruited through social media, commercial centers, health clinics, and community settings in Montreal, Qc, Canada. Participants accessed e-platform content tailored to their assigned hypothetical BC risk level, and then completed study e-questionnaires including the user Mobile Application Rating Scale (uMARS), an e-platform quality scale (i.e., in terms of engagement, functionality, aesthetics, and information). A subsample (n = 18) was randomly selected for an individual follow-up semi-structured interview. Results: The e-platform overall quality was high, with mean M = 4.01 (out of 5) and SD = 0.50. A total of 87% (n = 26) agreed or strongly agreed that PREVENTION increased their knowledge and awareness of BC risk, and 80% would recommend it to others while reporting likelihood of following lifestyle recommendations to decrease their BC risk. Follow up interviews indicated that participants perceived the e-platform as a trusted source of BC information and a promising means to connect with peers. They also reported that while the e-platform was easy to navigate, improvements were needed for connectivity, visuals, and the organization of scientific resources. Conclusion: Preliminary findings support PREVENTION as a promising means to provide personalized BC information and support. Efforts are underway to further refine the platform, assess its impact in larger samples and gather feedback from BC specialists. Full article

Sleep problems are common among United States (U.S.) veterans and are associated with poor health, mental health, and functioning. Yet, little is known about insufficient sleep and factors contributing to sleep disparities among veterans experiencing homelessness. We conducted semi-structured interviews to better understand the clinical, environmental, and structural factors contributing to insufficient sleep among veterans and to improve care for this population. Interviews were conducted with 13 providers caring for veterans experiencing homelessness, including physicians, psychologists, nurses, social workers, and peer support specialists. Providers worked at a West Coast VA institution serving a large population of veterans experiencing homelessness. Interviews were analyzed for themes pertaining to sleep using the social-ecological model as a framework. On an individual level, factors influencing sleep included psychiatric disorders and use of substances. On an interpersonal level, factors included safety concerns while sleeping. On an environmental level, factors included noise and proximity to others as barriers to sleep. On the organizational level, logistical issues scheduling sleep clinic appointments and lack of transportation to attend sleep clinic appointments were identified as treatment barriers. These findings can inform future research studying the impact of sleep on health and housing outcomes and interventions addressing sleep among veterans experiencing homelessness. Full article
